[Radial nerve entrapment in the spiral groove. Nerve block as a diagnostic and therapeutic tool].

The radial nerve has a long and sinuous course in the upper limb from the axilla to the hand and fingers. There are several possible areas of compression along this trajectory, the most frequent being on the Arcade of Frohse, with entrapment of its terminal nerve, the posterior interosseous nerve. We report the case of a patient with radial nerve entrapment in the spiral groove and describe how ultrasound and nerve blocks could be useful in diagnosis and treatment. In our patient, nerve block at the main radial nerve in the spiral groove was insufficient. A second nerve block was needed in the

Tendon Transfers, Nerve Grafts, and Nerve Transfers for Isolated Radial Nerve Palsy: A Systematic Review and Analysis.

BACKGROUND: Isolated radial nerve palsy is a debilitating injury that may potentially be reconstructed with either tendon transfers, nerve grafts, or nerve transfers. Currently, there is no consensus on the optimal technique for reconstruction. We performed a systematic review and analysis to determine which surgical intervention provides the best clinical outcomes. METHODS: A systematic review was conducted according to PRISMA guidelines. Twenty-nine papers met inclusion criteria. Grading scales of function and strength were converted into a tripartite scoring system to compare outcomes betw

Sural nerve autografts for high radial nerve injury with nine centimeter or greater defects.

PURPOSE: The purpose of this study is to present the outcome of sural nerve grafting that focuses on motor fascicles in 6 patients with high radial nerve injuries with defects of more than 9 cm. METHODS: Six patients with a high radial nerve complete injury were treated with a sural nerve autograft during a 6-year interval from June 1994 to June 2000. The gap of the radial nerve, which was measured in the surgical field, was 9 cm in 1 patient, 10 cm in 4 patients, and 11 cm in 1 patient. There were 4 men and 2 women with an average age of 32 years (range, 14-49 years). The causes of the injur

Radial motor nerve conduction study in posterior interosseous nerve syndrome and multifocal motor neuropathy.

BACKGROUND: Finger extension weakness could be a presentation of either posterior interosseous nerve (PIN) syndrome or multifocal motor neuropathy (MMN). However, there is a delay in the diagnosis of MMN in cases with a selective radial weakness, as they are frequently misdiagnosed as PIN. OBJECTIVE: To analyze which variables in nerve conduction studies could aid in the early diagnosis of MMN. METHODS: We reviewed charts of patients with diagnoses of MMN or PIN syndrome, from 2014 to 2022, in a single Brazilian reference center. Electrophysiological parameters included in the analysis were

Radial Nerve Palsy in the Setting of Humeral Shaft Fracture.

The upper limb has a complex anatomy comprised of many nerve and vascular structures, making humeral shaft fractures extremely important. Injury to the humeral shaft commonly occurs due to trauma and affects younger male or older female patients. The radial nerve travels along the spiral groove of the humerus, placing it at an increased risk of damage in humeral shaft fractures. If injured, there are a variety of classifications of radial nerve injury, different indications for exploration, and treatment methods that orthopedic surgeons have available in treating these injuries. This review ai

Surgical anatomy of the radial nerve in the arm: a cadaver study.

PURPOSE: The purpose of this study was to analyse the anatomic course of the radial nerve (RN) in the arm, in order to minimize the potential risk of surgical injury. METHODS: The study was performed in 19 embalmed upper extremities of 11 adult human cadavers. We measured: distance from deltoid insertion (DI) into the humerus to lateral epicondyle (LE); distance from RN piercing point into the lateral intermuscular septum (LIS) to three other points-DI, LE and RN division into superficial and deep terminal branches; distance between the LE and the RN division. To assess variability, we correl

Management of radial nerve palsy following fractures of the humerus.

