INTRODUCTION: Cardiogenic shock and cardiac arrest are associated with high risk of mortality despite advances in resuscitation techniques and supportive care. A growing body of evidence implicates a systemic inflammatory response syndrome (SIRS) in the pathogenesis of post-arrest vasoplegia and end-organ injury. Methylene blue (MB), a nitric oxide pathway inhibitor, has shown efficacy in vasoplegic and septic shock, yet its role in cardiogenic shock or cardiac arrest is not well established.
METHODS: We conducted a systematic review of peer-reviewed studies examining methylene blue in cardio

Systemic inflammation is a major feature of the post-cardiac arrest syndrome. The three monocyte subpopulations are thought to play an important role in this inflammatory state because they are endowed with numerous pattern recognition receptors, such as CD14, that have been associated with ischemia-reperfusion injury. By contrast, an exaggerated antiinflammatory response has also been described following cardiac arrest, which may be mediated by downregulation of antigen presentation receptor HLA-DR. We report the composition of monocyte subpopulations and the expression of CD14 and HLA-DR fol

BACKGROUND: Long hospital stays for neonates following cardiac surgery can be detrimental to short- and long-term outcomes. Furthermore, it can impact resource allocation within heart centres' daily operations. We aimed to explore multiple clinical variables and complications that can influence and predict the post-operative hospital length of stay.
METHODS: We conducted a retrospective observational review of the full-term neonates (<30 days old) who had cardiac surgery in a tertiary paediatric cardiac surgery centre - assessment of multiple clinical variables and their association with post

Neurological prognostication in cardiac arrest survivors is difficult, especially when the primary etiology is respiratory arrest. Prognostic factors designed to have zero false-positive rates to robustly confirm poor outcome are usually inadequate to rule out poor outcomes (i.e., high specificity and low sensitivity). One of the least understood prognosticators is generalised status myoclonus (GSM), with case reports confusing GSM, isolated myoclonic jerks and post-hypoxic intention myoclonus (Lance Adams syndrome [LAS]). With several prognostic indicators (including status myoclonus) having

Chronic post-hypoxic myoclonus, also known as Lance-Adams syndrome (LAS) is a neurological complication characterized by uncontrolled myoclonic jerks following cardiac arrest. In this article, clinical manifestation and symptomatic treatment options are discussed especially concerning the rationale of use of levatiracetam in patients with Lance-Adams syndrome. Clinical presentation is action myoclonus associated with cerebellar ataxia, postural imbalance, and very mild intellectual deficit. An 18-year-old female patient was admitted to our intensive care unit in a coma. She had a cardiorespira

Tako-tsubo transient cardiomyopathy (TCM) is a syndrome of specific myocardial wall motion abnormalities that appear in response to extreme physiological or emotional stress. The patients have normal coronary arteries and usually present as new-onset heart failure or acute coronary syndrome. We report on a 48-year-old woman with post-community cardiac arrest presenting as an acute myocardial infarction. An emergency physician–performed echocardiogram showed significant wall motion abnormalities, suggesting myocardial infarction. Cardiac catheterization revealed patent epicoronary circulation,

Although cardiac arrests are common, most survivors do not develop post-hypoxic myoclonus (Lance-Adams syndrome, LAS), suggesting that hypoxia alone may be insufficient. We retrospectively identified LAS cases from electronic records and video archives (2000-2025) and reviewed the literature. Among 17 patients, 12 had peri-event blood gases, all showing severe acidemia (mean pH, 7.08), with respiratory acidosis in seven and metabolic acidosis in five; the remaining five had respiratory failure. In published reports, respiratory events commonly preceded LAS. Systemic acidemia associated with re

BACKGROUND AND PURPOSE: Cerebrovascular autoregulation (CVAR) is impaired following cardiac arrest. We describe near-infrared spectroscopy (NIRS)-derived CVAR parameters following cardiac arrest and characterize their evolution over time.
METHODS: This was a retrospective analysis of a prospectively collected clinical and physiological cardiac arrest data repository at a single academic medical center between March 2022 and October 2023. Eligible patients were connected to neuromonitoring within 72 h of cardiac arrest, had greater than four hours of monitoring, and had no confounding neurolog

AIM: The objective of this study was to determine whether survival and post-arrest procedural utilization following in-hospital cardiac arrest (IHCA) differ in patients with and without comorbid cancer.
METHODS: We retrospectively reviewed all adult (age ≥18 years old) hospital admissions complicated by IHCA from 2003 to 2014 using the National Inpatient Sample (NIS) dataset. Utilizing propensity score matching using age, gender, race, insurance, all hospital level variables, HCUP mortality score, diabetes, hypertension and cardiopulmonary resuscitation use, rates of survival to hospital disc

