BACKGROUND: Paradoxical reaction (PR) refers to a worsening of existing or new TB-related lesions after initiating effective anti-tuberculosis therapy. While commonly observed in lymph node or CNS TB, PR involving the pericardium is exceedingly rare in immunocompetent patients.
CASE PRESENTATION: A 22-year-old Vietnamese man with mediastinal tuberculous lymphadenitis and endobronchial TB was started on standard anti-TB therapy. After initial improvement, he developed pericardial effusion three months into treatment. Cultures and PCR were negative, but elevated ADA levels were detected. Based

BACKGROUND: Tuberculous pericarditis is the most common cause of pericardial disease in tuberculosis-endemic areas. Accurate and early diagnosis of tuberculous pericarditis is crucial due to its high mortality; however, the diagnostic accuracy of current methods remains suboptimal. This study aims to investigate the clinical application value and imaging characteristics of 18F-FDG PET/CT in tuberculous pericarditis.
METHODS: A total of 11 patients with tuberculous pericarditis were retrospectively analyzed in this study. Patients were categorized into two groups based on the presence or absen

Pericardial inflammation secondary to Mycobacterium tuberculosis infection is a rare condition, but its incidence is increasing in parallel with human immunodeficiency virus infection. Recrudescence of various types of tuberculosis should alert the clinician to the possibility of tuberculous pericarditis. The authors present the case of a 27-year-old white male, seropositive for the human immunodeficiency virus, presenting with large volume pericardial effusion and unusual echocardiographic features, global heart failure and clinical suspicion of tuberculosis. After anti-tuberculous chemothera

Tuberculous pericarditis is often difficult to diagnose because of atypical symptoms and insufficient etiology examination. We present a case wherein the patient initially presented with a high fever accompanied by recurrent pericardial effusion. Notably, the etiology and genetic diagnosis of tuberculosis were negative. With the combined application of multimodal imaging techniques such as transthoracic echocardiography, transesophageal echocardiography, contrast-enhanced ultrasound and enhanced CT, clinical diagnosis and treatment can provide sufficient evidence to support tuberculous pericar

Tuberculosis (TB) and human immunodeficiency virus/acquired immunodeficiency syndrome have reached epidemic proportions, particularly affecting vulnerable populations in low- and middle-income countries of sub-Saharan Africa. TB pericarditis is the commonest cardiac manifestation of TB and is the leading cause of constrictive pericarditis, a reversible (by surgical pericardiectomy) cause of diastolic heart failure in endemic areas. Unpacking the complex mechanisms underpinning constrictive haemodynamics in TB pericarditis has proven challenging, leaving various basic and clinical research ques

BACKGROUND: Opportunistic infections are frequent in people living with the human immunodeficiency virus who either do not have access to antiretroviral therapy (ART) or use it irregularly. Tuberculosis is the most frequent infectious disease in PLHIV and can predispose patients to severe fungal infections with dire consequences.
CASE PRESENTATION: We describe the case of a 35-year-old Brazilian man living with human immunodeficiency virus (HIV) for 10 years. He reported no adherence to ART and a history of histoplasmosis with hospitalization for 1 month in a public hospital in Natal, Brazil.

Humoral immune reactions were analyzed in 12 patients with exudative tuberculous pericarditis, 10 patients with constrictive pericarditis due to former tuberculosis, 10 patients with viral pericarditis, 20 patients with pulmonary tuberculosis, and 98 healthy donors. Pericarditis occurred in 12.5% of the patients with tuberculosis, whereas the incidence of tuberculosis in the 149 patients with pericarditis was 8%. Repeated pericardial puncture and pericardial effusions of greater than 500 ml with impending cardiac tamponade had to be performed in 4 patients. Clinical data indicated probable myo

INTRODUCTION: Tuberculous pericarditis is a severe, high-mortality manifestation of extrapulmonary tuberculosis. Its diagnosis is hindered by the invasive nature of specimen collection and the limited sensitivity of conventional microbiological tests. Third-generation sequencing (TGS) offers a promising, rapid and comprehensive molecular approach for detecting Mycobacterium tuberculosis, but its specific diagnostic accuracy for tuberculous pericarditis has not been systematically evaluated. This planned systematic review and meta-analysis aims to precisely determine the diagnostic performance

