Cerebral infarction following pituitary apoplexy--case report.

A 29-year-old man presented with lethargy, headache, high fever, and visual disturbance. Neurological examination showed mydriatic pupil, ptosis, diminished light reflex, and ophthalmoplegia on the left. Magnetic resonance (MR) imaging showed the typical findings of pituitary apoplexy, and cerebral angiography disclosed mild narrowing of the A1 segment of the left anterior cerebral artery (ACA). Transsphenoidal tumor resection was performed. Transient severe right hemiparesis occurred directly after the operation. Computed tomography demonstrated cerebral infarction in the territory of the lef

Pituitary apoplexy after anticoagulation for unstable angina.

OBJECTIVE: To describe a patient with an undiagnosed pituitary macroadenoma, in whom pituitary apoplexy developed after heparin anticoagulation for treatment of unstable angina. METHODS: We chronicle the clinical course, treatment, and outcome in a 53-year-old woman with pituitary apoplexy. Potential risk factors are also discussed. RESULTS: In a patient with coronary artery disease and unstable angina, heparin anticoagulation therapy was initiated. Shortly thereafter, severe frontal headaches, nausea and vomiting, high fever, diplopia, and visual field deficits developed. A workup for infec

Subacute pituitary apoplexy: MR and CT appearance.

We report three cases of subacute hemorrhage into a pituitary adenoma. Two patients each had visual disturbance and hypopituitarism treated with corticosteroids. No patient received bromocriptine. Because it is more exact than CT in displaying the metabolic products of hemorrhage, magnetic resonance is the preferred modality for radiographic investigation of subacute or chronic pituitary apoplexy in patients with prolonged headache and visual disturbance.

Visual recovery after blindness from pituitary apoplexy.

In a series of 11 patients with classic pituitary apoplexy, two patients had the acute onset of bilateral blindness. After transsphenoidal decompressive surgery both patients had useful recovery of vision. Although impairment of neurologic function appears irreversible when central nervous tissue has been compressed to the point of total loss of function it would appear that the optic chiasm may be an exception to this experience. Our review of the literature and our experience suggests that decompression can be worthwhile even late in the course of this disease and visual loss should not be t

Gestational Pituitary Apoplexy With Persistent Hypogonadotropic Hypogonadism.

BACKGROUND/OBJECTIVE: Pituitary apoplexy (PA) is a rare and life-threatening emergency that remains prone to misdiagnosis, especially in pregnancy. Few cases of gestational PA have been reported in the literature. CASE REPORT: This study reports a case of a 26-year-old woman presenting at 35 weeks of gestation with PA. DISCUSSION: Initial presentation included severe headache, blurry vision, and dizziness, and conservative medical management was opted. This included stress-dose hydrocortisone, cabergoline, and levothyroxine, followed by cesarean delivery at 36 weeks of gestation. The patient

[Pituitary apoplexy in pediatric patients: systematic review].

INTRODUCTION: Necrosis or hemorrhage associated with pituitary tumors is an infrequent complication. The term pituitary apoplexy is related to the development of an inflammatory process that leads to irritation of the meningeal and periselar areas. Regardless of the mechanism (hemorrhage or infarction), the extent of the hemorrhage and necrosis will produce an increase in intraselar pressure which, in turn, will lead to a more or less pronounced compression of neighboring structures, thus explaining the broad spectrum clinical. METHODS: We carried out a systematic review of cases published in

Pituitary apoplexy and panhypopituitarism following acute leptospirosis.

Leptospirosis is a common tropical febrile illness which may manifest with the hepatorenal syndrome and systemic hemorrhagic manifestations. Pituitary apoplexy is a rare but life-threatening condition characterized by a hemorrhage within the pituitary gland or a pituitary adenoma. Apoplexy is very rarely associated with some inducing events such as infectious diseases such as dengue hemorrhagic fever, Hantaan virus, Puumala virus have also been reported to cause pituitary apoplexy. We present the first case of pituitary apoplexy in a patient who was being treated for leptospirosis and discuss

Pituitary Adenoma Apoplexy: Review of Personal Series.

BACKGROUND: Pituitary apoplexy is a life-threatening event with unspecific clinical background and no standardized treatment. MATERIALS AND METHODS: The authors retrospectively analyzed seventeen patients affected by pituitary adenoma apoplexy and treated in a 10-year period. Thirteen patients underwent surgery through transsphenoidal route while four patients have been treated conservatively. RESULTS: The endoscopic surgical procedure showed a better result in term of complete removal of the tumor while in the "conservative" group less frequent evidence of hormones' deficiency has been regi

Myasthenia gravis imitating pituitary apoplexy in macroprolactinoma.

