Finding a common language for patient safety in CKD.

Improving patient safety has become a primary objective of health systems; however, the frequency of adverse safety events continues to be unacceptable despite the attention and dedicated efforts of many stakeholders. CKD that does not require dialysis increases the risk for adverse safety events, and adverse safety events can account for a substantial portion of the poor outcomes typical of CKD. Because much of CKD care occurs outside the typical health care setting, systems designed to detect and reduce adverse safety events are not necessarily effective in this population. [About safety parameters for patient-controlled analgesia (PCA) devices].

During the course of preparation of an opioid prescription, the nurse in charge became aware that the patient-controlled analgesia (PCA) syringe driver did not permit programming for the delivery as required: a maximum bolus number (Bmax) was indicated but only a maximum cumulative dose (Dcmax) could be programmed. The prescription dose criteria were consistent with the guidelines of the French societies of palliative care, anesthesiology, and reanimation (Société française d'accompagnement et de soins palliatifs [Sfap] and Société française d'anesthésie réanimation [Sfar]). A Dcmax dose simul

Patient safety in interventional radiology: a CIRSE IR checklist.

Interventional radiology (IR) is an invasive speciality with the potential for complications as with other invasive specialities. The World Health Organization (WHO) produced a surgical safety checklist to decrease the morbidity and mortality associated with surgery. The Cardiovascular and Interventional Society of Europe (CIRSE) set up a task force to produce a checklist for IR. Use of the checklist will, we hope, reduce the incidence of complications after IR procedures. It has been modified from the WHO surgical safety checklist and the RAD PASS from Holland.

Intravenous fluid therapy - an under-recognized patient safety opportunity.

Inappropriate fluid regimens are rarely documented as being responsible for patient harm, although there is considerable circumstantial evidence to suggest that fluid overload may be a considerably under-estimated source of patient morbidity and mortality. The GIFTASUP fluid management guidelines published in 2008 offer a valuable opportunity to consider a more standardised approach to fluid management, possibly in the context of developing routine hospital services to support good practice.

Effect of critical access hospital conversion on patient safety.

BACKGROUND: The Medicare Rural Hospital Flexibility Program of the 1997 Balanced Budget Act allowed hospitals meeting certain criteria to convert to critical access hospitals (CAH) and changed their Medicare reimbursement mechanism from prospective payment system (PPS) to cost-based. OBJECTIVE: To examine the impact of CAH conversion on hospital patient safety. DATA SOURCE: Secondary data on hospital patient safety indicators (PSIs), hospital CAH status, patient case-mix, and market variables, for 89 Iowa rural hospitals during 1997-2004. STUDY DESIGN: We employed quasi-experimental designs

Age-specific differences in patient safety incidents: focus on falls and medication using the Korean patient safety incident reporting and learning system (2016-2023).

BACKGROUND: Patient safety incidents (PSIs) pose a significant global healthcare challenge, especially for vulnerable populations such as children and young adults (CYAs, aged 0–19 years), middle-aged adults (MAs, aged 20–60 years), and older adults (OAs, aged 20–60 years). Falls and medication errors are common hospital-based incidents with serious health impacts. Targeted prevention strategies are needed due to differing age-related risks. Therefore, we aimed to analyze the characteristics and key risk factors of falls and medication incidents across different age groups using 8 years (2016–

Patient and Family Perspectives on Statutory Duty of Candour Following Serious Adverse Patient Safety Events.

BACKGROUND: The Statutory Duty of Candour (SDC) legally requires healthcare organisations to notify patients, families, and carers who experience a serious adverse patient safety event (SAPSE), provide an apology, and explain steps taken to prevent recurrence. However, despite growing international adoption of SDC, evidence on its implementation and impact on patients and families ('consumers') remains limited. This study examined consumer awareness, understanding, and lived experiences of SDC in Victoria, Australia, two years after its introduction. METHODS: A mixed-methods study integrated

Perioperative patient safety: correct patient, correct surgery, correct side--a multifaceted, cross-organizational, interventional study.

BACKGROUND: It is important to ensure a patient-safe environment in the perioperative setting. With this in mind, a "patient-safety first" philosophy was adopted within our operating room service. METHODS: During the first phase of the interventional study (2001-2002), we defined and executed the organizational and educational aspects of the intervention. Thereafter, the implementation phase (2003-2005) was performed. According to our zero tolerance policy, in the event that a major error in patient readiness for anesthesia and surgery was found in the operating room holding area, the patient

Report Quality Is Not Investigation Quality: Strengthening Patient Safety Learning.

Bowditch et al. provide an important national assessment of patient safety incident investigation reports. This letter distinguishes documentation quality from investigation quality and acknowledges the study's quality-assurance procedures while proposing independent duplicate scoring of a representative subset. Future assessment frameworks should connect report content with investigation-process evidence and measurable learning outcomes.

FACTFINDERS FOR PATIENT SAFETY: Minimizing risks with cervical epidural injections.

This series of FactFinders presents a brief summary of the evidence and outlines recommendations to minimize risks associated with cervical epidural injections. Evidence in support of the following facts is presented. Minimizing Risks with Cervical Interlaminar Epidural Steroid Injections - 1) CILESIs should be performed at C6-C7 or below, with C7-T1 as the preferred access point due to the more generous dorsal epidural space at this level compared to the more cephalad interlaminar segments. This reduces the risk of the minor complication of dural puncture and the major complication of spinal

Safety and Tolerability of Antimicrobial Agents in the Older Patient.

Older patients are at high risk of infections, which often present atypically and are associated with high morbidity and mortality. Antimicrobial treatment in older individuals with infectious diseases represents a clinical challenge, causing an increasing burden on worldwide healthcare systems; immunosenescence and the coexistence of multiple comorbidities determine complex polypharmacy regimens with an increase in drug-drug interactions and spread of multidrug-resistance infections. Aging-induced pharmacokinetic and pharmacodynamic changes can additionally increase the risk of inappropriate
