[Mucinous adenocarcinoma of the urachus and peritoneal pseudomyxoma].

The adenocarcinoma of the urachus is very rare tumor, with an incidence of 1/5.000.000 inhabitants, represents less than 0.001 of all types of bladder cancer. Peritoneal pseudomixoma is a rare neoplasm characterized by mucinous acites that involvement the peritoneal surface and omentum. Usually is associated with benign o malignant mucinous tumor of the appendix or ovary. In this paper, we present a case of peritoneal pseudomixoma caused by a mucinous adenocarcinoma of the urachus.

Mitochondrial DNA sequencing demonstrates clonality of peritoneal implants of borderline ovarian tumors.

Borderline ovarian tumors are rare low malignant potential neoplasms characterized by the absence of stromal invasion, whose main prognostic factors are stage and type of peritoneal implants. The latter are defined as invasive when cell proliferation invades the underlying tissue (peritoneal surface, omentum and intestinal wall), or noninvasive. It is still unknown if these implants are metastatic spread from the primary ovarian mass or a neoplastic transformation de novo of the peritoneal surface. Mitochondrial DNA sequencing was performed to assess clonality in eight patients presenting both

A case report of peritoneal malignant mesothelioma presenting as primary ovarian mass.

Malignant mesotheliomas are aggressive neoplasms arising from mesothelial cells lining the body cavities. Malignant peritoneal mesothelioma (MPM) account for about one-third of the cases. Though the ovarian involvement may be seen in the background of a diffuse peritoneal involvement, the presentation of MPM as a primary ovarian mass is rare. Here we present such a case who underwent surgery but had residual progressive lesion. She received further chemotherapy resulting in a complete response and is disease free for almost a year.

Case Report of Four Different Primary Mesenteric Neoplasms and Review of Literature.

INTRODUCTION: It is rare for primary tumors to arise from the mesentery. Lymphangiomas appear as congenital malformations of the lymphatic system or benign neoplasms as a large, thin-walled, often multilocular cyst. Mesenteric infiltration is common and during surgical treatment, adjustment of structures such as the bowel and resection of the spleen may be required. Cystic mesotheliomas are rare, benign tumors that originate from the peritoneal mesothelium and are more common in women. Mesenteric cysts are mostly benign and rare intra-abdominal tumors, and can be seen as occupying a large cyst

The robotic future of minimally invasive cytoreduction and hyperthermic intraperitoneal chemotherapy for peritoneal surface malignancies.

Cytoreductive surgery with hyperthermic intraperitoneal chemotherapy (CRS/HIPEC) has been increasingly utilized for peritoneal surface malignancies. This has been commonly utilized for a variety of neoplasms, but, especially mucinous neoplasms of the appendix, ovarian cancer, gastric cancer, colorectal cancer and mesothelioma. Traditionally, CRS/HIPEC has been performed as an open, extensive operation associated with prolonged hospitalization. However, when the peritoneal carcinomatosis index (PCI) is small (<10), minimally invasive approaches can be considered. Such less invasive approaches m

Low-grade appendiceal mucinous neoplasm vs. high-grade appendiceal mucinous neoplasm: similarities, differences, and current management challenges.

Appendiceal mucinous neoplasms are rare tumours whose terminology, staging and management remain difficult to align with conventional colorectal cancer paradigms. Low-grade appendiceal mucinous neoplasm (LAMN) is well established, whereas high-grade appendiceal mucinous neoplasm (HAMN) remains less clearly defined in clinical practice. This review summarises current evidence on the classification, pathological distinction, clinical behaviour, prognosis and management of LAMN and HAMN. Both entities share a non-infiltrative growth pattern, but HAMN is distinguished by high-grade cytologic atypi

Lung cancer diagnosis on ovary mass: a case report.

Metastatic neoplasms to the ovary often cause diagnostic problems, in particular those large ovarian masses mimicking primary tumors. Most of these tumors arise from digestive system or breast, while 37-year-old woman diagnosed as right adnexal complex mass, with a subpleural nodule in the apical part of the left lower lobe, at preoperative chest computed tomography scan. The patient underwent total abdominal hysterectomy with right salpingo-oophorectomy (ovarian mass 220 × 200 mm), total omentectomy, left ovarian biopsy, peritoneal random biopsies, and peritoneal washings for cytology. Appendiceal mucinous neoplasms: a clinicopathologic analysis of 107 cases.

