Postoperative Nausea and Vomiting Management, New Drugs in the Outpatient Setting.

Postoperative and postdischarge nausea and vomiting are common, costly complications that delay recovery and reduce satisfaction after ambulatory surgery. They stem from complex neurotransmitter pathways targeted by antiemetics. Accurate risk prediction is key, and multimodal prophylaxis is most effective.

Effects of Intravenous Dextrose Timing on Postoperative Nausea, Vomiting and Anxiety.

OBJECTIVE: Postoperative nausea and vomiting (PONV) is a significant issue encountered in surgical patients. This study aims to investigate the effects of dextrose infusion timing on PONV incidence. METHODS: Ninety patients undergoing laparoscopic cholecystectomy were included in this randomized controlled trial. Patients were assigned to one of three equal groups. In Group I, patients received an infusion of 400 mL of 0.9% saline 2 hours before surgery. In Group D, patients received 400 mL of 5% dextrose at the same infusion rate. Both Groups I and D received 0.9% saline at 10 mL kg-1 h-1 du

Intraoperative electroencephalography predicts postoperative nausea and vomiting in laparoscopic sleeve gastrectomy.

INTRODUCTION: Postoperative nausea and vomiting (PONV) is one of the most common causes for rehospitalization and emergency department visits after bariatric surgery. The current understanding of risk factors prediction for PONV remains still incomplete. METHODS: A total of 64 adult patients morbidly obese patients with selective laparoscopic sleeve gastrectomy (LSG) were prospectively enrolled. Perioperative electroencephalography (EEG) information was recorded separately at different brain regions (F7, F8, Fp1, and Fp2) using the SedLine root monitor. EEG was processed into five time points

Is Multimodal Anesthesia Effecting Postoperative Nausea and Vomiting in Laparoscopic Cholecystectomy?

Background: Multimodal anesthesia represents a technique that can improve analgesia and lower the occurrence of opioid side effects in the postoperative period, such as postoperative nausea and vomiting (PONV). It can be achieved by providing different types of medication during the intraoperative period which can decrease the need for opioids. PONV happens more often in patients who have received large amounts of opioids during laparoscopic cholecystectomy. In this study, our aim was to observe the occurrence of PONV between three different groups of patients who received lidocaine, ketamine

Evidence-based analysis of risk factors for postoperative nausea and vomiting.

BACKGROUND: /st> In assessing a patient's risk for postoperative nausea and vomiting (PONV), it is important to know which risk factors are independent predictors, and which factors are not relevant for predicting PONV. METHODS: /st> We conducted a systematic review of prospective studies (n>500 patients) that applied multivariate logistic regression analyses to identify independent predictors of PONV. Odds ratios (ORs) of individual studies were pooled to calculate a more accurate overall point estimate for each predictor. RESULTS: /st> We identified 22 studies (n=95 154). Female gender was

Evaluation of three risk scores to predict postoperative nausea and vomiting.

BACKGROUND: So far there are three different scores to predict postoperative vomiting (PV: Apfel et al., 1998) or postoperative nausea and vomiting (PONV: Koivuranta et al., 1997; Palazzo and Evans, 1993). All three scores used logistic regression analysis to identify and create weights for the risk factors for PV or PONV. In short, these were sex, age, history of previous PONV, motion sickness, duration of anaesthesia, and use of postoperative opioids. However, an external evaluation and a comparison of these scores has not been performed so far. METHODS: Patients undergoing a variety of sur

Gastropleural fistula: an unusual cause of intractable postoperative nausea and vomiting.

Gastropleural fistula is an uncommon finding (1). Gastropleural fistulae have been reported after pulmonary resection (1), perforated paraesophageal hernia (2), perforated malignant gastric ulcer at the fundus, and gastric bypass operation for morbid obesity. We present a case of gastropleural fistula that resulted acutely from intractable postoperative nausea and vomiting after ambulatory knee arthroscopic surgery under general anesthesia.

Severe bradycardia induced by postoperative nausea and vomiting: A case report.

INTRODUCTION: Postoperative nausea and vomiting is a common complication for patients after anesthesia and surgery, which may result in increased parasympathetic activity, such as diaphoresis, pallor, or bradycardia. However, few cases of fatal bradycardia induced by postoperative nausea and vomiting have been reported before. Clinicians generally attribute bradycardia to certain anesthetics, instead of postoperative nausea or vomiting. PATIENT CONCERNS: A fifty-year-old female with a history of well-controlled hypertension underwent elective radical mastectomy. When recovering from anesthesi

Consideration of Migraines Among Risk Factors for Postoperative Nausea and Vomiting.

The impact of migraine on postoperative nausea and vomiting (PONV) is controversial, and few studies have focused on their relationship. Thus, we investigated the impact of migraine, among other risk factors, on PONV in a large retrospective study. We analyzed 10 years of clinical data from the Smart Clinical Data Warehouse of Hallym University Medical Center. PONV was defined as nausea or vomiting within the first 24 h after surgery. Patients diagnosed by a neurologist and with a history of triptan use before surgery were enrolled into the migraine group. We enrolled 208,029 patients aged > 1

How can we best prevent or treat postoperative nausea and vomiting?

In pursuit of preventing or rapidly rescuing patients from postoperative nausea and vomiting, new clinical studies provide revised postoperative nausea and vomiting risk prediction tools, risk stratification formulae, comparative trials to identify the most appropriate cost-effective anti-emetics, and several trials recommending the addition of steroids in anti-emetic prophylactic multimodal approaches. Common variables that identify high-risk postoperative nausea and vomiting patients include female sex, a history of postoperative nausea and vomiting or motion sickness, young age, volatile an

Prediction of postoperative nausea and vomiting using a logistic regression model.

In a previous study, logistic regression analysis was used to determine the association of independent fixed patient factors with the incidence of postoperative nausea and vomiting (PONV). Female sex, previous history of PONV, use of postoperative opioids, previous history of motion sickness and an interaction between male sex and previous history of PONV were combined in an equation from which risk of PONV could be estimated. The present study was designed to test this equation in a group of patients with wide selection criteria. Data on 400 patients were collected in relation to pre-, per- a
