Nonmesothelial Spindle Cell Tumors of Pleura and Pericardium.

Spindle cell lesions of the pleura and pericardium are rare. Distinction from sarcomatoid mesothelioma, which has a range of morphologic patterns, can be difficult, but accurate diagnosis matters. This article provides practical guidance for the diagnosis of pleural spindle cell neoplasms, focusing on primary lesions.

The Diagnosis of Pleural Tumors Other Than Mesothelioma.

CONTEXT: - Pleural pathology has been dominated by discussions relating to the diagnosis, prognosis, etiology, and management of malignant mesothelioma. However, there exists a diverse group of other neoplasms that involve the pleura; the most common by far is metastatic carcinoma, usually of pulmonary origin. Other metastatic tumors of varied histogenesis do occur but are less common. Primary pleural neoplasms other than diffuse malignant mesothelioma are either uncommon or rare and have received less attention. OBJECTIVE: - To provide a review of those diverse tumors that can involve the pl

Malignant Mesothelioma: Overcoming Diagnostic Hurdles.

Malignant pleural mesothelioma, an aggressive neoplasm frequently linked to asbestos exposure, is often detected at an advanced stage. This report details the case of a 58-year-old mason who presented with left-sided chest pain, and shortness of breath, accompanied by weight loss for a month. A positron emission tomography (PET) scan revealed increased uptake along the pleural surface, as well as in several mediastinal lymph nodes and the left supraclavicular lymph node. Thoracoscopy revealed the presence of multiple nodules on the costal pleura. Despite repeated negative results from pleural

Diagnostic Challenges in Epithelioid Pleural Mesothelioma: Case Series with Support from Electron Microscopy.

The histological diagnosis of pleural epithelioid mesothelioma can be difficult in the case of rare variants or in the definition of neoplasm origin in patients with previous or concomitant tumours. Currently, several immunohistochemical reactions are available in the surgical pathologist's armamentarium that allow us to obtain a more sensitive and specific diagnosis of malignant pleural mesothelioma. However, in some cases, the final interpretation remains inconclusive. Historically, ultrastructural examination has represented a useful tool for the definition of the mesothelial nature of neop

Mesenchymal chondrosarcoma - A rare cause of pleural effusion.

Mesenchymal chondrosarcoma is a malignant neoplasm arising from cartilaginous bone or soft tissue. It is uncommon yet devastating. Our patient was a 21-year-old man who presented with pleuritic chest pain and weight loss. His chest radiograph showed left pleural effusion. His pleural effusion analysis was consistent with exudative pleural effusion. Tuberculosis workup was negative. Pleural fluid cytology did not yield malignant cells. Subsequently, his computed tomography of thorax showed left rib sclerotic lesion with soft tissue component. Biopsy of the soft tissue eventually confirmed the d

Glycosaminoglycan in malignant pleural mesothelioma.

The quantitative analysis on glycosaminoglycan (GAG) in the tumor tissues of five patients with malignant pleural mesothelioma and in the pleural fluid of two patients was performed with the use of biochemical methods. In the tumor tissues, it was found that the average of the total amount of GAG was more than 7.9 times as high as that in adenocarcinoma of the lung, and that hyaluronic acid and chondroitin sulfate were main constituents of mesothelioma GAG. However, there was no significant difference in the content of dermatan sulfate and heparan sulfate between this neoplasm and adenocarcino

[Prognostic value of positive cytology found in pleural lavage of patients with cancer of the lung. Prospective study].

Pulmonary neoplasms can deposit malignant cells in the pleural cavity by a variety of mechanisms, depending not only on histological type but also on clinical stage. We investigated the effect on survival rate of a finding of malignant cells in pleural lavage. We also investigated the effect of the presence of pleural metastasis on postoperative course of disease. Two hundred surgical patients at Hospital General Universitario in Valencia between 1 February 1990 and 30 March 1993 were studied. Two groups were formed. Group one: 150 patients with lung cancer, none of whom had had pleural effusi

Ventilatory failure due to asbestos pleurisy.

Seven patients are described who had a distinctive syndrome of chest wall restriction caused by asbestos-induced pleural fibrosis. All had severe dyspnea and predominant pleural disease on radiographic examination, with pulmonary function findings of reduced vital capacity, total lung capacity (measured in five patients), and maximal voluntary ventilation. Five patients had ventilatory failure with carbon dioxide retention; four of these have died and one is close to death. Examination of the thoracic organs in five patients showed minimal or no parenchymal fibrosis in three and less severe in

Non-asbestos-related malignant pleural mesothelioma.

Malignant pleural mesothelioma (MPM) is an uncommon tumor derived from mesothelial lining cells. MPM has been described as an insidious neoplasm because of its long latency period. The tumor is typically found in patients several decades after asbestos exposure. We herein describe a 26-year-old patient with MPM who presented with pleural effusion. The patient had not been exposed to asbestos or erionite. There are few case reports of non-asbestos-related MPM in young patients. We report this case to remind physicians to consider MPM in the differential diagnosis of pleural effusion in young pa

Extranodal interdigitating dendritic cell sarcoma presenting in the pleura: a case report.

