Case report and ultrastructural study of intracranial embryonal carcinoma.

A case of a primary intracranial embryonal carcinoma, the first with ultrastructural study, is reported. The tumor was associated with precocious puberty in a 6 1/2-year-old female. Characteristic embryoid bodies were present. At the ultrastructural level three cell types were noted: undifferentiated, differentiated, and intermediate types. The undifferentiated showed scanty cytoplasmic organelles and numerous free polysomes, while the differentiated cells contained well-developed mitochondria, Golgi apparatus, rough endoplasmic reticulum, and some contained secretory granules. The intermediat

[ADH-deficient diabetes insipidus in children. A study of 93 cases (author's transl)].

Ninety three cases of diabetes insipidus are reviewed after 1 to 20 years follow-up. A codified water deprivation test realized in 50 cases revealed 31 severe cases and 19 partial forms. Apart from surgical causes of diabetes insipidus (16%), tumors represent the most frequent etiology (33.5%), essentially craniopharyngiomas and pinealomas. Histiocytosis X (16%) is also an important cause. In one third no cause could be found. Decreased secretion of anterior pituitary hormones is very common in association with surgical and tumoral causes and can also be found in idiopathic forms. Thus, in our

Plasma TSH and prolactin response to insulin induced hypoglycemia in patients with hypothalamic pituitary disorders.

Plasma TSH and prolactin (PRL) responses to insulin-induced hypoglycemia (ITT) were examined in 37 patients with various hypothalamic pituitary disorders. Four patients showed a definite TSH response to ITT. They were 2 patients with suspected pinealoma with diabetes insipidus (DI), a patient with pituitary adenoma and a patient with acute prophyria. A suspected hypothalamic dysfunction and a slightly impaired pituitary function manifested as GH deficiency were their common endocrinological features. On the contrary, plasma PRL response to ITT was noted in about one third of the patients exami

[Intra- and postoperative dysregulations of blood pressure, heart rate, body temperature, respiratory frequency and blood glucose in operations of tumors in ventricles I-III].

218 patients who were operated on because of processes in the region of the ventricles I to III, among them 101 tumours of the third ventricle (29 pinealomas) were examined with respect to intra- and postoperative dysregulations of the vital parameters blood pressure, heart rate, respiration, temperature and blood sugar. The evaluation included anaesthetic and intensive therapy records. Among the intraoperative phenomena, derangements of the blood pressure and the heart rate were dominating, bradycardiac and tachycardiac reactions showed the same frequency. In the postoperative stage, regulati

A case of hypopituitarism with diabetes insipidus and loss of thirst. Role of antidiuretic hormone and angiotensin II in the control of urine flow and osmolality.

A 20-yr-old male was found to have diabetes insipidus is association with panhypopituitarism but without any focal neurological lesion being identified. He was initially treated with steroid supplements, the features of diabetes insipidus being controlled with a thiazide diuretic. Eighteen months later the patient lost thirst sensation and stopped treatment, subsequently being re-admitted with severe dehydration, oliguria and focal neurological signs. Further investigation, including brain biopsy, confirmed the presence of an atypical pinealoma which was considered inoperable. Measurements of

[Granulomatous myositis during collagenosis (one case) and suprasellar dysgerminoma (one case) (author's transl)].

Two cases of mainly interstitial myositis with epitheloid and giant cells are reported. In the first case, the late amyotrophic-type paralysis of the two girdles, of the clinical myositis type, was associated with severe myocardial damage, the atrioventricular conduction disorder requiring a pace-maker. This observation would appear to be a true polymyositis of the collagen diseases, in view of the associated skin affection, death occurring within a year, a large increase in serum muscle enzymes and urine creatine, the existence of a rheumatoid factor and striated muscle antibodies, and a larg

Widespread functional deficits in perception-related networks demonstrated by PET in a case with simple visual seizures.

PURPOSE: To study benzodiazepine receptor (BZR) density and functional deficits in occipital lobe epilepsy. METHODS: A 39-year-old man who had simple partial visual seizures after neurosurgical transtentorial extirpation of a pinealoma was studied by EEG, magnetic resonance imaging (MRI), and positron emission tomography (PET) of [18F]2-fluoro-2-deoxy-D-glucose (FDG) at rest and during visual activation task and[11C]flumazenil (FMZ). RESULTS: Electroencephalographic recordings were nonspecific, and MRI did not reveal any morphologic anomaly in the occipital lobe. Flumazenil-PET demonstrated

[Radiotherapy of cerebral tumor in the adult with the exception of hemispheric glioma].

The results obtained in this series of 120 patients with brain tumors, excepting hemispheric gliomas, appear particularly comforting. Radiosensitiveness of medulloblastomas, pinealomas and to some extent of ependymomas, was of course known. On the contrary, the radio-sensitiveness of some astrocytary processes, or supposed astrocytomas, well or moderately differentiated (gliomas of the IIIrd. ventricle, opto-chiasmatic gliomas, gliomas of the brain stem) appears to have been appreciable and it has been possible to obtain numerous stabilizations equivalent to cures. Dorsal midbrain syndrome with loss of motor fusion: a rare association.

INTRODUCTION: Dorsal midbrain syndrome (DMS) is characterized by upgaze paralysis, convergence-retraction nystagmus, and eyelid retraction. Impaired downgaze and pupillary light-near dissociation may co-exist, while less common features including exotropia and convergence paralysis have also been described. However, loss of motor fusion is not a well recognised finding in DMS. PURPOSE: To describe DMS associated with loss of motor fusion. METHODS: Retrospective case series. RESULTS: Five patients (age 21-80 years) presented with features of DMS due to severe head injury (n=1), brainstem str

[Tumors of the 3d ventricle, clinical aspects and treatment].

