What initiates a migraine attack? Conclusions from four longitudinal studies of quantitative EEG and steady-state visual-evoked potentials in migraineurs.

OBJECTIVES: Quantitative electroencephalograpic (QEEG) frequency spectra and steady-state visual-evoked potentials (SSVEP) are indicators of corticothalamic excitability (e.g., arousal). Increased interictal excitability is suggested to be an important element in the migraine pathophysiology. In this paper, we summarize our results from four studies of QEEG and SSVEP recordings in migraineurs interictally and in the days before an attack with the intention to shed light on attack-initiating mechanisms. MATERIAL AND METHODS: Thirty-two healthy controls, 33 migraineurs without and eight with au

[The effects of ordinary provocation on the power density spectrum. A clinical electroencephalographic study (author's transl)].

In the search for methods which might increase the discrimination of the EEG, the power density spectra of a random sample of patients during different conditions were analysed. Power density spectra were made during eyes closed, eyes open, after hyperventilation, during photic stimulation, and during auditory stimulation. It proved that the spectra may change under different provocation tests, sometimes in a rather peculiar way. The conclusion was, that these analyses improve explicit the EEG diagnosis, a problem however is the time required for spectra interpretation.

Functional clustering in EEG photic and auditory driving in schizophrenia.

The purpose of this study is to investigate the effects of photic and auditory stimuli on brain functions in schizophrenics by investigating the functional cluster (FC) of EEGs. We recorded EEGs using 16 electrodes on 10 schizophrenic patients and on 10 normal controls during photic and auditory stimuli. We estimated FC would characterize the strongly interactive brain regions among many brain regions. FC refers to the brain regions that interact much more strongly among themselves than with the rest of the brain. Brain regions that belong to the same cluster are therefore all functionally inv

Abnormal photic driving responses in never-medicated schizophrenia patients.

The present study was conducted to compare electroencephalogram (EEG) driving responses to 10 hertz photic stimulation in 14 drug-naive schizophrenia patients and 16 sex- and age-matched control subjects. The amplitude of photic driving responses (PDRs) recorded from the occipital region was significantly lower in schizophrenia patients than in controls. In eight schizophrenia patients (vs. none in the control group) the PDR amplitudes recorded at the frontal region were higher than those at the occipital region, and frontal to occipital PDR ratios were significantly larger in schizophrenia su

[Reflex epilepsy with seizures induced by mental calculation, playing chess and scrabble].

Reflex seizures induced by higher mental activity is rare. We report the case of a young man with myoclonic jerks and generalized tonico-clonic convulsion precipitated by calculation, playing chess and scrabble. Routine EEG, including hyperventilation and photic stimulation, showed no abnormality. Tests stimulation procedures, including spatial tasks, induced focal and generalized EEG spike-wave complex and myoclonic jerks. Valproate was effective in reducing epileptic seizures during a follow-up period of three years. Comparison of our case with previously reported reflex epilepsy with seizur

Transcranial doppler sonography diagnostic value for the cerebral flow velocity changes in the interictal phase of classic migraine.

BACKGROUND: An imbalance of the cerebrovascular response during functional activation of the brain has been postulated as a factor in the pathophysiology of migraine. The purpose of this study was to determine the transcranial doppler sonography (TDS) diagnostic value for the cerebral flow velocity changes in the interictal phase of classic migraine. METHODS: This study was carried out on 46 patients (23 cases and 23 controls). We used Doppler instrument via trans-temporal window and detected middle cerebral artery, anterior cerebral artery and posterior cerebral artery by 2 MHz probe. The fl

The influence of acute and chronic hypoxia on the electroencephalogram of human body.

This report deals with the study of electroencephalograms of 36 subjects who inhaled hypoxic mixture at the sea level (PO2 is as that at 7000 m above sea level) or in a hypobaric chamber with rapid decompression (as that at 5000-7500 m above sea level) and of 46 subjects who reached high altitude area (5000-8600 m) under chronic hypoxia. The characters of the EEGs at rest during photic stimulation, hyperventilation and convalescence after hypoxia were compared in two experiments. The result of experiments shows that in the same individual, the severer the hypoxia, the more obvious the changes

Pattern-induced negative occipital potentials (PINOP).

: We describe 24 subjects (16 female, 8 male; age range, 3 to 36 years [mean, 15.7]) who exhibited a hitherto inadequately described EEG response to visual scanning of geometric patterns. Like lambda waves (the normal physiologic response to visual scanning of pictures), the responses to pattern scanning were occipitally distributed and disappeared on eye closure. In contrast to lambda waves, they were surface negative, briefer in duration, and sharper in contour. To designate them, we have coined the term pattern-induced negative occipital potentials (PINOP). The prevalence of epilepsy was no

A new actimetry-based test of photic sensitization in a murine photosensitive magnesium depletion model.

Photic hypersensitivity may induce various signs of nervous hypersensitivity, including diurnal cephalalgias, anxiety, dyssomnia, seizures, fatigue and/or myalgias. The patients usually present both dishabituation and generalization in response to repetitive light stimuli instead of habituation as in normal subjects. These clinical manifestations appear when light intensity is maximum (daytime; spring and summer) in magnesium-depleted patients with hypofunction of the biological clock. The best photic hypersensitivity management involves darkness therapy, either darkness per se or darkness-mim

Epileptiform abnormalities in children with attention-deficit-hyperactivity disorder.

