Contralateral stimulation, using TENS, of phantom limb pain: two confirmatory cases.

OBJECTIVES: This study aims to evaluate the effectiveness of trans-electric nerve stimulation (TENS) for phantom limb pain applied to contralateral limb (nonamputated limb). DESIGN: Two detailed single case studies using TENS on the contralateral limb are reported in a longitudinal study with one-year follow-up. Five variables were measured across this period. The study comprised of five sequential stages (Pre-assessment, Preliminary baseline, Start of intervention, Extended assessment, One-year follow-up). SETTING AND PATIENTS: Patients were identified at the Rookwood Hospital in Cardiff. T

[Referred pain from amputation stump trigger points into the phantom limb].

OBJECTIVES: The aim of the study was to examine whether/how myofascial stump trigger points (TPs) after lower leg amputations are able to produce stump pain (SP), phantom pain (PP), and sensations (PS) in the phantom limb. METHODS: Palpation of the 5 most striking stump TPs of 30 leg amputees (12 transfemoral, 18 transtibial) was documented in a standardized manner. Patients were asked to localize SP, PP, and PS. RESULTS: Of 150 TPs, 14 produced involuntary stump movements and 10 stump fasciculations. Dorsal PP after ventral TP palpation occurred as well as PP in the toes from TPs near the h

Phantom limb pain: relief by application of TENS to contralateral extremity.

Three adult patients with below-knee amputation of various etiologies were treated at Norristown's Sacred Heart Hospital and Rehabilitation Center in the fall of 1983. The patients ranged in age from 48 to 64 years and two were men. All three had complaints of phantom limb pain originating from various anatomic sites of the amputated extremity. In all three cases the phantom limb pain was severe and hampered prosthetic training. The patients were treated solely by application of the TENS unit to the contralateral extremity at the sites where the phantom pain originated on the amputated limb. A

Phantom limb telescoping in individuals with limb loss: links to anxiety, depression, and pain-related measures.

Phantom limb pain (PLP) and residual limb pain (RLP) have been widely studied following limb loss; however, the role of telescoping, the perceived shortening of the phantom limb, remains poorly understood in pain and psychosocial outcomes. Using a cross-sectional observational design, this study examined whether PLP and RLP intensity, pain interference, and psychosocial functioning differ between individuals who report telescoping and those who do not. Fifty-one adults with limb loss (mean age = 49.5 years, SD = 15.4) completed measures of PLP and RLP intensity (0-10 numeric rating scale), tel

A scoping review of current non-pharmacological treatment modalities for phantom limb pain in limb amputees.

PURPOSE: Phantom limb pain (PLP) is a chronic neuropathic pain condition of a missing limb following amputation. Pain management is multi-modal, including various non-pharmacological therapies. The purpose of this scoping review was to investigate the evidence surrounding current non-pharmacological treatment modalities for PLP and provide insight into their clinical feasibility. METHOD: A systematic search was conducted using four databases (Medline, Embase, PsychInfo, and CINAHL) following the PRISMA-ScR method. Results from papers meeting the inclusion criteria were charted to summarize fi

Interplay Between Innovation and Intersubjectivity: Therapists Perceptions of Phantom Motor Execution Therapy and Its Effect on Phantom Limb Pain.

PURPOSE: Interpersonal processes, including therapeutic alliance, may modulate the impact of interventions on pain experience. However, the role of interpersonal context on the effects of technology-enhanced interventions remains underexplored. This study elicited therapists' perspectives on how a novel rehabilitative process, involving Phantom Motor Execution (PME), may impact phantom limb pain. The mediating role of therapeutic alliance, and the way PME influenced its formation, was investigated. METHODS: A qualitative descriptive design, using a framework method, was used to explore therap

Post-traumatic Visualized Supernumerary Phantom Limbs: A Case Presentation.

UNLABELLED: The experience of supernumerary phantom limbs (SPLs) is a rare phenomenon known to occur following a variety of neurological ailments. This case report details visualized supernumerary phantom arms and legs in a polytrauma patient with suspicion of seizure as the primary contributing factor. Fewer than 30 cases of SPLs have been previously described. SPLs are usually confined to the phantom proprioception/sensation of the limb, with only 6 prior cases reporting visualized SPLs, all of which occurred in the setting of isolated stroke. This case presentation is notable because it is

The effect of tactile and visual sensory inputs on phantom limb awareness.

Multiple sensory stimuli contribute to the conscious awareness of the body. It is well known that limb amputation can result in abnormal body awareness, but the manner in which the CNS constructs and updates a body schema after injury is largely unknown. The purpose of the present study was to systematically evaluate the effects of sensory inputs on phantom limb awareness (PLA) shortly after unilateral upper extremity amputation. The location, quality and intensity of spontaneous and tactile-evoked phantom sensations and awareness were assessed in 13 amputees who were referred sequentially for

Beyond re-membering: phantom sensations of congenitally absent limbs.

Phantom limbs are traditionally conceptualized as the phenomenal persistence of a body part after deafferentation. Previous clinical observations of subjects with phantoms of congenitally absent limbs are not compatible with this view, but, in the absence of experimental work, the neural basis of such "aplasic phantoms" has remained enigmatic. In this paper, we report a series of behavioral, imaging, and neurophysiological experiments with a university-educated woman born without forearms and legs, who experiences vivid phantom sensations of all four limbs. Visuokinesthetic integration of tach

Alterations in Functional Connectivity Density in Patients With Phantom Limb Pain.

