Bone metabolism and the perimenopause overview, risk factors, screening, and osteoporosis preventive measures.

In summary, FDA-approved therapies for prevention and treatment of osteoporosis are all antiresorptive agents. There are no approved therapies at this time that stimulate bone formation, although one such agent (PTH) is awaiting approval. Screening perimenopausal women at risk should identify osteopenic women early in the menopause before the accelerated bone loss of estrogen deficiency causes further irreversible erosion in bone density. The National Osteoporosis Foundation advocates initiating therapy to reduce fracture risk in postmenopausal women with T scores below -2 in the absence or fa

Change in bone mineral density and its determinants in pre- and perimenopausal Chinese women: the Hong Kong Perimenopausal Women Osteoporosis Study.

UNLABELLED: This 30-month study investigating bone change and its determinants in 438 perimenopausal Chinese women revealed that the fastest bone loss occurred in women undergoing menopausal transition but maintenance of body weight and physical fitness were beneficial for bone health. Soy protein intake also seemed to exert a protective effect. INTRODUCTION: This 30-month follow-up study aims to investigate change in bone mineral density and its determinants in Hong Kong Chinese perimenopausal women. METHODS: Four hundred and thirty-eight women aged 45 to 55 years were recruited through ran

Perimenopausal Nuances in the Aging Face: A Comprehensive Perspective.

INTRODUCTION: Perimenopause accelerates facial aging through hormonal, structural, and psychosocial changes, presenting unique challenges for facial plastic surgeons. OBJECTIVES: This viewpoint article integrates two key perspectives: (1) psychosocial impacts and patient-centered care, contributed by Dr. Mary Claire Haver, a board-certified obstetrician-gynecologist and menopause specialist; and (2) hormonal influences and strategic interventions during perimenopause as a critical window for rejuvenation, contributed by Dr. Lindsey Pennington, who emphasizes integrated surgical and non-surgic

Adult and perimenopausal acne and the nurse's role in management.

Adult female acne is increasingly recognised as a chronic inflammatory skin disorder affecting women well beyond adolescence. Hormonal fluctuation, inflammatory signalling, skin barrier impairment, and microbiome shifts contribute to its development and persistence. Perimenopause and menopause introduce further complexity due to declining oestrogen levels and relative androgen excess. This article provides an evidence-based overview of adult and menopausal acne, integrating epidemiology, underlying mechanisms, clinical presentation, assessment, management, and the crucial role nurses play in p

Drugs against osteoporosis in perimenopausal and postmenopausal women.

The prevalence of osteoporosis will continue to rise as the world's population ages. Since women's bones are smaller and thinner than men's, bone loss accelerates as estrogen levels fall. Based on their primary effects, drugs used to treat osteoporosis can be categorized into three groups: 1. Anti-resorptive medications. These primarily prevent osteoclasts from resorbing bone, which has a secondary effect on bone growth. Commonly used anti-resorptive drugs include bisphosphonates, denosumab, hormone replacement therapy, and raloxifene. 2. Anabolic medicines. These mainly stimulate osteoblasts

Risk factors for perimenopausal distal forearm fracture.

This prospective population-based cohort study investigated factors predicting distal forearm fracture (DFF) in perimenopausal women. The study population consisted of 11,798 women from the Kuopio Osteoporosis Risk Factor and Prevention (OSTPRE) Study in Finland. Mean baseline age of these women was 52.3 (SD 2.9) years (range 47-56 years) and 68% were postmenopausal. Three hundred and sixty-eight women (3.1%) had a validated DFF during the 5-year follow-up. Previous wrist fracture, postmenopausal state, age and nulliparity were independent predictors of DFF, while hormone replacement therapy (

Risk factors for perimenopausal fractures: a prospective study.

This prospective study was aimed at determining the risk factors for the development of fractures in perimenopausal women. The study group (n = 3068) was comprised of a stratified population sample of women aged between 47 and 56 years. During the follow-up period of 3.6 years, 257 (8.4%) of the women sustained a total of 295 fractures. After adjustment for covariates, the relative risk (RR) of sustaining a fracture was found to be 1.4 [95% confidence interval (CI) 1.2-1.6] for a 1 standard deviation (SD) decrease in the spinal and femoral neck bone mineral density (BMD). The correlation of vaginal flora and anxiety in perimenopause women: a prospective study.

BACKGROUND: Perimenopausal women experience significant mental changes that affect their health and lives. Emerging evidence suggests that alterations in the vaginal microbiota may be associated with mental health symptoms through a potential vagina-brain axis. Here, we aimed to investigate the relation between anxiety in perimenopausal women and the changes in vaginal flora and identify the related factors of anxiety. METHODS: A total of 120 perimenopausal women were included. Anxiety was diagnosed using a self-rating anxiety scale. Their sleep and life quality were evaluated using the Pitts

Cognitive Behavioral Therapy for Sexual Concerns During Perimenopause: A Four Session Study Protocol.

Background: During the menopausal transition, women often experience physical (e.g., vasomotor symptoms) and emotional (e.g., anxiety and depression) difficulties that significantly impact functioning and overall quality of life. Although sexual concerns (e.g., decreased sexual desire, orgasm), are reported by up to 87% of peri- and post-menopausal women, and are associated with adverse impact on functioning and distress, treatment options that directly target this area are limited, and most often involve medication (e.g., hormone replacement). Effectiveness of these treatments is often define

Perimenopause promotes neuroinflammation in select hippocampal regions in a mouse model of Alzheimer's disease.

Alzheimer's disease (AD) is the most common neurodegenerative disorder characterized by age-dependent amyloid beta (Aβ) aggregation and accumulation, neuroinflammation, and cognitive deficits. Significantly, there are prominent sex differences in the risk, onset, progression, and severity of AD, as well as response to therapies, with disease burden disproportionately affecting women. Although menopause onset (i.e., perimenopause) may be a critical transition stage for AD susceptibility in women, the role of early ovarian decline in initial disease pathology, particularly key neuroinflammatory

[Changes in the hemostatic system in anomalous uterine hemorrhages and uterine myoma at perimenopause].

Concentrations of some hemostasis system parameters in the total blood stream and in the menstrual fluid were measured in 58 patients of a perimenopausal age suffering from uterine bleedings and myomas. Plasminogen, fibrinogen, and fibrin/fibrinogen degradation product concentrations were increased in the menstrual fluid of patients with profuse uterine bleedings in comparison with patients with moderate menstrual discharge. These shifts were the most marked in the patients with submucous localization of uterine myoma. Gestrinon and danason exerted no pathologic effect on the studied hemostasi
