[Accompanied needling at Huantiao (GB30) as an adjunctive therapy for piriformis syndrome: a randomized controlled trial].

OBJECTIVE: To observe the clinical efficacy of accompanied needling at Huantiao (GB30) as an adjunctive therapy for piriformis syndrome (PS). METHODS: A total of 76 PS patients were randomly divided into an observation group(38 cases, 1 case dropped out, 1 case was eliminated) and a control group (38 cases, 2 cases dropped out). Both groups received routine acupuncture combined with shock wave along meridians. In the observation group, accompanied needling was adopted, Huantiao (GB30) on the affected side and 2 cun medial to Huantiao (GB30) were selected. Both groups received treatment once d

Unique formation of sciatic nerve below the piriformis muscle - a case report.

Dorsal and ventral divisions of ventral rami of lower lumbar and sacral spinal nerve were found to pass ventral and dorsal to the piriformis muscle respectively. These divisions joined each other below the piriformis muscle to form sciatic nerve. This low formation of sciatic nerve was observed in distal part of left gluteal region of a 50-year-old male cadaver. The sciatic nerve thus formed passed caudally into back of thigh and divided into tibial and common peroneal nerves in the upper part of popliteal fossa. In addition, a communicating nerve from the sciatic nerve was found to join the c

Ultrasound-guided interventional procedures for patients with chronic pelvic pain - a description of techniques and review of literature.

Chronic pelvic pain can present in various pain syndromes. In particular, interventional procedure plays an important diagnostic and therapeutic role in 3 types of pelvic pain syndromes: pudendal neuralgia, piriformis syndrome, and "border nerve" syndrome (ilioinguinal, iliohypogastric, and genitofemoral nerve neuropathy). The objective of this review is to discuss the ultrasound-guided approach of the interventional procedures commonly used for these 3 specific chronic pelvic pain syndromes. Piriformis syndrome is an uncommon cause of buttock and leg pain. Some treatment options include the i

Editorial Commentary: Piriformis Syndrome Is a Complex Condition That Requires Precise Diagnosis and Can Be Adequately Treated by Sciatic Neurolysis and Release of the Piriformis Tendon.

Piriformis syndrome is an extrapelvic compression of the sciatic nerve at the infrapiriformis canal. Recently, this condition has been included with other extrapelvic sciatic compression syndromes under the term deep gluteal syndrome. Patients usually present with posterior gluteal pain irradiated to the posterior thigh, which frequently is triggered or exacerbated by sitting over long periods of time (30 minutes or longer). Even though piriformis syndrome is well understood, its diagnosis is usually one of exclusion, in which other causes of sciatica, including lumbosacral pathology, are excl

Piriformis muscle injection using ultrasonography and motor stimulation--report of a technique.

The piriformis muscle syndrome has been described in the literature since 1947 and accounts for 6-8% of patients presenting with buttock pain, which may variably be associated with sciatica. Through the years, there have been attempts to find safe and effective ways of managing this condition, whether through conservative treatment or with the use of interventional procedures. Several authors have reported injection techniques using the following: nerve stimulation; fluoroscopy with electromyography; and fluoroscopy with muscle stimulation. We aim to describe an injection method which is effec

Four symptoms define the piriformis syndrome: an updated systematic review of its clinical features.

PURPOSE: To update the evidence on the clinical features of the piriformis syndrome since the first systematic review published in 2010. METHOD: A systematic review of all case, cross-sectional and prevalence studies. RESULTS: The commonest features reported were: buttock pain, pain aggravated on sitting, external tenderness near the greater sciatic notch and pain on any maneuver that increases piriformis muscle tension, and limitation of straight leg raising. The quality of case reports since the previous review has not improved with considerable under-reporting of presumed negative tests.

Piriformis syndrome as the only initial manifestation of septic sacroiliac osteomyelitis.

Piriformis syndrome refers to sciatica symptoms that do not originate from spinal root compression, but involve the overlying piriformis muscle. Diagnosis is clinical since there are no definitive tests to confirm the diagnosis. Piriformis syndrome is often misinterpreted as sciatica or other pains. Rarely, it could be one of the manifestations of pelvic osteomyelitis. Pelvic osteomyelitis is rare in adults, but it is associated with high morbidity and mortality. One of the severe complications is bacteraemia, which is life-threatening and may be associated with poor outcome. Acute pelvic oste

Successful Ultrasound-Guided Dry Needling for Treatment of Piriformis Syndrome.

Piriformis syndrome is an often overlooked cause of chronic buttock or low back pain, sometimes radiating to the posterior thigh. Despite various treatment options, a definitive standard therapy has not been established. Dry needling has recently gained attention as a non-pharmacological intervention, especially when guided by ultrasound to enhance accuracy and safety. A 69-year-old woman presented with a 9-month history of fluctuating right gluteal pain, aggravated by prolonged sitting and movements. Physical examination revealed tenderness in the piriformis area and positive provocative test

Bilateral eventration of sciatic nerve.

