Comprehensive evaluation of velopharyngeal function in myasthenia gravis patients.

PURPOSE: Hypernasality, which is a symptom of dysarthria, may be seen in patients with Myasthenia Gravis with bulbar symptoms. However, there is not enough evidence to show that these patients may have velopharyngeal dysfunction. This study investigates the features of velopharyngeal function in myasthenia gravis patients using objective and subjective measurement tools. METHODS: Ten adult myasthenia gravis patients with bulbar symptoms and ten adult myasthenia gravis patients without bulbar symptoms were recruited for this study. Ten healthy subjects were also included as the control group.

Long-term outcomes in unilateral vocal fold paralysis patients.

OBJECTIVES/HYPOTHESIS: At presentation, unilateral vocal fold paralysis (UVFP) patients have different treatment options, including conservative management (CM), injection laryngoplasty (IL) with a temporary agent, or permanent medialization (PM). This study evaluates long-term outcomes for UVFP patients relative to intervention. STUDY DESIGN: Retrospective chart review. METHODS: A retrospective chart review was performed of UVFP patients who presented to the University of California San Francisco Voice and Swallowing Center. Videolaryngostroboscopy examinations were reviewed. Maximum glotti

Diplophonia in unilateral vocal fold paralysis and intracordal cyst.

Diplophonia is the production by the voice of 2 separate tones through quasiperiodic variations in the vocal fold vibration (Ward PH, Moore GP. Ann Otol Rhinol Laryngol 1969;78:771-7). Clinically, diplophonia can be observed in patients with unilateral vocal fold paralysis with incomplete glottal closure and a mass lesion of the vocal fold, intracordal cyst, and granuloma (Kiritani S, et al. Ann Bull RILP 1991;25:55-62; Hirano M, et al. Ann Otol Rhinol Laryngol 1989;98:791-5). In this study we report 16 subjects with unilateral vocal cord paralysis or an intracordal cyst characterized perceptu

Glottal closure, transglottal airflow, and voice quality in healthy middle-aged women.

Seventeen healthy women, 45 to 61 years old, were examined using videofiberstroboscopy during phonation at three loudness levels. Two phoniatricians evaluated glottal closure using category and ratio scales. Transglottal airflow was studied by inverse filtering of the oral airflow signal recorded in a flow mask (Glottal Enterprises System) during the spoken phrase /ba:pa:pa:pa:p/ at three loudness levels. Subglottal pressure was estimated from the intraoral pressure during p occlusion. Running speech and the repeated /pa:/ syllables were perceptually evaluated by three speech pathologists rega

Use of perceptual memory as a performance validity indicator: initial validation with simulated mild traumatic brain injury.

INTRODUCTION: Many commonly employed performance validity tests (PVTs) are several decades old and vulnerable to compromise, leading to a need for novel instruments. Because implicit/non-declarative memory may be robust to brain damage, tasks that rely upon such memory may serve as an effective PVT. Using a simulation design, this experiment evaluated whether novel tasks that rely upon perceptual memory hold promise as PVTs. METHOD: Sixty healthy participants were provided instructions to simulate symptoms of mild traumatic brain injury (TBI), and they were compared to a group of 20 honest re

Ranking fluctuating asymmetry in a dot figure and the significant impact of imagining a face.

Fluctuating asymmetry and averageness is correlated with our perception of beauty in human faces. Yet, whether deviations of centrality in the positioning of the eyes, the nose, and the mouth have different effects on our perception of asymmetry in a holistic human face, is still uncertain. In this study we aimed to test the relative effect of decentralising the horizontal position of three sets of paired dots representing eyes, nostrils, or mouth from the vertical midline of ambiguous dot figures, vaguely resembling a face. The figures were ranked according to perceived asymmetry by human obs

Residual clefts in the hard palate: correlation between cleft size and speech.

OBJECTIVE: This study was conducted to evaluate the relationship between size of residual clefts in the hard palate and speech. SUBJECTS: Fifteen 7-year-old children born with complete cleft lip and palate were investigated. METHODS: All of the children were treated according to a surgical regimen involving early soft palate repair and delayed hard palate closure. Measures were taken of the area, length, and maximal width of the residual cleft in the hard palate about a year before its closure and correlated with a perceptual judgment of several speech variables. RESULTS: Significant positi

Neoglottic vibration in tracheoesophageal shunt phonation.

The purpose of the present study was to examine vibratory patterns of the neoglottis and to ascertain the neoglottic conditions that could cause failure in tracheoesophageal shunt phonation. We studied 30 tracheoesophageal shunt speakers while using "trap-door" type voice prostheses. Videostroboscopy was undertaken to investigate vibratory patterns of the neoglottis during phonation of the sustained vowel sound /e/ in these patients. The voice qualities of the patients were judged perceptually. A regular neoglottic vibration was observed in 21 patients. Nine patients had irregular neoglottic v

Speech outcome after early repair of cleft soft palate using Furlow technique.

