Association of a Prophylactic Surgical Approach to Stage IV Small Intestinal Neuroendocrine Tumors With Survival.

IMPORTANCE: Primary tumor resection and mesenteric lymph node dissection in asymptomatic patients with stage IV small intestinal neuroendocrine tumors (SI-NETs) are controversial. OBJECTIVE: To determine the association of locoregional surgery (LRS) performed at diagnosis with outcomes in patients with asymptomatic SI-NETs and distant metastases. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included asymptomatic patients with stage IV SI-NETs diagnosed from January 1, 1985, through December 31, 2015, and identified using the prospective database of SI-NETs from Uppsala University Hos

The determinants of angioedema attacks related to dental and gingival procedures in hereditary angioedema patients.

BACKGROUND: The factors causing acute attacks during dental and gingival procedures in patients with hereditary angioedema due to C1 inhibitor (HAE-C1INH) deficiency remain unclear. Our aim is to investigate the patients and dental/gingival procedure characteristics influencing acute attack risk. METHODS: A retrospective review of 638 dental interventions in 40 HAE-C1INH patients was conducted between June and September 2023 using questionnaires, diaries, and medical charts. Data included demographics, clinical details, intervention history, and types of procedures, aiming to understand acute

Assessing the impact of antibiotic prophylaxis in outpatient elective hand surgery: a single-center, retrospective review of 8,850 cases.

PURPOSE: Prophylactic antibiotics have been shown to prevent surgical site infection (SSI) after some gastrointestinal, orthopedic, and plastic surgical procedures, but their efficacy in clean, elective hand surgery is unclear. Our aims were to assess the efficacy of preoperative antibiotics in preventing SSI after clean, elective hand surgery, and to identify potential risk factors for SSI. METHODS: We queried the database from an outpatient surgical center by Current Procedural Terminology code to identify patients who underwent elective hand surgery. For each medical record, we collected p

Fluoroquinolones and surgical prophylaxis.

The objective of surgical prophylaxis is to prevent wound infections associated with surgery. The rates of wound infections vary according to the procedure: less than 3 infections per 100 for clean procedures; up to 4 per 100 for clean-contaminated procedures; and up to 9 per 100 for contaminated procedures. Surgical antimicrobial prophylaxis has been shown in many randomised clinical trials to reduce the incidence of postoperative wound infections. Such prophylaxis is actually recommended in many clean-contaminated and some clean procedures. Because of their antimicrobial, pharmacokinetic, an

When Antimicrobial Treatment and Surgical Prophylaxis Collide: A Stewardship Opportunity.

Background: In 2002, the Centers for Medicare and Medicaid Services (CMS) in collaboration with the Centers for Disease Control and Prevention (CDC) established the Surgical Infection Prevention (SIP) project for the purposes of developing and standardizing quality improvement measures known to reduce the rates of post-operative surgical site infections (SSIs). Four years later the Surgical Care Improvement Project (SCIP), an expansion of SIP, was published in governmental Specifications Manual for National Inpatient Quality Measures and provided several additional initiatives applicable to th

[Prophylactic contralateral orchiopexy in patients with testicular torsion].

The aim of the study was to examine the recommendations for prophylactic contralateral orchiopexy in patients presenting with unilateral testicular torsion in Denmark. A questionnaire was mailed to the 61 surgical departments treating acute urological patients. Prophylactic contralateral orchiopexy was performed in 60 of the 61 departments. Twenty-eight departments routinely performed a combined ipsilateral and contralateral operation. In 20 departments the contralateral procedure was performed immediately in the case of a vital ipsilateral testis, but delayed a median of five (3-12) weeks in

Reduction in Surgical Volume for Metastatic Disease to the Femur and Humerus during the COVID-19 Pandemic with an Associated Backlog of Surgeries.

The purpose of this study was to observe how the COVID-19 pandemic impacted surgical volume for metastatic bone disease to the femur and humerus and whether the proportion of surgeries performed prophylactically significantly changed during the pandemic (March 2020 to April 2021). Patients who underwent surgical management for metastatic disease of the humerus and femur from January 2018 to April 2021 were identified in the PearlDiver Database. These patients were further stratified to surgeries performed prophylactically or not. The monthly changes in surgical volume and percentage of surgeri

Two cases of avascular necrosis after prophylactic pinning of the asymptomatic, contralateral femoral head for slipped capital femoral epiphysis: case report and review of the literature.

BACKGROUND: In a child with a unilateral slipped capital femoral epiphysis (SCFE), the treatment of the radiographically normal, asymptomatic contralateral hip remains controversial. The risks of a subsequent slip have to be measured against the risks involved with an additional surgical procedure. Proponents of prophylactic pinning believe that fixation with a single cannulated screw is a safe method to prevent secondary arthrosis from a missed minor slip. Others argue that in most cases it is an unnecessary procedure and with careful follow-up, any issues with the asymptomatic, normal hip ca

Long-term results of atrial maze surgery in patients with congenital heart disease.

