Ventilators for noninvasive ventilation to treat acute respiratory failure.

The application of noninvasive ventilation (NIV) to treat acute respiratory failure has increased tremendously both inside and outside the intensive care unit. The choice of ventilator is crucial for success of NIV in the acute setting, because poor tolerance and excessive air leaks are significantly correlated with NIV failure. Patient-ventilator asynchrony and discomfort can occur if the physician or respiratory therapist fails to adequately set NIV to respond to the patient's ventilatory demand, so clinicians need to fully understood the ventilator's technical peculiarities (eg, efficiency

Understanding the Control of Breathing to Optimize Patient-Ventilator Interactions.

Control of breathing depends on a complex and intricate feedback control system that integrates both automatic and volitional aspects of ventilation. A "central pattern generator" integrates inputs from peripheral and central chemoreceptors, as well as receptors located in the lungs, airways, muscles, and joints, in conjunction with signals from various brain regions. The output of the central pattern generator-referred to as the respiratory drive-determines the characteristics of breathing, including magnitude, timing, and the specific effector muscles involved. Several conditions frequently

Patient triggered ventilation in chronically ventilator-dependent infants.

Patient triggered ventilation (PTV) has been assessed as a method of respiratory support in infants remaining ventilator-dependent beyond the 1st week of life. Sixteen preterm infants were studied who had a median gestational age of 26 weeks and postnatal age of 22 days. PTV was delivered using a ventilator incorporating an airway pressure trigger. PTV was only successfully maintained until extubation in 3 infants, failing to provide a satisfactory method of respiratory support in the remaining 13 infants after a median of 1 h (range 1-10). One of the 13 infants was persistently asynchronous a

Adaptive support ventilation with IntelliSync versus pressure support ventilation for non-invasive ventilation in acute exacerbation of COPD: A feasibility randomized trial.

BACKGROUND: While non-invasive ventilation (NIV) is recommended for managing acute exacerbations of chronic obstructive pulmonary disease (AECOPD), it has a 30-40% failure rate when delivered using pressure support ventilation (PSV). Adaptive support ventilation with IntelliSync (ASVi) synchronizes breath initiation and cycling to match patient's requirements. We conducted a feasibility study to inform design of a larger trial and compared NIV failure rates using PSV or ASVi in patients with AECOPD. MATERIALS AND METHODS: We conducted a single-center, randomized controlled trial from December

Description and impact on gas exchange of sleep related respiratory events for patients with chronic respiratory failure treated with non-invasive ventilation.

BACKGROUND: The rising prevalence of hypercapnic chronic respiratory failure (HCRF) raises questions about how to initiate and adjust noninvasive ventilation (NIV), which is the first-line treatment. The current literature regarding the prevalence of patient ventilator asynchronies (PVA) and their impact on patient gas exchange is limited. METHODS: This single center observational cohort study describes the monitoring of consecutive patients treated with a long-term indication for NIV. Polygraphy was used to assess leaks, upper airway obstruction (UAO), and PVA rate (% of recording time and e

Work of Breathing in Premature Neonates: Noninvasive Neurally-Adjusted Ventilatory Assist versus Noninvasive Ventilation.

BACKGROUND: We tested whether work of breathing in premature newborns estimated by phase angle (θ) by using respiratory inductance plethysmography is decreased during neurally-adjusted ventilatory assist (NAVA) noninvasive ventilation (NIV) versus NIV alone. METHODS: NAVA NIV and NIV were applied in random order while using respiratory inductance plethysmography to measure the phase angle. RESULTS: Patient-ventilator asynchrony was decreased during NAVA NIV; however, the phase angle was not different between the modes. A large number of repeated assists with switches to backup were found whe

Modelling ventilation with spontaneous breaths: Improving accuracy with shape functions and slice method.

