An integrated multi-omics approach for deciphering podophyllotoxin-induced nephrotoxicity via the microbiome-gut-kidney (MGK) axis based on the toxicological evidence (TEC) concept.

Podophyllotoxin (PPT), a potent antitumor natural lignan, is clinically limited by nephrotoxicity, with the microbiome-gut-kidney (MGK) axis's role unclear. Guided by the Toxicological Evidence Chain (TEC) framework, this study used multi-omics approaches in ICR mice (control, low/high-dose PPT) to explore mechanisms. The physiological indices of mice, health status, renal pathological changes, injury and proinflammatory biomarkers, gut microbiota, metabolism, and transcriptional changes were determined to collect toxicity evidence. Besides, an integrated multi-omics approach, including 16S rR

Liquid chromatography-high-resolution mass spectrometry-based integrated strategy combining in silico prediction and molecular networking for metabolite identification of podophyllotoxin in a fatal poisoning case.

Podophyllotoxin is an aryltetralin lignan with potent antimitotic and antiviral activities; however, its severe toxicity restricts clinical use to topical application, and systemic exposure to it can result in life-threatening poisoning. However, its metabolic profile remains largely uncharacterized, leaving a critical gap in the biomarkers available for confirming intoxication. Here, we used an integrated nontargeted screening strategy to investigate podophyllotoxin metabolism in post-mortem blood from a rare fatal case of podophyllotoxin poisoning. Two complementary nontargeted screening str

Podophyllotoxin-induced acute kidney injury via the HMGB1/TLR4/MyD88/NF-κB axis in SD rats based on the toxicological evidence chain (TEC) concept via multiomic analysis.

BACKGROUND: Podophyllotoxin (PPT) is a compound of significant clinical interest because of its anticancer properties; however, its systemic toxicity, particularly its nephrotoxicity, has not yet been fully elucidated. METHODS: Utilizing the toxicological evidence chain (TEC) approach, our team developed a rat model to investigate podophyllotoxin-induced acute kidney injury (AKI). Serum biochemistry and histopathology were used to assess systemic and renal toxicity. To explore underlying mechanisms, we performed 16S rRNA sequencing, Raman spectroscopy, and applied proteomic and metabolomic pr

Podophyllotoxin mediates hepatic toxicity via the C5a/C5aR/ROS/NLRP3 and cGMP/PKG/mTOR axis in rats based on toxicological evidence chain (TEC) concept by phosphoproteomic analysis.

BACKGROUND: Podophyllotoxin (PPT), a highly active compound extracted from the rhizome of Dysosma versipellis (DV), has been used as an effective anti-cancer drug clinically since the 1950s. It possesses various biological activities, including antiviral and antitumor effects. However, its clinical application is severely limited due to its hepatotoxicity, and the underlying mechanisms remain unclear. This study aims to elucidate the mechanisms of PPT-induced hepatotoxicity using tandem quality tag (TMT) based quantitative proteomics and phosphoproteomics, providing potential targets and direc

Cutaneous cytodestructive potency of lignans. I. A comparative evaluation of influence on epidermal and dermal DNA synthesis and on dermal microcirculation in the hairless mouse.

The four lignans: podophyllotoxin, 4'-demethyl-podophyllotoxin, alpha-peltatin and beta-peltatin represent the podophyllin ingredients believed to exert lesional necrosis after application to condylomata acuminata. This study comparatively evaluates cutaneous cytodestructive potency of these drugs, as evaluated by measurement of epidermal and dermal 3H-thymidine incorporation and injury of dermal microcirculation at intervals in the range of 9--168 h after application to hairless mouse skin. Influence on DNA-synthesis was measured using a disc technique for isolation of standardized skin areas

Quercetin and kaempherol: an argument against the use of podophyllin?

INTRODUCTION: Topical application of podophyllin is a routine procedure in patients with ano-genital warts. Podophyllin is a crude plant extract and is therefore not a well-defined product. It may contain variable amounts of the active lignan podophyllotoxin and the majority of the dry weight of podophyllin is made up of substances never identified. OBJECTIVE: The purpose of the study was to estimate in podophyllin 20% the amounts of two mutagenic substances, quercetin and kaempherol. METHODS: Using high-pressure liquid chromatography the amounts of quercetin and kaempherol were determined i

Health economic evaluation of topical treatments for external anogenital warts.

INTRODUCTION: No treatment hierarchy for external anogenital warts (AGW) is currently recommended, despite wide variations in the costs of available treatments. The aim of this study was to propose a treatment hierarchy based on a health economic analysis of local treatments for AGW from the perspective of the French health insurance system. METHODS: Thirteen treatments and 73 treatment sequences were evaluated for AGW clearance and absence of AGW recurrence at 3 months of follow-up. The cost per treatment included the cost of consultations, drugs, medical procedures, and dressings. The time

[Interest of lignans in prevention and treatment of cancers].

Lignans are diphenolic compounds widely distributed in the plant kingdom. They are mainly localised in lignified tissues, seeds and roots. These molecules are involved in plant defence mechanisms, but are also interesting for human health. Flax lignans belonging to the phytoestrogens are metabolised after ingestion into enterolignans that may offer a protection against the onset and development of hormono-dependant cancers. In vitro studies based on mammalian cellular models tend to confirm their beneficial effects observed during epidemiological studies and give us insights about their mechan

Dysosma versipellis is associated with cholestatic hepatotoxicity potentially involving FXR-SHP-CYP7A1/BSEP-MRP2 axis disruption: A multi-omics-driven mechanistic investigation based on the toxicological evidence chain (TEC) concept.

