Insurance Authorizations in Pediatric Disorders of Gut-Brain Interaction Treated With Auricular Neuromodulation: A Tertiary Center Experience.

OBJECTIVES: Auricular percutaneous electrical nerve field stimulation (PENFS) is a Food and Drug Administration-approved treatment for functional abdominal pain associated with irritable bowel syndrome and functional dyspepsia. Because PENFS is relatively new, insurance coverage has been inconsistent, with patients experiencing denials or delays in authorization. Thus, we aimed to evaluate insurance authorizations and their influence on outcomes in children with disorders of gut-brain interaction (DGBI) who undergo PENFS. MATERIALS AND METHODS: We reviewed charts of patients with DGBI aged se

Clinical Utility and Reimbursement of Next-Generation Sequencing-Based Testing for Myeloid Malignancies.

Next-generation sequencing is becoming increasingly important for the diagnosis, risk stratification, and management of patients with established or suspected myeloid malignancies. These tests are being incorporated into clinical practice guidelines and many genetic alterations now constitute disease classification criteria. However, the reimbursement for these tests is uncertain. This study analyzed the clinical impact, ordering practices, prior authorization, and reimbursement outcomes of 505 samples from 477 patients sequenced with a 50-gene myeloid next-generation sequencing panel or a 15-

Reproductive Autonomy and Insurer Denials of Care: The Fine Line Between Oversight and Interference.

AbstractReproductive autonomy is a fundamental ethical principle in healthcare, yet insurance denials of care often undermine patient decision-making. This article examines a case in which a 35-year-old patient with stage 4 endometriosis sought a total hysterectomy and bilateral salpingo-oophorectomy to manage severe, refractory pelvic pain. Despite the patient's informed and autonomous decision, her insurer denied authorization based on a paternalistic concern for her future fertility. Through an ethical analysis, this article critiques the role of insurers in reproductive decision-making, hi

Denying and Delaying Indicated Care: The True Cost.

Cost in health care is not simply the dollar amount of a transaction. When a claim for indicated surgery is denied or delayed, savings are not guaranteed and patient outcomes likely worsen. Delays resulting from prior authorization create an administrative burden, and patients bear the burden of prolonged conservative care, lost workforce participation, and added psychological toll. Delaying or denying indicated surgery does not reduce cost; it simply redistributes it.

Challenges in Implementing Revocation of Oncology Drugs Under Conditional Early Approval in Japan: Regulatory Insights From Comparisons With FDA/EMA.

Expedited pathways in the United States (U.S.), European Union (EU), and Japan (JP) enable earlier access to innovative therapies for rare and serious diseases. In 2025, Japan explicitly introduced revocation provisions to its Conditional Early Approval (CEA). Considering the challenges in a related prior scheme-conditional and time-limited approval for regenerative medical products-particularly withdrawals prior to full approval, this study examined oncology cases under the U.S. Accelerated Approval (AA) and EU Conditional Marketing Authorization (CMA) pathways to identify operational challen

Predictors of Medicaid managed care plan performance on opioid use disorder treatment quality metrics.

INTRODUCTION: Medicaid managed care plans (MCPs) and states play essential roles in supporting access to high-quality opioid use disorder (OUD) treatment services. This study aimed to identify MCP and state-level policies associated with better plan performance on indicators of quality OUD treatment. METHODS: Publicly available data on Medicaid MCPs' profit status, behavioral health contracting arrangements, market share, buprenorphine prior authorization and quantity limit policies and state Medicaid policies were linked with plan-level measures of OUD treatment quality from the National Com

Poor Antifungal Coverage for Onychomycosis in a Cross-Sectional Analysis of Medicaid Formularies.

