Phobias, other psychiatric comorbidities and chronic migraine.

BACKGROUND: Comorbidity of chronic migraine (CM) with psychiatric disorders, mostly anxiety and mood disorders, is a well-recognized phenomenon. Phobias are one of the most common anxiety disorders in the general population. Phobias are more common in migraineurs than non-migraineurs. The clinical profile of phobias in CM has never been studied. METHOD: We investigated the psychiatric profile in 56 patients with CM using the SCID I/P interview. RESULTS: Lifetime criteria for at least one mental disorder was found in 87.5% of the sample; 75% met criteria for at least one lifetime anxiety diso

An fMRI study examining effects of acute D-cycloserine during symptom provocation in spider phobia.

BACKGROUND: Exposure-based therapy for anxiety disorders is believed to operate on the basis of fear extinction. Studies have shown acute administration of D-cycloserine (DCS) enhances fear extinction in animals and facilitates exposure therapy in humans, but the neural mechanisms are not completely understood. To date, no study has examined neural effects of acute DCS in anxiety-disordered populations. METHODS: Two hours prior to functional magnetic resonance imaging scanning, 23 spider-phobic and 23 non-phobic participants were randomized to receive DCS 100 mg or placebo. During scanning, p

Primary and secondary social phobia: psychopathologic and familial differentiations.

We compared "primary" social phobic patients (n = 25) with patients whose social phobia developing in the setting of panic disorder ("secondary" social phobics, n = 26) and panic-agoraphobic patients (n = 82). Familial, demographic, and symptomatologic data suggest a categorical distinction between the two types of social phobia. Secondary social phobics were essentially similar in all parameters to panic disorder with agoraphobia. These data support DSM-III conventions.

OCD is Not a Phobia: An Alternative Conceptualization of OCD.

While the Diagnostic and Statistical Manual Version 5 (DSM-5) has separated obsessive-compulsive disorder (OCD) from other anxiety disorders, conceptualization and treatment of OCD is still identical to the conceptualization and treatment of phobias. Many differences exist between phobias and OCD justifying a different conceptualization and treatment modality for OCD. Implications of the phobic model for OCD are discussed and its limitations are shown. Ethical and counterproductive forms of treatment for OCD derived from the phobic model are also presented. An alternative conceptualization of

The Mediation Effect of Phobic Anxiety on the Treatment Outcome of Activity and Participation across Age: Comparison between Online and Face-to-Face Rehabilitation Aftercare of an RCT.

The efficacy of internet and mobile-based interventions (IMIs) has been demonstrated with different mental health disorders, but little is known about the mediating effect of phobic anxiety on activity and participation and the differential effect of age. The current study tested a moderated mediation model with short-term change in phobic anxiety mediating between treatment (IMI vs. face-to-face, F2F) and long-term change in activity and participation, and age of patients moderating this mediation. Participants (N = 142) were recruited from psychosomatic rehabilitation clinics and randomized

["Phobic postural vertigo". A further differentiation of psychogenic vertigo conditions seems necessary].

Due to the results of an interdisciplinary study on patients with vertigo as the chief symptom and on the background of psychodynamic theories concerning anxiety disorders the term of phobic postural vertigo (Brandt & Dieterich 1986) is discussed. It becomes obvious that phobic postural vertigo is a generalizing term which encompasses different forms of psychogenic vertigo. The authors plead for a more differentiated diagnosis and subgroup oriented classification of vertigo caused by psychiatric disorders.

Major mental disorders in Addis Ababa, Ethiopia. III. Neurotic and somatoform disorders.

This report presents estimates of the lifetime and point (one-month) prevalence of phobic anxiety, dissociative, other anxiety and somatoform disorders based on a survey conducted in Addis Ababa between September and December of 1994. An Amharic version of the CIDI was used to collect data from a random community sample of 1420 individuals aged 15 and above. Phobic anxiety disorders were found to be the most common of all anxiety disorders, with lifetime prevalence of 4.8%. The lifetime prevalence for dissociative disorders was 0.8% and for other anxiety disorders it was 2.7%. The prevalence o

The changing profile of eating disorders at a tertiary psychiatric clinic in Hong Kong (1987-2007).

OBJECTIVE: To examine the clinical profile of Chinese eating disorder patients at a tertiary psychiatric clinic in Hong Kong from 1987 to 2007. METHOD: Data on 195 consecutive patients were retrieved from a standardized intake interview by an eating disorder specialist. Patients seen between 1987-1997 (n = 67) and 1998-2007 (n = 128) and fat-phobic (n = 76) and nonfat-phobic (n = 39) anorexic patients were compared. RESULTS: Patients were predominantly single (91.8%), female (99.0%), in their early-20s and suffered from anorexia (n = 115; 59.0%) or bulimia (n = 78; 40.0%) nervosa. The number

Brain dynamics of visual attention during anticipation and encoding of threat- and safe-cues in spider-phobic individuals.

This study systematically investigated the sensitivity of the phobic attention system by measuring event-related potentials (ERPs) in spider-phobic and non-phobic volunteers in a context where spider and neutral pictures were presented (phobic threat condition) and in contexts where no phobic but unpleasant and neutral or only neutral pictures were displayed (phobia-irrelevant conditions). In a between-group study, participants were assigned to phobia-irrelevant conditions either before or after the exposure to spider pictures (pre-exposure vs post-exposure participants). Comorbidity and social phobia: evidence from clinical, epidemiologic, and genetic studies.

