Local Morphologic and Hemodynamic Analyses for the Prediction of Abdominal Aortic Aneurysm Rupture Based on Patient-Specific CTA and Computational Modeling.

OBJECTIVE: Extensive research has focused on the evaluation of rupture risks in abdominal aortic aneurysms (AAAs) through comprehensive morphologic and hemodynamic analyses, primarily considering the AAA as a whole entity. This study tried to identify the high-risk rupture sites of AAAs more precisely before the fatal process based on morphologic and hemodynamic analyses at the local segment. METHODS: Computed tomography angiography of a specific AAA patient was conducted at the follow-up 4 months before rupture, 1 day before rupture, the day of the rupture, and 15 days after endovascular aor

Patient-Specific Haemodynamic Modeling to Estimate the Extent of Microvascular Disease and Response to Pulmonary Endarterectomy in Chronic Thromboembolic Pulmonary Hypertension.

Chronic thromboembolic pulmonary hypertension (CTEPH) is a form of pulmonary hypertension that is caused by persistent obstruction of the pulmonary arteries by organized thrombi and associated microvascular disease. Pulmonary endarterectomy (PEA) is the gold standard treatment, but the extent of small vessel remodeling, which strongly influences treatment outcomes, remains difficult to quantify pre-operatively. We developed a multiscale, structure-based model of the pulmonary circulation using patient-specific vascular geometries from CT pulmonary angiography (CTPA) and haemodynamic data from

Objective Evaluation of 3D Morphology by Statistical Shape Modeling or Geometric Morphometrics Enabling Patient-Specific Treatment in Craniosynostosis-A Systematic Review.

Craniosynostoses are rare congenital conditions characterized by variable severity and a multifactorial relation between morphology and function that remains incompletely understood, impacting outcomes and prognosis. There is no consensus on how to objectively quantify severity. This systematic review aimed to evaluate the use of statistical shape modeling (SSM) and geometric morphometrics (GM) in addressing these challenges and explore their relevance in developing patient-specific treatments. A comprehensive search was conducted using PubMed, Scopus, and Web of Science to identify relevant s

Assessing the Feasibility and Utility of Patient-Specific 3D Advanced Visualization Modeling in Cerebrovascular Disease: Retrospective Analysis and Prospective Survey Pilot Study.

BACKGROUND: The prevalence, clinical burden, and health care costs (>US $100 billion) associated with cerebrovascular disease (CVD) will increase significantly as the US population grows and ages over the next 25 years. Existing 2D imaging modalities have inherent limitations in visualizing complex CVD, which may be mitigated with the use of patient-specific 3D advanced visualization (AV) technologies. There remain gaps in knowledge, however, regarding how and with what impact these technologies are being used in CVD. OBJECTIVE: The aim of this study was to characterize the clinical attribute

A Combined Computational Fluid Dynamics and Arterial Spin Labeling MRI Modeling Strategy to Quantify Patient-Specific Cerebral Hemodynamics in Cerebrovascular Occlusive Disease.

Cerebral hemodynamics in the presence of cerebrovascular occlusive disease (CVOD) are influenced by the anatomy of the intracranial arteries, the degree of stenosis, the patency of collateral pathways, and the condition of the cerebral microvasculature. Accurate characterization of cerebral hemodynamics is a challenging problem. In this work, we present a strategy to quantify cerebral hemodynamics using computational fluid dynamics (CFD) in combination with arterial spin labeling MRI (ASL). First, we calibrated patient-specific CFD outflow boundary conditions using ASL-derived flow splits in t

Towards a validated patient-specific computational modeling framework to identify failure regions in traditional growing rods in patients with early onset scoliosis.

