Use of surface electromyography in phonation studies: an integrative review.

INTRODUCTION: Surface electromyography has been used to assess the extrinsic laryngeal muscles during chewing and swallowing, but there have been few studies assessing these muscles during phonation. OBJECTIVE: To investigate the current state of knowledge regarding the use of surface electromyography for evaluation of the electrical activity of the extrinsic muscles of the larynx during phonation by means of an integrative review. METHOD: We searched for articles and other papers in the PubMed, Medline/Bireme, and Scielo databases that were published between 1980 and 2012, by using the foll

[Cephalometric study of the velocardiofacial syndrome: Impact of dysmorphosis on phonation].

INTRODUCTION: Velopharyngeal insufficiency is a frequent sign of the velocardiofacial syndrome (VCFS) but its origins are not well-documented. Our aim was to establish a correlation between this functional disorder and regional morphological anomalies. PATIENTS AND METHODS: Twenty-seven of 36 patients presenting with VCFS could be included retrospectively. We measured cavum depth and velum length on lateral orthodontic X-rays, and assessed the relationship between these two measures. We compared these measures to those of reference populations. Postoperative phonation was assessed with an aer

A computational study of the effect of vocal-fold asymmetry on phonation.

Unilateral laryngeal paralysis leads to tension imbalance and hence to asynchronous movements between the two vocal folds during phonation. In the current study, a computational model of phonation that couples a two-mass model of the vocal folds with a Navier-Stokes model of the glottal airflow, has been used to examine the dynamics of vocal fold configurations with tension imbalance and its implications for phonation. The simulations show that tension imbalance influences phonation onset, intensity as well as the fundamental phonation frequency. Distinct non-linear effects such as period-doub

Shortening of the front vibrating part of the vocal folds in phonation.

We have tried to make use of the opportunity of repeated observations through the analysis of the recorded movements of the phonating vocal folds by means of the phonating vocal folds through indirect videolaryngostroboscopic technique. The results of our investigation correspond, among others, with Hála and Sovák's diagrammatic description of continual and gradual changes in the vibration of the vocal folds in an uninterrupted sequence of tones of the human voice. In the initial chapter mention has been made of Hála and Honty's works and of Pressmann's work dealing with the described behaviou

Soft phonation in the male singing voice: a preliminary study.

Sustained high notes, diminishing gradually from the loudest to the softest phonation within a maneuver called messa di voce, are examined in two contrasting professional tenor voices. Signals of the sound pressure level, electroglottograph, and mean esophageal pressure are recorded, and similar maneuvers by the same subjects are examined stroboscopically. The lyric voice is found to make a gradual diminuendo while maintaining nearly constant posture of the vocal tract together with a phase of complete closure in the glottal cycle. The robust voice, by contrast, passes abruptly from a producti

Modifying electroglottograph-identified intervals of phonation: the effect on stuttering.

Single-subject experiments were conducted with an adolescent and an adult male who stutter to assess the effect on stuttering of changing the frequency of phonation intervals that were within prescribed duration ranges during spontaneous speech. Electroglottograph-identified intervals of phonation were measured using a computer-assisted biofeedback system. Both subjects demonstrated that their stuttering could be controlled by modifying the frequency of phonation intervals within short duration ranges. The experimental effects not only replicated earlier findings but were demonstrated to be in

Tracheotomy revision in infants: to facilitate extubation and restore phonation.

The formation of tracheal 'granuloma' in the tracheostomized infant, as a cause of aphonia and an obstacle to decannulation has long been recognized. Endobronchial or transtomal removal has been recommended; however, a number of these lesions were resected in conjunction with complete tracheostomy revision and found to be very much larger than suspected endoscopically and consisting of dense mature scar tissue. It is, therefore, suggested that resection of these lesions endoscopically or transtomally may be quite inadequate and an open procedure preferred.

Vocal Tract Configurations of Professional Operatic Singers During Sustained Phonation.

OBJECTIVES: In voice production, interactions occur between the oscillating vocal folds, the respiratory system, and the vocal tract. However, it is not yet sufficiently understood how the respiratory system could affect the vocal tract configuration. It is hypothesized that a reduction in tracheal pull, caused by decreasing lung volume, along with shifts in dominant exhalation forces (from inspiratory to expiratory muscles), leads to a larynx elevation with shortening of the vocal tract tube, and consecutively, articulatory adjustments to preserve consistent sound quality. METHODS: Seventeen

Phonation Threshold Pressure Revisited: Effects of Intrinsic Laryngeal Muscle Activation.

