Mental Health of Youths Who Use Puberty Blockers.

IMPORTANCE: Puberty blockers are an established, reversible intervention for central precocious puberty in children and gender dysphoria in adolescents. Despite broad clinical support, these treatments are currently politically contested. OBJECTIVE: To characterize the mental health of transgender youths prescribed puberty blockers compared with cisgender peers who received these medications for treatment of precocious puberty and with transgender and cisgender youths who did not receive these medications. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study used data from a US

[Plasma dehydroepiandrosterone sulfate (DHAS) in paediatric endocrinology (author's transl)].

The normal values of plasma DHAS were determined by measuring the levels in 637 normal subjects whose ages ranged from birth to adulthood. The changes induced by adrenal stimulation and suppression, and by testicular stimulation tests were studied. Knowledge of the normal values and the changes with age is essential for the interpretation of the results in children with endocrine disorders. Normal DHAS levels were found in cases of precocious puberty. High levels were found in pseudo-precocious puberty and in premature pubarche. Low levels were detected in primary or secondary adrenal insuffic

Growth and pubertal outcome of three-years medical treatment of peripheral precocious puberty in a boy with McCune-Albright Syndrome: a case report.

BACKGROUND: Peripheral precocious puberty (PPP) is an endocrine disorder characterized by premature, autonomous gonadal or extragonadal sexual steroid secretion. Accounting for about 20% of cases of precocious puberty, PPP is usually caused by rare diseases, including congenital adrenal hyperplasias, hormone-secreting gonadal and adrenal tumours, and McCune-Albright Syndrome (MAS). MAS is a rare, genetic disorder, mainly characterized by bone dysplasia, skin hyperpigmentation, and endocrine, hyperfunctioning disorders, including, as reported, PPP. Being a rare disease, PPP management in MAS pa

[Monitoring the treatment of precocious puberty with a GnRH-analog (buserelin): comparison of the GnRH-test with nocturnal pulsatility of LH, testosterone and 17-beta-estradiol].

We studied in 10 children (3 boys and 7 girls), affected by precocious puberty and treated for a period of more than 6 months with the GnRH-analog Buserelin, the efficacy of the GnRH test versus the study of spontaneous nocturnal secretion of LH, testosterone and 17 beta-estradiol as a means of testing the gonadotropin suppression. All the subjects have shown a total suppression of LH during the GnRH test as well as a nocturnal profile of LH, testosterone and 17 beta-estradiol of a prepubertal pattern. Clinically all the patients were treated successfully. Considering the effectiveness of both

Adrenal steroid secretion in girls with pseudoprecocious puberty due to autonomous ovarian cysts.

To evaluate the role of adrenal steroids in pseudoprecocious puberty due to large ovarian follicular cysts, we studied the serum 17-hydroxyprogesterone response to a combination of dexamethasone suppression followed by iv ACTH administration in two girls and compared the results to those in girls with premature thelarche, normal prepubertal girls, and a girl with true precocious puberty. Although basal serum 17-hydroxyprogesterone levels were normal in all subjects, there was incomplete suppression of 17-hydroxyprogesterone with dexamethasone in the two girls with pseudoprecocious puberty and

A simplified gonadotrophin-releasing hormone test for precocious puberty.

BACKGROUND AND OBJECTIVE: The gonadotrophin-releasing hormone (GnRH) test is important for the evaluation of children with disorders of puberty. Traditionally, the test determines the peak LH and the peak FSH responses from 5 to 8 blood samples, which makes it cumbersome and costly. We suspected that the test could be simplified to require fewer hormonal measurements. DESIGN: A retrospective clinical study. PATIENTS: Subjects were 44 girls and 7 boys with signs of precocious puberty, of whom 39 were diagnosed as definite central precocious puberty. Fifty-five diagnostic tests and 39 tests fo

Normal onset of puberty have definitions of onset changed?

Puberty is the sequence of events that culminates in the ability to procreate. It is widely accepted that the onset of puberty in girls occurs on average at 8 years of age and that onset prior to 8 years of age is precocious puberty. As a result of the cross-sectional study by the American Association of Pediatrics, a movement exists to change the age limit of the onset of puberty to 6 years of age in black girls and 7 years of age in white girls. We should be cautious in adhering to strict age limits when diagnosing precocious puberty. Also the rapidity and progression of puberty should be ev

Prevalence and effects on outcomes of intracranial tumors and structural alterations in children with central precocious puberty and early and fast puberty.

