The brains of 3 adult subjects suffering from normotensive hydrocephalus have been examined pathologically. The diagnosis of normotensive hydrocephalus was based on clinical symptoms, pneumoencephalography and isotope cisternography, in 1 case integrated with the results of the constant-infusion manometric test. Part of the neuropathological findings were common to the 3 patients: leptomeningeal non-obstructive fibrosis, ventricular ependymal disruption, subependymal glial reaction, periventricular demyelination and spongiosis. We describe the unusual association of two ocular motor abnormalities: upbeat nystagmus and internuclear ophthalmoplegia in a young woman originally believed to have demyelinating disease. Quantitative eye movement recordings documented a unidirectional defect in upward visual pursuit with preservation of the vertical vestibulo-ocular reflexes. A definition of "Hydrocephalus not completely arrested" is not easy to be given. Cerebral pneumography represents the most detailed neuroradiological examination. In spite of this, neither its analysis nor its measurements bring definite criteria for the favorable moment of locating a shunting system in that case. These criteria are reviewed systemetically. The clinical features in 268 patients with diffuse cerebral atrophy of initially unknown origin have been analysed. Seven patients with congenital generalized lipodystrophy and one patient with acquired generalized lipodystrophy have been followed up for 38 years. Various radiological techniques have been used, and the major findings are summarized. All patients had an advanced skeletal age. A sclerotic skeleton was a major feature in childhood in the congenital form, later turning into osteolytic lesions or more distinct sclerotic patches in adolescence or early adulthood. Comparative ventricular measurements were made on 43 small children and infants between the ages of 0 and 3 years, the methods used being those of pneumoencephalography and enchoventriculography. The number of cases that could be analyzed was 31. OBJECTIVE: To avoid misdiagnosis and misapplied therapy of acoustic neuroma.
METHODS: 92 cases of acoustic neuroma treated between 1983 and 1997 were retrospectively reviewed. Among them, 20 patients (21 ears, 21.7%) presented with sudden hearing loss as the starting symptoms. The diagnosis was based on audiological and radiologic examinations.
RESULTS: The clinical and audiological analysis demonstrated that 57.1% of the ears showed hearing loss over 71 dBHL, and all ears had deteriorated ABRs. Acoustic reflex was nonreactive in all 10 ears tested. Positive findings in CT scan were 88.89%.
