Transfusion-related acute lung injury following random donor platelet transfusion: a report of two cases.

OBJECTIVES: Transfusion-related acute lung injury (TRALI) following random donor platelet (RDP) transfusion is a rare complication of transfusion without any well-documented case reported in the English language literature. We describe 2 patients in whom TRALI occurred following RDP transfusion. METHODS: Conventional clinical and laboratory methods. RESULTS: Both patients developed acute shortness of breath 30-60 min after completion of RDP transfusion and required mechanical ventilatory support. Chest X-ray (CXR) in both cases revealed bilateral pulmonary infiltrates. Patient 1 required vas

Molecular confirmation of transfusion-transmitted dengue virus infection following platelet transfusion in the Chinese mainland.

BACKGROUND: Dengue virus (DENV) is considered an emerging threat to blood safety. While transfusion-transmitted DENV (TT-DENV) cases have been documented, TT-DENV has never been reported in Chinese mainland. In this study, a retrospective investigation was conducted after two patients developed dengue fever (DF) following apheresis platelet (PLT) transfusion from the same healthy donor. METHODS: Blood samples from the recipients and the donor were tested for DENV RNA by nucleic acid testing (NAT). Nonstructural protein 1 (NS1) antigen and anti-DENV immunoglobulin M/G (IgM/IgG) antibodies were

Testing platelet mass versus platelet count to guide platelet transfusions in the neonatal intensive care unit.

BACKGROUND: Platelet (PLT) transfusions can bestow significant benefits but they also carry risks. This study sought a safe means of reducing PLT transfusions to neonatal intensive care unit (NICU) patients with thrombocytopenia by comparing two transfusion guidelines, one based on PLT count and the other on PLT mass (PLT count times mean PLT volume). STUDY DESIGN AND METHODS: Using a prospective, two-centered, before versus after design, PLT transfusion usage and hemorrhagic events were contrasted during a period when PLT count-based transfusion guidelines were in use (Period 1) versus a per

Blood Utilization and Transfusion Reactions in Pediatric Patients Transfused with Conventional or Pathogen Reduced Platelets.

OBJECTIVES: To assess the safety and efficacy of a Food and Drug Administration-approved pathogen-reduced platelet (PLT) product in children, as ongoing questions regarding their use in this population remain. STUDY DESIGN: We report findings from a quality assurance review of PLT utilization, associated red blood cell transfusion trends, and short-term safety of conventional vs pathogen-reduced PLTs over a 21-month period while transitioning from conventional to pathogen-reduced PLTs at a large, tertiary care hospital. We assessed utilization in neonatal intensive care unit (NICU) patients,

ABO-mismatched transfusions are not over-represented in febrile non-haemolytic transfusion reactions to platelets.

ABO-mismatched plasma and platelet (PLT) transfusion have been associated with worse outcomes, including haemolysis and other reactions, compared to recipients of ABO-identical products. The immune complexes that form in a mismatched transfusion have been demonstrated to stimulate pyrogenic cytokine release in vitro. Comparing ABO identical vs. ABO mismatched PLT transfusions, we found no significant difference in the ABO compatibilities between the PLT doses implicated in causing febrile non-haemolytic transfusion reactions (FNHTR) in 162 recipients and both the baseline PLT donor/recipient A

Eltrombopag treatment promotes platelet recovery and reduces platelet transfusion for patients with post-transplantation thrombocytopenia.

Post-transplantation thrombocytopenia (PT) is a common and severe complication which usually leads to poor prognosis. Eltrombopag (EPAG), a novel oral thrombopoietin (TPO) receptor agonist, has shown promising effects in thrombocytopenia due to immune thrombocytopenic purpura (ITP) and refractory severe aplastic anemia (rSAA), while the effectiveness of EPAG for PT patients still needs to be evaluated. A total of 32 PT patients receiving EPAG were retrospectively analyzed between September 2017 and July 2019, including 15 patients with poor graft function (PGF) and 17 patients with secondary f

Clinical application of a modified platelet desialylation test for mechanistic characterization of platelet transfusion refractoriness.

BACKGROUND: Platelet transfusion refractoriness (PTR) is a major challenge in transfusion medicine and may result from both immune and non-immune mechanisms. Although alloantibodies are well-established contributors, Fc-independent pathways such as platelet desialylation have emerged as alternative mechanisms of clearance. STUDY DESIGN AND METHODS: In this prospective diagnostic study, 81 patients with suspected PTR were evaluated using an integrated approach combining the platelet immunofluorescence test (PIFT) to detect antibody-mediated refractoriness and a modified platelet desialylation

Circulating biomarkers predict platelet recovery in adult dengue patients with thrombocytopenia receiving prophylactic platelet transfusion.

OBJECTIVES: Prophylactic platelet transfusion is frequently used in managing hospitalized dengue patients. Our earlier study identified a subset of patients who, despite receiving prophylactic transfusions, exhibited delayed recovery of peripheral platelet counts. This study investigated the underlying mechanisms of this impaired response. METHODS: We measured cytokine/chemokine concentrations in plasma samples from 197 patients in the multicentre, randomized Adult Dengue Platelet (ADEPT) Study of prophylactic platelet transfusion. Patients were divided into a control group (supportive therap

A rapid platelet function assay used to regulate platelet transfusion prophylaxis following cardiopulmonary bypass surgery.

