Role of the posterior capsule in the aqueous-vitreous barrier in aphakic and pseudophakic eyes.

Clinical observations have shown that an intact posterior capsule between aqueous and vitreous may function as a barrier to low and high molecular weight substances, e.g., prostaglandins or the angiogenic factor. To determine if this protective barrier breaks down after posterior capsulotomy, we examined the distribution of fluorescein in human eyes after intracapsular cataract extraction (ICCE), extracapsular cataract extraction (ECCE) and intraocular lens (IOL) implantation with capsulotomy, and ECCE and IOL implantation without capsulotomy in 12 patients each. After ICCE and after ECCE and

[Femtosecond laser in cataract surgery. A critical appraisal].

BACKGROUND: The use of femtosecond lasers (FSL) is increasingly spreading in cataract surgery. Potential advantages over standard manual cataract surgery are the superior precision of corneal incisions and capsular openings as well as the reduction of ultrasound energy for lens nucleus work-up. Exact positioning and dimensioning of the anterior capsular opening should help reduce decentration and tilt of the intraocular lens (IOL) optics and thus achieve better target refraction. Together with the possibility to correct low-grade corneal astigmatism by precise arcuate incision, FSL technology

Extended focal length intraocular lens implantation in posttraumatic posterior capsular rupture.

We present a case of posterior capsular defect with traumatic cataract after blunt trauma in which an extended focal length intraocular lens (IOL) was implanted in the bag after conventional phacoemulsification and anterior vitrectomy. Anterior segment optical coherence tomography performed preoperatively aided in the confirmation and documentation of the capsular integrity. Intraoperative trocar anterior chamber (AC) maintainer allowed AC maintenance without further complications. IOL was well centered postoperatively at 6 months, and the unaided visual acuity was 20/20 for distance and N8 fo

Posterior capsule opacification and wrinkling in a case of capsular bag distension.

We describe a complication following anterior neodymium: YAG laser capsulotomy in a case of capsular bag distension which was diagnosed 21 months after cataract extraction with phacoemulsification and in-the-bag lens implantation. An anterior neodymium: YAG capsulotomy was performed and immediately after this the posterior capsule collapsed and wrinkled, causing a marked decrease in visual acuity that necessitated posterior neodymium: YAG capsulotomy.

Treatment of secondary posterior capsular membranes with the Nd:YAG laser in a pediatric population.

Thirty-two eyes of 28 pediatric patients were treated with the H. S. Meridian Microruptor III Nd:YAG laser for secondary posterior capsular membranes after cataract extraction either with or without posterior chamber intraocular lens implantation. This laser allows for 90-degree rotation of the laser delivery system to treat recumbent patients who may be under general endotracheal anesthesia. In all patients, at least a 5-mm axial capsulotomy was created. The energy requirements for the procedure were related to the density of the membrane, which correlated with the time lapse between cataract

Femtosecond laser-assisted cataract surgery in infants.

We describe a technique for performing cataract surgery with a femtosecond laser in infants. Anterior capsulotomy and lens division are performed by the laser. A fluid-filled interface makes it possible to re-dock the laser to the eye for posterior capsulotomy after the eye has been opened for lens aspiration without complications. The integrated 3-dimensional optical coherence tomography also visualizes the posterior capsule, allowing a centered central posterior capsulotomy followed by mechanical anterior vitrectomy. The anterior and the posterior capsule disks are removed without capsule te

Does Posterior Capsule Opacification Affect the Results of Diagnostic Technologies to Evaluate the Retina and the Optic Disc?

The visual outcome obtained after cataract removal may progressively decline because of posterior capsular opacification (PCO). This condition can be treated by creating an opening in the posterior lens capsule by Nd:YAG laser capsulotomy. PCO optical imperfections cause several light reflection, refraction, and diffraction phenomena, which may interfere with the functional and structural tests performed in different ocular locations for the diagnosis and follow-up of ocular disease, like macular and optic nerve diseases. Some parameters measured by visual field examinations, scanning laser po

Nd:YAG capsulotomy: an update.

BACKGROUND: Posterior capsule opacification (PCO) remains one of the most common late complications following cataract surgery and may lead to decreased visual acuity, glare, contrast sensitivity loss, and impaired visual quality. Nd:YAG laser posterior capsulotomy is currently accepted as the gold standard treatment for symptomatic PCO because of its non-invasive nature and rapid visual rehabilitation. However, despite its widespread use and generally favorable safety profile, concerns remain about procedural variability, postoperative management, and complications such as intraocular pressur
