Morphological spectrum of prenatal cerebellar disruptions.

There is increasing evidence that the cerebellum is susceptible to both prenatal infections and haemorrhages as well as being vulnerable in extremely preterm babies, but not to perinatal and postnatal hypoxic-ischaemic injuries. Starting with the imaging appearance we describe and illustrate a spectrum of prenatal cerebellar disruptions: cerebellar agenesis; unilateral cerebellar hypoplasia; unilateral cerebellar cleft; global cerebellar hypoplasia; vanishing cerebellum in myelomeningocele; and disruption of cerebellar development in preterm infants. We discuss neuroradiological characteristic

Prenatal Alcohol Exposure as a Potential Contributor to Hydranencephaly: Clinical and Neurodevelopmental Perspectives.

Hydranencephaly is traditionally attributed to prenatal vascular disruption; however, chronic prenatal alcohol exposure may contribute through neurotoxicity, placental dysfunction, oxidative stress, and vascular injury. Given the biological plausibility of this association, alcohol abstinence during pregnancy remains essential, and further studies are required to clarify its role in hydranencephaly.

Physical abuse during pregnancy and risk of low-birthweight infants among aborigines in Taiwan.

OBJECTIVES: The aim of this study was to assess whether physical abuse during pregnancy is associated with an increased risk of giving birth to a low-birthweight (LBW) infant. STUDY DESIGN AND METHODS: A cross-sectional survey was conducted and 1143 aboriginal women were recruited into this study. The Abuse Assessment Screen was used to collect information regarding maternal physical abuse, and infants' birth weights were obtained from hospital medical records. Multiple logistic regression was used to estimate the association of LBW with physical abuse during pregnancy, adjusting for behaviou

Sensitive Periods for Recovery from Early Brain Injury.

The developing brain is remarkably plastic as it changes in response to a wide range of experiences including sensory and motor experience, psychoactive drugs, peer relationships, parent-infant interactions, gonadal hormones, intestinal flora, diet, and injury. There are sensitive periods for many of these experiences, including cerebral injury. Comparisons across mammalian species (humans, monkeys, cats, rats, mice) show a sensitive period for good outcomes from cerebral injury around the time of intense synaptogenesis. This period is postnatal in humans, cats, and rats, but prenatal in monke

Chromatin Disruption After Prenatal Hypoxia Predicts Lasting Neuron Deficits.

Prenatal exposure to transient hypoxia can result in various developmental disabilities presumably by disrupting normal neurodevelopmental processes, even in milder injuries without detectable cell death or structural damage 1,2 . Emerging evidence suggests that disruption of the epigenome may be one of the most critical consequences of prenatal brain exposure to hypoxia 3-8 . However, the cell-type-selective effects of hypoxia on the developing brain's transcriptome and epigenome remain unknown. Here, we demonstrate that immediately after hypoxia, transcriptional and chromatin disruptions occ

Alcohol-induced behavioural problems in fetal alcohol spectrum disorders versus confounding behavioural problems.

Prenatal alcohol exposure is strongly associated with disruptive behaviour in childhood and antisocial behaviour later in life. There are numerous confounding risk factors in the lives of alcohol-abusing mothers that may contribute to the behaviour problems seen in their children, rather than direct brain injury by alcohol. In fact, many of these additional environmental and genetic risk factors for childhood behaviour problems co-occur with prenatal alcohol exposure and affect the same child, creating a confluence of risk. As a result, one cannot with any certainty attribute behaviour problem

Evaluation of delayed effects of ionizing radiation: an historical perspective.

It is widely assumed that after the bombing of Hiroshima and Nagasaki there were no lasting effects of the acute injuries (which included extensive damage to blood forming tissues by the radiation) or the massively high death rate (which was caused by environmental effects of the blast as well as personal injuries). However, close inspection of the dose response curves for non-cancer deaths has shown that this could be a false impression caused by one effect of marrow aplasia being confused with leukemia (defective erythropoiesis) and a second effect being confused with early selection in favo

CeRebrUm and CardIac Protection with ALlopurinol in Neonates with Critical Congenital Heart Disease Requiring Cardiac Surgery with Cardiopulmonary Bypass (CRUCIAL): study protocol of a phase III, randomized, quadruple-blinded, placebo-controlled, Dutch multicenter trial.

BACKGROUND: Neonates with critical congenital heart disease (CCHD) undergoing cardiac surgery with cardiopulmonary bypass (CPB) are at risk of brain injury that may result in adverse neurodevelopment. To date, no therapy is available to improve long-term neurodevelopmental outcomes of CCHD neonates. Allopurinol, a xanthine oxidase inhibitor, prevents the formation of reactive oxygen and nitrogen species, thereby limiting cell damage during reperfusion and reoxygenation to the brain and heart. Animal and neonatal studies suggest that allopurinol reduces hypoxic-ischemic brain injury and is card

Maternal deaths due to homicide and other injuries in North Carolina: 1992-1994.

