Delivering perinatal psychiatric services in the neonatal intensive care unit.

AIM: To describe characteristics of mothers who would likely benefit from on-site short-term psychiatric services while their infant is in the neonatal intensive care unit (NICU). METHODS: For 150 consecutive mothers who were referred for psychiatric evaluation and psychotherapeutic intervention in an innovative NICU mental health programme, baseline information was collected. Data regarding their referrals, diagnosis, treatments and infants were analysed. RESULTS: Most mothers were referred because of depression (43%), anxiety (44%) and/or difficulty coping with their infant's medical probl

[Vaccine prevention in perinatal health care: parents, children and professionals].

Recent legislative texts have changed vaccinal policy and reinforced the role of midwives in vaccine prevention in perinatal healthcare. Quite as paediatricians and obstetricians-gynecologists, midwives can now prescribe and carry out, for the mothers, vaccines against rubella, tetanus, poliomyelitis, diphtheria, hepatitis B, influenza and whooping-cough and for the newborns vaccines against hepatitis B and tuberculosis. Concerning vaccinations, practitioners have to respect the vaccination calendar and a collaborative action is useful and necessary. These national guidelines are regularly upd

[Interface management in perinatal palliative medicine - Multiprofessional perinatal palliative care consultation and support: Implementation hurdles and solution strategies].

Perinatal palliative care represents an interdisciplinary and multiprofessional interface within the care context of perinatology. The three distinct patient groups are: extremely premature infants at the limit of viability, infants prenatally diagnosed life-limiting conditions, and acutely and severely ill newborns. This paper presents the approach for prenatally diagnosed life-limiting conditions at a university Level 1 perinatal care center and contextualizes it within the existing literature. Central challenges include: the uncertainty of prognosis, integration into existing care structure

Bereavement care guidelines used in health care facilities immediately following perinatal loss: a scoping review.

OBJECTIVE: The objective of the scoping review was to explore the evidence and describe what is known about perinatal bereavement care guidelines provided within health care facilities prior to discharge. Additionally, the review sought to identify what is known about parents' mental health outcomes, and map these outcomes to the characteristics of the bereavement care guidelines. INTRODUCTION: Perinatal loss poses a serious risk of emotional trauma and mental health sequelae. Conflicting evidence for international bereavement care guidelines and inconsistent implementation, a lack of experim

Perinatal mortality disparities between public care and private obstetrician-led care: a propensity score analysis.

OBJECTIVE: To examine whether disparities in stillbirth, and neonatal and perinatal mortality rates, between public and private hospitals are the result of differences in population characteristics and/or clinical practices. DESIGN: Retrospective cohort study. SETTING: A metropolitan tertiary centre encompassing public and private hospitals. Women accessed care from either a private obstetrician or from public models of care - predominantly midwife-led care or care shared between midwives, general practitioners, and obstetricians. POPULATION: A total of 131 436 births during 1998-2013: 69 0

Acupressure in perinatal care: Results of a cross-sectional study in Malta.

INTRODUCTION: Acupressure, the application of firm pressure to specific acupoints, has been associated with benefits during the perinatal period, including reducing nausea, alleviating pain, inducing labor, supporting lactation and decreasing anxiety. While research has explored its effectiveness, little is known about midwives' knowledge and views regarding acupressure and its integration into practice. This study aimed to explore midwives' knowledge, perspectives and factors influencing their use of acupressure during the perinatal period. METHODS: A quantitative, cross-sectional survey des

Care of Patients After Bariatric Surgery in the Periconceptional and Perinatal Periods.

Obesity in women of reproductive age is a major issue. It is associated with reduced fertility and an increased risk of obstetric and perinatal complications. Bariatric surgery is the most effective treatment for severe obesity, leading to substantial weight reduction, improvement of metabolic disorders, and enhanced fertility. Consequently, an increasing number of women are becoming pregnant after undergoing bariatric surgery. The aim of this paper is to review current recommendations and research data regarding the care of women after bariatric surgery in the periconceptional and perinatal p

Maternal Satisfaction with Perinatal Care and Breastfeeding at 6 Months Postpartum.

