MR-guided navigation for botulinum toxin injection in the lateral pterygoid muscle. First results in the treatment of temporomandibular joint disorders.

INTRODUCTION: Hyperactivity of the lateral pterygoid muscle (LPM) is one of the presumed mechanisms of the onset of temporomandibular disorders (TMD) and explains why intramuscular injection of botulinum toxin (BT) may be indicated for treatment. Intramuscular injection without guidance is difficult because of its deep location. The objectives of this study were to determine the feasibility of MR-guided navigation for BT injection in the LPM in TMD and to report about our first results. PATIENTS AND METHODS: Six patients suffering from persistent myogenic TMD were enrolled in a prospective st

Magnetic resonance imaging evaluation of discal attachment of superior head of lateral pterygoid muscle in individuals with symptomatic temporomandibular joint.

OBJECTIVE: There has been much speculation on the role of the lateral pterygoid muscle (LPM) in temporomandibular joint (TMJ) internal derangement. As a result, this muscle has been the focus of many reports attempting to understand its possible role in TMJ internal derangement. STUDY DESIGN: Magnetic resonance (MR) imaging exams of 108 TMJ disorder patients were assessed for types of morphologic insertion of the superior head of LPM to the condyle-disc complex and possible correlation with disc position. The investigation was made on the sagittal T1-weighted MR images in closed- and open-mou

Treatment of severe temporomandibular joint clicking with botulinum toxin in the lateral pterygoid muscle in two cases of anterior disc displacement.

A novel treatment procedure is introduced for severe clicking of the temporomandibular joint (TMJ) associated with anterior disc displacement (ADD), using injections with botulinum toxin (BTX-A) in the lateral pterygoid muscle (LP). It has been suggested that ADD may be caused, precipitated, or maintained by LP activity, but the role of the LP in the dynamics of the TMJ clicking is uncertain. The case report includes 2 women, followed with clinical examinations, TMJ imaging, and electromyography (EMG), in whom local anesthetics in the LP could abolish the clicking for several days. BTX-A block

Quantitative MRI analysis of the lateral pterygoid muscle in temporomandibular joint disc displacement: attachment type, volume, and signal intensity.

The role of morphological and radiological characteristics of the lateral pterygoid muscle (LPM) in the progression of temporomandibular joint (TMJ) disc displacement remains uncertain. This study was performed to investigate the association between LPM attachment type, volume, and signal intensity ratios (SIRs) and disc displacement with reduction (DDwR) and without reduction (DDwoR). A total of 188 TMJs from 94 patients were categorized into three groups: healthy controls (n = 62), DDwR (n = 62), and DDwoR (n = 64). LPM attachment type, superior and inferior LPM volumes, and SIRs were assess

Ultrasound-guided lateral pterygoid muscle botulinum toxin: an injection for recurrent temporomandibular joint dislocation in a brain injury patient.

Botulinum toxin type A (BTX-A) injection using nerve stimulation or electromyography for recurrent temporomandibular joint (TMJ) dislocation has been reported for several years. However, using the available equipment like a nerve stimulator or an electromyograph is uncommon, and ultrasound guidance is convenient and requires no additional resources. In this report, we used ultrasound as a tool to achieve BTX-A injections in a patient with a traumatic brain injury to treat her TMJ dislocation. One week after the injections, she had no more dislocation. She remained symptom free during the 3 mon

Myositis Ossificans Traumatica of the Medial Pterygoid Muscle After Third Molar Tooth Extraction: A Case Report and Review of Literature.

Myositis ossificans (MO) is a rare non-neoplastic disorder characterized by heterotopic ossification in soft tissues, mainly muscles. MO traumatica is characterized by ossification of the soft tissues after acute or repetitive trauma, burns, or surgical intervention. Muscular or soft tissue trauma is usually present as the underlying etiology. MO traumatica usually involves the extremity muscles. The number of reported cases involving the masticatory muscles is extremely low. The most common clinical sign of this condition is progressive limitation of mouth opening. Evaluation of the ultrasonography-guidance for botulinum toxin type a injection into lateral pterygoid muscle in patients with TMJ anterior disc displacement with reduction.

Anterior disc displacement with reduction (ADDwR) is a common temporomandibular joint disorder associated with pain, joint sounds, and limited jaw movement. Conservative treatments are considered first-line but often yield unsatisfactory results. Botulinum toxin type A (BTX-A) injection into the lateral pterygoid muscle (LPM) offers a minimally invasive alternative, and ultrasonography enhances safety and accuracy through real-time, radiation-free guidance. This single-arm clinical trial included ten patients (20 TMJs), aged 19-38 years, diagnosed with ADDwR. Each patient received 25 units of

Evaluating the correlation between the lateral pterygoid muscle attachment type and internal derangement of the temporomandibular joint with an emphasis on MR imaging findings.

OBJECTIVES: Disc displacement is accepted as one of major findings in temporomandibular disorders. Correlation between lateral pterygoid muscle (LPM) attachment type to the disc-condyle complex and TMJ dysfunction has rarely been discussed and still not clarified. The purpose of this study was to assess the prevalence LPM attachment type to the disc-condyle complex, and to investigate whether these attachment types are linked to MR imaging findings of ID and TMJ dysfunction in a Turkish population. STUDY DESIGN: Ninety-eight TMJs in 49 patients (32 males, 17 females, mean age=36 years) with o

The effects of botulinum toxin A injection on the lateral pterygoid muscle in patients with a painful temporomandibular joint click: a randomized clinical trial study.

BACKGROUND: Temporomandibular disorder (TMD) is the main cause of non-dental pain in orofacial area. The most common symptoms of TMD are joint pain, joint sound and limitation of jaw function. Botulinum toxin (BTX) injection is considered a potential treatment for TMD due to its pain-relieving properties and its ability to reduce muscle activity. Most of the studies are case series and further investigations are required to prove the efficacy of this treatment modality. Thus, in this study, we aimed to investigate the effect of BTX-A injection on the lateral pterygoid (LP) muscle and to evalua
