BACKGROUND: Climate change is tightening the links between human, animal, and environmental health, especially in settings where systems are already stretched. One Health is now widely recognised in policy circles, but in many cases, it remains more of an intention than something that is consistently practised.
METHODS: This paper draws on a qualitative policy analysis of academic literature, national policy documents, and selected comparative examples from Kenya, Bangladesh, and Germany. The aim was not to be exhaustive, but to focus on how implementation unfolds, paying attention to coordin

During the COVID-19 pandemic, a key challenge was designing control strategies that balance the benefits and costs of public health measures while preventing healthcare systems from becoming overwhelmed. This was often addressed by epidemiological modellers by implementing a binding (hard) constraint based on the maximum number of hospital beds that could be occupied at any given time. However, experience from the pandemic demonstrated that healthcare capacity depends not only on bed availability, but also on staffing and the availability of other resources. We argue that defining healthcare c

BACKGROUND: The Syrian conflict has severely damaged health infrastructure and displaced large portions of the skilled medical workforce. This study aims to assess the readiness of the health system in Damascus Governorate for the reconstruction phase and to identify priority areas for recovery in similar post-conflict contexts. METHODS: In February 2025, we conducted an anonymous Delphi-based online survey (Cronbach’s α = 0.89) among physicians, dentists, pharmacists, and nurses working in hospitals across Damascus Governorate. Five domains were assessed: health infrastructure, medical workfo

Public health surveillance for infectious diseases is highly compatible with the practice of primary care medicine and is enhanced by contextual and population-based elements when grounded in primary care. Moreover, there have been long and successful partnerships between primary care and public health for influenza monitoring. Surveillance programs can be based on sentinel, laboratory, or mechanistic approaches and need to reflect the needs of clinicians and the realities of the primary care environment. Participation in, and access to, surveillance information improves patient care through s

CONTEXT: Rural populations face greater barriers to care and worse health outcomes than urban populations. Local health departments (LHDs) serving rural populations may play a key role in bridging the clinical care gap within their communities.
OBJECTIVE: To assess and describe the differences in services provided by rural and urban LHDs, as well as the contextual factors contributing to those differences.
METHODS: Data from the 2022 National Profile of Local Health Departments was used to estimate the provision of various clinical and population-based services and types of occupations emplo

INTRODUCTION: National health security depends on a trained field epidemiology workforce. This study evaluates the Tunisia Field Epidemiology Training Program (T-FETP) from its launch in 2018 through 2024, with a focus on workforce capacity, geographic coverage, and structural barriers to program sustainability.
METHODS: We conducted a descriptive cross-sectional analysis of the national T-FETP graduate roster. Territorial coverage was assessed against the international benchmark of one Intermediate-level FETP graduate per 200,000 inhabitants. Density and capacity gaps were analyzed for each

Mental health and psychosocial support remain excluded from many emergency preparedness frameworks despite predictable psychological consequences of crises. This commentary presents four policy examples from African health systems arguing that mental health readiness should function as core health security infrastructure. Sierra Leone embeds mental health professionals in emergency governance; Liberia designates psychotropic medicines as essential with buffer stocks; Uganda deploys community health workers trained in mhGAP-HIG with supervision during floods; Ethiopia integrates mental health s

BACKGROUND: Unilateral economic sanctions are increasingly recognized as factors associated with structural determinants of health, yet their implications for health service delivery require in-depth examination. In Iran, these pressures appear to extend beyond medicine shortages to affect financing, procurement, infrastructure, workforce capacity, and continuity of care.
MAIN BODY: This policy commentary offers an in-depth analysis of how economic sanctions influence health service delivery in Iran, drawing upon a synthesis of existing literature and expert understanding of the Iranian conte

SUMMARYAfrica's ongoing struggles with emerging epidemics and antimicrobial resistance (AMR) underscore the urgency of integrating pathogen genomics and surveillance systems into the continent's One Health strategy, particularly given the existing limitations in preparedness and technological resources. This review brings together current evidence on the growth of sequencing infrastructure, the development of regional genomic hubs, and the establishment of governance frameworks, while identifying critical challenges in data integration, bioinformatics capacity, and sustainable financing. BACKGROUND: The Covid-19 pandemic highlighted a critical lesson for global health systems: effective preparedness depends on timely access to reliable and interconnected health data. Across many countries, fragmented information systems limited the rapid exchange and use of data needed for outbreak detection, response, and decision-making. In Uganda, the recurrent emergence of infectious disease outbreaks, including Ebola, Marburg virus disease, and cholera, has reinforced the importance of resilient and interoperable health information systems. Although substantial progress has been made in s

We documented the value and utility associated with staffing a Response Ready Epidemiologist (RRE) in state, territorial, and local health departments (STLs). Recipients of the Centers for Disease Control's Epidemiology and Laboratory Capacity Cooperative Agreement provided feedback showing that RREs are an efficient way to provide STLs discretion to use staffing to meet acute or urgent needs that arise. Other funding mechanisms can adopt similar disease-agnostic flexible models in lieu of disease-specific, siloed, and prescriptive funding to increase efficiency. (Am J Public Health. 2026;116(

Lessons and best practices from outbreaks during 2022-25 in Africa were not comprehensively documented or shared to inform future outbreak responses. We conducted a narrative review of published articles and outbreak response reports of mpox, cholera, Ebola virus disease, and Marburg virus disease and captured experts' perspectives and lessons. We analysed and presented the data in themes. Evidence indicates that effective responses are built on routine investments maintained between outbreaks, particularly in decentralised laboratories, digital surveillance systems, community structures, and

BACKGROUND: Ethiopia's Health Extension Program (HEP) aims to improve primary care access through community-based delivery of essential preventive and promotive services. Despite progress, some health outcomes remain suboptimal, underscoring the need to optimize HEP implementation. Identifying barriers of the HEP and its Health Extension Workers (HEWs) is critical for enhancing program effectiveness. This scoping review aims to identify and synthesize evidence on the barriers impacting the performance of the Ethiopian HEP and HEWs.

The COVID-19 pandemic did not take us completely by surprise. The experts in public health worldwide heard the warnings and tried to alert and act. In many places, surveillance systems to monitor the emergence of new pathogens were set up. Many countries worldwide worked on prevention and preparedness for epidemic-prone infectious diseases. The rapidity of the identification of a novel human virus, and the quick sharing of its full sequence compared to past epidemics is proof of the continuous improvement of our systems worldwide. Immediately after the emergence of SARS-CoV-2, the first diffic

Canada's healthcare system is approaching structural limits: nearly $400 billion in annual spending, while nearly one in five Canadians is without a regular healthcare provider, emergency departments overflow and health inequities persist that universal coverage alone cannot resolve. This paper argues that social prescribing, a structured referral system connecting people to community resources through trained link workers, offers not merely an additional program but a paradigm shift. By making visible the economic and social value of community-based health work, social prescribing functions a

BACKGROUND: Health systems worldwide are vulnerable to both acute shocks, which strain the health system for a relatively short period of time, and chronic stressors, which place persistent pressure on the health system. Given Nepal’s frequent exposure to public health threats, recent transition to a federal structure and systemic constraints, strengthening resilience at the local government level has emerged as a critical priority. We assessed the health system resilience capacities and identified gaps at the local government level to inform later interventions. METHODS: We conducted a cross-