During routine dissection of a 60 years male cadaver, it was observed that the two divisions of sciatic nerve were separate in the gluteal region on both the sides with the tibial nerve passing below the piriformis and the common peroneal nerve piercing the piriformis muscle. The abnormal passage of the sciatic nerve (SN), the common peroneal nerve (CPN), and the tibial nerve (TN), either through the piriformis or below the superior gemellus may facilitate compression of these nerves. Knowledge of such patterns is also important for surgeons dealing with piriformis syndrome which affects 5-6%

Impact of Physiotherapy Intervention on Pain, Quality of Life, and Function in Low Back Pain Associated With Piriformis Syndrome: Protocol for Systematic Review.

BACKGROUND: Piriformis syndrome is a neuromuscular condition with hip and buttock pain and other symptoms, including referred pain towards the lower back and leg and radiating towards the foot's medial aspect. Similarly, low back pain caused by piriformis syndrome is undetected or difficult to diagnose because of similar symptoms of lumbar disc herniation, lumbar stenosis, or radiculopathy, as well as neurogenic pain. A study conducted in 2013 found 2910 patients experienced low back pain with sciatica, which is the most common cause of low back pain, because of piriformis muscle stiffness. Th

Piriformis Tenotomy and Sciatic Neurolysis via a Mini-open Approach: A Technique Guide.

Piriformis syndrome is a common, treatable cause of deep gluteal pain. Treatment often starts with conservative measures such as nonsteroidal anti-inflammatories, muscle relaxants, physical therapy, and injections, but some patients progress to needing surgical intervention. Several surgical techniques are currently used to treat piriformis syndrome, with an endoscopic approach gaining popularity because of its less invasive nature and, therefore, esthetic and symptomatic benefits. However, an open approach remains more effective in releasing severely adherent sciatic nerves, in addition to be

Intrapelvic entrapment of sacral nerve roots by abnormal bundles of the piriformis muscle: description of an extra-spinal cause of sciatica and pudendal neuralgia.

Piriformis syndrome is a well-known extra-spinal cause of sciatica characterized by the entrapment of the sciatic nerve by variant bundles of the piriformis muscles in the deep gluteal space. In this case series, we describe the entrapment of intrapelvic portions of the sacral nerve roots by a variant bundle of the piriformis muscle originating medially to the sacral foramina, the surgical technique for the laparoscopic treatment of this condition, and the outcomes of the first eight cases treated with this technique. Five female and three male patients presenting with sciatica, pudendal pain

Piriformis syndrome: treatment of a rare cause of posterior hip pain with fluoroscopic-guided injection.

INTRODUCTION: Piriformis syndrome involves the irritation of the piriformis muscle due to various reasons that are primarily related to anatomical variation or sciatic nerve compression due to contraction caused by overuse. In this study, we aimed to define an injection method that is easy to apply, safe, simple and repeatable. MATERIALS AND METHODS: We administered percutaneous lidocaine and depomedrol injections into the hips of 28 patients (14 men and 14 women) under fluoroscopic control. Bipolar injections of the piriformis muscle were performed at the medial pole, the intersection of the

Ultrasound appears to be a reliable technique for the diagnosis of piriformis syndrome.

INTRODUCTION: Piriformis muscle syndrome (PS) is a disorder encompassing a constellation of symptoms, including buttock and hip pain. In this study we aimed to assess the value of ultrasound (US) in the diagnosis of PS. METHODS: Thirty-three clinically diagnosed PS patients and 26 healthy volunteers underwent a clinical PS scoring examination and US and MRI assessment of the bilateral piriformis muscles. The areas under the receiver operating characteristic curves (AUROCs) of the US parameters (i.e., increased thickness [iTh] and increased cross-sectional area [iCSA]) for piriformis muscle we

Composite Anatomical Variations between the Sciatic Nerve and the Piriformis Muscle: A Nepalese Cadaveric Study.

Piriformis syndrome is a rare syndrome which is one of the main causes of nondiscogenic sciatica causing severe low back pain due to entrapment of sciatic nerve either by the hypertrophy or by inflammation of the piriformis muscle. We have carried out dissection in 20 Nepalese cadavers. Out of 40 dissected gluteal regions, 37 exhibited typical appearance of sciatic nerve, piriformis muscle, and their relations resembling type-a, whereas 3 gluteal regions showed composite structural variations resembling type-b and type-c based on Beaton and Anson's classification. Knowledge pertaining to such

Deep Gluteal Space Anatomy, Pathology, Clinical Presentation, and Management.

The deep gluteal space is a complex anatomic region associated with posterior hip symptoms. The clinical presentation involves symptoms and signs attributable to the deep gluteal space itself, in addition to other periarticular structures. Imaging involves both 2 and 3 dimensional modalities. Collision-detection software may be highly valuable for dynamic assessment. Nonsurgical treatment involves education-driven activity modification and physical therapy, combined with medications and selective injections. Surgical treatment involves endoscopic or open management, performed in isolation or c