Lance-Adams syndrome (chronic post-hypoxic myoclonus) is a rare syndrome occurring in patients after cardiopulmonary resuscitation. Awareness of this condition is important to distinguish it from myoclonic status epilepticus, which is a poor prognostic sign. We present the case of a 32-year-old woman who developed Lance-Adams syndrome after an episode of hypoxic cardiac arrest from severe pneumonia. Brain computed tomography, magnetic resonance imaging, and an electroencephalogram were used to rule out other causes of myoclonus. In this report, we discuss the diagnosis, treatment, and prognosi

CLINICAL INTRODUCTION: A 50-year-old woman presented with an inferoposterior ST-elevation myocardial infarction (STEMI) and underwent emergency percutaneous coronary intervention (PCI). Angiography revealed acute occlusion of the circumflex and right coronary (RCA) arteries. PCI was uncomplicated. Her medical history included asthma, hypertension and chronic sinusitis.Three months later, she presented with a non-STEMI (NSTEMI), and angiogram showed a new focal stenosis in the left anterior descending artery. Pressure wire assessment induced severe coronary spasm. After liberal intracoronary ni

INTRODUCTION: Brugada syndrome is an inherited channelopathy characterized by arrhythmia and an increased risk of sudden cardiac death (SCD). Implantation of a defibrillator for primary or secondary prevention is the only effective strategy to decrease the risk of SCD in Brugada syndrome. We present a case in which a cardiac donor had a pathogenic variant for Brugada syndrome, discovered on genetic testing after transplantation.
CASE REPORT: A young child with dilated cardiomyopathy underwent orthotopic heart transplantation from a donor with in-hospital cardiac arrest in the context of fever

Pioneer works on therapeutic hypothermia (TH) half a century ago already showed promising results but clinical application was limited by a lack of understanding of the underlying pathophysiology, lack of reliable method for temperature control and lack of intensive care facilities to deal with possible complications. More recently, 2 studies in 2002 supported the application of moderate TH (32.0-34.0℃) in post-cardiac arrest patients. Although the studies included only patients suffering from out-of-hospital VF, many ICUs world-wide are applying the therapy to all post-cardiac arrest patients

It is not common for a patient who survives cardiac arrest to experience significant neurologic impairment such as acute and chronic post-hypoxic myoclonus, known as Lance-Adams syndrome. This syndrome is predominantly characterized by myoclonus that starts days to weeks after cardiopulmonary resuscitation in patients who regained consciousness. Although several cases of LAS were reported, the decisive treatment method has not been established. We report a 43 year old man with Lance-Adams syndrome who showed long-term improvement through treatment with anti-myoclonic agents and participation i

BACKGROUND: Besides temperature management, there is currently no effective therapy to decrease the burden of post-cardiac arrest syndrome. The pleiotropic effects of vitamin C may improve clinical outcome.
PURPOSE: This trial investigated whether early administration of 3 g or 10 g intravenous vitamin C attenuated organ dysfunction post-cardiac arrest.
METHODS: In this double-blind, multi-center, phase 2 trial, comatose adults resuscitated from shockable out-of-hospital cardiac arrest randomly received intravenously placebo, supplementary (3 g) or supraphysiological dose (10 g) vitamin C da

Long term survival of successfully resuscitated patients is primarily determined by their post-cardiac arrest neurological function. If the patient undergoes a long-term resuscitation or remains comatose as part of the post-cardiac arrest syndrome (PCAS), organ-specific intensive care is urged to aim hemodynamic stabilisation, normalisation of organ perfusion and prevention of injuries at cellular level. One of the basic measures of PCAS intensive care is to prevent hypoxic brain injury by mild therapeutic hypothermia (THT). The physiological changes of the human body at hypothermic conditions

Severe hypoxic-ischemic brain injury following cardiac arrest can result in persistent and evolving neurologic, cognitive, and visual impairment, with later clinical decline that may be difficult to distinguish from superimposed neurodegenerative disease. A prolonged and evolving pattern of neurologic, cognitive, and visual impairment is documented in a woman throughout her 70s and 80s who survived a perioperative cardiac arrest requiring an extended resuscitation interval. Her recovery was complicated by Anton syndrome, prosopagnosia, gait apraxia, chronic microvascular disease with progressi

BACKGROUND: Diffuse brain injury is a key component of post-cardiac arrest syndrome reported in 30-80% of survivors of out-of-hospital cardiac arrest (OHCA). It is responsible for a high mortality rate, and is a common cause of cognitive and neurological deficits and disability. Symptom variability and dynamics and the rehabilitation potential remain poorly understood.
AIM: To investigate symptom prevalence, type, and severity and the natural course of recovery within 12 months after OHCA, and to estimate neurorehabilitation needs.
METHODS: Study participants were selected from OHCA survivor