Our last 30 patients, who were operated upon 3rd-ventricle-tumors, are discussed on symptomatology, diagnosis and therapy. Depending on the localization we classify oral, basal and caudal tumors. The peak of these tumors is found in the first ten years of life. They are distributed as followed: spongioblastomas, ependymomas, pinealoma and other rare tumors. Mainsymptom is the sudden diffuse headache depending on this skull's position. The three groups show different symptomatic features, the oral one sepecially that headache described above. The symptoms of the caudal group are due to signs of

Pineal region tumors: endocrinological aspects.

The pineal gland and the superior cervical ganglia represent an integration center for hormonal and neural signals, and play an important role in modulating basic hormonal secretions. The prevalence of pineal tumors in the male sex and their frequent association with precocious puberty has been known for a long time. However, the rarity of ascertained cases investigated by means of direct and sensitive methods (RIA; tests applying hypothalamic releasing factors) is still preventing the proper knowledge of the endocrinological aspects of pinealomas. On the basis of the recent literature, the im

Delayed cerebral radiation necrosis.

The clinical features and long-term outcome of seven patients with delayed cerebral radiation necrosis (DCRN) are described. Radiotherapy had been given for pituitary tumour (1), astrocytoma (2), pinealoma (2), craniopharyngioma (1) and parotid carcinoma (1). The mean latency to onset of the first neurological symptoms was 22 months (range 6-40 months), and mean duration of follow-up was 86 months (range 60-126). Three patients died at a mean of 84 months after radiotherapy (range 62-98). A fourth patient probably died from metastatic disease. Three patients remain alive, albeit severely disab

Occurrence of Pineal Gland Tumors in Combined Chronic Toxicity/Carcinogenicity Studies in Wistar Rats.

Pineal gland tumors are very rare brain lesions in rats as well as in other species including humans. A total of 8 (out of 1,360 examined) Wistar rats from 3 different combined chronic toxicity/carcinogenicity or mere carcinogenicity studies revealed pineal gland tumors. The tumors were regarded to be spontaneous and unrelated to treatment. The morphology and immunohistochemical evaluation led to the diagnosis malignant pinealoma. The main characteristics that were variably developed within the tumors were the following: cellular atypia, high mitotic index, giant cells, necrosis, Homer Wright

Primary chiasmal germinoma. A case report and review of the literature.

A case is presented in which an adult man with painless progressive loss of vision subsequently was found to have a primary suprasellar/perichiasmal germinoma (ectopic pinealoma). A review of the literature revealed 93 similar cases of germinoma occurring in the perichiasmal region and these are tabulated. The diagnosis and management of this lesion are discussed, including the recognition of the characteristic neuroendocrinologic triad of diabetes insipidus, visual changes, and hypopituitarism. Of 64 cases from the literature wherein presenting symptoms were reported, 56 (87.5%) had diabetes

[Therapy of orbital and retroorbital space occupying processes with optic nerve compression].

Space occupying lesions involving the optic nerve require individual management. Graves' disease is first treated with cortisone: failures of treatment require radiation and operative decompression. Good results have been achieved with cytostatic drugs and, in acute progressive loss of vision, with plasmapheresis. Idiopathic pseudo-tumor orbitae disappears with cortisone, failures of treatment indicate malignant development of a lymphoma, which requires management with cytostatic drugs or radiation. Granulomatous infiltration of the optic nerve in sarcoidosis should be treated first of all wit

Diagnosis and surgical treatment of childhood brain tumors.

As the most frequent solid tumor occurring in childhood, brain tumors constitute an important segment of pediatric oncology. Neurologic manifestations may be deceptively mild and easily overlooked or misinterpreted, particularly in the very young, because of the remarkable resiliency of the immature central nervous system and the skull's ability to expand throughout the pre-adolescent years. The majority of childhood tumors produce increased intracranial pressure, usually the consequence of obstructive hydrocephalus. Specific neurologic deficits correspond to the tumor's location. The posterio

Paroxysmal tonic upgaze of childhood--a review.

Ouvrier and Billson (1988) were apparently the first to describe this entity. In the four original cases, the clinical features were as follows: (1) onset usually under 1 year of age, (2) episodes of variably sustained conjugate upward deviation of the eyes, with neck flexion (chin down) apparently compensating for the abnormal eye position, (3) downbeating saccades in attempted downgaze, (4) normal horizontal eye movements, (5) diurnal fluctuation of symptoms, (6) frequent relief by sleep, (7) exacerbation with febrile illnesses, (8) varying degrees of ataxia, (9) neurological examination usu

An unusual case of headache.

Pinealgland tumors comprise 0.5%-1% of adult brain tumors. Pineoblastomas constitute less than half of these pineal gland tumors. Due to the key anatomic location, these tumors produce hydrocephalus by pressing on the aqueduct of Sylvius. Headache is a very common symptom. We describe the case of a 20-year-old female who presented with new onset persistent headache. Workup revealed a pineoblastoma. These tumors can be detected by MRI and diagnosed on cytology of cerebrospinal fluid (CSF) showing blue round tumor cells with specific tumormarkers. Combined chemo radiation therapy is the treatmen

Paroxysmal tonic upgaze: physiopathological considerations in three additional cases.

Paroxysmal tonic upgaze of childhood has been described as a benign distinctive syndrome of abnormal ocular movement, with or without concomitant ataxia. After the first observation of four children, a further 29 patients have been reported with a wide spectrum of neurologic abnormalities such as ataxia, unsteady of gait, learning disabilities and mental retardation at follow-up. Electroencephalograms were normal in all the subjects and magnetic resonance imaging showed deficient myelination in only one patient. Recently it has been suggested that paroxysmal tonic upgaze could be a heterogeneo
