Biomechanics of common fixation devices for first tarsometatarsal joint fusion-a comparative study with synthetic bones.

BACKGROUND: Hallux valgus disease is a common deformity of the forefoot. There are currently more than 100 surgical approaches for operative treatment. Because hypermobility of the first tarsometatarsal joint is considered to be causal for hallux valgus disease, fusion of the tarsometatarsal joint is an upcoming surgical procedure. Despite the development of new and increasingly stable fixation devices like different locking plates, malunion rates have been reported in 5 to 15% of cases. METHODS: Biomechanical comparison of three commonly used fixation devices (a dorsal locking plate, a plant

Second MTP joint instability: grading of the deformity and description of surgical repair of capsular insufficiency.

A crossover second toe is a commonly seen forefoot problem, usually occurring in women aged > 50 years, and often in association with a bunion deformity. The plantar plate is the principal static stabilizer of the second metatarsophalangeal (MTP) joint. Different authors have proposed classifications to define instability of the second MTP joint, but only describe clinical progression of the deformity. Once a plantar plate tear has developed, conservative treatment can eliminate the symptoms and prevent progression of the deformity but cannot achieve correction or realignment of the deformity.

Metatarsophalangeal joint instability of the lesser toes.

Metatarsophalangeal joint instability of the lesser toes is a common finding and a common cause of metatarsalgia. The clinical presentation can include swelling without digital deformity; however, often, this can progress to the development of coronal and transverse plane malalignment. In some cases, frank metatarsophalangeal joint dislocation can develop. The treatment regimen has historically focused on indirect surgical realignment using soft tissue release, soft tissue reefing, tendon transfers, and periarticular osteotomies. An improved understanding of the plantar plate has recently led

Metatarsophalangeal joint pathology in crossover second toe deformity: a cadaveric study.

BACKGROUND: Ligamentous and capsular insufficiency of the second metatarsophalangeal joint has been surgically treated for over two decades, mainly with indirect surgical repairs, which stabilize adjacent soft tissue and shorten or decompress the osseous structures. While ligamentous insufficiency has been described and recognized, degeneration of the plantar plate and tears of the capsule have rarely been documented. The purpose of this study was to document and describe the presence and pattern of plantar plate tears in specimens with crossover second toe deformities, and based on this, to d

Metatarsophalangeal Joint Instability: Anatomy and Physiopathology.

Metatarsophalangeal (MTP) instability is a common cause of metatarsalgia of the lesser toes. Instability often presents as pain in the plantar forefoot, progressing to coronal and transverse plane malalignment. MTP joint dislocation can develop in some cases. The plantar plate plays an integral role in MTP joint stabilization, and injury to the plantar plate is the main contributor to the instability noted at the MTP joint.

Functional characteristics of the foot and plantar aponeurosis under tibiotalar loading.

Structurally the foot is equivalent to a twisted plate. The hindpart is located in the sagittal plane and the forepart in the transverse plane. The transition induced by the twist creates the transverse and the longitudinal arches. Under vertical loading of the foot plate by the tibiotalar column, compressive forces are created on the dorsum and tensile forces on the plantar aspect of the foot plate. The plantar aponeurosis acting as a tie-rod when under tension relieves the tensile forces from the plantar aspect of the foot plate. The increased tension in the plantar aponeurosis in the weight

Stability of Locking Plate and Compression Screws for Lapidus Arthrodesis: A Biomechanical Comparison of Plate Position.

Lapidus (first tarsometatarsal joint) arthrodesis is an established and widely used procedure for the management of moderate to severe hallux valgus, especially in cases involving hypermobility of the first tarsometatarsal joint. Multiple fixation methods are available, and several previous investigations have studied the relative strengths of these methods, including dorsomedial and plantar plating comparisons. However, these studies compared plates of varying designs and mechanical properties and used varying modes of compression and interfragmentary screw techniques. The present study mecha

Ultrasound Relationship of Plantar Fat and Predislocation Syndrome.

BACKGROUND: Plantar fat plays a crucial role in protecting and cushioning the metatarsals. Its degeneration is a risk factor for the development of metatarsalgia and, consequently, predislocation syndrome. OBJECTIVES: To evaluate the relationship between plantar fat thickness and predislocation syndrome in an adult population, and to determine a possible association between a decrease in forefoot plantar fat and the presence of symptoms. MATERIAL AND METHODS: A retrospective observational study was conducted, including records of patients who visited the podiatry clinic between December 2022

Lesser Metatarsophalangeal Joint Instability: Open Surgery Treatment Alternatives.

