Plasmablastic lymphoma of the oral cavity with breast recurrence: a case report.

BackgroundPlasmablastic lymphoma is an aggressive variant of diffuse large B cell lymphoma, mostly found in the oral cavity and associated with human immunodeficiency virus. There are no clear guidelines for its treatment. Therapies more intensive than cyclophosphamide, doxorubicin, vincristine, and prednisone are not associated with a prolonged survival. Lymphomas of the breast are rare, in one series representing 0.14% of all female breast malignancies, with diffuse large B cell lymphoma comprising up to 55% of all cases. Only one case of plasmablastic lymphoma involving the breast has been

IG/MYC rearrangements are the main cytogenetic alteration in plasmablastic lymphomas.

Plasmablastic lymphoma (PBL) is an aggressive lymphoma characterized by a terminally differentiated B-cell phenotype that usually occurs in the immunocompromised or elderly patients. Although the clinical and pathologic characteristics of these tumors have been defined, the genetic alterations involved in their pathogenesis are not well known. In this study, we have investigated the chromosomal alterations of MYC, BCL2, BCL6, MALT1, PAX5, and IGH loci using fluorescence in situ hybridization in 42 PBL and 3 extracavitary primary effusion lymphomas. MYC rearrangements were identified in 20 of 4

Lymphomas with plasmablastic features: a report of the lymphoma workshop of the 20th meeting of the European Association for Haematopathology.

Lymphomas with plasmablastic features are a heterogeneous group of aggressive and mostly uncommon neoplasms of varied aetiologies, presenting in immunocompetent individuals as well as in immunodeficiency, associated with EBV and Kaposi sarcoma virus infections, and some as progression from indolent B-cell lymphomas. They show overlapping diagnostic features and pose a differential diagnosis with other aggressive B-cell lymphomas that can downregulate the B-cell expression programme. The spectrum of rare reactive proliferations and all lymphomas defined by plasmablastic features, together with

Epstein-Barr virus-positive mucocutaneous ulcer, plasmablastic type, associated with nodal CD4+ angioimmunoblastic T-cell lymphoma and generalised pruritus: a self-limiting lymphoproliferative disorder resembling cutaneous plasmablastic lymphoma.

A woman in her 80s reported of generalised pruritus, which was treated with phototherapy and steroid administration. Two months after onset, lymph node biopsy revealed CD4+ angioimmunoblastic T-cell lymphoma with systemic superficial nodal involvement. Intractable prurigo was judged as T-cell lymphoma related. After effective chemotherapy (7 months later), skin nodules appeared multifocally, including on the lip, thumb and lower leg. The biopsied infiltrative lesion on the right lower leg microscopically revealed subcutaneous growth of atypical plasmablasts with nearly 100% Ki-67 labelling and

An Unusual Clinical Manifestation of Plasmablastic Lymphoma in a Renal Transplant Recipient.

Plasmablastic lymphoma is a rapidly progressive CD20 negative large cell non-Hodgkin lymphoma with poor outcome. It occurs mostly in immunocompromised individuals and has a predilection for extranodal sites. They need to be differentiated from other entities sharing similar morphological features like poorly differentiated carcinoma, Burkitt's lymphoma, Alk positive large B cell lymphoma, Diffuse large B cell lymphoma, and anaplastic myeloma. EBV negativity in recipients, type, intensity, and duration of immunosuppressives used are certain risk factors in development of posttransplant lymphopr

A rare presentation of plasmablastic lymphoma as cutaneous nodules in an immunocompromised patient.

Plasmablastic lymphoma is a rare entity accounting for around 2.7% of all AIDS-related lymphomas. The oral cavity and gastrointestinal tract are the most common sites involved. We report a case of a 34-year-old HIV-positive woman with a rare presentation of cutaneous nodules all over the body. Due to overwhelming tumour burden, she developed tumour lysis syndrome during her hospital stay and succumbed to the illness.

Plasmablastic lymphomas may occur as post-transplant lymphoproliferative disorders.

