Proton pump inhibitor use increases the risk of peritonitis in peritoneal dialysis patients.

Peritonitis is a major and the most significant complication of peritoneal dialysis (PD). Although some predictors of peritonitis in PD patients are known, the association between proton pump inhibitor (PPI) use and peritonitis has not been characterized. Here, we examined whether PPI use is a risk factor for the development of peritonitis, based on a single-center retrospective analysis of 230 consecutive Japanese PD patients at Narita Memorial Hospital. We assessed the association between PPI use and subsequent first episode of peritonitis using multivariate Cox proportional hazards models,

An exploratory analysis of survival with AIDS using a nonparametric tree-structured approach.

We illustrate an analysis with classification and regression trees applied to survival data. Through this application, we provide a description of the opportunistic diseases and sociodemographic factors that contribute to survival among people with human immunodeficiency virus disease. The analyses are based on 43,795 cases reported to the Centers for Disease Control between January 1, 1984, and December 31, 1987. We used vital status as of December 31, 1989, to estimate mortality rates. We identified Kaposi's sarcoma and opportunistic diseases causing central nervous system damage (cryptococc

Comparison of waitlist and post-transplant outcomes in patients supported with total artificial heart versus continuous biventricular assist devices.

BACKGROUND: Durable biventricular support may be necessary to bridge patients with end-stage biventricular failure to heart transplantation. This study compares waitlist and post-transplant outcomes between patients supported with continuous flow, durable biventricular assist devices (BiVAD), and total artificial heart (TAH). METHODS: Using the UNOS registry, we analyzed adult (≥18 years old), first-time transplant candidates with TAH or BiVAD at the time of listing or transplantation from 10/1/2010-10/31/2020, with follow-up through 3/31/2022. Recurrent injury event-time analysis.

Public health decision making based on data sources that are characterized by a lack of independence and other complicating factors requires the development of innovative statistical techniques. Studies of injuries in occupational cohorts require methods to account for recurrent injuries to workers over time and the temporary removal of workers from the 'risk set' while recuperating. In this study, the times until injury events are modelled in an occupational cohort of employees in a large power utility company where employees are susceptible to recurrent events. The injury history over a ten-

Estimating Short-Term and Long-Term Survival for Patients With Kidney Failure Using a Mixture Survival Model.

RATIONALE & OBJECTIVE: Traditional survival models assume all patients receiving kidney replacement therapy (KRT) may be grouped into one population, overlooking long-term survivors, particularly successful transplant recipients, and may fail to appreciate the disparities in minority populations. On the other hand, a mixture survival model allows for the estimation of hazard and odds ratios of all-cause mortality in patients with kidney failure undergoing either dialysis or transplantation. STUDY DESIGN: This retrospective cohort study analyzed survival outcomes using a proportional hazards m

Prediction of Hepatocellular Carcinoma by Liver Stiffness Measurements Using Magnetic Resonance Elastography After Eradicating Hepatitis C Virus.

INTRODUCTION: Liver fibrosis stage is one of the most important factors in stratifying the risk of developing hepatocellular carcinoma (HCC). We evaluated the usefulness of liver stiffness measured by magnetic resonance elastography (MRE) to stratify the risk of developing HCC in patients who underwent MRE before receiving direct-acting antivirals (DAAs) and subsequently achieved sustained virological response (SVR). METHODS: A total of 537 consecutive patients with persistent hepatitis C virus who underwent initial MRE before DAA therapy and achieved SVR were enrolled. Factors associated wit

Risk factors for upper tract urothelial recurrence following local excision of bladder cancer.

The mechanism of upper tract recurrence after local excision of bladder cancer remains unknown. This study was designed to identify risk factors for upper tract urothelial recurrence following local tumor excision of bladder cancer. To identify 76 597 bladder cancer patients, comprising 76 537 nonrecurrence and 60 recurrence patients, the Surveillance, Epidemiology, and End Results database was used. Kaplan-Meier analysis and univariate and multivariate Cox proportional hazards regression models were used to determine the risk factors. Compared with the nonrecurrence group, the recurrence grou

On semiparametric accelerated failure time models with time-varying covariates: A maximum penalised likelihood estimation.

The accelerated failure time (AFT) model offers an important and useful alternative to the conventional Cox proportional hazards model, particularly when the proportional hazards assumption for a Cox model is violated. Since an AFT model is basically a log-linear model, meaningful interpretations of covariate effects on failure times can be made directly. However, estimation of a semiparametric AFT model imposes computational challenges even when it only has time-fixed covariates, and the situation becomes much more complicated when time-varying covariates are included. In this paper, we propo

Prognostic value of Glasgow prognostic score in patients undergoing hemodialysis.