OBJECTIVES/HYPOTHESIS: Phonation threshold pressure (Pth ) is the minimum subglottic pressure required to reach phonation onset and is considered a marker for vocal efficiency and health. We investigated the effects of intrinsic laryngeal muscle (ILM) activation on Pth . STUDY DESIGN: In vivo animal study. METHODS: In an in vivo canine phonation model, laryngeal adductor muscles were activated together by stimulation of the recurrent laryngeal nerves (RLNs) and individually via stimulation of respective terminal nerve branches. Cricothyroid (CT) muscles were activated via stimulation of the

Decoding phonation with artificial intelligence (DeP AI): Proof of concept.

OBJECTIVE: Acoustic analysis of voice has the potential to expedite detection and diagnosis of voice disorders. Applying an image-based, neural-network approach to analyzing the acoustic signal may be an effective means for detecting and differentially diagnosing voice disorders. The purpose of this study is to provide a proof-of-concept that embedded data within human phonation can be accurately and efficiently decoded with deep learning neural network analysis to differentiate between normal and disordered voices. METHODS: Acoustic recordings from 10 vocally-healthy speakers, as well as 70

Vocal instabilities in a three-dimensional body-cover phonation model.

The goal of this study is to identify vocal fold conditions that produce irregular vocal fold vibration and the underlying physical mechanisms. Using a three-dimensional computational model of phonation, parametric simulations are performed with co-variations in vocal fold geometry, stiffness, and vocal tract shape. For each simulation, the cycle-to-cycle variations in the amplitude and period of the glottal area function are calculated, based on which the voice is classified into three types corresponding to regular, quasi-steady or subharmonic, and chaotic phonation. The results show that vo

A computational study of asymmetric glottal jet deflection during phonation.

Two-dimensional numerical simulations are used to explore the mechanism for asymmetric deflection of the glottal jet during phonation. The model employs the full Navier-Stokes equations for the flow but a simple laryngeal geometry and vocal-fold motion. The study focuses on the effect of Reynolds number and glottal opening angle with a particular emphasis on examining the importance of the so-called "Coanda effect" in jet deflection. The study indicates that the glottal opening angle has no substantial effect on glottal jet deflection. Deflection in the glottal jet is always preceded by large-

Frequency and amplitude perturbation analysis of electroglottograph during sustained phonation.

Electroglottography (EGG) was used to monitor vocal fold vibration patterns in normal subjects and patients with various laryngeal disorders. In order to evaluate the regularity of vocal fold vibration, frequency and amplitude perturbation of EGG waves during sustained phonation were measured with a laboratory computer. The data were compared to the degree of hoarseness evaluated by auditory perception and by sound spectrographic analysis. Frequency and amplitude perturbation measures showed some overlap between normal and pathological groups. However, there was a close relation between pertur

Dynamic Study of Oesophageal Function during Phonation: Simple but Effective.

Secondary aphonia significantly affects the quality of life of the laryngectomy patient despite the excellent success of the operation. Vocal rehabilitation often does not achieve the desired results, given the innumerable prognostic variables. Tracheo-oesophageal speech is considered the standard gold method of voice restoration, but a possible spasm of pharyngo-oesophageal segment is the prominent cause of rehabilitation failure. However, the phenomenon is difficult to identify and underestimated, and diagnostic methods are inquisitive or expensive. To propose a phonatory and swallowing eval

Drilling speaking valves to promote phonation in tracheostomy-dependent children.

OBJECTIVES/HYPOTHESIS: Placement of a Passy-Muir speaking valve is considered best practice for infants and children with a tracheostomy. The Passy-Muir valve enables phonation by redirecting exhaled air via the glottis. Poor tolerance of the Passy-Muir valve is associated with excessive transtracheal pressures on exhalation due to upper airway obstruction. Drilling a small hole in the side of the Passy-Muir valve creates a pressure relief port to allow partial exhalation through the tracheostomy tube while enabling phonation. STUDY DESIGN: A retrospective case series is presented of 10 aphon

Treatment of functional ventricular fold phonation by temporary suture lateralization.

Ventricular fold phonation (VFP) is a phenomenon characterized by phonation using the false vocal folds. Besides a substitution voice due to loss of the true vocal folds-so called compensatory type-the noncompensatory types have a psychogenic, functional, or habitual background. Therapeutic options for these cases so far comprise voice therapy, pharmacological therapy (injection of anesthetics or botulinum toxin), and, in case of failure, surgical resection of the ventricular folds. Indication for aggressive surgical interventions is usually difficult, as there is always a risk of producing an