AIMS: To describe four cases of plasmablastic lymphoma arising in the unusual setting of a post-transplantation lymphoproliferative disorder (PTLD). METHODS AND RESULTS: Four cases were encountered over 2 years in human immunodeficiency virus (HIV)-negative patients following renal, heart or bone marrow transplantation. The cases were routinely processed and immunohistochemistry was performed. The cases showed blastic non-Hodgkin's lymphoma morphology and plasma cell-like immunophenotypic features: minimal or absent expression of leucocyte common antigen and CD20, variable CD79a and VS38 posi

A "Double-Hit" Lymphoma With Plasmablastic Features and Aberrant Keratin Expression Likely Transforming From Follicular Lymphoma.

Large B-cell lymphomas may exhibit morphology typical of a diffuse large B-cell lymphoma (DLBCL) or a Burkitt lymphoma. Rarely, they may exhibit either a blastoid morphology or features that are intermediate between a DLBCL and Burkitt lymphoma (the so-called intermediate/blastoid morphology); the term high-grade B-cell lymphoma (HGBL) is used to describe the latter. The presence of MYC and BCL2 rearrangements can be seen either in HGBL or in lymphomas with DLBCL morphology (so-called double-hit lymphoma) and rarely in plasmablastic lymphomas. Herein, we report a rare occurrence of a double-hi

When immature plasma cells form lymphoma: how to improve on diagnostics and treatment of plasmablastic lymphoma?

Plasmablastic lymphoma (PBL) is a rare and aggressive, usually CD20-negative, B-cell lymphoma with features between multiple myeloma and diffuse large B-cell lymphoma. Approximately 120 new cases are diagnosed annually in the United States, and there is a strong association with HIV infection and other immunosuppressive states. It affects mostly adult males, though PBL has been diagnosed in children and immunocompetent individuals. Epstein-Barr virus infection, MYC gene rearrangements, and other mechanisms appear to play a role in PBL lymphomagenesis. Given its rarity, PBL poses distinct diagn

Transformation of double-hit follicular lymphoma to plasmablastic lymphoma: a partial role of MYC gene rearrangement.

Follicular lymphoma (FL) is genetically characterized by BCL2/IGH translocation. Some FL cases histologically transform to high-grade lymphoma, and the majority of cases transform to diffuse large B-cell lymphoma. We report herein an unusual FL case that transformed to plasmablastic lymphoma (PBL) with MYC gene rearrangement as early as 12 months after FL diagnosis. IGH/MYC translocation, the most common cytogenetic abnormality seen in de novo PBL, was also detected in the transformed tumor (double-hit lymphoma). The patient became resistant to chemotherapy and died 4 months after transformati

Cutaneous plasmablastic lymphoma: retrospective comparative study of primary and secondary skin involvement.

AIMS: Cutaneous plasmablastic lymphoma (cPBL) is a rare and aggressive neoplasm that presents as skin nodules. Despite occurring in ~5% of PBL cases, the World Health Organization (WHO) does not distinguish primary cutaneous PBL (pcPBL) from secondary cutaneous PBL (scPBL), and their clinical differences remain poorly defined. To determine whether pcPBL represents a distinct clinical entity, we compare the clinicopathologic features, immunohistochemical profiles, and survival outcomes of pcPBL and scPBL. METHODS AND RESULTS: Retrospective comparative study analysing 40 cases of cPBL (6 newly

Anal plasmablastic lymphoma with complete response to antiretroviral therapy - a case report.

A 40-year-old male, known to be positive with the human immunodeficiency virus (HIV), presented with a large perianal mass and faecal obstruction. His CD4 count was 121 cells/mm3 suggesting advanced HIV disease, and he was started on second-line antiretroviral therapy. Magnetic resonance imaging (MRI) confirmed a large mass involving the anorectal junction. A biopsy of the tumor revealed a plasmablastic lymphoma (PBL). A diverting colostomy was performed for faecal obstruction. While awaiting chemotherapy treatment, the tumour rapidly decreased in size with complete clinical resolution. PBL is

BCMA CAR-T induces complete and durable remission in refractory plasmablastic lymphoma.