The proportion of children with attention-deficit-hyperactivity disorder and epileptiform abnormalities is compared with an historic control group of normal school-aged children. The medical records of 655 children 5-16 years of age referred to a single pediatric neurologist (M.S.) from January 1991 to December 1999 with school problems, behavior problems, or hyperactivity were retrospectively reviewed. Clinical criteria for attention-deficit-hyperactivity disorder were satisfied in 476 of these children. An electroencephalogram was obtained from 347 patients and coded as epileptiform in 6.1 +

Multimodal tests of cerebrovascular reactivity in migraine: a transcranial Doppler study.

Altered cerebral vasoreactivity (CVR) has been implicated in migraine. To test this hypothesis, we studied CVR as measured by transcranial Doppler ultrasound (TCD) in 11 migraineurs and 12 healthy controls of similar age. Mean flow velocities (MFV) in the middle cerebral artery (MCA) were recorded during a cognitive and two motor tasks. MFV in the posterior cerebral artery (PCA) were measured during photic stimulation and observation of complex images. The increase of MFV in the MCA during the cognitive task was greater in migraineurs than in controls (9.1% vs 5.0%; P = 0.06). The increase of

Pinealectomy reduces optic nerve but not intergeniculate leaflet input to the suprachiasmatic nucleus at night.

The suprachiasmatic nucleus (SCN) of the hypothalamus regulates circadian rhythms in mammals. It receives, among others, direct inputs from the retina and from the thalamic intergeniculate leaflet (IGL). The former sends photic signals to the SCN, whereas the latter probably integrates photic and nonphotic information. To characterise these inputs in vivo, extracellular single-unit recordings were made from the SCN of rats under urethane anaesthesia during electrical stimulation of the optic nerve (OptN) or the IGL region. Cell responses were evaluated by creating peri-stimulus time histograms

Activation procedures in the electroencephalograms of healthy and epileptic cats under propofol anaesthesia.

The current study evaluated the diagnostic value of electroencephalographic recordings (EEG) in cats with epilepsy under special consideration of photic stimulation and hyperventilation. EEGs in six healthy cats were recorded under light (mean dose of 0.23 mg/kg/min) and deep (mean dose of 0.7 mg/kg/min) propofol anaesthesia, whereas EEGs in 13 diseased cats were recorded under a propofol anaesthesia which was kept as light as possible (mean dose of 0.39 mg/kg/min). Paroxysmal discharges were detected in six of 13 cats suffering from seizures (two cats with idiopathic epilepsy and four cats wi

Rhythmic auditory stimulation in generalized epilepsy.

Activation of spike-wave discharges by photic stimulations (PS) and rhythmic auditory stimulations (AS) was investigated in 40 patients with generalized epilepsy and 25 controls. PS produced spike-wave discharges in 23 (55%) and AS in 12 (33%) cases. In 4 patients AS activated spike-wave discharges but PS failed to do so. AS may be occasionally used in generalized epilepsy as an activating method when standard procedures such as hyperventilation and PS fail.

Autosomal dominant cortical tremor, myoclonus, and epilepsy (ADCME): Probable first family from India.

Autosomal dominant cortical tremor, myoclonus, and epilepsy (ADCME) is an extremely rare syndrome characterized by familial occurrence of postural and action-induced tremors of the hands but showing electrophysiologic findings of cortical reflex myoclonus. Patients also have cognitive decline and tonic-clonic seizures, often precipitated by sleep deprivation or photic stimulation. We describe probably the first family from India of this ill-defined syndrome.

MDMA use is associated with increased spatial BOLD fMRI visual cortex activation in human MDMA users.

Previous animal studies have demonstrated that 3,4-methylenedioxymethamphetamine (MDMA) exposure causes serotonin axotomy that is greatest in occipital cortex (including primary visual cortex) where serotonergic axons innervate neurons and blood vessels. Human MDMA users have altered serotonergic function and reduced gray matter density in occipital cortex. The fMRI BOLD method is potentially sensitive to both the neuronal and vascular consequences of MDMA-induced serotonin toxicity. To test the hypothesis that MDMA users have altered visual system function, we used the fMRI BOLD technique to

Effects of prolonged exposure to darkness on circadian photic responsiveness in the mouse.

Circadian rhythms in mammals are generated by an endogenous pacemaker but are modulated by environmental cycles, principally the alternation of light and darkness. Although much is known about nonparametric effects of light on the circadian system, little is known about other effects of photic stimulation. In the present study, which consists of a series of five experiments in mice, various manipulations of photic stimulation were used to dissect the mechanisms responsible for a variation in the magnitude of light-induced phase-shifts that results from prolonged exposure to darkness. The resul

[The resonance EEG reactions in rhythmical photostimulation and the changes in functional status].

Changes in EEG fine structure in the occipital areas, heart rate, and breath pattern were recorded in 10 male volunteers under photic stimulation with slowly varying frequency in the range of lower or higher EEG alpha-subbands. Self-ratings (on the scale SAN-self-assessment of state of health, activity, and mood) of the functional state, general activity, and mood were obtained before and after stimulation. EEG effects of such photostimulation were represented by the resonance EEG responses of discrete spectral EEG components, frequencies of which periodically coincided with varying rate of fl

The EEG activity of two patients with occlusion of the vertebral and basilar arteries was analyzed. Power maps, peak power frequency, and field power differences during both photic drive and verbal command were evaluated. Clinical findings of one patient fit the criteria of locked-in syndrome. Photic stimulation and event related desynchronization paradigm was used for testing the reactivity of EEG. Averaged EEG epochs during intended movement after verbal command showed significant alpha and sub-alpha power reduction. Regional differences of EEG reactivity were assumed secondary to the underl
