Respiratory depression with concomitant use of opioids and cannabinoids: a case report.

BACKGROUND: The United States has had a 15-fold increase in daily cannabis use in the last 30 years. There is a growing importance for palliative care physicians to understand the pharmacological interactions between high-dose opioids and cannabinoids when treating patients with cancer-related pain who are increasingly concomitantly using cannabinoids. We present a case of respiratory depression following high‑dose THC and opioids. We discuss proposed mechanistic interactions, the importance of cannabinoid use in social history, and the role of proactive counseling of patients with concomitant

Neurophysiological Evidence of Enhanced Cognitive Control Following Auditory Control Enhancement (ACE) Therapy: A Pilot RCT in Schizophrenia-Spectrum Disorders with Persistent Auditory Hallucinations.

Schizophrenia (Sz) involves positive symptoms such as auditory hallucinations, paranoia, and delusions, and are commonly linked to deficits in cognitive control. These deficits appear in tasks like the A-X version of the continuous performance test (AX-CPT), which reflects reduced proactive control, and in diminished auditory steady-state responses (ASSR) during selective attention. Notably, in Sz, ASSR to sounds presented during diverted attention increases with hallucination severity. We report results from a single-blind randomized controlled trial evaluating Auditory Control Enhancement (A

Perioperative normothermia during major surgery: is it important?

PH caused by anesthesia-induced thermoregulatory inhibition and exposure to cold operating room environments still occurs in a significant proportion of patients undergoing major surgery. Although the association between specific perioperative temperatures (in and of themselves) and postoperative morbidity remains unclear, there is fair evidence to suggest that perioperative active warming may reduce the risk of postoperative cardiac events, bleeding, and SSIs. As such, proactive efforts by surgical teams to prevent PH are warranted and have become the standard of care at many institutions. Co

Role of the substantia nigra pars reticulata in sensorimotor gating, measured by prepulse inhibition of startle in rats.

The substantia nigra pars reticulata (SNR) is one of the major output nuclei of the basal ganglia. It connects the dorsal and ventral striatum with the thalamus, superior colliculus and pontomedullary brainstem. The SNR is therefore in a strategic position to regulate sensorimotor behavior. We here assessed the effects of SNR lesions on prepulse inhibition (PPI) of the acoustic startle response (ASR), stereotypy and locomotion in drug-free rats, as well as after systemic administration of the dopamine agonist DL-amphetamine (2 mg/kg), and the NMDA receptor antagonists dizocilpine (0.16 mg/kg)

Developing treatment strategies to optimize outcomes in patients with psoriatic arthritis.

INTRODUCTION: Achieving sustained remission in psoriatic arthritis (PsA) remains challenging despite the availability of multiple biologic and targeted synthetic disease-modifying antirheumatic drugs (b/tsDMARDs). Heterogeneous disease expression, comorbidities, and variable durability of response limit treatment effectiveness. AREAS COVERED: This review summarizes contemporary and emerging pharmacologic approaches to PsA, spanning established therapies targeting tumor necrosis factor (TNF), the interleukin-23/interleukin-17 (IL-23/IL-17) axis, and Janus kinase (JAK) signaling, as well as eme

Response Inhibition in borderline personality disorder assessed with a gamified stop signal task.

Borderline personality disorder (BPD) is characterized by pervasive difficulties with self-image, interpersonal relationships, emotion regulation and impulse control. Impulsive behaviors can be assessed using tasks such as the stop-signal task (SST). Traditional SSTs are repetitive and cognitively demanding, requiring sustained attention and effort over extended periods. This can be challenging, particularly for clinical populations who often experience difficulties with attention, emotional regulation, and frustration tolerance. In this study, we examined whether a gamified version of the SST

The initial phase of the dextran-induced anaphylactoid reaction in the rat: a comparison of inhibitors of the blood pressure fall.