This paper reviews evidence from clinical, epidemiologic, and family studies regarding the association between social phobia and other syndromes. Social phobia is strongly associated with other anxiety disorders, substance abuse, and affective disorders in both clinical and community samples. An average of 80% of social phobics identified in community samples meet diagnostic criteria for another lifetime condition. Social phobia is most strongly associated with other subtypes of anxiety disorders, with an average of 50% of social phobics in the community reporting a concomitant anxiety disorde

[Differentiated types of chronic agoraphobia].

The study included 60 patients aged 18-65 years with anxious-phobic disorder (APD)--agoraphobia with and without panic disorders. Diagnosis of the disease was performed according to ICD-10. It was established that dynamics and outcome of APD with stable agoraphobia depend on some comorbid psychic disorders (neurotic, nonpsychotic affective, personality disorders, slow-progredient schizophrenia). Influence of such disorders on APD depends on level of realization of the comorbid correlations: symptomatic (nondelirious hypochondria) and intersyndromal ones (affective and personal disorders as wel

Fear induced complexity loss in the electrocardiogram of flight phobics: a multiscale entropy analysis.

In this study we explored the changes in the variability and complexity of the electrocardiogram (ECG) of flight phobics (N=61) and a matched non-phobic control group (N=58) when they performed a paced breathing task and were exposed to flight related stimuli. Lower complexity/entropy values were expected in phobics as compared to controls. The phobic system complexity as well as the heart rate variability (HRV) were expected to be reduced by the exposure to fearful stimuli. The multiscale entropy (MSE) analysis revealed lower entropy values in phobics during paced breathing and exposure, and

[New proposals for the development of the concepts of neurotic anxiety, major depression and panic attack].

An accumulation of research results, laboratory and clinic, leads to revise the classification of disorders of anxious nature (anxiety disorders according to the DSM-III). An empirical confirmation allows to separate anxious states from panic attack and phobic behavior constituting a component separated from generalized anxiety, exaggerated emotional reactions to stress agents and obsessive-compulsive disorders. The response to pharmacotherapeutic agents and the important number of anxiety depression and mixed depression tends to make permeable the separating line between the nosological entit

Comorbid psychiatric disorders in subjects with panic attacks.

Several psychiatric disorders are associated with panic disorder (PD), although the nature of their relationships is unknown. The purpose of this study was to a) document comorbid associations with both PD and infrequent panic (IP), and b) investigate the nature of the relationships among these disorders. This community-based study included 97 adults who met DSM-III-R criteria for panic attacks compared with 97 matched controls. Psychiatric comorbidity was assessed using the SCID and SCL-90. Subjects with either PD or IP had higher rates of psychiatric comorbidity than controls. PD differed fr

Do recognizable lifetime eating disorder phenotypes naturally occur in a culturally asian population? A combined latent profile and taxometric approach.

BACKGROUND: We examined whether empirically derived eating disorder (ED) categories in Hong Kong Chinese patients (N = 454) would be consistent with recognizable lifetime ED phenotypes derived from latent structure models of European and American samples. METHOD: We performed latent profile analysis (LPA) using indicator variables from data collected during routine assessment, and then applied taxometric analysis to determine whether latent classes were qualitatively versus quantitatively distinct. RESULTS: Latent profile analysis identified four classes: (i) binge/purge (47%); (ii) non-fat-

Individual and familial predictors of impairment in childhood anxiety disorders.

OBJECTIVE: To determine which correlates of childhood anxiety disorders are predictive of impaired functioning as judged by clinicians. METHOD: Seventy-four families of children with anxiety disorders attending a hospital outpatient clinic completed questionnaires measuring child symptoms of anxiety and depression, maternal psychopathology, maternal ratings of child psychopathology, and developmental and environmental difficulties. Clinicians completed the Global Assessment of Functioning (GAF) for each child, and correlations between it and the other measures were ascertained. RESULTS: Mate

[Diagnosis and assessment of anxiety disorders].

Anxiety disorders tend to become chronic if untreated and often have considerable social consequences. Therefore, early recognition of anxiety disorders at the primary health care level is of great importance. This article gives a short overview of important aspects of the diagnosis of anxiety disorders in general medical practice. Panic disorder, generalised anxiety disorder and phobic disorders are describer and phobic disorders are described based on ICD-10 criteria. Anxiety disorders have important differential diagnostic distinctions from somatic disorders and other psychiatric disorders.

Stability of childhood anxiety disorder diagnoses: a follow-up naturalistic study in psychiatric care.

Few studies have examined the stability of major psychiatric disorders in pediatric psychiatric clinical populations. The objective of this study was to examine the long-term stability of anxiety diagnoses starting with pre-school age children through adolescence evaluated at multiple time points. Prospective cohort study was conducted of all children and adolescents receiving psychiatric care at all pediatric psychiatric clinics belonging to two catchment areas in Madrid, Spain, between 1 January, 1992 and 30 April, 2006. Patients were selected from among 24,163 children and adolescents who r
