BACKGROUND: Reconstruction of the anterior cruciate ligament (ACL) commonly involves patellar tendon (PT) or hamstring tendon(s) (HT) autografts. There is no consensus with respect to the choice between these two grafts in ACL surgery.
OBJECTIVES: This review compared the outcomes of ACL reconstruction using PT versus HT autografts in ACL deficient patients.
SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (April 2008), the Cochrane Central Register of Controlled Trials (2008, Issue 2), MEDLINE (1966 to April 10 2008), EMBASE (1980 to April 1

The quadriceps tendon autograft can be used for primary and revision anterior cruciate ligament (ACL) reconstruction. Despite several successful clinical reports, graft fixation issues remain, and the ideal technique for fixation continues to be controversial. We present a technique of ACL reconstruction with quadriceps tendon autograft (QTA) using a patellar bone block. The tendon end is fixed in the femoral tunnel and the bone plug in the tibial tunnel using reabsorbable interference screws. The advantages of this technique are related to the increase in stiffness of the graft, the achieveme

PURPOSE OF THE STUDY: Injury to infrapatellar branches of the medial saphenous nerve are incriminated in disorders of the anterior aspect of the knee, particularly following bone-tendon-bone reconstruction. We demonstrated in prior anatomic work the usefulness of using a double-incision minimal approach for harvesting the patellar transplant in order to spare the nerve branches.
MATERIAL AND METHODS: The patellar transplant is harvested via two vertical incisions, one on the apex of the patella and the other along the protrusion of the anterior tibial tubercle. After harvesting the bony trans

This article describes a medial patellofemoral ligament (MPFL) reconstruction procedure using an artificial ligament for recurrent patellar dislocation. The MPFL has been shown biomechanically to be the primary restraint among the medial patellar stabilizers. Although various predisposing factors are involved in lateral patellar dislocation, we believe that the MPFL, as a primary restraint, should undergo primary reconstruction for patellar dislocation. The results of MPFL reconstruction using a mesh-type artificial ligament and medial retinaculum slip coverage for recurrent patellar dislocati

In a prospective study on ACL reconstruction we treated 17 patients with a "central third-bone-patellar tendon-bone" autograft (PT group) and 32 patients with a doubled semitendinosus/doubled gracilis autograft (HS group). Patients were reviewed at 6 weeks, and at 3, 6, and 12 months postoperatively and the groups compared. Up till 6 months, the KT-1000 side-to-side difference was significantly greater in the HS group (P<0.05) and at 6 and 12 months isokinetic hamstrings strength was lower (P<0.05). No significant differences were noted at any stage with respect to quadriceps strength, functio

PURPOSE: The purpose of this study was to examine the strength of the knee flexors and knee extensors after two surgical techniques of ACL reconstruction and compare them to an age and activity level matched control group.
METHODS: Twenty-four subjects who had undergone ACL reconstruction greater than 1 yr previously were placed into one of two groups according to autograft donor site: patellar tendon (BPB; N = 8) and hamstring (H; N = 16), and compared with an active, control group (N = 30). Knee flexor and extensor strength was evaluated using isovelocity dynamometry (5 speeds, eccentric an

The results of intraarticular anterior cruciate ligament reconstruction with either the patellar tendon or the semitendinosus and gracilis tendons (four strands) were prospectively compared in a consecutive series of 60 patients with chronic injuries. A single surgeon performed arthroscopically assisted reconstructions in an alternating sequence. Preoperative and operative data revealed no significant differences between the two groups. After 28 months of followup there were no significant differences in the incidence of symptoms, and recurrent giving way was present in only one knee with semi

A new surgical technique to uniformly harvest the middle one-third of the patellar tendon for anterior cruciate ligament reconstruction is described. The technique uses a guide system of 8 templates of varying widths to guide the saw cuts and "collared," depth-controlled saw blades and drill bits to produce a standard cutting and drilling depth. This kind of template system lessens the risks of graft harvesting complications such as patella fracture, bone graft fragmentation, suture pull out, and judgment errors pertaining to graft depth, length, or width. The Patellar Tendon Graft Guide was u

BACKGROUND: Medial patellar luxation is a common orthopedic condition in small breed dogs, though its etiology remains poorly understood. The aim of this study was to assess the morphometry of the femoral trochlea, patella and patellar ligament in small breed dogs with and without grade 2 medial patellar luxation. This retrospective study evaluated small breed dogs weighing ≤ 15 kg including 80 stifles with Grade 2 MPL and 80 controls. The femoral trochlear length to patellar length ratio (TL: PL), trochlear curvature to condylar curvature ratio (TC: CC) and patellar ligament length to patella

