A novel in vitro experimental design for biomechanical testing of patellofemoral joint kinetics and kinematics.

INTRODUCTION: Complications arising from the patellofemoral joint (PFJ) represent the third most common cause for revision in total knee arthroplasty (TKA). Previous in vitro biomechanical studies have altered the native attachments of muscles controlling the PFJ. The purpose of this study was to design an in vitro biomechanical setup that would allow testing of both native and arthroplasty knee joints, specifically the PFJ, without disturbing the native attachments of the quadriceps and hamstrings muscles. METHODS: After finalising a prototype, a pelvis-to-toe human cadaver specimen was test

Morphological analysis of three-dimensional MR images of patellofemoral joints in asymptomatic subjects.

The existing methods for analyzing patellofemoral (PF) osteoarthritis (OA) are limited. Our purpose was to clarify the frequency, localization, and morphological progression of PFOA by observing three-dimensional (3D) magnetic resonance (MR) images from a cohort population. The subjects were 561 patients aged 30-79 years from the Kanagawa Knee Study who had not visited a hospital for more than three consecutive months for knee symptoms. MR images of the PF joints, separated into the medial and lateral types, were presented in order of the highest to lowest patella cartilage area ratios. Cartil

The influence of varus and valgus deviation on the contact area of patellofemoral joint in healthy knees.

OBJECT: Varus-valgus lower alignment is a risk factor for patellofemoral osteoarthritis, but malalignment alone affect not only the tibiofemoral joint but also the patellofemoral joint. The aim of the present study was to analyse the contact area of patellofemoral joint in varus alignment and valgus alignment of healthy subjects using magnetic resonance imaging. METHODS: Twenty-six healthy subjects with valgus lower limb alignment (Group I, n = 26) and twenty-six volunteers with varus lower limb alignment (Group II, n = 26) was performed. An MRI scan was used to capture and measure the patell

Induction of osteoarthritis by injecting monosodium iodoacetate into the patellofemoral joint of an experimental rat model.

This study aimed to investigate the histopathological changes in the patellofemoral joint using a rat model of osteoarthritis that was induced using monosodium iodoacetate, and to establish a novel model of patellofemoral osteoarthritis in a rat model using histopathological analysis. Sixty male rats were used. Osteoarthritis was induced through a single intra-articular injection of monosodium iodoacetate in both knee joints. Animals were equally divided into two experimental groups based on the monosodium iodoacetate dose: 0.2 mg and 1.0 mg. Histopathological changes in the articular cartilag

A cadaveric investigation into the demographic and bony alignment properties associated with osteoarthritis of the patellofemoral joint.

BACKGROUND: Patellofemoral joint osteoarthritis is common, although circumstances dictating its evolution and pathogenesis remain unclear. Advances in surgical technique have improved the ability to modify long-bone alignment in the coronal, sagittal, and axial planes. However, to our knowledge, there is no significant long-term data available in regard to the relationship between anatomic alignment parameters most amenable to surgical modification and patellofemoral joint osteoarthritis. METHODS: Five-hundred and seventy-one cadaveric skeletons were obtained from the Hamann-Todd osteological

Arthroscopic evaluation of patellofemoral congruence with rotation of the knee joint and electrical stimulation of the quadriceps.

PURPOSE: The aim of this study was to investigate the pathoanatomic features of patellar instability by arthroscopically comparing patellofemoral congruence with rotation of the knee joint and/or electrical stimulation of the quadriceps (ESQ) between knees with and without patellar instability. METHODS: We retrospectively examined 83 knee joints in 83 patients. The joints were classified into 2 groups: group 1 comprised those without a history of patellar dislocation and included 59 patients (25 male and 34 female patients), and group 2 comprised those with a history of patellar dislocation a

Structural parameters of the vastus medialis muscle and its relationship to patellofemoral joint deterioration.

Vastus medialis (VM) muscle dysfunction and abnormal limb alignment are commonly observed in patients who experience changes in patellofemoral joint (PFJ) function, leading many clinicians to assume that there is a direct relationship between VM structural parameters, leg alignment, and PFJ dysfunction. This study tested the hypothesis that there is a relationship between structural parameters of the VM muscle, limb alignment, and the location and severity of patellofemoral joint deterioration (PFJD). The dissection study used 32 limbs from 24 intact cadavers. Data were collected on limb align

Association between radiological findings and symptoms at the patellofemoral joint in advanced knee osteoarthritis.

We investigated the association between the radiological findings and the symptoms arising from the patellofemoral joint in advanced osteoarthritis (OA) of the knee. Four radiological features, joint space narrowing, osteophyte formation, translation of the patella and focal attrition were assessed in 151 consecutive osteoarthritic knees in 107 patients undergoing total knee replacement. The symptoms which were assessed included anterior knee pain which was scored, the ability to rise from a chair and climb stairs, and quadriceps weakness. Among the radiological features, only patellar transla

Relationship between knee abduction moment with patellofemoral joint reaction force, stress and self-reported pain during stair descent in women with patellofemoral pain.