The histamine H2-receptor antagonist cimetidine (25-100 mg/kg) caused a partial inhibition of the pronounced blood pressure fall induced by dextran (Macrodex, 40-100 mg/kg) in the rat. The inhibition by cimetidine could not be distinguished from the inhibition achieved by the serotonin D-receptor antagonist bromolysergic acid diethylamide (BOL, 1-4 mg/kg). Doses of cimetidine and BOL that gave submaximum inhibitory effects separately, showed approximately additive effects when combined. The combined effect of these drugs never exceeded the maximum effects of the drugs separately, whereas injec

Palmatine as a potent immunomodulator: Enhancing resistance to Micropterus salmoides rhabdovirus in largemouth bass through innate immune activation and viral suppression.

Micropterus salmoides rhabdovirus (MSRV) poses a significant threat to aquaculture, causing substantial economic losses. In this study, we evaluated the antiviral efficacy and immunomodulatory potential of palmatine, a plant-derived monomer, against MSRV infection in largemouth bass. Our results demonstrated that palmatine significantly inhibited MSRV replication, with a reduction in viral nucleoprotein expression by 85 % at a safe concentration. Additionally, palmatine pre-treatment of EPC cells enhanced their antiviral capacity, with a maximum inhibition rate of 82 % following 24 h pre-incub

Midbrain hypoactivation and mesocortical hypoconnectivity in inhibitory control learning in autism.

Repetitive behaviour, resulting from impaired inhibitory control and error monitoring, represents a core manifestation of autism, with a poorly understood neural basis. Our primary hypothesis was that a striatum-midbrain framework could provide a theoretical basis for understanding neural changes underlying response inhibition in autism. This conceptual approach considered two critical neurobehavioural factors: (a) inhibition as a dynamic process requiring trial-and-error learning, and (b) efficient error learning relying on the striatum receiving dopaminergic signals from the midbrain-a frame

Targeting Plasmodium glutathione S-transferases to overcome antimalaria drug resistance in Southern Africa.

Southern Africa remains burdened by malaria, with growing concern about the emergence of antimalarial drug resistance. While artemisinin resistance has not yet been observed in the region, molecular surveillance in sub-Saharan Africa has identified Plasmodium falciparum k13 mutations known to confer resistance, making it vitally important to take proactive and creative measures to protect current treatment regimens. Glutathione S-transferases (GSTs), the multifunctional enzymes involved in detoxification and redox control, have emerged as key contributors to drug resistance in Plasmodium speci

Understaffed and beleaguered: a national survey of chiefs of police about the post-George Floyd era.

PURPOSE –: The 2020 murder of George Floyd resulted in challenges to policing in the United States of America, but little is known about how police chiefs perceive them. At the same time, chiefs of police wield great influence over public perceptions of crime and disorder, the state of their profession, the laws and policies that govern the conduct of police officers and municipal public safety budgets. It is therefore critical to understand how police perceive the changes to their profession post-Floyd. DESIGN/METHODOLOGY/APPROACH –: This study surveyed a randomly selected national sample of

Redirecting Tumor Evolution with Nanocompiler Precision for Enhanced Therapeutic Outcomes.

Precisely programming the highly plastic tumor expression profile to render it devoid of drug resistance and metastatic potential presents immense challenges. Here, a transformative nanocompiler designed to reprogram and stabilize the mutable state of tumor cells is introduced. This nanocompiler features a trio of components: 2-deoxy-d-glucose-modified lipid nanoparticles to inhibit glucose uptake, iron oxide nanoparticles to induce oxidative stress, and a deubiquitinase inhibitor to block adaptive protein profile changes in tumor cells. By specifically targeting the hypermetabolic nature of t

How Do Drug-Death-Bereaved Parents Adjust to Life Without the Deceased? A Qualitative Study.

Knowledge about how bereaved persons grieve can enhance quality in providing the support and potential services that they need. We aimed to identify ways in which drug-death-bereaved Norwegian parents go on with their lives and what inhibits or promotes adaptation during their grieving. Reflexive thematic analysis was used to analyze 14 semistructured in-depth interviews. We generated three themes: (I) processing grief emotions, (II) proactive coping, and (III) giving and receiving support and assistance. Processing guilt rumination, reflections on blame and a burden of grief emotions characte

Necrosulfonamide ameliorates intestinal inflammation via inhibiting GSDMD-medicated pyroptosis and MLKL-mediated necroptosis.

