In the health systems of many countries, there is neither a requirement to collect a minimum set of demographic information during patient registration nor a standard way of identifying patients. This impedes the provision of integrated, good-quality care for individual patients and, at the system level, prevents generation of the high-quality data necessary for effective management and continuous improvement. Assigning each patient a unique identifier (UID) to create a master patient index (MPI) is therefore essential to ensure data interoperability across all the points of patient care withi

BACKGROUND: The blood product administration process has been subject to various quality improvement initiatives aimed at reducing errors, including blood product labels that are missing, inaccessible, unreadable, or mismatched to orders and/or patients. This article reports the results of a formal simulation-based usability test of two comparable technologies designed to reduce blood product administration errors.
STUDY DESIGN AND METHODS: Nineteen nurses and three anesthesia providers evaluated one of the two products during simulated use in realistic scenarios during 90-minute test session

When a physician decides that a diabetic patient requires insulin treatment, more teaching is necessary than when initiating most other medications. It has been found advantageous, in treating persons with diabetes mellitus who are not severely compromised metabolically, to introduce them in an outpatient setting to the administration of insulin. The objective of this article as a teaching guide is to enable the health-care provider to cover all essential points clearly and effectively in a minimal period of time and thereby allow the patient to confidently inject himself/herself properly, and

OBJECTIVES: Specimen labeling defects within the perioperative environment are a known patient safety risk that carries the potential for adverse outcomes. These outcomes are a result of errors that occur when unsuspecting providers operate within poorly designed processes with little control over the specimen collection context. Many costly outcomes resulting from labeling errors may include patient harm, inappropriate treatments, lengthy investigations, corrective actions, and, at times, legal action.
METHODS: This improvement initiative to identify and reduce the risk of specimen labeling

BACKGROUND: It is important to ensure a patient-safe environment in the perioperative setting. With this in mind, a "patient-safety first" philosophy was adopted within our operating room service.
METHODS: During the first phase of the interventional study (2001-2002), we defined and executed the organizational and educational aspects of the intervention. Thereafter, the implementation phase (2003-2005) was performed. According to our zero tolerance policy, in the event that a major error in patient readiness for anesthesia and surgery was found in the operating room holding area, the patient

A new, simple, and reliable forensic identification system has been described. It permits the rapid and positive identification of victims of catastrophies such as airplane accidents, battles, floods, and fires. An electronic microprocessing unit directs a mechanical engraver to encode up to 13 alphanumeric characters on a small gold disk 0.25 mm thick and 2.0 mm in diameter. The identification chip is sealed in a 0.8-mm deep cavity prepared with a specially designed diamond burr in the lingual enamel of a molar tooth. The sealant is a stained composite material shown experimentally to be leak

Wrong-patient errors cause serious harm in newborns. These errors involve ordering and administering tests, procedures, medications, and breast milk to an unintended patient. Newborns receiving care in neonatal intensive care units (NICUs) are at particularly high risk. Although more distinct newborn naming conventions as recommended by the Joint Commission significantly reduce wrong-patient orders, name similarities among multiple-birth infants and truncation of differentiating information in some electronic health record (EHR) systems contribute to this persistent increased risk. Accordingly

OBJECTIVE: To identify and analyze errors in drug prescriptions of patients treated in a "high resolution" hospital by applying a Failure mode and effects analysis (FMEA).Material and methods A multidisciplinary group of medical specialties and nursing analyzed medical records where drug prescriptions were held in free text format. An FMEA was developed in which the risk priority index (RPI) was obtained from a cross-sectional observational study using an audit of the medical records, carried out in 2 phases: 1) Pre-intervention testing, and (2) evaluation of improvement actions after the firs

The transfusion chain is susceptible to error at every step. Accurate patient registration is a key first step that links a patient with their historical medical profile, yet patient registration is marked by its own challenges. Registration errors are deviations from standard operating procedures that occur during the process of patient registration. A frequent consequence of registration errors is the obfuscation of historical information and patient misidentification. Through duplicate registrations, patient information can be spread across multiple records and through hybrid registrations

Magnetic resonance (MR) imaging is generally considered a safe procedure. Contraindications include the presence of foreign objects in or on the body, which may be subject to electromagnetic fields associated with the MR system. Most of these objects are well known and are routinely screened for prior to the procedure. The authors report an unusual adverse event that appears to have been caused by a unique combination of factors involving an identification bracelet, an item not previously known to present any risks. To the authors' knowledge, this is the first report in the literature of a sev

Errors on identity of patients during their registration may lead to non-compliance with a transfusion procedure, a non-issue adjusted blood and thus a transfusion risk. The main mistake is the misknowledge of an antibody, secondarily a loss of transfusion information and a redundancy of examinations. The creation of a working group "identitovigilance" helped sensitize the staff of health establishments and clinical chemistry laboratories. In this area of computerization of medical and transfusion records, shared folders and networking, identification of patients is a real issue of risk manage

The correct wristband identification of patients is essential to prevent acute, hemolytic transfusion reactions from incompatible transfusion. We compared wristband identification errors for 712 hospitals. Phlebotomists checked patient wristbands on 2,463,727 occasions, finding 67,289 errors; in 33,308 instances, patient wristbands were missing entirely. The median total error rate was 2.2%; 10% of participants had error rates of 10.9% or greater. Absent wristbands represented 49.5% of errors; multiple wristbands with different information, 8.3%; wristbands with incomplete data, 7.5%; erroneou

BACKGROUND: Blood component mistransfusion is generally due to preventable clerical errors, specifically pretransfusion misidentification of patient/blood unit at bedside. Hence, electronic devices such as barcode scanners are recommended as the standard instrument used to check the patient's identity. However, several healthcare facilities in underdeveloped countries cannot afford this instrument; hence, they usually perform subjective visual assessment to check the patient's identity. This type of assessment is prone to clinical errors, which precipitates significant level of anxiety in the

In 2012, 110 hospitals reported on consent policy against national guidelines, with 105 (95%) including a statement on blood transfusion. Of the 103 hospitals (1788 transfusion episodes) that participated in the consent practice component, 89 specified the method of consent used; 46 (52%) had a specific transfusion consent form and achieved compliance of 90% compared to 43-69% for most other methods. The consent audit shows that a specific blood consent form achieved a high rate of medical record documentation compliance.

BACKGROUND: Fatal hemolytic transfusion reaction due to ABO incompatibility occurs mainly as a result of clerical errors. Blood sample drawn from the wrong patient and labeled as another patient's specimen will not be detected by the blood bank unless there is a previous ABO grouping result.
METHODS: In Hong Kong, we had designed a transfusion wristband system--portable barcode scanner system to detect such clerical errors. The system was well accepted by the house staff and had prevented two BO mismatched transfusion. Other current system of patient's identification may have similar results,

Occult specimen provenance complications (SPCs), which occur when there is an absence of any direct or indirect indication that a specimen switch or contamination may have occurred, constitute a significant patient safety and medical-legal problem because they can lead to misdiagnosis. However, the rate at which occult SPCs occur is unknown because, by definition, this category of errors is not identified by standard laboratory practices. In this study, we evaluated a data set comprising almost 13,000 prostate biopsies that were prospectively tested for specimen provenance errors as part of ro
