[Effect of thiazide therapy in the prophylaxis of calcium lithiasis].

OBJECTIVE: To analyze the role of thiazide therapy in the prevention of recurrent calcium nephrolithiasis, considering the hypocalciuric action of this drug. METHODS: A prospective study before and after treatment, with a 3-year follow-up, was carried out on 150 patients with recurrent calcium renal stone. The patients were divided into three groups: (A) no treatment, (B) treatment with thiazide 50 mg/day and (C) treatment with thiazide and potassium citrate. The treated groups were randomly assigned to the treatment arms. Control analytical tests and radiological assessments were performed r

Milk pH, acid base status, and growth in babies.

Metabolic acidosis is common in babies fed cows' milk-based formulae. Therefore the effects of adding alkaline salts (sodium and potassium citrate) to a demineralised whey formula were studied in vitro and in 26 low birthweight babies fed on the formula or formula plus citrate. The alkali altered the pH and titratable acidity to a value nearer human milk but it increased the buffering capacity to a value further away. This may effect the bacterial flora of the intestine. The babies fed on formula plus citrate did not make greater gains in weight, length, head circumference, skinfold thickness,

Sub-Acute Toxicity Evaluation and Antiurolithiatic Activity of Tribulus terrestris Ethanol Extract in Rats.

BACKGROUND: Tribulus terrestris (T. terrestris) is traditionally used in the management of urinary disorders; however, evidence regarding its safety upon repeated administration and its antiurolithiatic efficacy remains limited. This study aimed to evaluate both the sub-acute toxicity and antiurolithiatic potential of the ethanol extract of T. terrestris (EETT) in experimental rats. METHODS: Sub-acute toxicity was assessed in Wistar rats (n = 30; 5 males and 5 females per group) following 28-day repeated oral administration of EETT (500 and 1000 mg/kg). Antiurolithiatic activity was evaluated

Effectiveness of the ketogenic diet used to treat resistant childhood epilepsy in Scandinavia.

BACKGROUND: This Scandinavian collaborative retrospective study of children treated with ketogenic diet (KD) highlights indications and effectiveness over two years follow-up. METHODS: Five centres specialised in KD collected data retrospectively on 315 patients started on KD from 1999 to 2009. Twenty-five patients who stopped the diet within four weeks because of compliance-problems and minor side-effects were excluded. Seizure-type(s), seizure-frequency, anti-epileptic drugs and other treatments, mental retardation, autism-spectrum disorder and motor-dysfunction were identified and treatmen

Case of hypoparathyroidism, deafness and renal dysplasia (HDR) syndrome associated with nephrocalcinosis and distal renal tubular acidosis.

Hypoparathyroidism, deafness and renal dysplasia (HDR) syndrome is an autosomal dominant disorder characterized by hypoparathyroidism, sensorineural deafness and renal dysplasia. Herein, we report a case of HDR syndrome associated with nephrocalcinosis and distal renal tubular acidosis. A 34-year-old woman was admitted to investigate recurrent stone formation and bilateral nephrocalcinosis. As a 3-year-old child, she had been diagnosed with HDR syndrome without chromosome evaluation. She had spontaneous stone passages on several occasions. On laboratory examination, serum calcium and intact pa

Creatinine, calcium, citrate and acid-base in spinal cord injured patients.

The aim of this work was to recognise factors responsible for reduced citrate excretion, previously reported in patients with spinal cord lesions and possibly related to the occurrence of urinary tract stone or catheter blockage. Inter alia, a reference range for creatinine in plasma (34-88 mumol/l) was also obtained. Two groups of subjects were studied. The first group consisted of 64 male inpatients with spinal cord lesions and 20 male control subjects. The second group were 342 spinal patients who attended an outpatient clinic and 31 control subjects. Plasma calcium was within the normal ra

Clinical features of hypocitraturic kidney stone formers: an observational and retrospective real-life study in an Italian kidney stone center.

Low urinary excretion of citrate is a risk factor for calcium nephrolithiasis, but clinical features of hypocitraturic stone formers remain unclear. Therefore, the present study explored clinical characteristics of hypocitraturic stone formers. It retrospectively analysed clinical characteristics of hypocitraturic patients in a series of 668 adult stone formers refering to a stone outpatient clinic in Milan. Hypocitraturic stone formers (n = 227, 34%) had lower excretion of calcium, phosphate, uric acid, sodium, and potassium than normocitraturic patients. Considering quartiles of citrate excr

Kidney Effects of Potassium Additives in Health and Disease.