OBJECTIVE: The earlier closure of palatal cleft is the better the speech outcome and the less compensatory articulation errors, however dissection on the hard palate may interfere with facial growth. In Furlow palatoplasty, dissection on the hard palate is not needed and surgery is usually limited to the soft palate, so the technique has no deleterious effect on the facial growth. The aim of this study was to assess the efficacy of Furlow palatoplasty technique on the speech of young infants with cleft soft palate. METHODS: Twenty-one infants with cleft soft palate were included in this study

Endoscopic and stroboscopic presentation of the larynx in male-to-female transsexual persons.

BACKGROUND: Male-to-female transsexual (MFT) persons often attempt to produce a female-sounding voice as part of the transition process. Endoscopic and stroboscopic data about how they accomplish this with an anatomically male larynx are lacking. OBJECTIVES: To describe vocal fold activity in MFT persons producing their feminine voice and identify signs of vocal misuse or hyperfunction in MFT speakers, if any. STUDY DESIGN: Prospective, nonrandomized, descriptive study of a convenience sample of MFT persons. METHODS: All MFT persons had endoscopic and stroboscopic procedures completed. Imag

Information in speech: observations on the perception of [s]-stop clusters.

A series of experiments is reported that investigated the pattern of acoustic information specifying place and manner of stop consonants in medial position after [s]. In both production and perception, information for stop place includes the spectrum of the fricative at offset, the duration of the silent closure interval, the spectral relationship between the frequency of the stop release burst and the following periodically excited formants, and the spectral and temporal characteristics of the first formant transition. Similarly, the information for stop manner includes the duration of silent

Breath-Holding Maneuvers and Airway Closure in Healthy Adults and Parkinson's Disease: Effects of Maneuver Type, Health Status, and Vocal Fold Bowing.

INTRODUCTION: Breath-holding maneuvers are frequently used to improve airway closure and swallowing safety in people with dysphagia. While their effects on airway closure have been studied in healthy adults, little is known about their effects in people with Parkinson's disease (PwPD). This study aimed to: (1) characterize glottic and supraglottic airway closure during light and hard breath-holding maneuvers in healthy adults and PwPD; (2) determine whether glottic and supraglottic closure changes as an effect of maneuver type (light vs. hard breath-hold) and health status (healthy adults vs.

Ansa cervicalis-to-recurrent laryngeal nerve anastomosis for unilateral vocal fold paralysis: experience of a single institution.

OBJECTIVES: One treatment option for unilateral vocal fold paralysis (UVFP) is ansa cervicalis-to-recurrent laryngeal nerve (ansa-RLN) anastomosis to provide reinnervation to the affected vocal fold. The advantages of this treatment approach are that it 1) provides vocal fold tone, bulk, and tension, 2) is technically simple, and 3) does not preclude other medialization procedures. We present all patients who have undergone ansa-RLN anastomosis for UVFP at our institution. METHODS: An Institutional Review Board-approved retrospective chart review was performed to include all patients who had

Contour-integration deficits on the intact side of the visual field in hemianopia patients.

OBJECTIVE: Visual impairments in hemianopia are thought to be exclusively caused by the reduced visual field size. However, the primary lesion may affect the contralateral hemisphere through damage of interhemispheric projections. The question therefore arises if the presumed "intact" hemifield is perceptually impaired. METHODS: Three hemianopia patients and three matched controls carried out a Yes/No figure detection task with their intact side of the visual field. The figure (square) contours were composed of non-contiguous Gabor patches embedded in a random patch array of different backgro

Autologous Fat Injection through the Cricothyroid Membrane Improves Voice in Patients with Unilateral Vocal Fold Paralysis.

BACKGROUND: Traditional fat injection techniques still have some limitations, such as inevitable fat resorption, operative fat overflow, and the lack of standardized procedural techniques, which result in worse voice improvement. This study aims to present our experience with a modified autologous fat injection technique through the cricothyroid membrane, designed to improve glottic closure and voice function in patients with unilateral vocal fold paralysis (UVFP). METHOD: Sixteen consecutive cases of UVFP, 36.9 years (range, 18-71 years), underwent autologous fat injection through the cricot

Spatial Ability in Children with Mathematics Learning Disorder (MLD) and Its Impact on Executive Functions.

Mathematics Learning Disorder (MLD) has been found to be related to impairments in cognitive functions. Spatial ability (SA), which is made up of eight single but unified elements can be assumed to have a leading part in different areas of math skills. The first objective of this study was to explore differences in SA factors in 128 school children of third to sixth grade with and without MLD. The Flexibility of Closure, Closure Speed, Perceptual Speed, Visualization, Spatial Relation, Spatial Orientation, Spatial Temporal, and Wayfinding were measured through eight tasks. The second objective
