Abdominal compartment syndrome and acute pancreatitis.

Significant visceral edema associated with massive fluid resuscitation, paralytic ileus and formation of pancreatic ascites in patients with severe acute pancreatitis (SAP) can lead to abdominal compartment syndrome (ACS) that can contribute to the early development of multiple organ dysfunction syndrome (MODS), especially in the early stages of the disease. The prevalence of intra-abdominal hypertension (IAH) in SAP is about 40% and a manifest ACS occurs in about 10% of the patients warranting close monitoring of intra-abdominal pressure (lAP) in all patients with the severe form of the disea

Cardiovascular abnormalities in special conditions of advanced cirrhosis. The circulatory adaptative changes to specific therapeutic procedures for the management of refractory ascites.

Advanced liver disease is characterized by decreased arterial blood pressure and peripheral vascular resistances, increased cardiac output and heart rate in the setting of a hyperdynamic circulatory pattern favoured by total blood volume expansion, circulatory overload and overactivity of the endogenous vasoactive systems. Reduced heart responses to stressful conditions such as changes in loading conditions of the heart in presence of further deterioration of liver function such as refractory ascites, hepatorenal syndrome, spontaneous bacterial peritonitis and bleeding esophageal varices have

Treatment of Ascites.

Ascites is the most common complication of cirrhosis and occurs in more than half of all patients with cirrhosis. The development of ascites indicates progression of the underlying cirrhosis and is associated with a 50% 2-year survival rate. Conventional therapies used for the treatment of ascites include sodium restriction (<88 mmol/d), diuretics, and large volume paracentesis (LVP) (>5 L). The most effective diuretic combination is that of a potassium-sparing, distal-acting diuretic (eg, spironolactone) with a loop diuretic (eg, furosemide). LVP provides rapid resolution of symptoms with min

Minimizing ascites. Complication of cirrhosis signals clinical deterioration.

Ascites is the most common complication of cirrhosis. Its development is associated with a grave prognosis; 50% of patients die within 2 years of diagnosis. An understanding of the analysis of ascitic fluid is essential for the appropriate management of patients with liver disease and ascites. The management of patients with ascites involves a combination of dietary, medical, and surgical approaches. Furthermore, patients with ascites are at risk for ascitic fluid infections and neurohormonal dysregulation that can lead to hepatorenal syndrome. Early recognition of these complications allows t

Ascites.

Ascites is the most common presentation of decompensated cirrhosis, and its development heralds a poor prognosis, with a 50% 2-year survival rate. Effective first-line therapy for ascites includes sodium restriction (2 g/d), use of diuretics, and large-volume paracentesis (LVP). Ideally, a combination of a loop-acting diuretic (eg, furosemide) and a distal-acting diuretic (eg, spironolactone) is used. LVP has the advantage of producing immediate relief from ascites and its associated symptoms. When 5 L or more ascitic fluid is removed, albumin (6 to 8 g per liter of fluid removed) should be ad

Chylous ascites after oncological abdominal surgery: incidence and treatment.

AIMS: Chylous ascites can be a problem after oncological abdominal surgery. The aim of this study was to report the incidence and the management of the problem. METHODS: A retrospective study over a 2-year period of all oncological patients undergoing abdominal surgical procedures was carried out. Patients with resections in the upper abdomen and retroperitoneum were studied in more detail. RESULTS: Twelve (7.4%) of 163 patients with complex surgical procedures developed a chyloperitoneum. Chylous ascites stopped in time with conservative management in nine patients. Three patients had a per

[Venous angiomatous hamartoma of the mesenteric root. Therapy of chylous ascites abd chylothorax--case report].

Hamartomas are rare benign tumors appearing very often without any symptoms. In case of diagnostic detection the operative exploration is indicated for a correct histological diagnosis and resection. In this report we describe a very rare case of mesenterial hamartoma. During the late postoperative course a refilling chylothorax and chylous ascites occurred. With total parenteral nutrition, excluding short- and long-chain fatty acids, chylous leakage was successfully treated. The chylous exudation was stopped totally; only intraabdominally did minimal ascites persist. The therapy was continued

[The treatment of patients with complications of liver cirrhosis at the decompensation stage].

