Bartonella henselae infection inducing hemophagocytic lymphohistiocytosis in a kidney transplant recipient.

Bartonella henselae (Bh) is the cause of cat-scratch fever. When infection is symptomatic, it typically presents with singular lymphadenitis and fever. Less commonly, the infection can become disseminated and cause endocarditis, osteomyelitis, and micro-abscesses in multiple sites including liver, spleen, eyes, and brain, especially in immunocompromised patients. Hemophagocytic lymphohistiocytosis (Hlh) is a rare and severe multisystem disorder that may be triggered by infections. In one prior case, Bh, like other infections, has induced Hlh, an immune-mediated disease that can be characterize

Current understanding of pathogenesis of functional dyspepsia.

Functional dyspepsia (FD) is a disorder in which upper abdominal symptoms occur in the absence of organic disease that explains them. Many pathogenic factors have been proposed for FD, including motility abnormalities, visceral hypersensitivity, psychosocial factors, excessive gastric acid secretion, Helicobacter pylori, genetics, environment, diet, lifestyle, and post-infectious FD. Many of those pathogenic factors are also common to irritable bowel syndrome and other functional gastrointestinal disorders, so understanding FD offers a glimpse into the nature of functional gastrointestinal dis

The promise of a prophylactic Epstein-Barr virus vaccine.

The worldwide burden of disease due to Epstein-Barr virus (EBV) infection is enormous. Diseases include endemic Burkitt lymphoma, infectious mononucleosis, cancers after transplantation, Hodgkin lymphoma, and nasopharyngeal carcinoma. A prophylactic EBV vaccine has the potential to significantly reduce the incidence and/or the severity of all these diseases. Infectious mononucleosis can be nasty and prolonged with a median duration of 17 days. Patients, especially children, undergoing bone marrow or solid organ transplantation may develop post-transplant lymphoproliferative disorder (PTLD). Pr

Admission of foreign citizens to the general teaching hospital of Bologna, northeastern Italy: an epidemiological and clinical survey.

BACKGROUND: The emergency regarding recent immigration waves into Italy makes continued healthcare monitoring of these populations necessary. METHODS: Through a survey of hospital admissions carried out during the last five years at the S. Orsola-Malpighi General Hospital of Bologna (Italy), all causes of admission of these subjects were evaluated, together with their correlates. Subsequently, we focused on admissions due to infectious diseases. All available data regarding foreign citizens admitted as inpatients or in Day-Hospital settings of our teaching hospital from January 1, 1999, to Ma

Assessment and Management of Acute Disseminated Encephalomyelitis (ADEM) in the Pediatric Patient.

Acute disseminated encephalomyelitis (ADEM) is an inflammatory demyelinating disease of the central nervous system that typically presents in childhood and is associated with encephalopathy and multifocal brain lesions. Although ADEM is thought to be a post-infectious disorder, the etiology is still poorly understood. ADEM is often a monophasic disorder, in contrast to other demyelinating disorders such as multiple sclerosis and neuromyelitis optica spectrum disorder. With increasing awareness, understanding, and testing for myelin oligodendrocyte glycoprotein antibodies, this disease is now k

Electrophysiological features and outcomes of post-infectious myoclonus-ataxia syndrome: a case report and literature review.

BACKGROUND: Myoclonus has become one of the neurological manifestations associated with coronavirus disease 2019 (COVID-19); however, the origin and pathophysiological mechanism remained uncertain. CASE PRESENTATION: A rare case of myoclonus-ataxia syndrome associated with COVID-19 was presented. A 52-year-old Asian woman exhibited generalized myoclonus, ataxia, nystagmus, and dysarthria two weeks after a fever episode, which deteriorated rapidly within a few days. Electromyography (EMG) revealed synchronized bursts in both hands concurrent with myoclonic jerks. The absence of somatosensory e

Para-infectious and post-vaccinal encephalomyelitis.

The incidence of encephalomyelitis in association with acute specific fevers and prophylactic inoculations is discussed. Available statistics are inaccurate, but these conditions are of considerable importance—it is likely that there are about 400 cases of measles encephalitis in England and Wales in an epidemic year. The pathology of these neurological complications is discussed and emphasis placed on the distinction between typical perivenous demyelinating encephalitis, and the toxic type of encephalopathy which occurs mainly in young children. The clinical syndromes occurring in association

Guillain-Barré syndrome (GBS).

Guillain-Barré syndrome (GBS) is an autoimmune and post-infectious immune disease. The syndrome includes several pathological subtypes, the most common of which is a multifocal demyelinating disorder of the peripheral nerves. In the present review, the main clinical aspects and the basic features of GBS are discussed along with approaches to diagnosis and treatment. Furthermore, the pathophysiology of GBS is reviewed, with an emphasis on the production of symptoms and the course of the disease.

The effect of social adversity on the fatigue syndrome, psychiatric disorders and physical recovery, following glandular fever.

Two hundred and fifty patients attending primary care with glandular fever or an upper respiratory tract infection were studied prospectively up to 6 months after onset. Of these patients 228 were interviewed with the Life Events and Difficulties Schedule and the Schedule for Affective Disorders and Schzophrenia, giving Research Diagnostic Criteria for psychiatric disorders. The experience of severe social adversity (provoking agents) had a significant association with psychiatric disorder at 2 months (odds ratio = 5.3) and 6 months (odds ratio = 5.8) after onset of infection. This association

["Benign" intracranial hypertension due to bilateral thrombosis of the lateral sinuses treated with venous bypass (author's transl)].

