Proton Therapy May Reduce the Risk of Cancer Progression During Immune Checkpoint Inhibitor Therapy: A Propensity Score-Matched Analysis of Intensity Modulated Proton Versus Photon Radiation Therapy.

PURPOSE: Intensity modulated proton therapy (IMPT) may preserve the immune response more effectively than intensity modulated photon radiation therapy (IMRT) owing to its dosimetric advantages, making it a potentially superior modality in immunotherapy. This study aimed to evaluate the clinical benefits of IMPT versus IMRT during immune checkpoint inhibitors (ICIs) treatment. METHODS AND MATERIALS: We retrospectively analyzed the data of 466 patients (IMPT group, n = 109; IMRT group, n = 357) who received radiation therapy (RT) during ICI therapy between July 2022 and September 2024. High-energy proton beam radiation therapy for gynecologic malignancies. Potential of proton beam as an alternative to brachytherapy.

Fifteen patients with advanced gynecologic malignancies were treated with high-energy proton beam radiation therapy (RT) at the Particle Radiation Medical Science Center (PARMS), Tsukuba University, Japan, from 1983 to 1987. The potential of proton beam RT as an alternative to conventional brachytherapy was evaluated. Except for one local recurrence, 14 of 15 patients were locoregionally controlled for 15 to 57 months. Two-year local control rate and 2-year survival rate were 92.3% and 93.3%, respectively. Two cases of transient, radiation-induced proctitis (neither of which required surgical

Multi-Institutional Proton Beam Therapy Outcomes for Hepatocellular Carcinoma From the Proton Collaborative Group REG001-09 Registry Study.

PURPOSE: External beam radiation therapy (EBRT) for hepatocellular carcinoma (HCC) is commonly delivered with x-rays, although proton beam therapy (PBT) offers a unique dosimetric advantage that for some patients may translate into meaningful clinical benefit. Most published outcomes of PBT for HCC come from Asian institutions, whereas there is a scarcity of data from Western countries. METHODS AND MATERIALS: We identified patients with nonmetastatic HCC patients treated with definitive PBT at 9 institutions in the United States and enrolled on the Proton Collaborative Group REG001-09 prospec

Quantitative assessment of anatomical change using a virtual proton depth radiograph for adaptive head and neck proton therapy.

The aim of this work is to demonstrate the feasibility of using water-equivalent thickness (WET) and virtual proton depth radiographs (PDRs) of intensity corrected cone-beam computed tomography (CBCT) to detect anatomical change and patient setup error to trigger adaptive head and neck proton therapy. The planning CT (pCT) and linear accelerator (linac) equipped CBCTs acquired weekly during treatment of a head and neck patient were used in this study. Deformable image registration (DIR) was used to register each CBCT with the pCT and map Hounsfield units (HUs) from the planning CT (pCT) onto t

Proton beam therapy for the treatment of uveal melanoma in Scotland.

BACKGROUND AND AIMS: In Scotland, uveal melanomas are primarily treated with eye-preserving modalities with a low primary enucleation rate. Proton beam therapy (PBT) is reserved for those not suitable for brachytherapy. The authors' aim was to review the outcome of Scottish patients undergoing treatment for uveal melanoma with PBT. METHOD: All ciliary body or choroidal melanomas treated initially with PBT between 1 January 1993 and 30 March 2008 were identified. Data were gathered from retrospective review of case notes, database information, PBT planning data, ultrasound and pathology report

Ultrafast proton delivery with pin ridge filters (pRFs): repurposing single-energy delivery for motion management in proton therapy.

OBJECTIVE: Highly conformal proton stereotactic body radiation therapy (SBRT) remains vulnerable to respiratory interplay, and standard mitigation techniques are either patient-dependent or enlarge the irradiated volume. Long delivery times dominated by energy-layer switching (ELS) limit the use of breath-hold (BH). We propose using pin-ridge filters (pRF) to eliminate ELS, shorten beam-on time, and reduce margins. APPROACH: A monoenergetic pRF plan was constructed, with the highest clinical energy (Emax) obtained from the reference internal target volume (ITV)-based intensity modulated proto

Preliminary assessment of proton linear energy transfer distribution in patients with MRI-guided proton therapy: a simulation study.

Objective. Magnetic resonance imaging (MRI)-guided proton therapy is under development as an advanced technique that combines proton therapy with real-time MRI imaging, offering improved tumor targeting and better protection of adjacent healthy tissues. However, clinically relevant interactions between magnetic fields and linear energy transfer (LET) remain unexplored. This study investigated the LET distributions of primary and secondary protons with an emphasis on the influence of magnetic fields on both tumors and organs at risk.Approach.Monte Carlo simulations were performed using the Gean

Advances in Proton Therapy for the Management of Head and Neck Tumors.

Proton therapy (PBRT) is a form of external beam radiotherapy with several dosimetric advantages compared with conventional photon (x-ray) radiotherapy. Unlike x-rays, protons deposit most of their dose over a finite range, with no exit dose, in a pattern known as the Bragg peak. Clinically, this can be exploited to optimize dose to tumors while delivering a lower integral dose to normal tissues. However, the optimal role of PBRT is not as well-defined as advanced x-ray-based techniques such as intensity-modulated radiotherapy.

Rethinking the Combination of Proton Exchanger Inhibitors in Cancer Therapy.

Microenvironmental acidity is becoming a key target for the new age of cancer treatment. In fact, while cancer is characterized by genetic heterogeneity, extracellular acidity is a common phenotype of almost all cancers. To survive and proliferate under acidic conditions, tumor cells up-regulate proton exchangers and transporters (mainly V-ATPase, Na⁺/H⁺ exchanger (NHE), monocarboxylate transporters (MCTs), and carbonic anhydrases (CAs)), that actively extrude excess protons, avoiding intracellular accumulation of toxic molecules, thus becoming a sort of survival option with many similarities

Dosimetric impact and safety of uninterrupted fluoroscopic-gated proton therapy.

BACKGROUND: Fluoroscopic-gated proton therapy (FGPT) enables precise dose delivery to tumors affected by respiratory motion by tracking internal fiducial markers and delivering the proton beam when the marker is within a gating window. However, scatter radiation from fluoroscopic x-rays may be detected by the dose monitor (DM) and mistakenly counted as proton monitor units (MU). To mitigate this issue, proton beam delivery is typically interrupted during the fluoroscopy pulse, a method known as interrupted continuous delivery (ICD). If the contamination from scattered fluoroscopic x-rays is su

Proton beam therapy induces protective immunity via HMGB1-dependent signaling.

Proton beam therapy is widely regarded for its cost-effectiveness, precision, and protection of normal tissues. Emerging evidence shows that conventional radiotherapy can inhibit primary tumors and promote immunogenicity of distant tumors, possibly via damage-associated molecular patterns (DAMPs) like calreticulin (CRT) and high mobility group box 1 (HMGB1) released during immunogenic cell death (ICD). However, the role of proton beams in inducing DAMPs and enhancing immunogenicity remains unclear. This study aimed to investigate the effects of proton beam-induced DAMPs on the colonization of

Deep learning based linear energy transfer calculation for proton therapy.

Objective. This study aims to address the limitations of traditional methods for calculating linear energy transfer (LET), a critical component in assessing relative biological effectiveness (RBE). Currently, Monte Carlo (MC) simulation, the gold-standard for accuracy, is resource-intensive and slow for dose optimization, while the speedier analytical approximation has compromised accuracy. Our objective was to prototype a deep-learning-based model for calculating dose-averaged LET (LETd) using patient anatomy and dose-to-water (DW) data, facilitating real-time biological dose evaluation and LE
