[Steps in the recovery from severe depression. On the psychosomatic aspects of a convalescent process].

In retrospect, several phases of the healing process are easily distinguishable: Initiated with a terminal inpatient antidepressant therapy, continued by a tranquilizer-hypnotic withdrawal treatment, the psychotherapeutic mastering of the life-crises which provoked the mental disturbance in the first place, then the patient's going back to work, and, finally, the total mental stabilization on a mature level. The psychopathologic and psychosomatic symptoms appearing in the process are interpreted as expressions of spontaneous healing and not to be confounded with symptoms of withdrawal and/or r

Depression in Older Adults: A Treatable Medical Condition.

Depression is not a normal part of the aging process. Depression in older adults is a treatable medical condition; a variety of psychotherapeutic options are available. Electroconvulsive therapy is a useful treatment. Older patients must be viewed in their medical, functional, and social context for effective management. Cognition must be assessed along with mood in the older patient with depression.

Client resistance predicts outcomes in cognitive-behavioral therapy for panic disorder.

Objective: Little is known about how therapy processes relate to outcome in cognitive-behavioral therapy (CBT) for panic disorder (PD). This study examined whether client resistance predicts CBT for PD outcomes beyond the effects of established pre-treatment predictors. A secondary aim was to assess the consistency of resistance over treatment. Method: Data were from 71 adults participating in up to 24 biweekly sessions of CBT in a randomized controlled trial. Panic severity was assessed before, during (at Weeks 1, 5, and 9), and at termination of treatment (Week 12) using the Panic Disorder S

The power of the group: How neuroscience supports expanding the therapeutic dyad through group psychotherapy.

Group psychotherapy represents a therapeutic modality that provides unique affordances for patients, allowing layers of engagement by the patient in the psychotherapeutic process that are not available in dyadic psychotherapeutic treatment. This article considers psychodynamic group psychotherapy through the lens of current neuroscientific concepts. In doing so, we offer a framework for clinicians to consider how beliefs and habits that are originally adaptive become maladaptive and persist, and why patients hold on to maladaptive beliefs about the world and their own agency that are not refle

Dementia in the aged.

Dementia can be feature of several distinct pathologic processes, including extracranial and intracranial disease, iatrogenesis, and depression. In approximately 20 per cent of cases, dementia is caused by a potentially reversible disease; in the remaining 80 per cent, it is associated with primary brain disease for which no specific treatment is yet known. In all cases, thorough medical and psychiatric evaluation is indicated. In a large percentage of cases of primary dementia, a multiple therapeutic approach, which includes psychopharmacologic and psychotherapeutic modalities, can arrest and

Treatment engagement in the early phase of cognitive-behavior therapy for panic disorder: A grounded theory analysis of patient experience.

Exposure-based cognitive-behavior therapy (EBCBT) is the treatment of choice for panic disorder (PD). However, little is known about early treatment processes that facilitate retention in treatment and positive outcomes of PD treatment. We studied the development of early treatment process with semi-structured individual in-depth interviews with 12 patients with PD, conducted post session 3. Grounded theory was used to analyze the transcripts. The development of early treatment process was captured by four thematic categories: approaching the problem, easing in and opening up, building trust a

An intensive case analysis of client metacognition in a good-outcome psychotherapy: Lisa's case.

The authors analyzed the successful case of Lisa, a client with major depression, using the Metacognitive Assessment Scale (MAS). Consistently with the literature on depression, the authors hypothesize that Lisa's ability to reflect on mental states--here metacognition--is marginally affected. The authors found that Lisa was better at describing her own mind rather than understanding the mind of the others. Furthermore, the most severe impairment was in using metacognition to cope with mental state source of distress and to enact strategies to solve interpersonal problems. During the therapy s

Early therapy interpersonal process differentiating clients high and low in outcome expectations.

The present study compared early interpersonal process, during resistant and cooperative segments of session 1, between clients who went on to have low (n=9) versus high (n=8) treatment outcome expectations. Baseline outcome expectations were assessed prior to any therapist contact. During resistance episodes, there was substantially less affiliative reciprocity (i.e., complementarity), and greater client separation and hostility in the low versus high expectations group. During cooperation episodes, therapists of low versus high outcome expectation clients engaged in lower levels of affirming

Goal setting in psychotherapy: the relevance of approach and avoidance goals for treatment outcome.