OBJECTIVE: To investigate intracranial lesion prevalence in children with central precocious puberty (CPP) and early and fast puberty (EFP), and assess impacts on progression and GnRHa response. METHODS: This retrospective study included 1,087 CPP/EFP patients (2011-2023) who underwent brain MRI. Patients were stratified by MRI findings: no lesion, other (incidental) lesions, and pathogenic lesions (hypothalamic hamartoma/glioma). Clinical, hormonal, bone age, and growth parameters were compared. GnRHa dosing and therapeutic responses were evaluated. RESULTS: Intracranial lesions were detect

Neuroendocrine effects of haloperidol in an adolescent with Gilles de la Tourette's disease and delayed onset of puberty.

The effects of haloperidol on the release of prolactin, growth hormone, and luteinizing hormone during sleep were studied in an adolescent male who had Gilles de la Tourette's disease and delayed onset of puberty. At doses of 5 and 2 mg, haloperidol led to an increase of prolactin secretion and a suppression of luteinizing hormone release. Growth hormone was unaffected. Despite these changes, the patient had normal secondary sexual development consistent with puberty.

New concepts of the growth spurt of puberty.

The timing of the growth spurt of normal children does not affect their final height attainment. Growth rate during the growth acceleration of puberty correlates with changes in GH, and not sex steroid, secretion. GH secretion during puberty is pulse amplitude, and not frequency, modulated. In both central precocious puberty and normal puberty suppressed with a GnRH analogue, the rate of epiphyseal maturation is decreased as are GH pulse amplitude and growth, but with no influence on final height.

Laboratory monitoring of children with precocious puberty.

It is necessary not only to perform laboratory tests for the correct diagnosis of children with precocious puberty, but also to monitor laboratory tests to ensure adequacy of therapy. Careful laboratory testing is a requisite in the differentiation of central from peripheral precocious puberty. It is also required to determine whether the patient who presents with early physical changes of pubertal development with peripheral precocious puberty has evidence of pubertal hormonal secretion. The most useful single test is gonadotropin-releasing hormone (gonadorelin) stimulation of luteinizing hor

Diagnostic value of urinary luteinizing hormone levels in the monitoring of precocious puberty treatment.

Objective To determine whether first-voided urinary LH (FV-ULH) - level measurement can adequately assess pubertal suppression as much as standard tests can. Subjects and methods The study group included patients with central precocious puberty and rapidly progressing early puberty who received up to 3 - 4 doses of GnRHa therapy monthly and did not have adequate hormonal suppression after GnRH stimulation (90-minute LH level > 4 IU/L). Design: All of the participants underwent an LHRH test just after admission to the study. According to the stimulated peak LH levels, the patients were divided

[Giant aneurysm of the abdominal aorta and iliac arteries].

BACKGROUND/AIM: Central precocious puberty (CPP) is more common in females than in males. During the last few decades a new group of patients with CPP has been seen frequently in Northern Europe, namely children adopted from developing countries. GnRH analogue preparations, administered either as intranasal spray or as depot preparations, are the drugs of choice for inhibiting the release of gonadotropins. The aim of this study was to compare the effect of buserelin given by intranasal spray with that of the same compound given as a subcutaneous depot preparation. METHODS: The study group com

A delay in pubertal onset affects the covariation of body weight, estradiol, and bone size.

The skeletal system functions as a locomotive organ and a mineral reservoir and combinations of genetic and environmental factors affect the skeletal system. Although delayed puberty is associated with compromised bone mass, suppression of estrogen should be beneficial to cortical strength. The purpose was to employ path analysis to study bone strength and delayed puberty. Forty-five female rats were randomly assigned to a control group (n = 15) and an experimental group (n = 30) that received injections of gonadotropin releasing hormone antagonist (GnRH-a). Causal models were constructed by s

Pathogenesis and epidemiology of precocious puberty. Effects of exogenous oestrogens.

Precocious puberty is generally defined as the appearance of secondary sex characteristics before age 8 years in girls (or menarche before age 9 years) and before 9 years in boys. The overall incidence of sexual precocity is estimated to be 1:5,000 to 1:10,000 children. The female-to-male ratio is approximately 10:1. In addition to the psychosocial disturbances associated with precocious puberty, the premature pubertal growth spurt (with less time for prepubertal growth) and the accelerated bone maturation result in reduced adult height. Precocious puberty may be gonadotrophin-dependent [i.e.

The NIH experience with precocious puberty: diagnostic subgroups and response to short-term luteinizing hormone releasing hormone analogue therapy.

Between 1979 and 1983, 129 children (95 girls) with precocious puberty were referred to the National Institutes of Health and received treatment for at least 6 months with the long-acting LHRH analogue D-Trp6-Pro9-NEt-LHRH. The majority (107 of 129) of the children had central precocious puberty mediated by activation of the hypothalamic-pituitary-gonadal axis in association with hypothalamic hamartomas (24 of 107) or other central nervous system lesions (21 of 107), or idiopathic precocious puberty (62 of 107). Hypothalamic hamartomas or other central nervous system lesions were a frequent ca
