Effects of positive end-expiratory pressure on intracranial pressure in dogs with intracranial hypertension.

Positive end-expiratory pressure (PEEP) is used to improve oxygenation in patients with the adult respiratory distress syndrome. Nevertheless, this treatment may increase intracranial pressure (ICP) and be detrimental to certain neurosurgical patients. This clinical situation was simulated by administering PEEP to dogs with normal and elevated ICP. Increases in PEEP increased ICP in all animals. However, the presence of intracranial hypertension diminished the increase in ICP seen at a given level of PEEP. Cerebral perfusion pressure also fell less in the presence of intracranial hypertension

THE STATUS OF RESPIRATION IN THE METHODS OF DIFFERENTIAL PRESSURE COMPARED WITH THAT UNDER THE METHOD OF INTRATRACHEAL INSUFFLATION.

The maintenance of life of an individual with an open double pneumothorax under differential pressure depends essentially upon the normal position of the lower lobes of the lungs and their close approximation to the diaphragm, especially of the posterior parts of the lobes. A complete dislodgment of both lower lobes leads invariably to the death of the individual, which may occur in a very short time, or after fifteen to twenty-five minutes. In all cases the respiration is affected first; it slows almost at once and stops invariably before the heart. The result is the same whether the vagi are

A rare case of tension pneumocephalus after head trauma.

OBJECTIVES: We present a rare case of tension pneumocephalus (TP) after head trauma not involving mask ventilation but based on pathological respiration pattern, kussmaul breathing. TP is rare condition and exceptionally rare when positive airway pressure has not been applied. In this particular case, the vacuum and one-valve intracranial effects causing pneumocephalon were instead accentuated by a pathological pattern of respiration due to severe ketoacidosis - a condition not previously reported in the literature. METHODS: This is a case report showing a rare cause of pneumocephalon caused

Airway occlusion pressure to titrate positive end-expiratory pressure in patients with dynamic hyperinflation.

BACKGROUND: Although the use of external positive end-expiratory pressure (PEEP) is recommended for patients with intrinsic PEEP, no simple method exists for bedside titration. We hypothesized that the occlusion pressure, measured from airway pressure during the phase of ventilator triggering (P0.1t), could help to indicate the effects of PEEP on the work of breathing (WOB). METHODS: Twenty patients under assisted ventilation with chronic obstructive pulmonary disease were studied with 0, 5, and 10 cm H2O of PEEP while ventilated with a fixed level of pressure support. RESULTS: PEEP 5 signif

Spontaneous breathing with continuous positive airway pressure in adult respiratory distress syndrome.

The authors describe three systems of spontaneous respiration with positive end expiratory pressure (P.E.E.P.) : the system of continuous positive airway pressure (C.P.A.P.), the bubble and the intermittent mandatory ventilation with C.P.A.P. (I.M.V.-C.P.A.P.) by Servo-Ventilator 900 B. The results are conclusive with the C.P.A.P. unit used for post-traumatic adult patients in acute respiratory distress and for patients who have been anesthetized for more than three hours. The PaO2 significantly increase in subjects whose PaO2 in the ambient air without P.E.E.P. is lower than 70 mm Hg. The bub

Use of high frequency jet ventilation during mechanical hyperventilation to reduce intracranial pressure in patients with multiple organ system injury.

Eleven patients with multiple organ system injury, including significant closed head injury, all required positive end-expiratory pressure (PEEP) for treatment of their pulmonary pathological condition. Additionally, the need for intracranial pressure (ICP) monitoring had previously been established on clinical evaluation by the Neurosurgery Service. Seven of the 11 patients met the criteria for invasive hemodynamic monitoring. Hemodynamic monitoring data are supplied for these 7 patients. All patients, after the initial institution of conventional means of hyperventilation, were transitioned

Acute Respiratory Distress Syndrome.

Acute respiratory distress syndrome (ARDS) occurs in up to 10% of patients with respiratory failure admitted through the emergency department. Use of noninvasive respiratory support has proliferated in recent years; clinicians must understand the relative merits and risks of these technologies and know how to recognize signs of failure. The cornerstone of ARDS care of the mechanically ventilated patient is low-tidal volume ventilation based on ideal body weight. Adjunctive therapies, such as prone positioning and neuromuscular blockade, may have a role in the emergency department management of

New parachute cuff and positive end-expiratory pressure to minimize tracheal injury and prevent aspiration.