Potassium homeostasis is maintained primarily by the kidney, yet potassium is increasingly recognized not only as a regulated electrolyte but also as a key determinant of renal physiology and kidney health. Experimental, epidemiologic, and clinical evidence supports a reciprocal relationship in which kidney function governs potassium balance, whereas dietary potassium intake and plasma potassium concentration influence renal sodium transport, blood pressure regulation, and susceptibility to kidney injury. Both potassium deficiency and excess are associated with adverse cardiorenal outcomes, em

Urinary Supersaturation in a Randomized Trial Among Individuals With Recurrent Nephrolithiasis Comparing Empiric vs Selective Preventive Therapy: The URINE Trial.

PURPOSE: We compare a strategy using testing to guide treatment, also known as selective therapy, with a strategy of initiating interventions without testing, termed empiric therapy, on urinary supersaturation of calcium oxalate and calcium phosphate. MATERIALS AND METHODS: In this single-center trial, adult individuals with recurrent idiopathic calcium stone disease were randomized to either an empiric or selective strategy. Participants received 24-hour urine testing at baseline, 4 weeks, and 8 weeks. Treatment in the empiric arm comprised dietary and fluid counseling and pharmacologic trea

Natriuretic response to an acute oral potassium load in healthy individuals and patients with chronic kidney disease: a randomized controlled trial.

BACKGROUND: The health benefits of dietary potassium have been linked to potassium-induced natriuresis. However, it is unknown whether potassium loading in patients with chronic kidney disease (CKD) induces natriuresis and to what degree it increases plasma potassium. We hypothesize that potassium-induced natriuresis remains intact and that potassium loading is safe in patients with CKD. METHODS: In this placebo-controlled crossover study, 18 healthy individuals and 9 patients with non-diabetic CKD stage G3b-G4 received an acute oral load of potassium chloride (KCl), potassium citrate (K-cit)

Continuous infusion of calcium gluconate in 5% albumin is safe and prevents most hypocalcemic reactions during therapeutic plasma exchange.

While therapeutic plasma exchanges (TPEs) performed with 5% albumin are considered safe, concerns regarding venous access and hypocalcemic toxicity remain. We reviewed the frequency of complications during TPEs performed with 5% albumin supplemented with calcium gluconate and potassium chloride for a 5 year period in our institution. Eighty-four adult patients (46 males and 38 females) underwent 581 plasma exchanges during the study period. The most common indications were myasthenia gravis (37%), acute inflammatory demyelinating polyradiculoneuropathy (31%), and chronic inflammatory demyelina

Hypokalaemic myopathy associated with iatrogenic hypercorticism in a cat.

CASE SUMMARY: A 12-year-old castrated male domestic shorthair cat was referred for investigation of lethargy, hindlimb weakness with plantigrade stance and ventroflexion of the neck. The cat was fed a balanced diet and had received methylprednisolone acetate at a dose of 20 mg intramuscularly every 6 months for 6 years. On blood work, severe hypokalaemia and marked elevation of muscle enzymes were noticeable. The findings were suggestive of hypokalaemic myopathy. Urine fractional excretion of potassium (FEk) was moderately high (9.04%), and serum aldosterone was below the reference interval. A

An unusual case of renal tubular acidosis.

A 40-year-old female with a history of glue sniffing and intravenous drug use presented to hospital with a week's history of feeling generally unwell. She had had multiple admissions to hospital with similar presentations in the past. On examination, the only significant clinical finding was that of a reduced level of consciousness. Laboratory investigations revealed a hyperchloremic normal anion gap metabolic acidosis with a positive urine anion gap and a urine pH of 6.5 combined with a severely low hypokalaemia. She was subsequently diagnosed with renal tubular acidosis type 1, secondary to

Distal renal tubular acidosis with severe hypokalaemia, probably caused by colonic H(+)-K(+)-ATPase deficiency.

We describe a 21 month old male infant who presented with failure to thrive associated with severe hypokalaemia and metabolic acidosis, together with hypomagnesaemia. Evaluation revealed marked renal and probable faecal potassium wasting, distal renal tubular acidosis, mild urinary magnesium wasting, and a normal gastric pH (gastric H(+)-K(+)-ATPase). Hypokalaemic forms of metabolic acidosis, such as diabetic ketoacidosis and proximal renal tubular acidosis were ruled out from the clinical picture. The hypokalaemia of distal renal tubular acidosis usually improves with alkali therapy, but this

Pathogenesis of sudden unexplained nocturnal death (lai tai) and endemic distal renal tubular acidosis.

Sudden unexplained nocturnal death (SUND), a disorder of unknown cause that occurs in otherwise healthy young adults, mostly male, during their sleep, is prevalent in the north-east region of Thailand, where it has been known for generations as lai tai. It occurs in the same population and area where hypokalaemic periodic paralysis (HPP), endemic distal renal tubular acidosis (EdRTA), and renal stones are also endemic. SUND has occurred in families of patients with EdRTA, and HPP can present as sudden onset of muscle parlysis with potentially lethal cardiac arrhythmias and respiratory failure