BACKGROUND: While growing rods are an important contribution to early-onset scoliosis treatment, rod fractures are a common complication that require reoperations. A recent retrieval analysis study performed on failed traditional growing rods revealed that there are commonalities among patient characteristics based on the location of rod fracture. However, it remains unknown if these locations correspond to high stress regions in the implanted construct. METHODS: A patient-specific finite element scoliotic model was developed to match the pre-operative (pre-op) scoliotic curve of a patient as

Real-time interactive simulations of large-scale systems on personal computers and cell phones: Toward patient-specific heart modeling and other applications.

Cardiac dynamics modeling has been useful for studying and treating arrhythmias. However, it is a multiscale problem requiring the solution of billions of differential equations describing the complex electrophysiology of interconnected cells. Therefore, large-scale cardiac modeling has been limited to groups with access to supercomputers and clusters. Many areas of computational science face similar problems where computational costs are too high for personal computers so that supercomputers or clusters currently are necessary. Here, we introduce a new approach that makes high-performance sim

Patient-Specific Cardio-Respiratory Model for Optimization of Cardiac Radioablation.

Stereotactic Arrhythmia Radioablation (STAR) is a promising treatment for refractory ventricular tachycardia. However, its precision may be hampered by cardiac and respiratory motions. Multiple techniques exist to mitigate the effects of these displacements. The purpose of this work was, based on cardiac and respiratory dynamic CT scans, to generate a patient-specific dynamic model of the structures of interest, that enables simulation of treatments for evaluation of motion management methods. Deep learning-based segmentation was used to extract the geometry of the cardiac structures, whose de

Patient-Specific Mobilization: A Structured, Systemwide Model of Mobility Care.

Hospital-associated functional decline resulting from immobility and prolonged bed rest is a widespread yet underrecognized complication in acute care. Despite strong evidence linking immobility directly to severe in-hospital complications and significant postdischarge impairments - including reduced independence, diminished quality of life, and impaired ability to perform activities of daily living (ADLs) - many U.S. hospitals lack standardized mobility care programs. Mobility practices vary widely and are typically driven by individual clinician knowledge, institutional culture, and informal

A patient-specific engineered tissue model of BAG3-mediated cardiomyopathy.

Pathogenic mutations in Bcl2-associated athanogene 3 (BAG3) cause genetic dilated cardiomyopathy (DCM), a disease characterized by ventricular dilation, systolic dysfunction, and fibrosis. Previous studies have demonstrated that BAG3 mediates sarcomeric protein turnover through chaperone-assisted selective autophagy to maintain sarcomere integrity in the human heart. Although mouse models provide valuable insights into whole-organism effects of BAG3 knockout or pathogenic variants, their utility is limited by species-specific differences in pathophysiology, which often do not translate to huma

A Patient-Specific Morphoelastic Growth Model of Aortic Dissection Evolution.

The human aorta undergoes complex morphologic changes that indicate the evolution of disease. Finite element analysis enables the prediction of aortic pathologic states, but the absence of a biomechanical understanding hinders the applicability of this computational tool. We incorporate geometric information from computed tomography angiography (CTA) imaging scans into finite element analysis (FEA) to predict a trajectory of future geometries for four aortic disease patients. Through defining a geometric correspondence between two patient scans separated in time, a patient-specific FEA model c

A 1D-3D Hybrid Model of Patient-Specific Coronary Hemodynamics.

PURPOSE: Coronary flow is affected by evolving events such as atherosclerotic plaque formation, rupture, and thrombosis, resulting in myocardial ischemia and infarction. Highly resolved 3D hemodynamic data at the stenosis is essential to model shear-sensitive thrombotic events in coronary artery disease. METHODS: We developed a hybrid 1D-3D simulation framework to compute patient-specific coronary hemodynamics efficiently. A 1D model of the coronary flow is coupled to an image-based 3D model of the region of interest. This framework affords the advantages of reduced-order modeling, decreasing

Generation of a Patient-Specific Model of Normal Sagittal Alignment of the Spine.