BACKGROUND: Onychomycosis is the most common nail disease seen in clinical practice. Medication safety, severity of disease, comorbidities, concomitant medications, patient age, and cost are all important considerations when treating onychomycosis. Because cost may affect treatment decisions, we sought to analyze Medicaid formulary coverage of onychomycosis antifungals. METHODS: Public state Medicaid formularies were searched for coverage of US Food and Drug Administration-approved onychomycosis medications and off-label oral fluconazole. Total drug cost for a single great toenail was calcula

A Word to the WISeR: Why Otolaryngologists Must Lead to Preserve Access to Hypoglossal Nerve Stimulation.

The Centers for Medicare & Medicaid Services' Wasteful and Inappropriate Service Reduction (WISeR) model introduced prior authorization review for hypoglossal nerve stimulation (HGNS) and took effect on January 1, 2026, in six states. HGNS was the only otolaryngology procedure included in the model, reflecting the rapid growth in utilization and potential payor concern about continued access to the procedure. This policy shift presents an opportunity for otolaryngologists to define and demonstrate high-quality, evidence-based stewardship of HGNS.

The Role of Panobinostat Plus Bortezomib and Dexamethasone in Treating Relapsed or Relapsed and Refractory Multiple Myeloma: A European Perspective.

Panobinostat is an oral pan-histone deacetylase inhibitor developed by Novartis. Panobinostat acts via epigenetic modification and inhibition of the aggresome pathway. In August 2015, the European Commission authorized panobinostat for use in combination with bortezomib and dexamethasone for the treatment of relapsed or relapsed and refractory multiple myeloma (MM) in patients who have received ≥2 prior regimens including bortezomib and an immunomodulatory drug. In January 2016, the National Institute for Health and Care Excellence recommended panobinostat for use in the same combination and p

The UW Experience: Feasibility of De Novo Letermovir for Primary Prophylaxis After Abdominal Solid Organ Transplant.

BACKGROUND: Letermovir is approved for primary prophylaxis of cytomegalovirus (CMV) in high-risk kidney transplant recipients. However, many experts suggest the drug be reserved as a second-line agent when valganciclovir is not tolerated or fails. OBJECTIVE: The purpose of this study was to describe the feasibility of a de novo letermovir prophylactic approach for CMV high-risk and seropositive abdominal solid organ transplant patients. METHODS: Retrospective review of abdominal transplant recipients who required CMV prophylaxis between June 6, 2023, and June 6, 2024. The purpose was to eval

Primary Biliary Cholangitis: Medical and Specialty Pharmacy Management Update.

BACKGROUND: Chronic liver disease and cirrhosis are a leading cause of morbidity and mortality in the United States. Primary biliary cholangitis (PBC), previously known as primary biliary cirrhosis and which has been designated an orphan condition, is a chronic autoimmune disease resulting in the destruction of the small bile ducts in the liver. Without effective treatment, disease progression frequently leads to liver failure and death. Until May 2016, the only FDA-approved treatment for PBC was ursodiol (UDCA), an oral hydrophilic bile acid, which can slow progression of liver damage due to

Genetic testing for children at risk to be hemophilia carriers.

Carriers for hemophilia are at risk for bleeding despite normal or mildly reduced factor 8 or factor 9 activity levels. Genetic testing is necessary to determine carrier status in those at risk and early identification of carriers can inform their bleeding risk. The aims of this single-center retrospective study were to determine the uptake of genetic testing in children at risk to be hemophilia carriers and identify barriers to completion of testing. We identified 64 unique at-risk children assigned female sex at birth under 18 years old, with at least one caregiver participating in a visit b

National perspectives of barriers by insurance and pharmacy benefit managers in pediatric inflammatory bowel disease.

OBJECTIVES: Early biologic initiation, dose optimization, and therapy modification based on disease phenotype are key to improving outcomes in pediatric inflammatory bowel disease (IBD). Enacting optimized therapy is often impeded by the lack of United States Food and Drug Administration (FDA) approval for pediatric use of newer advanced therapies or intensified dosing regimens. These barriers often result in initial payor denial of coverage and added prior authorization burden on physicians, leading to patient delays in medication initiation and therapy optimization, and development of diseas
