Atelectasis during anesthesia: pathophysiology and treatment.

BACKGROUND AND METHODS: The incidence of intraoperative pulmonary collapse is elevated in patients undergoing surgery under general anesthesia with muscle relaxation/paralysis. This complication is associated with worsening intraoperative gas exchange and, in some cases, the need for prolonged postoperative respiratory support. The objective of this report was to review the pathophysiological aspects of atelectasis during general anesthesia and possible therapeutic maneuvers that could prevent and treat this complication. CONTENTS: This review discusses the concepts about the incidence of int

Shrinking pleuritis with atelectasis.

During a 10-year period 28 patients with shrinking pleuritis with atelectasis (SPA) were observed and operated upon. This lesion has been given different names in the literature, for instance rounded atelectasis, pleuroma, pulmonary pseudotumour, and lung folding. All patients except two were operated upon because of a diagnosis of pulmonary tumour. However, at operation no tumour was found. The aetiology and pathogenesis of SPA are discussed on the basis of X-ray, operative, and histopathological findings.

[Rounded atelectasis of the lung].

Rounded atelectasis is an uncommon but increasingly recognized form of pulmonary collapse that often mimics a pulmonary neoplasm. It is suggested that these lesions can occur many years after exposure to asbestos or exudative pleural effusions. The authors report 16 cases of rounded atelectasis seen during a period of over 10 years, 9 of which in the last 18 months. They describe the plain film, tomographic and CT appearances of the lesion. The radiographic features that allow a diagnosis are rounded or oval shadows lying along the posterior surface of the lower lobe, adjacent to thickened ple

Rounded atelectasis of the lung: A pictorial review.

Rounded atelectasis of the lung is well described in medical literature, but still difficult to diagnose. Since lesions give no clinical symptoms in patients, radiologists are often the first to recognize the round lesion in an X-ray picture or a CT scan. Rounded atelectasis is an atypical form of lung collapse that usually occurs adjacent to scarred pleura and can be mistaken for lung cancer. Patients with rounded atelectasis have a history of asbestos exposure or pleural effusion due to various causes. When characteristic imaging findings are present, the diagnosis is rarely dubious and no f

Effect of different fraction of inspired oxygen on development of atelectasis in mechanically ventilated children: A randomized controlled trial.

BACKGROUND: The use of high fraction of inspired oxygen (FI O2 ) can cause direct pulmonary toxicity and pulmonary complications. The purpose of this study was to evaluate the effect of different FI O2 on development of intraoperative atelectasis in mechanically ventilated children using lung ultrasound. METHODS: In this randomized controlled, patient- and sonographer-blinded trial, 86 children (≤6 years) undergoing noncardiac surgery were allocated into a low (n = 43) or high (n = 43) FI O2 group. The low FI O2 group consistently received 30% air-oxygen mixture during preoxygenation, ultraso

The radiological aspects of chest complication in childhood measles.

Chest radiographs of 335 Nigerian children presenting with pulmonary and cardiac complications of measles were studied systematically. Perihilar (88%) and diffuse (69%) pulmonary infiltrates were the commonest findings. Other abnormalities included segmental consolidation (34%), emphysema (20%), cardiomegaly (12%) and atelectasis (10%). Pneumothorax and pleural effusion were seen in 24 (7%) and 14 (4%) children respectively. Eight children (2.5%) had subcutaneous and mediastinal emphysema. In patients with atelectasis and segmental consolidation there was a striking preponderance of right side

Pathogenesis of shrinking pleuritis with atelectasis--"rounded atelectasis".

The pathogenesis of "shrinking pleuritis with atelectasis" or "rounded atelectasis" is discussed on the basis of 37 operated patients and on experiments on cadaver lungs. Peroperative dissections with microscopic examinations and the results of experiments with the cadaver lungs support the concept that the lesion is caused by an inflammatory reaction in the visceral layer of the pleura, caused by asbestos fibers. The inflammation occurs in stages, with deposition of connective tissue that shrinks and causes considerable atelectasis of the underlying pulmonary parenchyma. Compression of the lu

Repeated endotracheal intubations and bronchial lavage in the postoperative treatment of tracheoesophageal fistula.

