Bioavailability enhancement of atovaquone using hot melt extrusion technology.

Emerging parasite resistance and poor oral bioavailability of anti-malarials are the two cardinal issues which hinder the clinical success of malaria chemotherapy. Atovaquone-Proguanil is a WHO approved fixed dose combination used to tackle the problem of emerging resistance. However, Atovaquone is a highly lipophilic drug having poor aqueous solubility (less than 0.2 μg/ml) thus reducing its oral bioavailability. The aim of the present investigation was to explore hot melt extrusion (HME) as a solvent-free technique to enhance solubility and oral bioavailability of Atovaquone and to develop a

[A collective malarial infestation during a humanitarian mission in west Africa].

Four truck drivers involved in a humanitarian mission across the Sahara towards Mali fell ill 15 days after their return. Plasmodium falciparum malaria (thankfully, non pernicious) was diagnosed with 3 to 4 days delay. The four drivers had been treated with chloroquine and proguanil but the dosage may have been insufficient with regard to their body weight (average weight = 110 kg). These 4 travelers had all slept outside (in Tintane, near Kiffa in Mauritania), without any anti-vectorial protection, whereas their other 8 companions (none of whom caught malaria) had slept in their vehicles. Polymicrobial Infection in an Immigrant Female at the United States-Mexico Border.

Malaria is a highly infectious disease transmitted through the bite of the Anopheles mosquito carrying the parasite of the Plasmodium genus; it presents with cyclical fevers, myalgias, and headaches. In the United States, the vast majority of malaria cases are reported in people who travel abroad, mainly to Africa. These cases are predominantly linked to Plasmodium falciparum or ovale and can be medically treated with artemisinin, chloroquine, or atovaquone-proguanil. We discuss a case of a 38-year-old female immigrant from Venezuela living at an immigration facility who presented to a hospita

Advanced Molecular Detection of Malarone Resistance.

The rapid emergence of drug-resistant malaria parasites during the course of an infection remains a major challenge for providing accurate treatment guidelines. This is particularly important in cases of malaria treatment failure. Using a previously well-characterized case of malaria treatment failure, we show the utility of using next-generation sequencing for early detection of the rise and selection of a previously reported atovaquone-proguanil (malarone) drug resistance-associated mutation.

Frequency of severe neutropenia associated with amodiaquine prophylaxis against malaria.

6 out of 7 patients with severe neutropenia associated with the use of amodiaquine for malaria prophylaxis amodiaquine (400 mg weekly) plus proguanil (200 mg daily); 1 of these patients had also taken cotrimoxazole and another had taken sulphaguanidine. The 7th patient had taken amodiaquine alone, but at a higher dose. A retrospective analysis suggests that the frequency of severe neutropenia complicating amodiaquine taken prophylactically may be as high as 1 in 2000.

Lord of the "rings": A case of Plasmodium falciparum.

Plasmodium falciparum is the most common species to result in severe malaria infection. Examination of blood sample with thick and thin smear is the gold standard diagnostic test and repeat peripheral smear monitoring every 12-24 h should be performed after initiation of treatment to ensure parasite clearance in severe infection. Treatment for severe P. falciparum infection should be initiated without delay, and if artemisinin products are not available, atovaquone-proguanil can be used as an alternative.

Stevens-Johnson syndrome associated with Malarone antimalarial prophylaxis.

To the best of our knowledge, Stevens-Johnson syndrome (SJS) has not been reported previously as an adverse reaction to Malarone, which is a combination of atovaquone and proguanil hydrochloride used for antimalarial prophylaxis and therapy. We describe a 65-year-old patient who had SJS with typical clinical and histopathological findings associated with the use of Malarone prophylaxis for malaria. This report should alert physicians to this severe cutaneous reaction, and Malarone should be added to the list of drugs that can potentially cause SJS.

Amodiaquine induced agranulocytosis: inhibition of colony growth in bone marrow by antimalarial agents.