BACKGROUND AND OBJECTIVE: Accurate detection of spontaneous breathings (SBs) and respiratory asynchronies during mechanical ventilation (MV) is essential for optimizing patient care and preventing lung injuries. Conventional models often fail to capture these events with sufficient accuracy. To address this gap, this study introduces new equations incorporating custom shape functions and the Slice method, aiming to deliver a more robust, "bedside" model with potential applications in real-time asynchrony detection. METHODS: Three new equations were developed to incorporate shape functions acc

Neurally adjusted ventilatory assist improves patient-ventilator synchrony in a patient with tetanus and unstable diaphragmatic electrical activity.

A 79-y-old man with generalized tetanus was admitted to the ICU. A left-forearm wound was surgically debrided, and the wound was closed. On postoperative day 1, after the patient experienced opisthotonos and convulsions, endotracheal intubation was performed. Propofol and diazepam were infused but failed to stop the convulsions. Morphine, midazolam, dantrolene, and rocuronium were used to ameliorate the muscle spasms. Magnesium sulfate was also infused. On postoperative day 15, patient-ventilator asynchrony was apparent. The patient showed recurrent tachypnea and bradypnea, which seemed typica

Myorelaxants in ARDS patients.

Neuromuscular blocking agents (NMBAs) inhibit patient-initiated active breath and the risk of high tidal volumes and consequent high transpulmonary pressure swings, and minimize patient/ ventilator asynchrony in acute respiratory distress syndrome (ARDS). Minimization of volutrauma and ventilator-induced lung injury (VILI) results in a lower incidence of barotrauma, improved oxygenation and a decrease in circulating proinflammatory markers. Recent randomized clinical trials did not reveal harmful muscular effects during a short course of NMBAs. The use of NMBAs should be considered during the

Effect of respiratory filter resistance on respiratory mechanics and human-machine synchrony during invasive jet atomization: a prospective self-controlled study.

BACKGROUND: To reduce the contamination of the breathing circuit, respiratory filters are used in patients receiving mechanical ventilation. However, their use increases the resistance and dead space volume of the breathing circuit, which potentially leads to asynchrony events, especially when patients are receiving jet nebulization. However, the impact of respiratory filter resistance on respiratory mechanics and human-machine synchronization during invasive ventilation with jet nebulization is not clear. Therefore, this study aims to investigate the question above and optimize the strategy o

Patient-Ventilator Interaction During Sleep.

Sleep in the ICU is subjectively and objectively poor. Patients in the ICU have short sleep duration and highly fragmented sleep. Poor sleep in the ICU is associated with worse outcomes, particularly the development of ICU delirium. Patients undergoing noninvasive or invasive mechanical ventilation struggle with sleep. For those patients on noninvasive ventilation, settings may need to be adjusted compared to wake to account for the physiological changes that occur with the sleep state. Specifically, upper airway collapsibility (ie, obstructive sleep apnea) must be considered as do changes in

Neurally Adjusted Ventilatory Assist in Newborns.

Patient-ventilator asynchrony is very common in newborns. Achieving synchrony is quite challenging because of small tidal volumes, high respiratory rates, and the presence of leaks. Leaks also cause unreliable monitoring of respiratory metrics. In addition, ventilator adjustment must take into account that infants have strong vagal reflexes and demonstrate central apnea and periodic breathing, with a high variability in breathing pattern. Neurally adjusted ventilatory assist (NAVA) is a mode of ventilation whereby the timing and amount of ventilatory assist is controlled by the patient's own n

A diaphragmatic electrical activity-based optimization strategy during pressure support ventilation improves synchronization but does not impact work of breathing.

BACKGROUND: Poor patient-ventilator synchronization is often observed during pressure support ventilation (PSV) and has been associated with prolonged duration of mechanical ventilation and poor outcome. Diaphragmatic electrical activity (Eadi) recorded using specialized nasogastric tubes is a surrogate of respiratory brain stem output. This study aimed at testing whether adapting ventilator settings during PSV using a protocolized Eadi-based optimization strategy, or Eadi-triggered and -cycled assisted pressure ventilation (or PSVN) could (1) improve patient-ventilator interaction and (2) red

Neurally adjusted ventilatory assist in patients with acute respiratory failure: study protocol for a randomized controlled trial.