Subsequent to cardiac surgery with cardiopulmonary bypass (CPB), patients are at risk of postoperative bleeding caused by acquired defects associated with the procedure. As such, many patients receive prophylactic blood product transfusion. The effectiveness of measuring platelet function using a near-patient platelet function analyzer for the purpose of regulating platelet transfusion and potentially modulating other blood products including fresh frozen plasma (FFP) and cryoprecipitate (CRYO) transfusion was evaluated prospectively in patients undergoing CPB. A 6-month prospective study was

Beyond transfusion: Platelet-derived therapeutic products as a new frontier for blood establishments and transfusion medicine.

BACKGROUND AND OBJECTIVES: Allogeneic blood components and plasma-derived medicinal products (PDMPs) are cornerstone therapies in modern medicine, setting standards for quality, safety and efficacy. While platelets are traditionally transfused to prevent or control bleeding, they also serve as reservoirs of bioactive molecules with regenerative, anti-inflammatory, anti-oxidative and neuroprotective properties. This review examines how pooled human platelet lysates (HPLs) and platelet-derived extracellular vesicles (p-EVs) could be developed as therapeutic products, building on the principles e

Impact of restrictive platelet transfusion strategies on transfusion rates: A cohort study in very preterm infants.

BACKGROUND: Evidence supports a restrictive platelet transfusion threshold in preterm neonates. We aimed to describe the effect of implementing this threshold on transfusion rates. STUDY DESIGN AND METHODS: This retrospective observational cohort study included all very preterm infants (born <32 weeks' gestation) admitted to a neonatal intensive care unit between 2004 and 2022, divided into three epochs. Platelet transfusion thresholds changed from 30 × 109/L for stable neonates and 50 × 109/L for unstable neonates (January 2004 to December 2009) to 20 × 109/L for stable neonates and 50 × 109

Incompatible crossmatch: First sign of a hemolytic transfusion reaction due to out-of-group platelet transfusion.

Platelet (PLT) transfusion is undertaken in a variety of clinical settings with thrombocytopenia, with or without bleeding. Since PLTs are most often stored in donor plasma, group-specific PLT transfusions are preferred to out-of-group transfusions. PLTs adsorb ABO antigens over their surface from the plasma. In major ABO-incompatible PLT transfusions, anti-A/B from the patient plasma react with the ABO antigens on transfused PLTs and can potentially cause adverse reactions or PLT refractoriness. Transfusion of PLTs with major ABO incompatibility, though effective in preventing clinical bleedi

Platelet Lipidome Characteristics of Adverse Reactions Associated with Irradiated Aphaeresis Platelets Transfusion in Children.

BACKGROUND: In clinical practice, we have observed that compared with red blood cells and plasma, platelet trans-fusion is more likely to cause transfusion adverse reactions, especially in children. Platelets may suffer different degrees of structural and biochemical damage during processing and preservation, and the lipid components contained in platelets may have a significant impact on the generation of transfusion adverse reactions. Therefore, the objective of this study is to identify the key bioactive lipid components that cause adverse blood transfusion reactions especially allergic ras

Immediate adverse reactions to platelet transfusions: whole blood derived versus apheresis platelets.

The transfusion of whole blood derived platelets (WBDPs) or apheresis platelets (APs) is standard support for cancer patients. However, disputes remain about which type of platelets are ideal in terms of efficacy, cost, and the risk of adverse reactions. This cross sectional study included 141 cancer patients who underwent chemotherapy or hematopoietic progenitor cell transplantation and received platelet transfusions at The University of Texas M.D. Anderson Cancer Center between 2002 and 2003 were retrospectively evaluated. A total of 141 patients who did not differ significantly in terms of

Triggers for prophylactic use of platelet transfusions and optimal platelet dosing in thrombocytopenic dogs and cats.

Prophylactic platelet transfusions are frequently given to human patients with hypoproliferative thrombocytopenia. For several decades, the most common transfusion trigger was 20,000/microL, but the trend is now to use 10,000/microL in the absence of other risk factors for bleeding. This trigger seems to reduce the number of transfusions without increasing the risk of severe bleeding. Most studies involved in establishing platelet transfusion policies have involved patients with acute leukemia, with fewer studies involving patients undergoing hematopoietic stem cell transplantation or aggressi

Utility and Safety of Romiplostim as an Alternative to Platelet Transfusion for Neonates.

Thrombocytopenia is a common challenge in the neonatal intensive care unit, traditionally managed with platelet transfusions. However, transfusions are associated with significant risks, including increased mortality and neurodevelopmental impairment. This case series highlights the use of romiplostim, a thrombopoietin receptor agonist, in three neonates with thrombocytopenia. Each case demonstrated notable improvements in platelet counts without adverse events, reducing dependency on transfusions. These findings underscore romiplostim's potential as a safer alternative for managing neonatal t

Neonatal thrombocytopenia-causes and outcomes following platelet transfusions.

UNLABELLED: We evaluated the causes for neonatal thrombocytopenia (NT), the duration of NT, and the indications of platelet transfusions (PT) by means of a retrospective cohort study over a 23-year period. Neonates with NT were identified via ICD-10 code D69.6. Of 371 neonates (1.8/1000 live births) with NT, the majority (312; 84.1%) had early onset thrombocytopenia, and 282 (76%) were preterm born. The most frequent causes for NT were early and late onset sepsis and asphyxia. The mean duration of thrombocytopenia was 10.2 days and was negatively correlated (KK = - 0.35) with the number of PT.