Recurrent pulmonary atelectasis occurs often in neonates following primary repair of esophageal atresia, leading to massive pneumonia and respiratory failure. The atelectasis is caused by tenacious mucous plugs which are presumably formed as a result of ciliary mucosal damage secondary to reflux of gastric fluid through a tracheo-esophageal fistula. Early aggressive treatment of the atelectasis by repeated gentle tracheal intubation with bronchial lavage and suction is advocated. This treatment is well tolerated and is preferred to continuous intubation with its inherent dangers for the neonat

Use of lung-preserving surgery in left inflammatory bronchial occlusion and distal atelectasis: preliminary results.

OBJECTIVES: Lung-preserving surgery was proved to be effective and safe to treat patients with benign bronchial strictures. However, this surgical treatment has been rarely reported in patients with complete occlusion in the left main bronchus. The aim of this study was to assess the value of this procedure and report our experience in the treatment of these patients with left atelectasis caused by inflammatory bronchial occlusion. METHODS: We reviewed and analysed the medical records of 8 patients who had undergone left main bronchus sleeve resection for symptomatic left atelectasis caused b

A 23-year-old man with left lung atelectasis treated with a targeted segmental recruitment maneuver: a case report.

BACKGROUND: Lung atelectasis are nonventilated parts of lung tissue and occur as a result of the collapse of the pulmonary parenchyma (alveoli). Various therapeutic procedures for inflating the collapsed pulmonary parenchyma, such as bronchial aspiration and/or standard recruitment maneuvers, are not always successful. CASE PRESENTATION: We report a case of a 23-year-old Croatian man with a parapharyngeal abscess on the left side of the neck with spreading of infection in the mediastinum and left side of the thorax and consequent major atelectasis of the left lung. The patient was mechanicall

The Effect of Different Levels of PEEP on the Occurrence of Atelectasis After CABG: A Retrospective Study From Palestine.

Respiratory complications are among the most common issues post coronary artery bypass grafting (CABG), with atelectasis being one of the most serious respiratory consequences. This study aims to evaluate the association between positive end-expiratory pressure (PEEP) levels and post-CABG atelectasis, investigate demographic risk factors associated with atelectasis, and determine the timing pattern of atelectasis development. A retrospective analysis was conducted on data from 268 CABG patients. Three PEEP levels-5, 8, and 10 cm H2O were considered. Demographic information and postoperative ou

The incidence of postoperative atelectasis following gastrointestinal procedures in pediatric population.

BACKGROUND: Postoperative pulmonary complications including atelectasis are common complications after surgery. However, the incidence of postoperative atelectasis in pediatric surgical population is not yet well delineated. METHODS: Using electronic medical record, we identified pediatric patients who underwent gastrointestinal surgery from January 2016 to September 2019 and determined the presence or absence of postoperative atelectasis by postoperative X-ray read. Risk factor analysis of postoperative atelectasis was performed using logistic regression analysis. RESULTS: We found that 25.

Wegener's Granulomatosis: CT evolution of pulmonary parenchymal findings in treated disease.

OBJECTIVE: To determine the computed tomography (CT) evolution of various pulmonary manifestations of Wegener's Granulomatosis (WG) following appropriate pharmacologic treatment of the disease. METHODS: Eleven patients with WG were identified, each of whom had had at least two CT examinations. CTs were reviewed retrospectively to identify pulmonary lesions of WG. Lesions were categorized as nodules, cavities, lobar atelectasis, pulmonary bands, or infiltrates. To determine the evolution of each individual lesion following pharmacologic treatment, the authors compared the lesions at presentati

Rounded atelectasis of the lung, just a benign disorder.

Three patients with rounded atelectases are described. One of them developed a malignant non-Hodgkin lymphoma 6 months after presentation with rounded atelectasis. His rounded atelectasis could be followed during 20 months and was unrelated to the appearance, complete remission after chemotherapy and relapse of a malignant non-Hodgkin lymphoma. Rounded atelectasis of the lung is a little-known form of peripheral pulmonary collapse which may mimic a neoplastic tumour. It might be formed either because of a folding in a basal lung segment caused by temporarily pleural effusion, or because of ini