INTRODUCTION: Usefulness of the Glasgow prognostic score (GPS), modified GPS (mGPS), and high-sensitivity mGPS (HS-mGPS) in the prognosis of patients undergoing hemodialysis remains unclear. This study aimed to investigate this. METHODS: The GPS, mGPS, and HS-mGPS were calculated retrospectively in 339 patients undergoing hemodialysis; their association with all-cause mortality was analyzed using the Kaplan-Meier method and Cox proportional hazards models. RESULTS: Survival rates decreased according to the GPS (0, 1, and 2), but were similar between the mGPS and HS-mGPS. In the multivariate

Survival analysis of left-truncated and right-censored HIV data: comparison of Cox regression and alternative models.

BACKGROUND: This study examined the impact of delayed entry in HIV cohorts by analyzing left-truncated and right-censored (LTRC) survival data from 69 HIV-positive male patients (24 deaths) followed at a tertiary infectious diseases center. The primary objective was to empirically compare commonly used survival models in estimating survival and identifying prognostic factors under LTRC conditions. METHODS: Three LTRC-adapted modeling frameworks were applied with identical covariates: the semiparametric Cox proportional hazards model, accelerated failure time (AFT) models, and parametric propo

Exosurf versus Survanta surfactant preparations: proportional-hazards regression analysis of time to successful extubation and discontinuation of oxygen therapy.

Time to successful extubation (VENT-DC) and time to discontinuation of oxygen therapy (OX-DC) for infants treated with Exosurf and Survanta surfactant preparations were compared in an observational study. Proportional-hazards (Cox) regression was used to adjust for confounding by differences in baseline risk factors between treatment groups. The Exosurf/Survanta hazard ratios, representing the relative instantaneous probabilities of the next event (VENT-DC or OX-DC) occurring in the Exosurf versus Survanta groups, were close to the null value of 1.0 and were not statistically significant. No cure for cure models: killed by competing risks.

Cure models have become increasingly popular over the last couple of decades. An appealing element of cure models is the idea that part of the population is immune to the event of interest. We argue that, in medical applications, the use of cure models is limited. Thus, true cure cannot exist if the event of interest includes death, as death is inevitable, and if death is excluded then competing risks must be accounted for. In this case, we find it more natural to model the cumulative incidence of both the event of interest and competing events over time, avoiding unreliable estimates at infin

Fitting high-dimensional mixture cure models using the hdcuremodelsR package.

BACKGROUND AND OBJECTIVE: Time-to-event outcomes are often of interest in biomedical studies. When the dataset includes long-term survivors or subjects who will not experience the event of interest, mixture cure models (MCMs) should be fit. Further, it is clinically relevant to identify molecular features from high-throughput assays that are associated with time-to-event outcomes, both to elucidate important pathways and to identify molecular features that may be therapeutic targets or for developing improved risk stratification systems. Herein, we describe our hdcuremodelsR package that can b

A practical overview and decision tool for analyzing recurrent events in mental illness: A review.

Mental illnesses are chronic conditions in which an individual will often experience recurrent outcomes such as hospitalization, symptomatic relapse or self-harm behaviours. Most clinical research in psychiatry considers only the first event, and does not analyze subsequent recurrent events. Methods exist to analyze recurrent events; however, these methods are underused in the psychiatric research literature. This review identifies that recurrent events can be analyzed using a time homogenous or time-to-recurrent-event (TTRE) framework. The TTRE framework is underutilized in psychiatric resear

Fitting a shared frailty illness-death model to left-truncated semi-competing risks data to examine the impact of education level on incident dementia.

BACKGROUND: Semi-competing risks arise when interest lies in the time-to-event for some non-terminal event, the observation of which is subject to some terminal event. One approach to assessing the impact of covariates on semi-competing risks data is through the illness-death model with shared frailty, where hazard regression models are used to model the effect of covariates on the endpoints. The shared frailty term, which can be viewed as an individual-specific random effect, acknowledges dependence between the events that is not accounted for by covariates. Although methods exist for fitting

causalCmprsk: An R package for nonparametric and Cox-based estimation of average treatment effects in competing risks data.

BACKGROUND AND OBJECTIVE: Competing risks data arise in both observational and experimental clinical studies with time-to-event outcomes, when each patient might follow one of the multiple mutually exclusive competing paths. Ignoring competing risks in the analysis can result in biased conclusions. In addition, possible confounding bias of the treatment-outcome relationship has to be addressed, when estimating treatment effects from observational data. In order to provide tools for estimation of average treatment effects on time-to-event outcomes in the presence of competing risks, we develope

Variable selection for high-dimensional partly linear additive Cox model with application to Alzheimer's disease.

Variable selection has been discussed under many contexts and especially, a large literature has been established for the analysis of right-censored failure time data. In this article, we discuss an interval-censored failure time situation where there exist two sets of covariates with one being low-dimensional and having possible nonlinear effects and the other being high-dimensional. For the problem, we present a penalized estimation procedure for simultaneous variable selection and estimation, and in the method, Bernstein polynomials are used to approximate the involved nonlinear functions.