Lesser metatarsophalangeal joint plantar plate degeneration and tear typically presents as an attritional pattern of capsuloligamentous deficiency in middle-aged patients or sports-related chronic injuries. Knowledge of the anatomy, pathophysiological basis, common patterns, grading and classification of these injuries, and indications for surgery will aid imaging interpretation in the preoperative setting. The acuity and extent of injury, tissue quality, and functional requirements of the patient influence clinical decision-making with respect to surgical management. This article provides an

Biomechanical investigation of two plating systems for medial column fusion in foot.

BACKGROUND: Arthrodesis of the medial column (navicular, cuneiform I and metatarsal I) is performed for reasons such as Charcot arthropathy, arthritis, posttraumatic reconstruction or severe pes planus. However, the complication rate is still high and mainly resulting from inadequate fixation. Special plates, designed for medial column arthrodesis, seem to offer potential to reduce the complication rate. The aim of this study was to investigate biomechanically plantar and dorsomedial fusion of the medial column using two new plating systems. METHODS: Eight matched pairs of human cadaveric low

Coupled experimental-computational study of Lapidus plates for the high-performance of surgical techniques for hallux hypermobility correction.

BACKGROUND: For the correction of Hallux hypermobility through surgery, Lapidus plates have been studied as surgical fixation devices to evaluate the procedure's success and its implications for biomechanical foot performance. However, significant knowledge gaps persist regarding the behavior of these devices and their biomechanical characteristics following implantation. METHODS: This work studies two types of Lapidus plates through a coupled experimental-computational analysis. The first is a plantar fixation device, and the second is a helical device; both are used as fixation tools in Cun

An atraumatic turf toe in an elite soccer player--a stress related phenomenon?

Plantar plate injuries to the hallux in elite athlete could potentially be career threatening. Reports in the literature are invariably linked to a significant traumatic episode. The occurrence of an atraumatic severe plantar plate injury in the presence of a bipartite sesamoid may suggest a stress related phenomenon. We present a case in an elite soccer player who was treated surgically and returned to top-level competition. The case is reported in detail and differences to other reports in the literature discussed.

Comparing the stability of a novel h-shaped bone plate and commercial endosteal plating systems in hallux valgus correction.

BACKGROUND: It is common for physicians to opt for surgical correction of hallux valgus deformities using implants, but post-operative complications are frequently reported. A novel h-shaped plate developed by the authors offers both endosteal and lateral fixation, helping to resist displacement in multiple directions. This study aims to assess the mechanical properties and stability of the h-shaped plate in comparison to various commercially available endosteal plating systems in a simulated hallux valgus correction model using finite element analysis. METHODS: Finite element models of four

High-Resolution MRI of the First Metatarsophalangeal Joint: Gross Anatomy and Injury Characterization.

The first metatarsophalangeal joint (MTPJ) is vital to the biomechanics of the foot and supports a weight up to eight times heavier than the body during athletic activities. The first MTPJ comprises osseous and cartilaginous surfaces along with a complex of supporting structures, including the dorsal extensor tendons, collateral ligaments, and a plantar plate complex. In contradistinction to the lesser MTPJ plantar plates, a single dominant fibrocartilaginous capsular thickening does not exist at the first MTPJ. Instead, the plantar plate complex comprises a fibrocartilaginous pad that invests

Lesser Metatarsophalangeal Joint Instability: Minimally Invasive Surgery Treatment Alternatives.

Lesser metatarsophalangeal (MTP) joints instability is usually produced by the rupture of the plantar plate. The treatment algorithm may change in patients with advanced stages of the plantar plate rupture, and surgical treatment is proposed to avoid MTP dislocation. Palliative surgery with minimally invasive surgery (MIS) can offer a less aggressive approach and enable excellent clinical correction and a good function. The authors proposed an algorithm for MIS treatment of MTP instability based on a combination of different osseous and/or soft tissues surgical procedures that the authors will

Spatiotemporal mapping of plantar shear forces during human walking using a flexible thin-film sensor.

During walking, the foot is subjected to complex vertical and shear ground reaction forces, which play critical roles in gait mechanics and the development of foot pathologies. However, plantar shear stress has been less investigated due to measurement difficulties. This study aimed (1) to evaluate the accuracy of a newly developed thin-film shear force distribution sensor in measuring plantar pressure and shear stress during walking, and (2) to characterize the spatiotemporal features of plantar shear loading throughout the stance phase. Five healthy male adults participated in walking experi
