Intensive Care Unit Sedation, Patient Memories, and Post-Intensive Care Psychological Distress.

BACKGROUND: Post-intensive care syndrome affects 1.5 million US patients yearly, and understanding the modifiable risk factors for this syndrome can reduce its incidence. OBJECTIVES: To investigate whether delusional memories after critical illness are associated with psychological distress and whether sedation practices affect the presence and character of intensive care unit (ICU) memories. METHODS: This was a retrospective study of patients in the Critical Illness Survivorship Clinic from October 2021 through August 2023. Clinic documentation included ICU memory descriptions and screening

Health care work environments, employee satisfaction, and patient safety: Care provider perspectives.

BACKGROUND: Experts continue to decry the lack of progress made in decreasing the alarming frequency of medical errors in health care organizations (Leape, L. L., & Berwick, D. M. (2005). Five years after to err is human: What have we learned?. Journal of the American Medical Association, 293(19), 2384-2390). At the same time, other experts are concerned about the lack of job satisfaction and turnover among nurses (. Keeping patients safe: Transforming the work environment of nurses. Washington, DC: National Academy Press). Research and theory suggest that a work environment that facilitates p

[Activity in an emergency care unit: experience of a patient care team].

An emergency and intensive care medical team (physicians, specialised nurses in anesthesia, paramedics) works in a mobile intensive care unit and in a Emergency room. The functions are to provide intensive care, to validate the pre-hospital diagnosis and also to prepare the optimal hospital admission. This organisation realize a "short route" to emergency hospitalisation. A questionnaire to this medical team enquired for their appreciation of the activity, for their implication and feeling, individual and collective, of death's experiences, of works organisation, and of requirements. Mainly, i

Patient-centered Care: Insights into Satisfaction Drivers.

BACKGROUND: Patient satisfaction is a key indicator of healthcare quality, influencing treatment adherence, clinical outcomes, and hospital reputation. The relationship between length of hospital stay (LOS) and patient satisfaction in neurological inpatients remains understudied, particularly regarding sex differences and neurological diagnosis types. OBJECTIVES: To understand how LOS influences satisfaction and patient experience. To evaluate care strategies and improve communication in neurological settings. METHODS: We conducted a retrospective observational study of 181 patients admitted

Patient Health Care Disparities in Shoulder Arthroplasty.

PURPOSE OF REVIEW: Shoulder arthroplasty (SA) is an effective surgical procedure to treat advanced degenerative conditions of the shoulder as well as tumors or fractures of the proximal humerus. Utilization of SA is increasing in the United States as indications have expanded, however, health care disparities around utilization and clinical outcomes following SA also exist. This review examines current literature on patient related health care disparities in SA. RECENT FINDINGS: Within SA, patient related health care disparities are highly influenced by race, ethnicity, socioeconomic status,

[Structured patient handover in intensive care medicine].

Intensive care units must always be prepared to admit or accept critically ill patients for intensive care monitoring or therapy. External admissions can take place via the emergency physician or transfers from other hospitals, while internal patients can be transferred, for example from normal wards, the emergency department or functional areas such as the operating room (OR), cardiac catheterization laboratory, interventional radiology. All relevant information should be communicated during these handovers to ensure maximum patient safety. We would like to raise awareness of the possible dan

Newborn Hearing Screening Success… Patient Care Failure.

OBJECTIVE: To assess the efficacy of a newborn hearing screening program in an urban hospital 24 years after a longitudinal study identified failings in its program and recommended solutions. METHODS: A hospital-specific, Pennsylvania database was queried to determine rates of in-hospital testing, referral, and diagnosis for Temple University Hospital (TUH), and to compare these with reporting sites statewide. At TUH, interviews with newborn hearing screeners and audiologists were conducted to identify barriers to care. RESULTS: In the year 2022, 2,006/2,069 newborns (97%) were successfully

Patient-centred stoma care support: ileostomy patients.

Stoma patients require continuous support throughout their entire journey with a stoma. Although many Stoma Care Services across the UK offer patient follow-up pathways, there is not one unified pathway. Patients may not be prepared for life with a stoma because, depending on their stoma type, they will have specific needs, and if patients and healthcare professionals are not prepared to manage these stoma-specific needs, complications and hospital readmissions may occur, worsening patients' outcomes and quality of life. Ileostomy patients are known to be more likely to experience complication

Care of the patient after valve intervention.

This review aims to outline the current evidence base and guidance for care of patients post-valve intervention. Careful follow-up, optimisation of medical therapy, antithrombotics, reduction of cardiovascular risk factors and patient education can help improve patient outcomes and quality of life. Those with mechanical valves should receive lifelong anticoagulation with a vitamin K antagonist but in certain circumstances may benefit from additional antiplatelet therapy. Patients with surgical bioprosthetic valves, valve repairs and transcatheter aortic valve implantation also benefit from ant

Neurologic Assessment of the Neurocritical Care Patient.

Sedation is a ubiquitous practice in ICUs and NCCUs. It has the benefit of reducing cerebral energy demands, but also precludes an accurate neurologic assessment. Because of this, sedation is intermittently stopped for the purposes of a neurologic assessment, which is termed a neurologic wake-up test (NWT). NWTs are considered to be the gold-standard in continued assessment of brain-injured patients under sedation. NWTs also produce an acute stress response that is accompanied by elevations in blood pressure, respiratory rate, heart rate, and ICP. Utilization of cerebral microdialysis and brai

Improving Communication in Heart Failure Patient Care.

BACKGROUND: Although implantable cardioverter-defibrillators (ICDs) reduce sudden death, these patients die of heart failure (HF) or other diseases. To prevent shocks at the end of life, clinicians should discuss deactivating the defibrillation function. OBJECTIVES: The purpose of this study was to determine if a clinician-centered teaching intervention and automatic reminders increased ICD deactivation discussions and increased device deactivation. METHODS: In this 6-center, single-blinded, cluster-randomized, controlled trial, primary outcomes were proportion of patients: 1) having ICD dea

The Perioperative Care of the Transgender Patient.

An estimated 25 million people identify as transgender worldwide, approximately 1 million of whom reside in the United States. The increasing visibility and acceptance of transgender people makes it likely that they will present in general surgical settings; therefore, perioperative health care providers must develop the knowledge and skills requisite for the safe management of transgender patients in the perioperative setting. Extant guidelines, such as those published by the World Professional Association for Transgender Health and the University of California San Francisco Center of Excelle

Peripartum Care of the Jehovah's Witness Patient.

Postpartum hemorrhage is a leading cause of maternal mortality. Jehovah's Witnesses (JWs) refuse allogeneic blood transfusion, and thus treatment of hemorrhagic complications in this patient population is complex. Recognition and respect for religious beliefs are essential when caring for the JW parturient and several ethical and legal considerations must be taken into account. Interdisciplinary teamwork and communication as well as active patient participation are critical in the decision-making process for JW patients. Various techniques and technologies are available to optimize red blood c

Sentinel node biopsy and improved patient care.

Sentinel lymph node biopsy (SLNB) is based on the hypothesis that the sentinel lymph node (SLN) reflects the lymph-node status and a negative SLN might allow complete axillary lymph node dissection (ALND) to be avoided. Past and current sentinel lymph node clinical trials for breast carcinoma have addressed the prognostic and therapeutic benefits of this technique and as such, SLNB has become a standard of care for select breast cancer patients. This article reviews the history of SLNB as well as current guidelines and recent controversies.