Introduction: Xpert MTB/RIF is a rapid diagnostic instrument for pulmonary tuberculosis (TB). However, studies reported varied accuracy of Xpert MTB/RIF in detecting Mycobacterium tuberculosis in pericardial effusion. Methods: We performed a systematic review of literature in PubMed, published up to February 1, 2020, according to PRISMA guidelines. We screened cross-sectional studies, observational cohort studies, and randomized control trials that evaluated the accuracy of Xpert MTB/RIF in diagnosing TB pericarditis. BACKGROUND: Pericardial constriction is a serious complication of tuberculous pericardial effusion that occurs in up to a quarter of patients despite anti-tuberculosis chemotherapy. The impact of human immunodeficiency virus (HIV) infection on the incidence of constrictive pericarditis following tuberculous pericardial effusion is unknown.
METHODS AND RESULTS: We conducted a prospective observational study to determine the association between HIV infection and the incidence of constrictive pericarditis among 185 patients (median age 33 years) with suspected tuberculous pericardial effusion. T

BACKGROUND: Tuberculosis remains an important cause of chronic constrictive pericarditis (CCP) in developing countries. It is a surgically treatable cause of diastolic heart failure. Without surgery, it is associated with high morbidity and mortality.
METHODS: We conducted a retrospective observational study of clinical presentations and perioperative outcomes of pericardiectomy in all patients operated from July 2015 to December 2018 for tuberculous CCP.
RESULTS: A total 14 patients (mean age - 38 ± 13.3 years, 10 male), underwent pericardiectomy via median sternotomy without cardiopulmonar

Background/Objectives: Tuberculous constrictive pericarditis (TCP) is characterized by "backward failure" and systemic venous congestion, leading to secondary multi-organ dysfunction. The Model for End-Stage Liver Disease (MELD) 3.0 score assesses liver, renal, and nutritional status, yet its prognostic utility in TCP remains undefined. This study aimed to evaluate the preoperative MELD 3.0 score as a predictor of time to discharge in patients undergoing pericardiectomy for TCP. Methods: We analyzed 190 patients undergoing pericardiectomy for TCP between 2018 and 2024. The association between

Tuberculous pericarditis is a serious extrapulmonary complication of tuberculosis (TB) that is difficult to diagnose and can lead to cardiac tamponade. This report describes the case of an immunocompetent man who developed cardiac tamponade due to a paradoxical response during TB treatment. No pericardial effusion was observed at the time of diagnosis. However, he developed cardiac tamponade 35 days after the initiation of anti-TB drugs. He was treated with emergency pericardial drainage and systemic corticosteroid therapy, after which his hemodynamic status improved. Although there was no rec

Tuberculous (TB) pericarditis is a rare but life-threatening form of extrapulmonary tuberculosis. The diagnostic strategy and optimal therapy for TB pericarditis are not well established. We retrospectively analyzed the diagnostic data, clinical characteristics, treatment and outcome in a total of 19 patients with TB pericarditis treated from January 1988 to July 2002. Based on the finding of echocardiography, 8 of these patients were classified as having early stage and 11 as having advanced-stage disease. There were 15 men and 4 women, with a mean age of 65 years (range, 34 to 80 years). All

BACKGROUND: Although Italy is a low-incidence country, global migration patterns necessitate consideration of tuberculosis (TB), particularly extrapulmonary forms, in the differential diagnosis.
CASE PRESENTATIONS: We report 2 cases of tuberculous pericarditis in young, immunocompetent, HIV-negative men from high TB-incidence regions who were hospitalized at the Infectious Diseases Unit of Policlinico Umberto I, Rome. In both patients, the diagnosis of TB was made presumptively based on clinical presentation, positive immunological markers, and characteristic findings on multimodal imaging. B

OBJECTIVE: Low cardiac output syndrome (LCOS) is the leading cause of death after cardiac surgery. Studies have shown that 24% of postoperative mortality in patients undergoing pericardiectomy is attributed to LCOS. It is necessary to explore the risk factors of LCOS after pericardiectomy in patients with tuberculous constrictive pericarditis (CP).
METHODS: Patients undergoing pericardiectomy for tuberculous CP were included in the study. The personal and clinical data of these patients with LCOS and without LCOS were collected and compared. Logistic regression analyses were conducted to iden

BACKGROUND: Perioperative complications following pericardiectomy in patients with constrictive pericarditis can significantly affect cardiac function recovery and postoperative outcomes. The prognostic nutritional index (PNI), a well-established nutritional marker, has been shown to predict outcomes in various diseases. However, its role as a predictive factor in patients with tuberculous constrictive pericarditis undergoing pericardiectomy remains unclear. This study aimed to evaluate the association between preoperative PNI and adverse perioperative outcomes in this patient population.