AIMS: Recurrent atrial tachycardia is common after repair of many types of congenital heart disease, and surgical ablation with a maze procedure represents a potential treatment strategy. The objective of this study is to report a single-centre 19 years' experience with maze surgery in congenital heart patients. METHODS AND RESULTS: Patients undergoing maze procedure concomitantly with cardiac surgical procedures were retrospectively analysed. The maze procedure was classified as therapeutic if the patient demonstrated preoperative atrial arrhythmias, or as prophylactic if done because the pa

Prevention of infection after induced abortion: release date October 2010: SFP guideline 20102.

One known complication of induced abortion is upper genital tract infection, which is relatively uncommon in the current era of safe, legal abortion. Currently, rates of upper genital tract infection in the setting of legal induced abortion in the United States are generally less than 1%. Randomized controlled trials support the use of prophylactic antibiotics for surgical abortion in the first trimester. For medical abortion, treatment-dose antibiotics may lower the risk of serious infection. However, the number-needed-to-treat is high. Consequently, the balance of risk and benefits warrants

Decision-making surrounding ovarian cancer risk-reducing surgery: perspectives from a diverse population.

OBJECTIVE: Individuals with BRCA1/2 pathogenic variants (PV) are recommended risk-reducing salpingo-oophorectomy between 35 and 45 years of age to prevent ovarian cancer. Trials evaluating the oncologic safety of risk-reducing salpingectomy are ongoing. One's decision on the type and timing of surgery has implications for fertility, quality of life, and long-term health. Published literature on patient preferences has focused on White women from North America and Europe; thus, findings are not necessarily generalizable to patients from other backgrounds. To address this gap, we sought to asses

[Does ovarian cancer start in the fallopian tubes? Possible implications for preventive adnexal removal].

BACKGROUND: Recent insights in high-grade serous ovarian cancer development are pointing to the fallopian tubes as likely place of origin and not the ovaries themselves. This may have consequences for patients with increased risk of ovarian cancer. Adnexal removal is currently recommended for this patient group at an age of 35-45, which leads to premature menopause. CASE DESCRIPTION: In a 55-year-old woman with a BRCA1 germ line mutation, a high-grade serous carcinoma was unexpectedly diagnosed in both fallopian tubes during preventive adnexal removal. Her ovaries did not have any abnormaliti

The Cost-Effectiveness of Salpingectomies for Family Planning in the Prevention of Ovarian Cancer.

OBJECTIVE: Ovarian cancer is the most lethal gynaecologic cancer. Disease prevention may be the only method to reduce the incidence of ovarian cancer. The Society of Gynecologic Oncology advised that salpingectomies may be an appropriate and feasible strategy for ovarian cancer risk reduction. This study conducted an economic evaluation from a societal perspective of bilateral salpingectomies versus conventional sterilization techniques in the prevention of ovarian cancer. STUDY DESIGN: We performed a micro-cost analysis comparing laparoscopic tubal coagulation, tubal clips and bilateral salp

Effect of prophylactic balloon occlusion of internal iliac artery in pregnancies complicated by placenta previa and accreta.

BACKGROUND: Placenta previa and accreta are serious obstetric conditions that are associated with a high risk of intraoperative massive hemorrhage, the prophylactic intravascular balloon occlusion technique is increasingly used in managing uncontrolled hemorrhage in cesarean section (CS). We aim to examine the clinical effectiveness of prophylactic balloon occlusion of the internal iliac artery (PBOIIA) during CS in improving maternal outcomes for patients with placenta previa and accreta. METHODS: A total of 420 women with placenta previa and accreta who underwent CS from January 2014 to Dec

Utility of the Current Procedural Terminology Codes for Prophylactic Stabilization for Defining Metastatic Femur Disease.

INTRODUCTION: Cohorts from the electronic health record are often defined by the Current Procedural Terminology (CPT) codes. The error prevalence of CPT codes for patients receiving surgical treatment of metastatic disease of the femur has not been investigated, and the predictive value of coding ontologies to identify patients with metastatic disease of the femur has not been adequately discussed. METHODS: All surgical cases at a single academic tertiary institution from 2010 through 2015 involving prophylactic stabilization of the femur or fixation of a pathologic fracture of the femur were

Bone Reconstruction in the Diabetic Foot.

Surgical bone reconstructive procedures of the foot and ankle in diabetic patients must be considered when performing evaluation of a diabetic foot for patients with preulcerative lesions and preexisting wounds. Preventive deformity correction can reduce the potential risk of ulceration, infection, and possible amputation in a patient with an at-risk foot type. It can also expedite wound healing and prevent further breakdown in a patient with lower extremity ulcerations. This article discusses different types of surgical bone reconstructive procedures as preventive and prophylactic deformity c

Intergroup statement: opportunistic salpingectomy-molecular pathology, clinical outcomes and implications for practice (German Ovarian Cancer Commission, the North-Eastern German Society of Gynecologic Oncology (NOGGO), AGO Austria and AGO Swiss).

Opportunistic salpingectomy is defined as the removal of both fallopian tubes as part of a surgical procedure planned for other reasons. The goal is primary prevention of ovarian cancer. The procedure is offered to patients who are not known to be at increased risk of developing ovarian cancer. This is in contrast to high-risk patients with a germline mutation, particularly BRCA1/2, for whom risk-reducing salpingo-oophorectomy is generally recommended. Premalignant cells and early occult cancers have been detected in RRSO specimens in the fimbrial funnel region, but not on the ovarian surface.
