[Clinical diagnosis and treatment of chronic dizziness].

Chronic dizziness is defined as a complex of symptoms lasting months or years, including oscillopsia, nystagmus, and postural instability. Diagnostic search includes peripheral vestibulopathy - chronic unilateral vestibulopathy resulting from vestibular neuronitis, schwannoma of the 8th pair of cranial nerves, medical or surgical ablation of the labyrinth, and bilateral vestibulopathy; central vestibulopathy - syndromes accompanied by central vertical nystagmus, small vessel disease in patients with chronic cerebral ischemia; general somatic diseases, peripheral polyneuropathy, side-effects of

Update on functional vestibular and gait disorders.

PURPOSE OF REVIEW: Three functional neurological disorders are encountered in neuro-otologic practice, persistent postural-perceptual dizziness (PPPD), which is the commonest cause of chronic vestibular and balance symptoms, mal de debarquement (MdDS), a rarer but potentially debilitating disorder, and functional gait disorder, an often overlooked but treatable condition. RECENT FINDINGS: Recent investigations of PPPD suggested that there may be subtypes or subthreshold variants that merit further investigation. Studies of pathological mechanisms continue to offer new insights into the comple

What patients do when they feel dizzy: behavioural signatures reveal diagnostic clues in common vestibular disorders.

BACKGROUND: Diagnosis of vestibular disorders relies heavily on clinical history, yet how patients adapt their behaviour in response to dizziness remains relatively unexplored. We investigated whether adopted avoidance behaviours and coping strategies provide additional diagnostic information and insights into disease mechanisms. METHODS: Consecutive patients attending a specialist dizziness clinic underwent a structured interview before clinical assessment. Participants reported dizziness triggers, avoidance behaviours, and coping responses. Final diagnoses were established independently by

Chronic Dizziness.

PURPOSE OF REVIEW: Determining the etiology of disorders that manifest with chronic dizziness can seem a daunting task, but extracting some basic elements of the patient's history can reduce the differential diagnosis significantly. This includes determining initial triggers, timing of symptoms, associated features, and exacerbating factors. This article covers distinct causes of chronic dizziness including persistent postural perceptual dizziness, mal de débarquement syndrome, motion sickness and visually induced motion sickness, bilateral vestibulopathy, and persistent dizziness after mild c

[Accident-related inner ear diseases - clinical aspects and conditions].

Trauma-related inner ear disorders following head impacts can have various causes. The use of the more recent classification of temporal bone fractures into "otic capsule sparing" (OCS) and "otic capsule violating" (OCV) is useful in the context of medical assessment. The available imaging is often insufficient, and the consequences of otic capsule fractures are underestimated in emergency management. Therefore, indirect signs (e. g., hemotympanum, cerebrospinal fluid otorrhea, facial palsy) often must be evaluated. Even without radiological evidence of a fracture, temporary inner ear damage c

Impact of proprioceptive cervical dizziness in chronic neck pain syndromes on gait and stance during active head-turn challenges.

An earlier observational study described selected patients with acute neck pain syndrome, who experienced short bursts of cervical vertigo elicited by rapid head movements. The current study on a larger cohort of 20 patients with chronic or frequently recurring neck pain syndrome and age-matched controls focused on two major questions: (1) Can head movements in subjects with exacerbations of neck pain and restrictions of neck mobility also elicit bursts of vertigo? (2) What is the impact on postural balance measured by analysis of body sway and locomotion? A detailed questionnaire was applied,

Visual snow syndrome, the spectrum of perceptual disorders, and migraine as a common risk factor: A narrative review.

OBJECTIVE: The aim of this narrative review is to explore the relationship between visual snow syndrome (VSS), migraine, and a group of other perceptual disorders. BACKGROUND: VSS is characterized by visual snow and additional visual and nonvisual disturbances. The clinical picture suggests a hypersensitivity to internal and external stimuli. Imaging and electrophysiological findings indicate a hyperexcitability of the primary and secondary visual areas of the brain possibly due to an impairment of inhibitory feedback mechanisms. Migraine is the most frequent comorbidity. Epidemiological and

Dizziness: When Is It Migraine, and When Is It Not?

PURPOSE OF REVIEW: Dizziness is a frequent neurologic symptom in patients with headache, which overlaps with many etiologies beyond vestibular migraine. This review aims to help neurologists distinguish when dizziness is due to vestibular migraine (VM) versus other vestibular syndromes. RECENT FINDINGS: VM is now recognized as the most common cause of spontaneous episodic vertigo, with expanding diagnostic criteria, new pathophysiological insights involving CGRP, and emerging treatment strategies including anti-CGRP therapies. It overlaps with benign paroxysmal positional vertigo (BPPV), Meni

The Pharmacological Treatment of Pediatric Vertigo.