Myasthenia gravis (MG) is an autoimmune disease affecting neuromuscular transmission that manifests with muscle weakness and typically involves the eye muscles, often producing diplopia and ptosis. Recent studies suggest that hyperprolactinaemia may have a role in the development of MG, although its association with prolactinoma is extremely rare. We report the unusual case of a 71-year-old male affected with macroprolactinoma, who presented at our Center 2 weeks after starting cabergoline treatment because of acute onset of headache, diplopia, and ptosis. On admission, he presented with drows

Idiopathic granulomatous hypophysitis presenting as pituitary apoplexy.

Idiopathic granulomatous hypophysitis (IGH) is an extremely rare chronic inflammatory lesion of the pituitary gland. This condition typically presents with chronic onset of headache and slow development of visual deficits. Pituitary apoplexy is a clinical syndrome characterized by sudden onset of headache, vision loss, opthalmoplegia, and signs of meningeal irritation. Although IGH has been previously described in the literature, its presentation as clinical pituitary apoplexy is novel. We report, to our knowledge, the first patient with IGH manifesting as clinical pituitary apoplexy. Physicia

Asymptomatic pituitary apoplexy after aortocoronary bypass surgery.

Pituitary apoplexy usually presents with acute neuro-ophthalmological complications that require urgent neurosurgical intervention. We present a case of pituitary apoplexy following aortocoronary bypass surgery that was asymptomatic until the patient presented with features of hormonal deficiency three months later. Only one case of pituitary apoplexy has been described in the literature following cardiac surgery that did not require operative intervention. We discuss the aetiology of pituitary apoplexy and the possible mechanisms for such an event after cardiac surgery. Although this is rare,

[Stress-induced pituitary apoplexy in 2 phases].

A case of pituitary apoplexy occurring in the context of physical effort is reported. A 47-year-old man presented headache after a foot-race. Secondary visual deficit developed. On admission, bilateral cavernous sinus syndrome with bitemporal hemianopia was observed. The CT scan showed an enlarged sella eroded by an intrasellar soft tissue mass. Magnetic resonance imaging (MRI) confirmed the presence of the mass with peripheral enhancement following gadolinium injection. Decompression was carried out by transsphenoidal surgery. The microscopic study demonstrated infarction of the anterior hypo

Hypothalamic hypopituitarism after pituitary apoplexy in acromegaly.

Pituitary apoplexy in acromegaly is an uncommon event having been recorded approximately 30 times in the English literature. This report records two additional cases that included growth hormone measurements and an assessment of pituitary function. The apoplectic event developed spontaneously in one, and in the other it developed within two weeks of completing a course of radiotherapy to the pituitary gland. Autocure of the acromegaly was apparent. Basal levels of growth hormone were in the normal range but failed to change with provocative stimuli. Luteinizing hormone and follicle-stimulating

Correlation between intrasellar pressure, pituitary adenoma invasiveness, pituitary dysfunction, and apoplexy.

BACKGROUND: Few studies have reported the association between intrasellar pressure (ISP) and tumor invasiveness, hypopituitarism, or pituitary apoplexy in patients with pituitary adenomas. This study aimed to investigate the relationship between intraoperatively measured ISP and pituitary adenoma invasiveness, as well as to assess whether elevated ISP is associated with hypopituitarism and pituitary apoplexy. METHODS: We retrospectively analyzed 84 patients with newly diagnosed pituitary adenomas who underwent endoscopic transsphenoidal surgery at the First Affiliated Hospital of Bengbu Medic

Pituitary metastasis presenting as ischemic pituitary apoplexy following heparin-induced thrombocytopenia.

Pituitary apoplexy (PA) typically results from infarction or hemorrhage in a pituitary adenoma, while PA in nonadenomatous pituitary gland is uncommon. Prothrombotic states have never been recognized as precipitating factors for PA. The authors report a case of an elderly female who received prophylactic fractionated heparin therapy due to sepsis, consequent rhabdomyolysis, and overt disseminated intravascular coagulation. On the seventh day of heparin therapy, she reported sudden vision loss, ptosis, diplopia, and severe headache. Severe thrombocytopenia and positive antibodies to the complex