The present study is the first aimed at investigating the influence of goal definition on treatment outcome in a sample of depressed patients. Data from 657 inpatients admitted to a psychosomatic clinic in Germany being treated in a cognitive-behavioral therapy program were analyzed. Treatment goals were identified as either approach or avoidance, and the sample was classified accordingly. Patients who identified approach goals only were placed in the approach group, and those who identified at least one avoidance goal were placed in the avoidance group. Results showed that framing goals using

When ELIZA meets therapists: A Turing test for the heart and mind.

"Can machines be therapists?" is a question receiving increased attention given the relative ease of working with generative artificial intelligence. Although recent (and decades-old) research has found that humans struggle to tell the difference between responses from machines and humans, recent findings suggest that artificial intelligence can write empathically and the generated content is rated highly by therapists and outperforms professionals. It is uncertain whether, in a preregistered competition where therapists and ChatGPT respond to therapeutic vignettes about couple therapy, a) a p

A systematic review of interoception and therapeutic outcome research: Implications for psychotherapeutic practice.

UNLABELLED: Interoception has been recognized as a key process linking bodily regulation, emotional experience and psychological well-being, yet its role within psychotherapy remains theoretically and empirically fragmented. Beyond outcome variability, this fragmentation reflects differences in how interoception is conceptualized and operationalized in psychotherapy research. OBJECTIVE: This systematic review examined associations between interoceptive processes and psychotherapeutic outcomes-including interoception as a pre-therapy client factor, as a pre-post change measure and as a therape

Efficacy of a psychotherapeutic group intervention for patients with Post-COVID-19 condition: a randomized controlled trial (PsyLoCo study).

BACKGROUND: Persistent physical symptoms such as fatigue following SARS-CoV-2 infection are common and often accompanied by cognitive complaints. Alongside biological mechanisms, psychosocial processes can contribute to symptom persistence. Psychotherapeutic interventions could be beneficial in symptom reduction. However, evidence of group-based psychotherapies tailored to Post-COVID-19 condition (PCS) remains limited. METHODS: In this randomized waitlist-controlled crossover study in an outpatient psychosomatic setting, adults with prior SARS-CoV-2 infection and persisting functionally impai

The importance and limitations of polysomnography in insomnia disorder-a critical appraisal.

The importance polysomnography (PSG) in the diagnosis and treatment process of insomnia disorder (ID) remains highly disputed. This review summarises the state of the science regarding PSG indications and findings in ID, and the indications to conduct PSG in ID as stated by relevant guidelines. It then highlights the most relevant questions regarding the topic, including the relevance of ID subtyping, to allow an individualised pharmacological or psychotherapeutic treatment approach.

Change processes in psychotherapy for patients presenting with histrionic personality disorder.

OBJECTIVE: Histrionic personality disorder (HPD) with a lifetime prevalence rate of 1.8% is an under-researched psychiatric diagnosis. The present study therefore aimed to investigate both the processes and outcomes of psychotherapy for HPD in a non-controlled study. METHODS: A total of 159 patients diagnosed with HPD were recruited and received clarification-oriented psychotherapy. Sessions 15, 20, and 25 were video-recorded and analysed using the Process-Content-Relationship Scale. Therapy outcome was assessed with symptom measures at intake and discharge. Hierarchical linear modelling was

The relationship between therapist competence and homework compliance in maintenance cognitive therapy for recurrent depression: secondary analysis of a randomized trial.

Meta-analyses reveal that homework compliance is associated with a better treatment outcome. However, little is known about the processes that could be responsible for patient compliance with homework. It has been proposed that therapist competence, in particular with respect to reviewing homework, is highly relevant for homework compliance. The present study is a secondary analysis of a randomized controlled trial. Altogether, 54 patients with recurrent depressive disorder (currently in remission) who were treated with maintenance cognitive therapy (MCT), were considered. One videotaped treat

Interpersonal predictors of group therapy outcome for complicated grief.

This study investigated three aspects of patients' interpersonal functioning as predictors of outcome for two forms of group psychotherapy for complicated grief. Patients presented with a variety of death losses and met criteria for complicated grief. The three aspects of interpersonal functioning were the patient's (1) attachment to the lost person, (2) quality of object relations (QOR), and (3) level of recent social role functioning. A more secure attachment to the lost person and better social role functioning were associated with more favorable outcome in both forms of therapy. In additio

Problems of internalization: a button is a button is-not.

Analysts hope to help the patient internalize a relationship with the analyst that contrasts with the original archaic object relation. In this paper, the author describes particular difficulties in working with a patient whose defenses and anxieties were bulimic, her movement toward internalization inevitably undone. Several issues are considered: how does the nonsymbolizing patient come to internalize the analyst's understanding, and when this does not hold, what is the nature of the patient's subsequent methods of dispersal? When the patient can maintain connection to the analyst as a good