Radial nerve palsy is the most common peripheral nerve injury following a humerus fracture, occurring in 2% to 17% of cases. Radial nerve palsies associated with closed humerus fractures have traditionally been treated with observation, with late exploration restricted to cases without spontaneous nerve recovery at 3 to 6 months. Advocates for early exploration believe that late exploration can result in increased muscular atrophy, motor endplate loss, compromised nerve recovery upon delayed repair, and significant interval loss of patient function and livelihood. In contrast, early exploratio

[Radial nerve palsy as a presenting feature of neuralgic amyotrophy].

A 28-year-old man noticed weakness in his left arm when he woke up. He was diagnosed as left radial nerve palsy and managed conservatively at a local hospital. A few days later, severe pain of the brachium appeared. Although severe pain improved in a year, dysesthesia and muscle atrophy remained. On admission, muscle weakness and atrophy were found in muscles innervated predominantly by the left radial nerve. In addition, needle-electromyography and computed tomography revealed the involvement of muscles innervated by the left suprascapular, long thoracic and axillary nerves, and we diagnosed

Analysis of Tsuge's procedure for the treatment of radial nerve paralysis.

Radial deviation and limited flexion of the wrist joint and a lack of abduction of the thumb have been noticed after the Riordan's procedure. Therefore, Tsuge et al. modified the Riordan's procedure, and their procedure includes transfer of the pronator teres to the extensor carpi radialis brevis, the flexor carpi radialis (FCR) to the extensor digitorum communis (EDC), and the palmaris longus to the extensor pollicis longus, along with tenodesis of the abductor pollicis longus. We reviewed the charts of 21 patients with isolated radial nerve paralysis who were treated with the Tsuge's procedu

Surgical management and outcome in patients with radial nerve lesions.

OBJECT: The goal of this paper was to review surgical management and outcomes in patients treated for radial nerve (RN) lesions at Louisiana State University Health Sciences over a period of 30 years. METHODS: Two hundred sixty patients with RN injuries were evaluated. The most common mechanisms of injuries involving the RN included fracture of the humerus, laceration, blunt contusions, and gunshot wounds. One hundred and eighty patients (69%) underwent surgery. Lesions not in continuity required primary or secondary end-to-end suture repairs or graft repairs. With the use of direct intraoper

Influence of electrical stimulation on regeneration of the radial nerve in dogs.

The effects of biphasic electric fields on nerve regeneration that follows injury to the left radial nerve were studied in dogs by electromyography (EMG). Left and right radial nerves were crushed with a serrated haemostat. Stimulating electrodes were positioned proximally and distally to the site of the injury. The left nerves received rectangular, biphasic and current pulses (30 microA, 0.5 Hz) through the injury for two months. The right radial nerves were treated as controls and regenerated without electrical stimulation. EMG activities were recorded intramuscularly from the left and right

Radial and axillary nerves. Anatomic considerations for humeral fixation.

Because the axillary and radial nerves can be injured during operative exposure and fixation of the humerus, accurate delineation of their location is vital to avoid complications. The authors investigated the relationship of the radial and axillary nerves for radiographically and surgically identifiable bony landmarks. Fifty fresh human cadaveric upper extremities were dissected to identify the nerves as they crossed the lateral intermuscular septum and the humeral surgical neck, respectively. Longitudinal distances between the nerves and the superior aspect of the humeral head, the surgical

Intraneural Nodular Fasciitis of the Radial Nerve: A Case Report.

CASE: We report a case of intraneural nodular fasciitis in the forearm initially suspected as a schwannoma, emphasizing the importance of accurate diagnosis. A 40-year-old woman presented with mass on the lateral aspect of her right forearm and radial neuropathy symptoms for 2 months. An excisional biopsy and histopathological examination confirmed nodular fasciitis. Postoperative evaluation at 4.5 years found no pain, paralysis, or recurrence. CONCLUSION: Awareness of nodular fasciitis is crucial to prevent misdiagnosis and unnecessary treatment. Despite its rapid growth, nodular fasciitis g