OBJECTIVE: To determine the role of homicide and other injuries in maternal deaths in North Carolina over the three-year period from 1992 through 1994. METHODS: Maternal deaths were identified from death certificates that indicated a maternal death and through an enhanced surveillance system that matches death certificates with live-birth and fetal-death certificates. Deaths were classified as direct, indirect, medically unrelated, or injury-related. Patterns of prenatal care were ascertained from the matching live-birth or fetal-death certificates. Maternal death rates for whites and nonwhit

Accurate neurosonographic prediction of brain injury in the surviving fetus after the death of a monochorionic cotwin.

OBJECTIVE: To assess the feasibility of the prenatal diagnosis using fetal neurosonography of brain injuries in the surviving fetus after the demise of a monochorionic cotwin. METHODS: This was a retrospective observational study in the period 1990-2004 of monochorionic twin pregnancies with a single fetal demise. A detailed sonographic evaluation of the intracranial anatomy of the surviving twin had been performed whenever possible using a multiplanar approach and from 1999, fetal magnetic resonance imaging was offered as well. Postnatal follow-up was obtained in all cases. RESULTS: In six

Association between maternal smoking and increased symptom severity in Tourette's syndrome.

OBJECTIVE: Substantial evidence suggests that both environmental and genetic factors contribute to the development and clinical expression of Tourette's syndrome. Although genetic studies of Tourette's syndrome are common, studies of environmental factors are relatively few and have not identified consistent risk factors across studies. This study examines in a large cohort of subjects (N=180) the relationship between prenatal/perinatal adverse events with Tourette's syndrome severity as determined by tic severity and rates of commonly comorbid disorders such as obsessive-compulsive disorder (

Erythropoietin Modulates Cerebral and Serum Degradation Products from Excess Calpain Activation following Prenatal Hypoxia-Ischemia.

Preterm infants suffer central nervous system (CNS) injury from hypoxia-ischemia and inflammation - termed encephalopathy of prematurity. Mature CNS injury activates caspase and calpain proteases. Erythropoietin (EPO) limits apoptosis mediated by activated caspases, but its role in modulating calpain activation has not yet been investigated extensively following injury to the developing CNS. We hypothesized that excess calpain activation degrades developmentally regulated molecules essential for CNS circuit formation, myelination and axon integrity, including neuronal potassium-chloride co-tra

Management of prenatally detected fetal hydronephrosis.

The increased frequency of prenatal ultrasonography has resulted in an increase in the detection of fetal genitourinary abnormalities, many of which are of minimal clinical significance. Severe fetal urinary tract obstruction with associated oligohydramnios results in a recognizable constellation of physical findings, including renal dysplasia, pulmonary hypoplasia, and perinatal death. In selected cases, prenatal intervention to decompress urinary tract obstruction may reestablish amniotic fluid volume, prevent renal damage, and allow normal pulmonary development. After severe renal injury ha

[Damage to the brachial plexus in vaginal delivery from breech presentation--correlation with reducing the extended arm].

BACKGROUND: Brachial plexus injuries in foetuses delivered spontaneously by breech presentation is very rare. The aim of this study was to evaluate the frequency of brachial plexus injuries in connection with the rate of the reduction of the extended arm. MATERIALS AND METHODS: In our retrospective study we examined 1194 vaginally intended breech deliveries (> 32 completed weeks) for the frequency of extended arms and subsequent plexus brachialis palsies. RESULTS: One brachial plexus injury occurred in 796 vaginally delivered newborns (1.2@1000). The outcome corresponds with the frequency of

Prenatal tobacco smoke exposure and risk of brain abnormalities on magnetic resonance imaging at term in infants born very preterm.

BACKGROUND: Prenatal tobacco smoke exposure and preterm birth are associated with abnormal brain and neurodevelopmental outcomes in infants. Studies that can disentangle indirect mediating effects from direct effects of prenatal tobacco smoke exposure on sensitive early brain magnetic resonance imaging biomarkers in very preterm infants are needed. OBJECTIVE: This study aimed to determine whether prenatal tobacco smoke exposure in preterm infants posed any direct effects on magnetic resonance imaging-determined global brain abnormality score and secondary measures of brain abnormalities after

Neuroprotective effect of neotrofin in a neonatal rat model of periventricular leukomalacia.

Periventricular leukomalacia (PVL) is the dominant form of brain injury in premature infants and no specific treatment is currently available. Neotrofin, a neurotrophin agonist, has been shown to provide neuroprotection in several in vivo and in vitro studies. The aim of this study was to investigate the neuroprotective effect of neotrofin treatment after endotoxin induced PVL in a rat model. Wistar rat pups were divided into four groups as: (1) control, (2) lipopolysaccharide (LPS)-administered group, (3) LPS-administered and prenatal maternal neotrofin-treated group and (4) LPS-administered