Experience of mini-invasive interventions in 180 patients with hepatic cirrhosis (HC), complicated by portal hypertension syndrome in decompensation stage was summarized. High risk of laparotomy in this patients made its conduction impossible. Mini-invasive interventions included the truncus coeliacus branches embolization, peritoneovenous shunting, endoscopic sclerotherapy of esophageal varicose veins. Total mortality constituted 6%. There were few severe complications. Most frequent rate (81% of observations) of fair results in the late period was noted after conduction of consecutive interv

Peritoneal shunt migration into the pulmonary artery--case report.

A 48-year-old man underwent ventriculoperitoneal shunting for hydrocephalus secondary to subarachnoid hemorrhage due to left vertebral artery dissection, which had been successfully treated by trapping. The peritoneal catheter was correctly positioned via a right upper abdominal incision, and symptoms related to the hydrocephalus disappeared. One month later, the patient began to complain of pain on the right side of the neck. Chest radiography revealed that the peritoneal end of the catheter had migrated into the right pulmonary artery. The catheter route was explored through a small neck inc

Subclavian catheterization for the placement of a ventriculovenous shunt.

Repeated nonfunction of the distal end of a ventriculoperitoneal shunt led to an unsuccessful attempt to introduce the distal catheter into the vena cava through the jugular vein. Subclavian catheterization utilizing a cardiac pacemaker introducer set proved to be a simple and effective means of establishing a ventriculovenous shunt.

Treatment of ascites: old and new remedies.

Ascites is a common complication of chronic liver disease. Treatment of the underlying liver disease with modalities such as abstinence from alcohol in Laennec's cirrhosis, phlebotomy in hemochromatosis, copper removal in Wilson's disease, and steroids in autoimmune liver disease, can improve survival in many patients. In addition, therapy of ascites alleviates the symptoms and improves the quality of life of the patients, and probably decreases the incidence of life-threatening conditions including spontaneous bacterial peritonitis and hepatorenal syndrome. The mean survival rate at 2 years i

Transjugular intrahepatic portosystemic stent-shunt after orthotopic liver transplantation in a patient with early recurrence of portal hypertension of unknown origin.

A 65-year-old italian patient developed complicated portal hypertension immediately after orthtopic liver transplantation (OLT) necessitating shunt creation. One to five weeks after OLT, massive ascitic fluid losses of up to 121/day developed. Vascular and major hepatic-parenchymal abnormalities were excluded by duplexsonography, angiography and initial histology, respectively. A peritoneovenous shunt (Denver-shunt) on day 31 after OLT reduced (by about 50%) but did not stop ascitic fluid losses. Furthermore, three variceal bleedings occurred after implantation of the Denver-shunt. Direct port

Treatment of patients with cirrhosis and ascites.

The treatment of patients with cirrhosis and fluid overload has undergone substantial change in recent years, because of new information regarding old treatments, as well as new treatments. The goals of treatment are to maximize life expectancy and quality of life. Development of ascites is a landmark in the natural history of cirrhosis signaling poor life expectancy, in general. Patients who are appropriate candidates for liver transplantation should undergo evaluation for this procedure after development of ascites. Patients awaiting transplantation as well as non-candidates for this procedu

Postoperative chylous ascites--the urologist's view.

Postoperative chylous ascites is a rare complication of retroperitoneal surgery caused by incidental disruption of major retroperitoneal lymphatics and is associated with serious mechanical, nutritional and immunological consequences due to the constant loss of protein and lymphocytes. Different management schemes may be used successfully to treat chylous ascites. Treatment should be adjusted to the severity of the lymphatic leak and its consequences. The management algorithm presented in this article is based on a step-up approach aimed at reducing lymph flow and maintaining nutritional balan

Contemporary Management of Malignant Ascites.

INTRODUCTION: Malignant ascites (MA) develops when malignant disease of the peritoneum causes excess fluid to accumulate in the abdominal cavity. It portends a poor prognosis and is associated with debilitating symptoms. While several palliative therapies exist, none have proven curative or free from side effects and complications. This review article describes experimental therapies on the horizon and the contemporary management of MA. MATERIALS AND METHODS: A literature review was performed using MEDLINE/PubMed, in which studies of emerging or experimental therapies under investigation for