Phantom limb pain (PLP) is prevalent and challenging to treat in amputees, which likely stems from maladaptive cortical reorganization, but the precise mechanisms remain unclear. This study aimed to comprehensively investigate the cortical remodeling patterns in amputees experiencing PLP, pinpointing key brain regions and neural circuits directly associated with the pain. Eighteen PLP patients and 20 healthy participants were recruited for resting-state functional magnetic resonance imaging (rs-fMRI). The functional connectivity density (FCD) mapping method was used to identify PLP-related abn

A scoping review of spinal cord stimulation for phantom limb pain.

BACKGROUND: Phantom limb pain (PLP) is a debilitating condition that affects individuals following limb amputation. While medical management with anticonvulsants and physical therapy is the first-line treatment, spinal cord stimulation (SCS) has emerged as an option for patients with persistent, refractory pain. OBJECTIVES: This study aimed to evaluate the current literature on SCS for PLP, focusing on pain reduction and associated complications. METHODS: A systematic review was conducted on reports of adults (≥18 years) with phantom limb pain treated with spinal cord stimulation. Titles and

Phantom limb pain, traumatic neuroma, or nerve sheath tumor? Illustrative case.

BACKGROUND: Phantom limb pain and traumatic neuromas are not commonly seen in neurosurgical practice. These conditions can present with similar symptoms; however, management of traumatic neuroma is often surgical, whereas phantom limb pain is treated with conservative measures. OBSERVATIONS: A 77-year-old female patient with a long-standing history of an above-the-knee amputation experienced severe pain in her right posterior buttocks area for several years' duration, attributed to phantom limb pain, which radiated down the stump of her leg and was treated with a variety of conservative measu

Case Report: Virtual reality training for phantom limb pain after amputation.

Several reports have demonstrated the effectiveness of neurorehabilitation, such as mirror therapy or virtual reality, in treating phantom limb pain (PLP). This case study describes the effect of virtual reality training (VRT) on severe, long-term PLP and upper limb activity on the amputated side in a patient who underwent digit amputation 9 years prior. A woman in her 40 s underwent amputation of 2-5 fingers 9 years prior due to a workplace accident. She experienced persistent pain in the palms of her hand near the amputation sites. A single case design (ABA'B') was applied. Periods A and A'

[Role of p38MAPK signaling pathway in rats with phantom limb pain].

To investigate the role of p38MAPK signal pathway in spinal cord and dorsal root ganglion (DRG) in rats with phantom limb pain and the effects of specific inhibitors.
Methods: Healthy adult male SD rats (n=48) were cut off one side of the sciatic under anesthesia to establish a model of phantom limb pain. In addition, the healthy rats were taken as a sham group (group S, n=24). The animals were scored by observing the action of chewing (0=no chewing, 13=the worst chewing) after the operation and were sacrificed on the following day after the operation. The successful model of phantom limb pai

Motor and parietal cortex stimulation for phantom limb pain and sensations.

Limb amputation may lead to chronic painful sensations referred to the absent limb, ie phantom limb pain (PLP), which is likely subtended by maladaptive plasticity. The present study investigated whether transcranial direct current stimulation (tDCS), a noninvasive technique of brain stimulation that can modulate neuroplasticity, can reduce PLP. In 2 double-blind, sham-controlled experiments in subjects with unilateral lower or upper limb amputation, we measured the effects of a single session of tDCS (2 mA, 15 min) of the primary motor cortex (M1) and of the posterior parietal cortex (PPC) on

A Human Lower Limb Mechanical Phantom for the Testing of Knee Exoskeletons.

The development of assistive lower-limb exoskeletons can be time-consuming. Testing prototype medical devices on vulnerable populations such as children also has safety concerns. Mechanical phantoms replicating the lower-limb kinematics provide an alternative for the fast validation and iteration of exoskeletons. However, most phantoms treat the limbs as rigid bodies and fail to capture soft tissue deformation at the human/exoskeleton interface. Human soft tissue can absorb and dissipate energy when compressed, leading to a mismatch between simulated and human exoskeleton testing outcomes. We

Psychophysical Evaluation of the Capability for Phantom Limb Movement in Forearm Amputees.

A phantom limb is the sensation that an amputated limb is still attached to the body and is moving together with other body parts. Phantom limb phenomenon is often described on the basis of the patient's subjective sense, for example as represented using a visual analog scale (VAS). The aim of this study was to propose a novel quantification method for behavioral aspect of phantom limb by psychophysics. Twelve unilateral forearm amputees were asked to perform phantom wrist motion with various motion frequencies (60, 80, 100, 120, 140, 160, 180, 200, 220, 240% of preferred speed). The attainmen

The missing link: How is the phantom limb influenced by prosthesis wearing in people with lower-limb amputation?

BACKGROUND: Recent therapeutic and technological solutions aim to improve the daily living of people with limb amputation by considering various aspects of the phantom limb, in particular painless phantom sensations (PS) and voluntary phantom movements (VPM). OBJECTIVE: Although previous research has explored these phenomena mostly without considering the prosthesis, this study investigates the influence of prosthesis wearing on painless PS, painful PS, and VPM, in people with lower-limb amputation. STUDY DESIGN: Cross-sectional study based on semi-directed interviews. METHODS: Semi-directe