A new parachute cuff has been tested in combination with a positive end-expiratory pressure (PEEP) on mongrel dogs. During positive-pressure ventilation the intracuff and intratracheal pressures showed synchronous, identical pressure variations, and therefore theoretically with this type of cuff the pressure against the tracheal wall would be minimal. The cuff provided a seal against gas leakage from the lungs throughout the entire test period, i.e., for up to 7 h. To avoid aspiration of mouth contents during the passive exhalation phase, different amounts of PEEP were tested. A PEEP of 4.0--8

Tracheal pressure generated by high-flow nasal cannula in 3D-Printed pediatric airway models.

OBJECTIVE: Heated and humidified high flow nasal cannula (HFNC) is an increasingly used form of noninvasive respiratory support with the potential to generate significant tracheal pressure. The aim of this study was to quantify the pressure generated by HFNC within the trachea in anatomically correct, pediatric airway models. METHODS: 3D-printed upper airway models of a preterm neonate, term neonate, toddler, and small child were connected to a spontaneous breathing computerized lung model at age-appropriate ventilation settings. Two commercially available HFNC systems were applied to each ai

Bilevel positive airway pressure worsens central apneas during sleep.

STUDY OBJECTIVES: While most patients with sleep-disordered breathing are treated with continuous positive airway pressure (CPAP), bilevel positive airway pressure (BLPAP) is often used. Having observed that BLPAP therapy increased central apneas in some of our patients undergoing sleep studies, we conducted this study to evaluate the effects of BLPAP. DESIGN: Retrospective analysis of all sleep studies performed in an outpatient sleep center that used BLPAP over a 2-year period. We assessed the incidence and frequency of events during rapid eye movement (REM) sleep and non-REM sleep during b

Estimation of transpulmonary driving pressure during synchronized mechanical ventilation using a single lower assist maneuver (LAM) in rabbits: a comparison to measurements made with an esophageal balloon.

BACKGROUND: Mechanical ventilation is applied to unload the respiratory muscles, but knowledge about transpulmonary driving pressure (ΔPL) is important to minimize lung injury. We propose a method to estimate ΔPL during neurally synchronized assisted ventilation, with a simple intervention of lowering the assist for one breath ("lower assist maneuver", LAM). METHODS: In 24 rabbits breathing spontaneously with imposed loads, titrations of increasing assist were performed, with two neurally synchronized modes: neurally adjusted ventilatory assist (NAVA) and neurally triggered pressure support (

Expiratory Pause Maneuver to Assess Inspiratory Muscle Pressure During Assisted Mechanical Ventilation: A Bench Study.

BACKGROUND: The generation of excessive inspiratory muscle pressure (Pmus) during assisted mechanical ventilation in patients with respiratory failure may result in acute respiratory muscle injury and/or fatigue, and exacerbate ventilator-induced lung injury. A readily available noninvasive surrogate measure of Pmus may help in titrating both mechanical ventilation and sedation to minimize these risks. This bench study explored the feasibility and accuracy of using a ventilator's expiratory pause hold function to measure Pmus across multiple operators. METHODS: A standardized technique for ex

Mechanical Ventilation After Bidirectional Superior Cavopulmonary Anastomosis for Single-Ventricle Physiology: A Comparison of Pressure Support Ventilation and Neurally Adjusted Ventilatory Assist.

We evaluated the effects of different respiratory assist modes on cerebral blood flow (CBF) and arterial oxygenation in single-ventricle patients after bidirectional superior cavopulmonary anastomosis (BCPA). We hypothesized that preserved auto-regulation of respiration during neurally adjusted ventilatory assist (NAVA) may have potential advantages for CBF and pulmonary blood flow regulation after the BCPA procedure. We enrolled 23 patients scheduled for BCPA, who underwent pressure-controlled ventilation (PCV), pressure support ventilation (PSV), and NAVA at two assist levels for all modes i

Cheyne-Stokes respiration during sleep in congestive heart failure.

Cheyne-Stokes respiration (CSR) is a form of sleep-disordered breathing seen in approximately 40% of congestive heart failure patients with a left ventricular ejection fraction of < 40%. It is characterized by a crescendo-decrescendo alteration in tidal volume separated by periods of apnea or hypopnea. Sleep is generally disrupted, often with frequent nocturnal arousals. Clinical features include excessive daytime sleepiness, paroxysmal nocturnal dyspnea, insomnia, and snoring. Proposed mechanisms include the following: (1) an increased CNS sensitivity to changes in arterial PCO2 and PO2 (incr