BACKGROUND: Increased external knee abduction moment has been proposed as a risk factor for patellofemoral pain. This alteration is thought to be associated with elevated patellofemoral joint reaction force and stress, however these relationships remain poorly explored. Therefore, this study aimed at comparing knee abduction moment parameters (peak, rate of moment development and impulse), patellofemoral joint reaction force and stress of women with patellofemoral pain and pain-free controls during stair descent; and investigating the relationship among these variables with self-reported pain.

The influence of altered lower-extremity kinematics on patellofemoral joint dysfunction: a theoretical perspective.

Although patellofemoral pain (PFP) is recognized as being one of the most common disorders of the lower extremity, treatment guidelines and underlying rationales remain vague and controversial. The premise behind most treatment approaches is that PFP is the result of abnormal patellar tracking and/or patellar malalignment. Given as such, interventions typically focus on the joint itself and have traditionally included strengthening the vastus medialis oblique, taping, bracing, soft tissue mobilization, and patellar mobilization. More recently, it has been recognized that the patellofemoral joi

[The origin of femoral trochlear dysplasia: comparative anatomy, evolution, and growth of the patellofemoral joint].

We performed a comparative analysis of the femoropatellar morphology examining the evolutionary aspects to search for the origin of trochelar dysplasia. Trochlear dysplasia is frequent in the human population and, when associated with morphological and positional abnormalities of the patella, can lead to patellar pain syndrome in minor cases or patellar dislocation in severe cases. There is no strict relationship between the observed anomalies and clinical expression. The shape of the articular surfaces is variable in mammals depending on their type of locomotion: unguligrade, digitigrade, pla

Contact pressures in the patellofemoral joint during impact loading on the human flexed knee.

Currently, a bone fracture criterion is used by the automotive industry to assess the potential for a lower extremity injury from impact directed on the flexed knee. However, recent studies with animal models indicate irreversible damage to articular cartilage due to overpressures generated within the patellofemoral (P-F) joint without bone fracture, and suggest this injury may lead to a progressive, degenerative disease of the joint. The purpose of this investigation was to measure contact pressure in the human P-F joint during impact loading on the isolated, flexed knee. Loads were delivered

Patellofemoral Joint Anatomy Is Associated With Landing Mechanics After Operative Treatment for Patellar Instability.

BACKGROUND: Patellofemoral instability (PFI) has several known anatomic factors, including trochlear dysplasia and patellar maltracking. While studies have documented altered lower-extremity gait biomechanics in patients with PFI, the relationship between these biomechanical abnormalities and anatomy has not been thoroughly investigated, particularly during sport-related tasks. PURPOSE/HYPOTHESIS: The purpose of this study is to examine the associations between preoperative anatomic characteristics and lower-extremity kinematics at sports clearance in adolescents after medial patellofemoral l

Patellofemoral and tibiofemoral joint loading during a single-leg forward hop following ACL reconstruction.

Altered biomechanics are frequently observed following anterior cruciate ligament reconstruction (ACLR). Yet, little is known about knee-joint loading, particularly in the patellofemoral-joint, despite patellofemoral-joint osteoarthritis commonly occurring post-ACLR. This study compared knee-joint reaction forces and impulses during the landing phase of a single-leg forward hop in the reconstructed knee of people 12-24 months post-ACLR and uninjured controls. Experimental marker data and ground forces for 66 participants with ACLR (28 ± 6 years, 78 ± 15 kg) and 33 uninjured controls (26 ± 5 ye

Relationship between quadriceps strength and patellofemoral joint chondral lesions after anterior cruciate ligament reconstruction.

BACKGROUND: The incidence of the patellofemoral joint chondral lesions after anterior cruciate ligament reconstruction (ACLR) is disturbingly high. Few studies have assessed the factors affecting patellofemoral joint chondral lesions postoperatively. HYPOTHESIS: The recovery of quadriceps strength after ACLR could be associated with patellofemoral joint cartilage damage. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: A total of 88 patients who underwent arthroscopic anatomic double-bundle ACLR with hamstring autografts received second-look arthroscopy at the time of metal staple

Biomechanical predictors of patellofemoral and tibiofemoral joint loads during running: A cross-sectional study.

Clinicians lack accurate means to estimate patellofemoral (PFJ) and tibiofemoral (TFJ) kinetics outside the lab. We sought to identify biomechanical gait predictors of PFJ and TFJ kinetics in male and female runners. We used an idealized approach with 3-D motion capture via factors that are readily measured using clinical tools, i.e. 2-D video and wearable devices. Participants (n = 106; 53 females, 53 males) ran on an instrumented treadmill while kinematics, kinetics and temporospatial data were sampled. PFJ and TFJ contact forces were estimated via a musculoskeletal model. De-biased LASSO an
