Petrous bone cholesteatoma presenting as CSF rhinorrhea: An extremely rare case report.

Patients with petrous bone cholesteatoma most commonly present with hearing loss and facial paralysis. Other clinical presentations can include otorrhea, tinnitus, aural fullness, otalgia, and dizziness. Here, we report a patient with petrous bone cholesteatoma who presented with cerebrospinal fluid rhinorrhea. This patient was initially misdiagnosed with rhinogenic cerebrospinal fluid leakage, and was finally diagnosed with the supralabyrinthine type of petrous bone cholesteatoma. During the surgical repair through the transmastoid trans-superior semicircular canal approach, the fistula was f

Exteriorization of Petrous Bone Cholesteatoma by Endonasal Endoscopic Approach: A Case Report.

A 61-year-old woman presented with diplopia and headache. The patient had a longstanding history of petrous bone cholesteatoma (PBC) on the left side and had undergone multiple surgeries to address it. Computed tomography (CT) revealed a radiolucent lesion with bony destruction in the left petrous apex. Magnetic resonance imaging of the lesion revealed a hypointense area on T1-weighted images and a hyperintense area on T2-weighted and abnormal diffusion-weighted images. A diagnosis of recurrent petrous apex cholesteatoma was made. The patient was treated by exteriorization using an endoscopic

Giant Petrous Bone Cholesteatoma: Combined Microscopic Surgery and an Adjuvant Endoscopic Approach.

Petrous bone cholesteatomas (PBCs) are epidermoid cysts, which have developed in the petrous portion of the temporal bone and may be congenital or acquired. Cholesteatomas arising in this region have a tendency to invade bone and functional structures and the middle and posterior fossae reaching an extensive size. Traditionally, surgery of a giant PBC contemplates lateral transtemporal or middle fossa microscopic surgery; however, in recent years, endoscopic surgical techniques (primary or complementary endoscopic approach) are starting to receive a greater consensus for middle ear and mastoid

Spontaneous Cerebrospinal Fluid Leak through the Posterior Aspect of the Petrous Bone.

Spontaneous cerebrospinal fluid (CSF) leak through the posterior fossa (PF) aspect of the petrous bone is exceedingly rare. A case series allows analysis of etiologies and how they may differ from the more common middle fossa (MF) route of leakage. The design was a retrospective case series. The setting was a tertiary care institution. A series of three patients with PF spontaneous CSF leaks was identified. High-resolution imaging (CT and MRI) and intraoperative observations were evaluated. Both in this series and in previously reported cases, patients share the demographics typically found in

[A rare cause of otitis externa : Manifestation of myeloid sarcoma in the petrous bone].

A 45-year-old male patient presented with pronounced swelling of the external auditory canal and symptoms of otitis externa which did not respond to conservative treatment. Due to concomitant mastoiditis, mastoidectomy was performed in the later course, which did not provide conclusive histological results. In the absence of improvement, biopsy of the external auditory canal and mastoid revision with repeated pathological analysis were carried out. Ultimately, a myeloid sarcoma representing a relapse of previously remitted acute myeloid leukemia (AML) was identified.

Decision-making in Petrous Bone Cholesteatoma (PBC): Surgical options according to PBC classification.

INTRODUCTION: Petrous bone cholesteatoma (PBC) is a rare, potentially life-threatening condition requiring surgical management to prevent severe complications. While classification systems, such as Sanna's, are widely used to guide surgical planning, actual decision-making is often influenced by additional clinical factors beyond lesion type. MATERIAL AND METHODS: A retrospective analysis of 94 PBCs in 92 patients operated between 1995 and 2024 was conducted. Data included demographics, clinical presentation, PBC classification, surgical approach, use of endoscopic assistance, recurrence, fol

OBJECTIVE: To assess our experience in the management of the facial nerve and hearing rehabilitation in patients with petrous bone cholesteatoma (PBC). STUDY DESIGN: Retrospective case review. SETTING: Tertiary referral center. PATIENTS: PBC cases managed by the senior authors between 1995 and 2024. MAIN OUTCOME MEASURES: Clinical presentation, Sanna's classification, presurgical and postsurgical audiometry and facial function, surgical approach (including assisted endoscopic techniques), recurrence rates, intraoperative findings and facial nerve management, hearing rehabilitation, facial

Intraosseous Meningioma Along the Left Petrous Bone: A Rare Cause of Trigeminal Neuralgia.

Trigeminal neuralgia (TN) presents with symptoms of intense recurrent shock-like brief pain localized to specific areas of the face innervated by the fifth cranial nerve. The pathology of trigeminal neuralgia is attributed to the fifth cranial nerve compression or demyelination. Most cases of this diagnosis are not due to bony structures, making this case an uncommon presentation of trigeminal neuralgia. Herein, we present a case of trigeminal neuralgia due to an intraosseous meningioma that formed along the left petrous bone, resulting in trigeminal nerve compression. On head computed tomogra

Factors associated with blunt internal carotid artery injury in petrous temporal bone fractures.

INTRODUCTION: Traumatic internal carotid artery (ICA) injuries are an uncommon complication of petrous temporal bone (PTB) fractures that can have devastating consequences of stroke, haemorrhage and death. Current guidelines suggest that all PTB fractures should be screened for blunt cerebrovascular injury, however clinical practice varies. The purpose of this study was to identify features associated with PTB fractures that increase the likelihood of ICA injury. METHODS: A retrospective cohort study was performed on all patients with PTB fractures who were investigated with computed-tomograp

Can bone wax cause cholesterol granuloma in the petrous apex? A case report.

BACKGROUND: Cholesterol granuloma (CG) is a rare entity but is the commonest lesion in the petrous apex. They are associated with chronic ear disease and previous temporal bone surgery. While bone wax has been known to cause foreign body reaction due to its non-resorbable property in the mastoid, it has not been documented to cause CG formation. CASE PRESENTATION: We described a 43 years old male who presented with a right mastoid swelling, nine years after a right retro-sigmoid craniotomy and excision for a cerebellopontine angle meningioma. He also had multiple cranial neuropathies involvin

The deformed petrous bone: a new plain film sign of premature lambdoid synostosis.

Towne-view skull radiographs of 38 infants with unilateral lambdoid synostosis were reviewed. The diagnosis of lambdoid synostosis was based on other plain film findings, CT findings, or clinical examination. All patients had deformity of the ipsilateral petrous bone, presumably because of pressure caused by redirected brain growth. Specific changes included flattening of the petrous ridge and depression of the roof of the mastoid portion of the petrous bone and adjacent structures such as the semicircular canals. Flattening of the petrous ridge and depression of the roof of the mastoid portio

Petrous bone fractures and ossicular chain lesions detected by PMCT in burnt bodies.

Thermal fractures of the skull or even the complete loss of skull structures can make it extremely difficult or even impossible to assess additional mechanical head trauma in severely burnt bodies. However, as the petrous part of the temporal bone is very dense, it often remains more intact, even when other structures of the head are destroyed. It has been suggested in the past that certain lesions of the petrous bone, particularly transverse fractures, may indicate the presence of mechanical trauma. In this retrospective study, a total of 54 cases comprising 108 petrous bones with thermal dam