Plasmablastic lymphoma (PBL) is a rare subtype of aggressive large B-cell lymphoma, with a dismal prognosis despite aggressive therapies. New approaches are needed for those with refractory disease. PBL expresses antigens similar to multiple myeloma (MM), including B-cell maturation antigen (BCMA). Chimeric antigen receptor T-cell (CAR-T) therapy directed against BCMA has shown efficacy for the treatment of heavily pretreated MM with low rates of grades 3 and 4 cytokine release syndrome (CRS) and immune effector cell-associated neurotoxicity syndrome (ICANS) in a phase Ib/II trial (A Study of

Transformed Plasmablastic Lymphoma Presenting With Marked Lymphocytosis and Spontaneous Tumor Lysis Syndrome.

The clinicopathology entity of plasmablastic lymphoma (PBL), despite broad recognition by the World Health Organization (WHO), represents a diagnostic challenge due to its overlapping features and scarce occurrence. Often, PBL arises in immunodeficient, elderly male patients, most notably those who are human immunodeficiency virus (HIV)-positive. More infrequent, cases of transformed PBL (tPBL) evolved from another hematologic disease have been identified. Herein, we describe a case of a 65-year-old male transferred from a neighboring hospital with pronounced lymphocytosis and spontaneous tumo

CD3-positive plasmablastic lymphoma reported in two cases: A potential diagnostic caveat.

Plasmablastic lymphoma (PBL) is a rare aggressive subtype of mature large B cell lymphoma involving almost exclusively the extranodal regions particularly the oral cavity, frequently described in immunocompromised patients. PBL is characterized histologically by diffuse proliferation of large neoplastic cells resembling B immunoblasts or plasmablasts. The diagnosis of PBL can be difficult due to its ambiguous histopathological features mimicking most large cell lymphomas and lacking a distinctive immunophenotypic pattern. They typically lack expression of CD20 and CD79a but may express plasma

Testicular Plasmablastic Lymphoma in an HIV-Negative Patient: A Rare Case Presentation.

Plasmablastic lymphoma (PBL) is a very rare disease and it is usually considered a human immunodeficiency virus (HIV)-related B-cell lymphoma that carries a poor prognosis. It mostly involves the oral cavity, lungs, nasal cavity, gastrointestinal tract, lymph node, and skin. Therapeutic regimens like dose-adjusted etoposide, vincristine, doxorubicin, cyclophosphamide, and prednisone (DA-EPOCH) have shown better results in these aggressive lymphomas. We report a rare case of PBL in an HIV-negative patient who presented to the clinic with a complaint of left testicular swelling for 3 months. Ult

Fistulizing Epstein-Barr virus-positive plasmablastic lymphoma in an HIV-positive man.

Plasmablastic lymphoma (PBL) is an unusual subtype of non-Hodgkin lymphoma recently classified as a diffuse immunoblastic lymphoma with a plasma-cell immunophenotype. Originally described in the oral cavity of HIV-positive patients, it has also been recognized to occur rarely at other sites. We describe a previously unreported fistulizing presentation of Epstein-Barr virus (EBV)-positive PBL, reviewing its association with HIV-1 infection and its importance as an AIDS-defining malignancy.

Clinicopathological characteristics and long-term outcomes of plasmablastic lymphoma in British Columbia.

Plasmablastic lymphoma (PBL) is an aggressive and rare subtype of non-Hodgkin lymphoma with no standard-of-care therapy. We reviewed all patients diagnosed with histologically confirmed PBL in British Columbia, Canada between 1997 and 2019. Overall, 42 patients were identified, including 15 (36%) positive for HIV and nine (21%) on chronic immunosuppression. Curative-intent treatment consisting primarily of cyclophosphamide, doxorubicin, vincristine and prednisone was administered to 31 patients, of which 74% achieved response, however 61% relapsed after a median of 7.5 months. At a median foll