Interdigitating dendritic cell sarcoma (IDCS) is an extremely rare neoplasm arising from the antigen-presenting cells of the immune system. This disease usually involves the lymph nodes, and rarely, extranodal sites may be affected. The authors report a case of extranodal IDCS presenting in the pleura. A 32-yr-old man presented with progressive chest pain. Imaging studies showed diffuse pleural thickening with pleural effusion. Morphological and immunohistochemical analysis of an incisional biopsy of the pleura were consistent with a diagnosis of IDCS; tumor cells were positive for S100 and CD

Incidentally detected multifocal calcifying fibrous tumour of the pleura: a diagnostic enigma.

Calcifying fibrous tumour of the pleura (CFTP) is a rare, benign soft tissue neoplasm with uncertain aetiology. To our knowledge, only 36 cases (including this case) have been reported in English literature. A man in his early 30s presented with an incidental pleural lesion identified following investigation for a persistent cough. Imaging revealed left pleural thickening with calcification, raising concern for a possible malignant pleural process. CT-guided biopsy demonstrated fibrosis without malignancy, and a video-assisted thoracoscopic surgery excision was performed. Histopathology confir

Recurrent Pneumothorax and Pleural Effusion: A Case Report].

Well-differentiated papillary mesothelial tumor (WDPMT) is a rare neoplasm characterized by a distinct papillary architecture, bland cytological features, and a propensity for superficial spread without stromal invasion. We herein report the diagnosis and management of a patient with pleural WDPMT presenting predominantly with recurrent spontaneous pneumothorax and pleural effusion, along with a review of the current literature on this entity. The patient was a 58-year-old female who presented with a two-week history of chest tightness and dyspnea on a background of recurrent spontaneous pneum

Multidetector CT for Evaluation of the Extrapleural Space.

The extrapleural space (EPS) is an anatomic space at the periphery of the chest that can be involved in a number of disease processes. This space lies between the inner surface of the ribs and the parietal pleura and contains adipose tissue, loose connective tissue, lymph nodes, vessels, endothoracic fascia, and the innermost intercostal muscle. It is often overlooked on cross-sectional imaging studies and almost invariably overlooked on conventional radiographic studies. At conventional radiography, the EPS occasionally can be seen when there is extrapleural fat proliferation, which might be

A case of primary pleural epithelioid hemangioendothelioma achieving stable disease with paclitaxel treatment: A case report and literature review.

Epithelioid hemangioendothelioma (EHE) is a rare vascular neoplasm with a clinical behaviour that falls between a benign hemangioma and a high-grade angiosarcoma. Pleural EHE is exceptionally rare, and its prognosis is grim, with most patients experiencing survival of less than 1 year. Here, we present a case of pleural EHE in a 45-year-old woman with a month-long history of right-sided pleuritic chest pain. Chest computed tomography revealed consolidation, atelectasis of the right lung, right pleural thickening, and pleural effusion. She underwent video-assisted thoracoscopic surgery for deco

[Pleural effusion: diagnostic usefulness of pleural puncture biopsy].

To assess the diagnostic usefulness of thoracocentesis with pleural needle biopsies, we retrospectively studied 316 procedures performed in 254 patients between 1977 and 1984. Of these, 130 were ultimately found to have pleural malignant disease, with a diagnostic cytologic study in 60% of the patients, a positive pleural biopsy in 52.30% and both methods combined in 81.53% of the patients. The marginal gains from pleural biopsy in the presence of negative cytology results were 63.46%. In 59 patients, the primary neoplasm was lung cancer, the most frequent tumor (45.30%); in second place, 30 p

Cytological features of breast implant-associated anaplastic large cell lymphoma in pleural effusion.

Breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) is a very rare CD30-positive ALK-negative T-cell non-Hodgkin lymphoma included as a provisional entity in the 2017 WHO classification of lymphoid neoplasms. BIA-ALCL arises as proliferating cells over the surface of the implant. It is generally an indolent disease if confined within the fibrous capsule. In contrast, mass and/or infiltration beyond the capsule is much more aggressive. This report describes a case of infiltrative BIA-ALCL with massive pleural effusion containing hallmark BIA-ALCL cells showing the characteristic

Pleural Pseudomyxoma Secondary to Appendiceal Mucinous Adenocarcinoma: A Case Report.

INTRODUCTION: Appendiceal mucinous neoplasms are uncommon gastrointestinal tumors. In some cases, they evolve into pseudomyxoma peritonei (PMP), a condition characterized by mucinous ascites and peritoneal dissemination. Treatment typically requires comprehensive cytoreductive surgery (CRS), commonly combined with hyperthermic intraperitoneal chemotherapy (HIPEC). While PMP usually remains confined to the abdominal cavity, extra-abdominal spread is exceptional. Pleural involvement, often described as pleural pseudomyxoma, has been reported only rarely. CASE PRESENTATION: We report the case of

Progress in Mechanistic Research and the Use of Traditional Chinese Medicine in Treating Malignant Pleural Effusion.

Malignant pleural effusion (MPE) is characterized by the accumulation of fluid in the chest cavity, secondary to metastasis from a primary pleural tumor or malignant neoplasms originating from other anatomical sites. MPE is associated with a poor prognosis. Consequently, timely and effective prevention and management of MPE are critical. In Western medicine, the treatment of MPE primarily involves procedures such as surgical puncture for drainage, pleural fixation, chemotherapy, and targeted therapy. In contrast, traditional Chinese medicine (TCM) offers therapeutic modalities including oral d