Dysosma versipellis (DV), despite its broad-spectrum antitumor potential, faces clinical limitations due to cholestatic hepatotoxicity, prompting investigation into its mechanisms and environmental health implications. Using a toxicological evidence chain (TEC) approach in Sprague–Dawley rats administered 1.2 g/kg DV ethanol extract for 3 days, integrated analyses revealed characteristic cholestatic injury, including significantly reduced liver weight (P < 0.01), elevated serum biomarkers (ALT, AST, TBA; P < 0.01), and bile duct hyperplasia. Toxicokinetic profiling identified five hepatotoxic

Podophyllin, but not the constituents quercetin or kaempferol, induced genotoxicity in vitro and in vivo through ROS production.

The genotoxic potential of podophyllin (PD) was investigated in this study. PD increased bacterial revertants and abnormal chromosomal structures in a concentration-dependent manner, both with and without metabolic activating enzymes, and increased the incidence of micronuclei in imprinted control region mouse reticulocytes. Results from three studied constituents of PD, such as podophyllotoxin, kampferol, and quercetin, suggested that the mutagenic effect of PD was not due to the presence of podophyllotoxin, kampferol, and quercetin and might be related to other components and the formation o

LAV694, a new antiproliferative agent showing improved skin tolerability vs. clinical standards for the treatment of actinic keratosis.

The skin tolerability of the tubulin polymerisation inhibitor LAV694 was compared to that of 5% 5-fluorouracil (5-FU) and 0.5% podophyllotoxin in vitro using a human reconstructed epidermis (HRE), and in vivo using minipigs. Topical treatment of HRE for 1 or 3 days with a 0.2, 0.6 or 1% LAV694 cream or the placebo showed no signs of irritation in terms of morphology, cell viability (lactate dehydrogenase leakage) or interleukin-8 mRNA expression and release. 5-FU increased interleukin-8 production and induced morphological signs of irritation. The substances were also applied under occlusion t

Recalcitrant extragenital giant condyloma acuminatum: A need for combination therapy.

Giant condyloma acuminatum (GCA), also known as Buschke -Löwenstein tumor, is a huge, rapidly growing variant of condyloma found in the anogenital region. Extragenital GCA is rare and, in most cases, affecting the intertriginous area and treated by excision. We described a case of refractory extragenital GCA involving the large part of the left axilla area successfully treated with immunotherapy and podophyllotoxin. Considering the large size and the location, the lesions were not treated by wide excision because it might result in scar contracture with limitation of shoulder movement or axill

Fungal colonization and candidate metabolite-mediated toxicity of Fusarium solani KMZW-1 against adult Bactrocera dorsalis.

This study examined the interaction between Fusarium solani KMZW-1 and adult Bactrocera dorsalis, with emphasis on cuticular colonization, adult-stage pathological effects, and candidate metabolite-mediated toxicity. KMZW-1 showed Fusarium-like colony and conidial morphology and colonized mainly tarsal setae, joints, and recessed cuticular regions, where fungal conidia and hyphae progressively accumulated over time. Fungal treatment reduced adult survival in a concentration-dependent manner (P < 0.001) and lowered egg production per female, likely due to reduced male survival and fewer mating

A cross-sectional survey of treatment choices for anogenital warts.

This was a cross-sectional survey that collected data relating to management of anogenital warts (AGW) during a single-patient visit only at genitourinary medicine clinics. Single-agent use of cryotherapy, podophyllotoxin and trichloroacetic acid (TCA) were the most common treatment modalities, accounting for over two-thirds of all modalities used. Podophyllin, alone or in combination with other agents, was used for about 20% of first-line treatments. Podophyllin was included in about 15% of all treatment modalities. Guidelines for the management of AGW continue to recommend the use of podophy

Radioprotective and antioxidant properties of low-altitude Podophyllum hexandrum (LAPH).

The development of nontoxic yet effective radioprotectors is needed because of the increasing risk of human exposure to ionizing radiation. We have reported that high-altitude Podophyllum hexandrum (HAPH) confers a radioprotective effect in in vitro and in vivo models. The present study reports on the antioxidant and radioprotective properties of low-altitude Podophyllum hexandrum (LAPH), from which the toxic compound podophyllotoxin has been partially removed during the extraction process. Using HPLC,we estimated the relative content of two marker compounds, podophyllotoxin and podophyllotoxi

Patients with genital warts: how are they managed by general practitioners?

This study was carried out to assess the management by general practitioners of patients with genital warts. An anonymized postal questionnaire was sent to 2060 general practitioners in the north west of England; 1260 (61.2%) completed questionnaires were returned. A total of 76.69% of GPs saw one or more cases per quarter. Of 468 (37.1%) GPs who managed patients in-house, 393 (83.9%) used podophyllin of concentration between 0.5 and 50%; 169 (43%) did not specify the podophyllin concentration and 112(23.9%) used podophyllotoxin. Of 395 GPs (31.3%) prescribing patient self-treatment, 259 (65.6