Patellar instability commonly affects adolescents and young athletes and is associated with injury to the medial patellofemoral ligament (MPFL). MPFL reconstruction is a well-established treatment for recurrent instability; however, complications related to graft tensioning, fixation, and incomplete restoration of medial soft-tissue restraints persist. We describe a surgical technique for allograft MPFL reconstruction that emphasizes stepwise imbrication of the medial knee layers, anatomic femoral tunnel placement, and all-suture fixation to the patella. This approach restores both the static

Anterior cruciate ligament reconstruction using a bone-patellar tendon-bone autograft is widely regarded as the graft of choice for many sports medicine surgeons for young, active patients with anterior cruciate ligament injuries. It is favored for use in athletes because of its high return-to-play rates and low re-rupture rates. Despite its benefits, there are many pitfalls in the surgical technique, and the use of a careful surgical technique can avoid them. This article describes our preferred technique.

The medial patellofemoral ligament (MPFL) is the main restraining force against lateral patellar displacement in the first 20° of flexion and is disrupted after patellar subluxation or dislocation. Management of acute patellar dislocations is controversial, and many clinicians opt for conservative treatment in the acute phase. However, a traumatic rupture of the MPFL warrants surgical attention. Several considerations must be made by surgeons attempting reinsertion of the MPFL, including the choice of implant and timing of surgery, to restore the anatomy and biomechanics of the patellofemoral

Our paper presents a unique case of a 64-year-old patient after a fall, treated with oral antidiabetic drugs for type II diabetes mellitus. Following a series of examinations, a bilateral injury was diagnosed - patellar ligament tear on the right side and rupture of quadriceps femoris muscle on the left side. It is a rare injury, complicated by simultaneous involvement of both knee joints. The used therapy consisted of a bilateral surgery followed by gradual verticalisation, first with the support of a walking frame and later with the use of forearm crutches. During the final examination, the

Gait pattern alterations were previously reported in association with objective patellar instability (OPI). Gait pattern comparison between a series of patients having undergone medial patellofemoral ligament (MPFL) reconstruction and a sample of control subjects. Thirty patients at 6 months postoperatively after MPFL reconstruction and thirty control subjects were enrolled in the study for a clinical and biomechanical assessment including gait analysis at three selected walking rates using the GAITRite(®) system. The mean raw IKDC score was 73 (± 19), and the mean Kujala knee function was 84

BACKGROUND: The medial patellomeniscal ligament has been determined in vitro to be the only substantial distal-medial restraint to lateral patellar displacement. The purpose of this study of prospectively collected data was to determine if isolated deficiency of the medial patellomeniscal ligament, resulting in excessive lateral patellar translation and associated symptoms, could be identified. We hypothesized that, if symptoms were associated with isolated deficiency of that ligament, the laxity and symptoms should resolve after plication of that ligament alone.
METHODS: Testing for lateral

Patellar tendon rupture following use of its central third for anterior cruciate ligament (ACL) reconstruction is a rare disabling injury that is technically difficult to repair. We report one case of patellar tendon rupture after harvesting the mid-third for ACL reconstruction. A number of different surgical methods exist for reconstructing patellar tendon ruptures. Here we report a case using a bone-tendon-bone (BTB) allograft, followed by a multiple-wire loop reinforcement with no postoperative immobilisation. One year after operation, our patient had regained full knee extension and flexio

OBJECTIVE: To describe an alternative procedure of arthroscopic posterior cruciate ligament reconstruction using quadriceps tendon-patellar bone autograft and evaluate the early stage follow-up results.
METHODS: Twenty consecutive cases of isolated and combined posterior cruciate ligament deficiency were studied. Quadriceps tendon with patellar bone block was used for preparation of Y-shaped autograft. Double femoral tunnels and single tibial tunnel were precisely drilled respectively at their anatomical footprint. By using this technique, the anterolateral bundle was fixed and tensioned in 9

The purpose of this study was to determine whether two groups of patients showed any early (6 months postoperative) clinical differences when treated by arthroscopic-assisted or miniarthrotomy anterior cruciate ligament (ACL) reconstruction. Fifty-two consecutive arthroscopic-assisted ACL reconstructions (Group I) were matched with 52 miniarthrotomy ACL reconstructions (Group II). An autogenous midthird patellar tendon was used in all reconstruction procedures. Group I patients were operated on by one surgeon (A.C.R.) and all Group II patients by another (K.D.S.). Both groups were similar with

Using transmission electron microscopy, patellar tendons that had been autografted to replace the ACL were studied. Follow-up biopsies, performed at 6, 12 and 24 months after reconstructive surgery demonstrated that the autograft patellar tendons had undergone considerable change. After 6 months, the large collagen fibrils were less numerous and more widely spaced. The spaces were occupied by dense colonies of very small, newly-formed collagen fibrils and were rich in glycosaminoglycans. Small elastic fibres, fibrocytes, round cells with abundant rough endoplasmic reticulum and macrophage-like