The classification of appendiceal mucinous tumors is controversial and terminology used for them inconsistent, particularly when they lack overtly malignant features but are associated with extra-appendiceal spread. We reviewed 107 appendiceal mucinous neoplasms and classified them as low-grade appendiceal mucinous neoplasm (LAMN) (n = 88), mucinous adenocarcinomas (MACAs) (n = 16), or discordant (n = 3) based on architectural and cytologic features. LAMNs were characterized by a villous or flat proliferation of mucinous epithelium with low-grade atypia. Thirty-nine tumors were confined to the

The positive peritoneal effusion. A retrospective study of cytopathologic diagnoses with autopsy confirmation.

The distribution of 215 malignant tumors responsible for peritoneal effusions was investigated with respect to cytohistologic correlation and autopsy features. In males, cancer of the gastrointestinal tract, pancreas and liver were the most frequently observed forms, whereas in females tumors at gynecologic sites far outnumbered other neoplasms, such as stomach, bowel, pancreas, gallbladder and liver cancer. Cells from extra abdominal tumors, consistently seen in positive ascitic effusions, were from pleural mesothelioma and breast carcinoma, respectively, in males and females. The first diagn

Hyperleukocytosis, leukemoid reaction caused by malignant peritoneal mesothelioma: a case report and review of literature.

Paraneoplastic leukemoid reaction (PLR)-induced hyperleukocytosis remains a critical diagnostic challenge. Malignant peritoneal mesothelioma, known for its nonspecific clinical presentation, often evades early detection. We report a 75-year-old male with cirrhosis presenting with fever (38.2°C), progressive ascites, leukocytosis (109.9×109/L), and elevated CRP (247.41 mg/L). Initial diagnosis of spontaneous peritonitis failed to explain the leukocytosis, prompting bone marrow examination, including cytomorphology and genetic analysis, which turned out do not support a diagnosis of leukemia or

Targeted Genetic Sequencing Analysis of 223 Cases of Pseudomyxoma Peritonei Treated by Cytoreductive Surgery and Hyperthermic Intraperitoneal Chemotherapy Shows Survival Related to GNAS and KRAS Status.

BACKGROUND AND AIM: Pseudomyxoma peritonei (PMP) is an unusual condition with unique behaviour caused by a mucinous neoplasm, usually arising from the appendix. The aim of this study was to evaluate the prevalence of genomic alterations in clinical specimens of PMP using a targeted assay and correlate the findings with clinical, pathological and outcome data. Sequencing data from 223 patients were analysed. RESULTS: The median follow-up interval was 48 months. The primary neoplasm was appendiceal in 216 patients, ovarian in 4, urachal in 2 and renal in one. We confirmed common mutations in GN

"Pseudomyxoma Endometrii": Endometrial Deposition of Acellular Mucin from a Low-Grade Appendiceal Mucinous Neoplasm as a Rare Mimic of Myxoid Uterine Tumors.

Low-grade appendiceal mucinous neoplasms (LAMNs) are commonly associated with deposition of mucin, with or without admixed low-grade epithelium, on peritoneal surfaces (pseudomyxoma peritonei). We describe a very rare presentation of LAMN as extensive mucin deposition in the endometrium of a 43-yr-old woman initially mistaken for a primary uterine myxoid neoplasm. The patient underwent endometrial curettage that demonstrated abundant myxoid/mucoid material interspersed with small vessels, bland eosinophilic spindled cells, scattered foci of typical endometrial stroma, and occasional endometrio

Metastatic carcinomas in the cervix mimicking primary cervical adenocarcinoma and adenocarcinoma in situ: report of a series of cases.

Metastatic tumors within the cervix are uncommon if one excludes endometrial carcinoma, which involves the cervix by direct spread. A variety of other neoplasms rarely metastasize to the cervix and, in most cases, the diagnosis is straightforward because of a combination of clinical and pathologic parameters, common features of metastatic carcinoma within the cervix including predominant involvement of the deep stroma, absence of surface involvement and of an in situ component, and prominent lymphovascular permeation. We describe 6 cases of metastatic adenocarcinoma involving the cervix with s

A proposal of Brazilian Society of Surgical Oncology (BSSO/SBCO) for standardizing cytoreductive surgery (CRS) plus hyperthermic intraperitoneal chemotherapy (HIPEC) procedures in Brazil: pseudomixoma peritonei, appendiceal tumors and malignant peritoneal mesothelioma.