Bone marrow was cultured in vitro for colonies of granulocytes and macrophages five months after a patient had recovered from amodiaquine induced agranulocytosis. The addition of amodiaquine, chloroquine, and sulfadoxine to the culture was followed by a dose dependent inhibition of colony growth in the patient's marrow but not in normal control bone marrow. Colony growth was, however, unaffected by proguanil, pyrimethamine, and quinine. These findings show that in vitro marrow culture may have important predictive value in some cases of drug induced agranulocytosis.

[Antimalarial drug resistance].

Drug resistant malaria is mostly due to Plasmodium falciparum, the highly prevalent species in tropical Africa, Amazon, and Southeast Asia. P. falciparum is responsible for severe involvement of fever or anemia causing more than a million deaths per year. Rationale for treatment is becoming weak as multiple drug resistance against well-tolerated drugs develops. P. falciparum drug resistant malaria originates from chromosomal mutations. Analyses using molecular, genetic and biochemical approaches showed that: 1) impaired uptake of chloroquine by the parasite vacuole is a common characteristic o

[New drugs against parasitic diseases].

The gap in the antiparasitic armamentarium is still not filled in 2006 and few original and useful drugs were marketed in the last decade. Against malaria, artemether combined with lumefantrine is adopted as first line therapy in a majority of endemic countries but still not available in France and several european countries. As its tolerance and efficacy are satisfactory, atovaquone-proguanil, is taking a major place for prevention and treatment in travellers. Liposomal amphothericin B and miltefosine extended the treatment of visceral leishmaniasis. Nitazoxanide and triclabendazole may be us

[The synthesis of the epidemiology and treatment of malaria of the pregnant woman and the infant in subsaharan Africa].

This article summarizes informations regarding epidemiology, prophylaxis and treatment of Plasmodium falciparum malaria in pregnant women and children in subsaharian Africa. The authors recommend a regular antimalarial chemoprophylaxis using chloroquine and proguanil for pregnant women and particularly for primigravidae as they constitute a group at risk and considering the consequential effects on foetus. Precautions such as bed nets to avoid bites by infected mosquitoes are important. Chloroquine is the drug of choice for treatment in areas where P. falciparum remains sensitive. When chloroq

Infectious diseases in German military personnel after predominantly tropical deployments: a retrospective assessment over 13 years.

OBJECTIVES: Military deployments to the tropics are associated with specific infection risks. To add to the available epidemiological information, infectious disease risks in German military personnel returning from predominantly tropical deployments were assessed. METHODS: Since 2006, German soldiers returning from predominantly tropical deployments have been offered the opportunity of returnee screenings at the Department of Tropical Medicine and Infectious Diseases of the Bundeswehr Hospital Hamburg. Case files and diagnostic results recorded between 2006 and 2018 were retrospectively asse

[Malaise after a trip to Africa: amoebic liver abscess].

During a flight to Kenya, a 42-year-old man took a therapeutic dose of chloroquine because of fever. He regularly travelled to Africa and always took chloroquine and proguanil for malaria prophylaxis. The fever disappeared but he did not fully recover. He complained of malaise and weight loss. Fourteen years previously he had suffered from amoebic dysentery. One month after the onset of the patient's complaints, an amoebic liver abscess was suspected on the basis of his medical history, an elevated ESR and a slight leukocytosis. The diagnosis was confirmed by ultrasonography and serology.

Synthesis, Anticancer Activities, and Mechanism of N-heptyl-containing Biguanide Derivatives.

BACKGROUND: In recent years, the anticancer effects of biguanide drugs have received considerable attention. However, the effective concentration of biguanide drugs to kill cancer cells is relatively high. Thus, we focus on structural modification of biguanides to obtain better antitumor candidates. A previous study in our laboratory has found that a biguanide compound containing the n-heptyl group has potent anticancer activity. However, the effect of different substituents on the benzene ringside of the biguanides on the anti-proliferative activity is unknown. OBJECTIVE: A series of n-hepty