The authors report on a case of "benign" intracranial hypertension due to bilateral thrombosis of the lateral sinuses and internal jugular veins of infectious origin (otitis). A venous by-pass was established between one of the lateral sinuses and an external jugular vein. As early as the first post-operative week, the intracranial pressure was reduced and the visual disorders regressed. Such venous by-passes, now made possible by micromicrosurgical techniques, can be suggested for treating occlusions of the intracranial sinuses due to thrombophlebitis, traumatisms or tumours.

Steroid-Responsive, Post-infectious New Daily Persistent Headache With Mild Cerebrospinal Fluid Pleocytosis: A Case Report.

New daily persistent headache (NDPH) is a primary headache disorder characterized by a clearly remembered onset, with the headache becoming daily and unremitting within 24 hours and persisting for more than 3 months. Although antecedent infections have been reported as potential triggering events, the pathophysiology of post-infectious presentations of NDPH remains incompletely understood, and effective treatment options are limited. We report the case of a 42-year-old man who developed a continuous daily headache following a flu-like illness and was diagnosed with NDPH according to the Intern

Post-Infectious Cerebellitis: A Systematic Review of Aetiology, Clinical Presentation, Treatment, and Outcomes.

Post-infectious cerebellitis is an immune-mediated cerebellar inflammatory syndrome that follows a preceding infection (or, rarely, vaccination) after a latent interval. It spans a spectrum from benign post-infectious/acute cerebellar ataxia (ACA) through more severe acute cerebellitis (AC) with leptomeningeal enhancement and mass effect, to rare, life-threatening acute fulminant cerebellitis (AFC) with hydrocephalus risk. Evidence is largely limited to individual case reports and small case series, and no prior review has systematically pooled patient-level data across etiologies. A PRISMA-gu

[Reactivation of latent viruses in Neurology].

After establishing latent infection, some viruses can be reactivated by the alteration of host immunological conditions. First, we reviewed viruses that can cause neuronal damage by reactivation. Then we focused on the herpes simplex virus (HSV). The reactivation leads to neuronal damages through two possible mechanisms; "reactivation of a latent herpes virus" by which viruses can cause direct virus neurotoxicity, and "post-infectious immune inflammatory response" by which a focal reactivation of HSV leads to an inflammatory reaction. The former is radiologically characterized by cortical lesi

Perspective Article: Hyperammonemia without Liver Failure - Key Considerations for Intensivists.

Urea cycle disorders represent the most frequent cause of hyperammonemia in the absence of liver failure and may lead to life-threatening elevations in plasma ammonia, particularly when onset is acute and rapidly progressive. Although the etiologies and management principles of acute liver failure have been extensively addressed elsewhere, hyperammonemia must be recognized in critically ill patients as a potential cause of coma, especially when hepatic failure is not present. Non-hepatic causes, including urea cycle disorders, infectious diseases, drug-induced toxicity, and post-transplant com

Childhood renal disorders in Nigeria.

The common childhood renal disorders in Nigeria are post-infectious acute glomerulonephritis (PIAGN), nephrotic syndrome, hypertension, congenital anomalies and urinary tract infection. Children with PIAGN often present with hypertension, circulatory overload and hypoalbuminaemia. Nephrotic syndrome is characterized by a paucity of minimal change and a poor prognosis. Posterior urethral valves and hydronephrosis are the most frequent congenital anomalies. Children with congenital anomalies present late with advanced disease. The commonest malignancy is nephroblastoma, but Burkitt's lymphoma of

Infection-Related Mortality in Adults and Children Undergoing Allogeneic Hematopoietic Cell Transplantation: An Australian Registry Report.

Infection-related mortality (IRM) is the most common non-relapse-related cause of death reported after allogeneic hematopoietic cell transplantation (HCT). Information on the incidence and timing of specific infective organisms and the risk factors for IRM is essential to developing prevention strategies. This report provides the first account of IRM in adults and children undergoing HCT in Australia. Between 2013 and 2018, 2705 adult and 689 pediatric first HCTs were identified from the Australasian Bone Marrow Transplant Recipient Registry database, associated with 1075 (39.7%) total overall

Harm reduction for the treatment of patients with severe injection-related infections: description of the Jackson SIRI Team.

INTRODUCTION: Hospitalizations for severe injection-related infections (SIRI), such as endocarditis, osteomyelitis, and skin and soft tissue infections (SSTI) are increasingly common. People who inject drugs (PWID) experiencing SIRIs often receive inadequate substance use disorder (SUD) treatment and lack of access to harm reduction services. This translates into lengthy hospitalizations with high rates of patient-directed discharge, readmissions, and post-hospitalization mortality. The purpose of this study was to describe the development of an integrated "SIRI Team" and its initial barriers

A Review of Colonoscopy in Intestinal Diseases.

Since the development of the fiberoptic colonoscope in the late 1960s, colonoscopy has been a useful tool to diagnose and treat various intestinal diseases. This article reviews the clinical use of colonoscopy for various intestinal diseases based on present and future perspectives. Intestinal diseases include infectious diseases, inflammatory bowel disease (IBD), neoplasms, functional bowel disorders, and others. In cases of infectious diseases, colonoscopy is helpful in making the differential diagnosis, revealing endoscopic gross findings, and obtaining the specimens for pathology. Addition