Vertigo in children is a challenging topic. The lack of dedicated trials, guidelines and papers causes inhomogeneity in the treatment of vertigo in children. Meniere's disease, migraine equivalents, vestibular neuritis, paroxysmal positional benign vertigo (BPPV), persistent postural-perceptual dizziness (PPPD) and motion sickness may affect children with various degrees of incidence and clinical severity compared to adults. Several drugs are proposed for the management of these conditions, even if their use is subordinated to the child's age. In this review, we summarize the existing evidence

Visual function and symptom provocation in children and young adults with vestibular disorders: A prospective controlled study.

PURPOSE: To understand if visual function tests with moving stimuli were harder to process and caused more symptom provocation in persistent postural perceptual dizziness (PPPD) patients compared with those with other vestibular diagnoses or without any vestibular symptoms. METHODS: 61 patients aged 8-23 years participated in a prospective study with three groups: PPPD (n = 21); at least one vestibular disorder without PPPD (n = 20); control-without vestibular or vision symptoms (n = 20). Subjects underwent visual function tests with moving stimuli: near point of convergence (NPC), amplitude

Feasibility of gamified visual desensitisation for visually-induced dizziness.

Visually-induced dizziness (visual vertigo) is a core symptom of Persistent Perceptual Postural Dizziness (PPPD) and occurs in other conditions and general populations. It is difficult to treat and lacks new treatments and research. We incorporated the existing rehabilitation approach of visual desensitisation into an online game environment to enhance control over visual motion and complexity. We report a mixed-methods feasibility trial assessing: Usage and adherence; rehabilitation potential; system usability and enjoyment; relationship with daily dizziness. Participants played online with (

The Interrelations Between Different Causes of Dizziness: A Conceptual Framework for Understanding Vestibular Disorders.

BACKGROUND: According to population-based studies that estimate disease prevalence, the majority of patients evaluated at dizziness clinics receive a single vestibular diagnosis. However, accumulating literature supports the notion that different vestibular disorders are interrelated and often underdiagnosed. OBJECTIVE: Given the complexity and richness of these interrelations, we propose that a more inclusive conceptual framework to vestibular diagnostics that explicitly acknowledges this web of association will better inform vestibular differential diagnosis. METHODS: A narrative review wa

Therapeutic Management of Vestibular Disorders During Pregnancy: A Narrative and Evidence-Based Review.

Vestibular disorders such as vestibular migraine (VM), benign paroxysmal positional vertigo (BPPV), Ménière's disease (MD), persistent postural-perceptual dizziness (PPPD), and vestibular neuritis (VN) are not uncommon among women of reproductive age and may occur or worsen during pregnancy. However, the management of these disorders during gestation is complicated by physiological changes, the limited safety data on many medications, and ethical constraints in clinical research involving pregnant individuals. This review focuses on the therapeutic strategies employed in managing vestibular di

Advances in diagnosis and treatment of vestibular migraine and the vestibular disorders it mimics.

Dizziness is one of the most common chief complaints in both the ambulatory care setting and the emergency department. These symptoms may be representative of a broad range of entities. Therefore, any attempt at treatment must first start with determining the etiology. In this current perspective, we focus specifically on the diagnosis of and treatment of vestibular migraine, which is common and overlaps clinically with a variety of other diagnoses. We discuss the traditional treatments for vestibular migraine in addition to the recent explosion of novel migraine therapeutics. Because vestibul

Reduced betweenness centrality of a sensory-motor vestibular network in subclinical agoraphobia.

Agoraphobic patients feel dizzy in crowded open spaces and respond to this symptom with excessive fear and avoidance. These clinical features show great similitude with the newly defined syndrome of persistent postural perceptual dizziness (PPPD). Patients with PPPD show decreased activity and connectivity in regions of the vestibular cortex. Due to the great overlap between these two conditions, we hypothesized that individuals with sub-clinical agoraphobia would show reduction in the connectivity features of these regions. We selected a group of healthy individuals from the Human Connectome

[Paradigm shift in management of patients with vertigo and imbalance].

Dizziness is one of the most common causes of medical visits. Management of the dizzy patient may be challenging both for the general practitioner, in emergency departments, and special clinics, as behind a seemingly homogeneous clinical presentation several very different etiologies may occur. Research of the last two century enriched our knowledge about physiology and pathophysiology of the vestibular system. Much knowledge is now available about the labyrinth being able to sense head motions and gravity, processing of afferent vestibular stimuli, reflectory oculomotor and postural control,

Longitudinal Patient Outcomes in Chronic Dizziness: A Scoping Review.

BACKGROUND: Chronic dizziness can cause significant functional impairment. Outcome measures used in this patient population have not been examined systematically. Consequently, providers lack consensus on the ideal outcome measures to assess the impact of their interventions. OBJECTIVE AND METHODS: We conducted a scoping review to summarize existing literature on outcomes in chronic dizziness (with a minimum of 6 mo of patient follow-up). Among other details, we extracted and analyzed patient demographics, medical condition(s), and the specific outcome measures of each study. RESULTS: Of 19,
