Early diagnosis and prompt initiation of antibiotic therapy for group A beta-hemolytic streptococcal pharyngitis can shorten the duration of the clinical illness and reduce the period of infectivity. This in turn may expedite a patient's return to school or day care, as well as a parent's return to work. Antigen detection tests are available that can provide a rapid diagnosis of group A beta-hemolytic streptococcal pharyngitis and can facilitate the prompt initiation of antibiotic therapy. OBJECTIVE: To develop a typology of after-hours care (AHC) instructions and to examine physician and practice characteristics associated with each type of instruction.
DESIGN: Cross-sectional telephone survey. Physicians' offices were called during evenings and weekends to listen to their messages regarding AHC. All messages were categorized. Thematic analysis of a subset of messages was conducted to develop a typology of AHC instructions. BACKGROUND: Front desk staff are often responsible for triaging patients, despite lacking formal medical training. Incorrect triage recommendations could result in patients delaying care for emergent conditions. No research exists to characterize triage accuracy in the gynecologic outpatient setting.

The efficacy of the MicroTrak (Syva Co., Palo Alto, Calif.) direct immunofluorescence test for the detection of Chlamydia trachomatis was compared with cell culturing of fresh specimens obtained from patients attending a clinic on sexually transmitted disease and of frozen specimens delayed in transit from urban or remote physicians' offices and clinics. Direct immunofluorescence testing detected C. trachomatis more frequently than culturing of the same specimens when transit caused a delay in culturing.