Positive childbirth experiences increase breastfeeding in the early postpartum period. Using a diverse, clinic-based sample of predominately low-income women (n = 118) recruited at their first prenatal appointment in 2017-2018 and followed through six months postpartum, binary logistic regression analyses were used to examine the association between maternal satisfaction with perinatal care measured two weeks postpartum and breastfeeding at six months postpartum. Participants reported high satisfaction with perinatal care overall (mean = 25.7; range of 6-30), and 25% of participants reported b

Priorities for research in perinatal palliative care: an international Delphi study.

BACKGROUND: Perinatal palliative care (PPC) addresses the complex needs of families facing life-limiting fetal or neonatal conditions through an interdisciplinary, family-centered approach. Despite growing recognition of its importance, there is a lack of systematically defined research priorities to guide the development of evidence-based PPC practices. OBJECTIVE: To identify and establish research priorities for perinatal palliative care (PPC) using a Delphi method with international experts and parents who experienced perinatal loss. METHODS: This Delphi study involved three Delphi survey

[The perinatal palliative care development through the years: a longitudinal study].

Advances in perinatal medicine have contributed to significantly improved survival of newborns. While some infants die despite extensive medical treatment, a larger proportion dies after a decision to withdraw life-sustaining therapy is made. The approaches to these decisions have significantly changed over the years, and their practical implementation still varies greatly between different neonatal intensive care units. The aim of this study was to evaluate the circumstances surrounding all neonatal deaths in a university neonatal setting in Germany over a ten-year period and to document chan

The Role of Midwives in US Perinatal Palliative Care: A Scoping Review.

INTRODUCTION: Perinatal palliative care (PPC) is a rapidly growing and essential reproductive health care option for pregnant persons with a diagnosed life-limiting fetal condition who continue their pregnancy. The provision of PPC is within the scope of basic midwifery competencies, and midwives are well-positioned to make unique and valuable contributions to interprofessional PPC teams. However, little is known about midwives' past or current involvement in PPC in the United States. METHODS: This scoping review of the literature investigated what is known about the role of midwives in PPC i

Perinatal considerations for care of transgender and nonbinary people: a narrative review.

PURPOSE OF REVIEW: The visibility of the lesbian, gay, bisexual, transgender, and queer (LGBTQ+) communities, specifically the transgender and nonbinary (TGNB) communities, continues to grow. However, there is little description, much less guidance toward optimizing, the pregnancy-related care of TGNB people. The overarching goal of this paper is to provide guidance that aids in reimagining obstetrics to include people of all genders. RECENT FINDINGS: This article will review current literature and provide recommendations specific to the hospital birthing environment to help address the lack

European Guidelines on Perinatal Care - Oxytocin for induction and augmentation of labor.

OF RECOMMENDATIONS1. Oxytocin for induction or augmentation of labor should not be started when there is a previous scar on the body of the uterus (such as previous classical cesarean section, uterine perforation or myomectomy when uterine cavity is reached) or in any other condition where labor or vaginal delivery are contraindicated. (Moderate quality evidence +++-; Strong recommendation).2. Oxytocin should not be started before at least 1 h has elapsed since amniotomy, 6 h since the use of dinoprostone (30 min if vaginal insert) and 4 h since the use of misoprostol (Low quality evidence ++-

Pushing beyond the silos: the obstetrician's role in perinatal depression care.

Perinatal depression is one of the most common medical complications during and after pregnancy, occurring in up to 20% of new mothers in the USA. It can have both short- and long-term effects on the mother, child, and family. Multiple etiologies have been proposed for perinatal depression, and there are many reported genetic, epigenetic, environmental, socioeconomic, and psychosocial risk factors. Perinatal depression can be treatable but remains underdiagnosed and undertreated for a variety of reasons, including inconsistent clinical definitions, inconsistent screening, social stigma, and lo

Perinatal Transmission of Hepatitis C Virus: Defining the Cascade of Care.

OBJECTIVES: The US National Viral Hepatitis Action Plan calls for major efforts to expand hepatitis C virus (HCV) diagnosis and treatment; prenatal care settings are potential venues for expanding HCV testing. We aimed to characterize the HCV diagnostic cascade for women and infants and investigate factors associated with linkage and follow-up. STUDY DESIGN: We used electronic health records for a 10-year cohort of 879 women with opioid use disorder from an obstetric clinic serving women with substance use disorders. RESULTS: Altogether, 744 women (85%) were screened for HCV; 510 (68%) were