BACKGROUND: Patient-ventilator asynchrony is a common problem in mechanically ventilated patients with acute respiratory failure. It is assumed that asynchronies worsen lung function and prolong the duration of mechanical ventilation (MV). Neurally Adjusted Ventilatory Assist (NAVA) is a novel approach to MV based on neural respiratory center output that is able to trigger, cycle, and regulate the ventilatory cycle. We hypothesized that the use of NAVA compared to conventional lung-protective MV will result in a reduction of the duration of MV. It is further hypothesized that NAVA compared to

Trigger issues with a life support device in children.

Noninvasive ventilation (NIV) is widely used in children. Only a few devices are life support ventilators. The pressure support (PSV) mode is the most common used mode for home NIV, while assist-control pressure ventilation (PAC) is usually used in patients with abnormal central drive. Patient-ventilator asynchrony (PVA) is common during NIV and may have different causes, such as unintentional leaks, inadequate settings or misunderstanding of the settings. However, PVA may also be due to issues related to the NIV device, which is less common and is challenging. We report here the cases of 5 ch

New modalities for non-invasive positive pressure ventilation: A review article.

Efficiency of non-invasive positive pressure ventilation in the treatment of respiratory failure has been shown in many published studies. In this review article, we introduced new modalities of non-invasive ventilation (NIV), clinical settings in which NIV can be used and a practical summary of the latest official guidelines published by the European Respiratory Clinical Practice. Clinical trials and review articles in four databases up to 25 February 2018 about new modalities of non-invasive positive pressure ventilation were reviewed. Commonly used modalities for treatment of respiratory fa

Treatment of neonates with respiratory distress syndrome by proportional assist ventilation plus synchronized intermittent mandatory ventilation: a comparison study.

BACKGROUND: The proportion assisted ventilation (PAV) can improve patient-ventilator interaction, reducing the incidence of end-expiratory asynchrony and increasing the time of synchrony. PAV could compensate for the leaks by elastic and resistive unloading and thus is ideal for neonates with uncuffed airways. The aim of this study was to compare the relevant clinical parameters of neonates with respiratory distress syndrome (RDS) who are supported by PAV plus synchronized intermittent mandatory ventilation (SIMV) and SIMV. METHODS: Forty-six neonates diagnosed as RDS who required mechanical

Extracting ventilatory waveforms from screen recordings: a validated image processing methodology and its application to predictive modelling.

Access to high-fidelity ventilatory waveform data remains a significant challenge in respiratory mechanics research, particularly in resource-constrained environments. This study introduces and validates a novel, accessible framework for acquiring ventilatory data by applying image processing techniques to ventilator screen recordings. The accuracy of the framework was evaluated, demonstrating high fidelity (> 0.99) when compared with sensor-derived data from a laboratory emulator. Its reliability was further confirmed by demonstrating that image-derived patient data reproduced established cor

Nasal high-frequency oscillatory ventilation inhibits gastroesophageal reflux in the neonatal period.

Nasal high-frequency oscillatory ventilation (nHFOV) in neonates is increasingly considered due to enhanced alveolar ventilation, absence of patient-ventilator asynchrony and lessened ventilator-induced lung injury. Although any type of non-invasive respiratory support can lead to gastric distension via esophageal air passage and thus promote gastroesophageal refluxes (GERs), we have shown that nasal continuous positive airway pressure (CPAP; 6 cmH2O) and intermittent positive pressure ventilation (15/4 cmH2O) conversely inhibit GERs in lambs. The current objective was to test the hypothesis t

Sleep and mechanical ventilation in the intensive care unit.

Sleep is essential to a patient's well-being. The importance of sleep is highlighted by the adverse effects in the wake of its absence both physically and mentally. Sleep is difficult to achieve in the intensive care unit due to noise, patient care activities, illness, and mechanical ventilation. Activities related to mechanical ventilation such as suctioning, discomfort of the essential tremor, alarms, treatments and sedation effects can all alter sleep architecture. However, mechanical ventilation itself especially as it relates to asynchrony may also play a larger role than previously thoug