Cytoreductive surgery plus hypertermic intraperitoneal chemotherapy has emerged as a major comprehensive treatment of peritoneal malignancies and is currently the standard of care for appendiceal epithelial neoplasms and pseudomyxoma peritonei syndrome as well as malignant peritoneal mesothelioma. Unfortunately, there are some worldwide variations of the cytoreductive surgery and hypertermic intraperitoneal chemotherapy techniques since no single technique has so far demonstrated its superiority over the others. Therefore, standardization of practices might enhance better comparisons between o

Ovarian serous borderline tumors with noninvasive and invasive peritoneal implants: A case report each.

Serous borderline tumors (SBT) are defined by the World Health Organization (WHO) as serous neoplasms that show epithelial proliferation greater than that seen in serous cystadenomas, as evidenced by cellular stratification, cytologic atypicality, and epithelial tufting, but which exhibit no evidence of "destructive stromal" invasion and can show extra-ovarian implants. Characterization of invasive peritoneal implants from patients with noninvasive serous ovarian tumors has important prognostic and treatment implications. Peritoneal implants have been classified as either noninvasive or invasi

Peritoneal metastases from an atypical fibroxanthoma.

Atypical fibroxanthoma (AFX) is a mesenchymal neoplasm usually occurring in sun-exposed skin of elderly patients. The majority have an excellent prognosis, as recurrences are uncommon and metastases are rare. We present a case of an 81-year-old man who developed widespread peritoneal metastases from an AFX on his scalp, which was completely excised 3 years earlier. Histology of the scalp lesion showed a markedly pleomorphic neoplasm characteristic of AFX. Features associated with increased risk of metastasis, namely lymphovascular space invasion, deep invasion, and substantial necrosis, were n

Revisiting the necessity for routine appendectomies in mucinous neoplasms of the ovary: An evaluation of 460 mucinous ovarian tumors.

Appendectomies are not uncommonly performed following an intraoperative diagnosis of a mucinous ovarian neoplasm, although the evidentiary basis for the practice is relatively limited. The current study is a contemporary re-examination of the issue, based on an analysis of a large single institutional cohort. We assessed whether there are any composite of factors that may be associated with the finding of significant disease in the appendix in this setting following intraoperative consultation (IOC) diagnosis of a mucinous neoplasm on an ovary-based mass. Records for 460 consecutive patients w

SOX6 Expression Is Sensitive for Peritoneal Epithelioid Malignant Mesothelioma, But Not Specific in the Differential Diagnosis With Tubo-ovarian Serous Neoplasia.

Primary peritoneal malignant mesothelioma (MM) can demonstrate morphologic overlap with low-grade and high-grade tubo-ovarian serous neoplasms; it is also biologically and prognostically distinct from benign mesothelial proliferations. Currently, there is no single biomarker that can definitively distinguish these neoplasms. Sex-determining region Y box 6 (SOX6) immunohistochemistry has been recently described to differentiate pleural epithelioid MM from lung adenocarcinoma, but it has not been evaluated in the peritoneum. SOX6 immunohistochemistry was performed on 43 peritoneal epithelioid MM

Salvage cytoreductive surgery and HIPEC in chemoresistant juvenile granulosa cell tumour.

Juvenile granulosa cell tumours (JGCTs) are rare ovarian neoplasms with aggressive potential and limited responsiveness to chemotherapy. We present a case of a female in early adolescence with chemoresistant JGCT and extensive peritoneal recurrence following initial surgery and systemic therapy. The patient underwent cytoreductive surgery followed by hyperthermic intraperitoneal chemotherapy (HIPEC) with cisplatin, resulting in a favourable postoperative course. This report highlights the role of aggressive surgical management combined with HIPEC as a promising approach for selected cases of c

The Stomach: a Rare Site for Metastatic Solid Pseudopapillary Neoplasm of the Pancreas.

Metastatic solid pseudopapillary neoplasms of the pancreas are rare, and the stomach is also a rare site for metastases. We present a case of a lady with solid pseudopapillary neoplasm of the pancreas who previously had a distal pancreatectomy but subsequently had multiple hepatic, adrenal peritoneal and nodal metastases. She developed dyspepsia and on oesophagogastroduodenoscopy, was found to have gastric metastases as well. We believe this is the first reported case of metastatic solid pseudopapillary neoplasm of the pancreas to the stomach.