STUDY DESIGN: Mathematical modeling of normal sagittal spinal alignment. OBJECTIVE: To create a patient specific 3-dimensional (3D) model of normal adolescent spinal shape and alignment. SUMMARY OF BACKGROUND DATA: Recreating normal sagittal balance is a key goal in spinal deformity surgery. Because of the variation in normal sagittal alignment based on inherent pelvic parameters, it is difficult to know what is normal for a given patient who presents with spinal deformity. METHODS: Simultaneous biplanar 2-dimensional digital radiographs were taken for pediatric patients with no known spina

Patient-specific 3D microfluidic tissue model for multiple myeloma.

In vitro culturing of primary multiple myeloma cells (MMC) has been a major challenge as this plasma cell malignancy depends on the bone marrow environment for its survival. Using a microfluidic platform to emulate the dynamic physiology of the bone marrow microenvironment, we report here a new approach for culturing difficult to preserve primary human MMC. The system uses a three-dimensional ossified tissue to mimic the tumor niche and recapitulate interactions between bone marrow cells and osteoblasts (OSB). To this end, the human fetal OSB cell line hFOB 1.19 was cultured in an eight-chambe

Patient-Specific Sensitivity of Osteotomy Interface Modelling in Mandibular Reconstruction.

This study systematically investigates the influence of different interface modelling assumptions on biomechanical predictions using patient-specific models and evaluates their agreement with postoperative configurations. Five mandibular reconstruction cases using vascularised osseous free flaps were analysed. For each case, preoperative VSP-based finite element models incorporating four interface conditions, rigid osseous contact, soft-tissue compliance, initial osteotomy gap, and heterogeneous bone properties, were compared against corresponding postoperative models reconstructed from CT dat

Validation of patient-specific flatfoot models on finite element analysis.

Adult-acquired flatfoot causes various deformities. If a patient-specific foot model can be created using the finite element method, it can be used to study the appropriate surgical technique for each patient. Nine patient-specific flatfoot models were created, and loading simulations were performed. To validate the models, the patients' weight-bearing radiographs were compared with the parameters of the models. The CCC values ranged from 0.917 to 0.993 , all exceeding the moderate threshold according to the McBride criteria. Our model reproduces the biomechanics of a patient's foot under load

The technique for 3D printing patient-specific models for auricular reconstruction.

PURPOSE: Currently, surgeons approach autogenous microtia repair by creating a two-dimensional (2D) tracing of the unaffected ear to approximate a three-dimensional (3D) construct, a difficult process. To address these shortcomings, this study introduces the fabrication of patient-specific, sterilizable 3D printed auricular model for autogenous auricular reconstruction. METHODS: A high-resolution 3D digital photograph was captured of the patient's unaffected ear and surrounding anatomic structures. The photographs were exported and uploaded into Amira, for transformation into a digital (.stl)

In Vitro Validation of a Multiscale Patient-Specific Norwood Palliation Model.

In Norwood physiology, shunt size and the occurrence of coarctation can affect hemodynamics significantly. The aim of the study was to validate an in vitro model of the Norwood circulation against clinical measurements for patients presenting differing aortic morphologies. The mock circulatory system used coupled a lumped parameter network of the neonatal Norwood circulation with modified Blalock-Taussig (mBT) shunt with a three-dimensional aorta model. Five postoperative aortic arch anatomies of differing morphologies were reconstructed from imaging data, and the system was tuned to patient-s

Detection of growth-restricted fetuses using a patient-specific model.

Fetal growth restriction (FGR) is one of the major contributors to adverse perinatal outcome. The purpose of this work was to extend the use of Ultrasound Doppler measurements and allow early and accurate detection of FGR. To this end, a mathematical model was developed to represent the major fetal hemodynamic mechanisms involved. Based on model parameters' values, the forward model predicted flow waveforms at the locations where Doppler measurements are routinely performed. Blood velocity waveforms measured in 20 FGR and 20 normal fetuses were used as inputs to an inverse model solution to ob