Inflammatory bowel disease (IBD) is a chronic relapsing disorder of the gastrointestinal tract, while the present therapeutic efficacy is insufficient. In recent years, numerous studies have shown that necrosulfonamide (NSA) played a protective role in many inflammatory diseases by blocking mixed lineage kinase domain-like protein (MLKL) polymerization. However, the protective effect of NSA in dextran sodium sulfate (DSS)-induced colitis has not been reported. In the present study, we used DSS to establish mouse models of acute colitis to explore the proactive effect of NSA. Our study showed t

The Effect of Cerebellar tDCS On Inhibitory Control in Healthy Adults: Exploring Trait Moderation.

Transcranial direct current stimulation (tDCS) research on inhibitory control (IC) has largely focused on prefrontal targets, with inconsistent results, while the cerebellum's contribution to executive control networks remains underexplored. This study examines whether cerebellar anodal tDCS modulates IC in healthy adults, using both proactive (Go/No-Go) and reactive (Stop Signal) inhibition measures, and explores whether dispositional traits moderate stimulation effects. In a double-blind, sham-controlled, within-subjects design, 40 participants received active and sham cerebellar tDCS (2 mA,

Neurocognitive performance in obsessive-compulsive disorder before and after treatment with cognitive behavioral therapy.

BACKGROUND: Cross-sectional studies have reported neurocognitive performance deficits in obsessive-compulsive disorder (OCD), particularly on tasks assessing response inhibition and proactive control over stimulus-driven behaviors (task control). However, it is not clear whether these deficits represent trait-like markers of OCD or are state-dependent. METHODS: This study examined performance on two neurocognitive tasks in OCD patients (N = 26) before and after cognitive behavioral therapy (CBT) and matched healthy controls (HCs, N = 19). Tasks included the stop-signal task (assessing respons

Predictive Modeling of Pharmacokinetic Drug-Drug and Herb-Drug Interactions in Oncology: Insights From PBPK Studies.

Physiologically based pharmacokinetic (PBPK) modeling is increasingly used to anticipate, quantify, and strategically manage drug-drug (DDI) and herb-drug (HDI) interactions that can alter the exposure of chemotherapy agents together with co-administered phytochemicals or nutraceuticals. To evaluate current knowledge, we performed a comprehensive Google Scholar search (2003-2024) and selected studies that employed PBPK platforms, reported quantitative validation, and focused on chemotherapy-related interactions. From these reports, key modeling parameters, validation metrics, and clinically re

Complement activation in pemphigus vulgaris blister fluid.

Total haemolytic complement was reduced in blister fluids of four pemphigus vulgaris patients when compared to serum complement levels and other serum and blister fluid proteins. Complement levels in most control blister fluids, on the other hand, more closely approached their corresponding serum levels. Haemolytic C1, C4, C2, C3 and C5, measured in two pemphigus sera and blister fluids, were not measurable in one blister fluid and were extremely low in the second patient. C3 proactivator (C3PA) was absent from both of these blister fluids. Three of the blister fluids exhibited anti-complement

Targeting amyloid-β in Alzheimer's disease: A critical analysis of clinical trials and their implications for drug development.

Alzheimer's disease (AD), a prevalent neurodegenerative dementia, is characterized by amyloid-β (Aβ) plaques and neurofibrillary tangles, with Aβ playing a central pathogenic role. Approved AD drugs, primarily acetylcholinesterase inhibitors, only alleviate symptoms without modifying disease progression. Aβ-targeting strategies aim to inhibit Aβ production or enhance its clearance, leveraging early deposition for proactive intervention. Preclinical studies show Aβ reduction mitigates neurodegeneration, but clinical trials reveal challenges: γ-secretase inhibitors face off-target toxicities and
