The radial nerve is the biggest branch of the posterior cord of the brachial plexus and one of its five terminal branches. Entrapment of the radial nerve at the elbow is the third most common compressive neuropathy of the upper limb after carpal tunnel and cubital tunnel syndromes. Because the incidence is relatively low and many agents can compress it along its whole course, entrapment of the radial nerve or its branches can pose a considerable clinical challenge. Several of these agents are related to normal or variant anatomy. The most common of the compressive neuropathies related to the r

Radial nerve entrapment at the lateral intermuscular septum in the distal upper arm can cause debilitating pain and functional impairment. The nerve passes from the dorsal to the volar compartment through the LIS, approximately 8-10 cm proximal to the lateral epicondyle, making it susceptible to compression. Clinical signs include wrist drop, localized pain, and weakness in wrist and finger extension, often seen in manual laborers and athletes. A minimally invasive, ultrasound-guided, percutaneous technique using local anesthesia and no tourniquet allows precise nerve decompression while prese

Soft tissue swellings on the forearm can present with a range of clinical and histopathological diagnosis. Ancient Schawanoma is a rare benign condition that can develop over the flexor surface of the forearm as a cystic swelling and can involve the median or the ulnar nerve. However, the presentation of this condition on the extensor surface with involvement of the radial nerve is an extremely uncommon diagnosis. A 69 year old female presented at the outpatient department with a swelling on the extensor aspect of her right forearm for the past 2 years. Ultrasound examination showed a mixed cy

CASE: We report a case of intraneural nodular fasciitis in the forearm initially suspected as a schwannoma, emphasizing the importance of accurate diagnosis. A 40-year-old woman presented with mass on the lateral aspect of her right forearm and radial neuropathy symptoms for 2 months. An excisional biopsy and histopathological examination confirmed nodular fasciitis. Postoperative evaluation at 4.5 years found no pain, paralysis, or recurrence.
CONCLUSION: Awareness of nodular fasciitis is crucial to prevent misdiagnosis and unnecessary treatment. Despite its rapid growth, nodular fasciitis g

Radial nerve entrapment at the LIS is an aching pain in the distal, lateral upper arm and leads to weakness in wrist and thumb/index finger extension. Additionally, a positive scratch collapse test and localized pain on pressure at the entrapment site are observed. The described approach uses a minimally invasive percutaneous ultrasound-guided technique under local anesthesia.

BACKGROUND: Isolated radial nerve palsy is a debilitating injury that may potentially be reconstructed with either tendon transfers, nerve grafts, or nerve transfers. Currently, there is no consensus on the optimal technique for reconstruction. We performed a systematic review and analysis to determine which surgical intervention provides the best clinical outcomes.
METHODS: A systematic review was conducted according to PRISMA guidelines. Twenty-nine papers met inclusion criteria. Grading scales of function and strength were converted into a tripartite scoring system to compare outcomes betw

CASE: A 6-year-old boy sustained complete radial nerve palsy with a Gartland type III supracondylar humerus fracture (SCHF). Posteromedial displacement of the distal fragment was so severe that the tip of the proximal fragment protruded subcutaneously at the anterolateral aspect of the antecubital fossa. Immediate surgical exploration was performed to reveal radial nerve laceration. Neurorrhaphy after fixation of the fracture resulted in full recovery of radial nerve function 1 year postoperatively.
CONCLUSIONS: Severe posteromedial displacement with complete radial nerve palsy may warrant ac

Hourglass-like constriction (HLC) is an uncommon spontaneous mononeuropathy that is typically characterised by a sudden onset of pain followed by palsy, affecting branches of the radial (posterior interosseous nerve) and median nerves (anterior interosseous nerve). HLC of the radial nerve (RN) is rare, with only a few reported cases. Here, we report a case of a man who presented with acute wrist and finger drop due to the HLC of the RN. Surgery was recommended 5 months after clinical observation, when the lesion was resected and primarily repaired, resulting in satisfactory recovery. There is

There are very few descriptions of tendon transfers designed specifically to address the reconstruction of posterior interosseous nerve palsy (PINP). Unlike a radial nerve palsy (RNP), a patient with a PINP is able to extend their wrist but in radial deviation, because of the preserved innervation of the extensor carpi radialis longus (ECRL). Tendon transfers to restore finger and thumb extension in PINP have been extrapolated from tendon transfers to restore these functions in RNP, specifically using flexor carpi radialis, not flexor carpi ulnaris, so as not to further exacerbate the distinct

This study analyzes the effectiveness of ultrasound-guided hydrodissection (HD) perineural as a treatment for radial tunnel syndrome (RTS). A literature search was performed along with retrospective analysis of local cases to assess outcomes and safety of this procedure. In the case series, surgical candidates, defined as cases with over 80% but temporary relief after diagnostic injection, were treated with ultrasound-guided HD. Of 22 patients who received ultrasound-guided diagnostic injections, 11 proceeded to HD. All HD patients experienced complete and lasting symptom resolution for a mini

BACKGROUND: The aim of this study was to retrospectively evaluate patients who underwent surgery in our department for radial nerve lesions in terms of surgical outcomes.
METHODS: Thirty-eight patients were admitted to our department with radial nerve lesion. Twenty-seven of the patients had entrapment neuropathy and 11 had radial nerve injury secondary to other traumas. Various factors such as surgical results, time to surgical intervention, injury mechanism, and reconstruction technique were analyzed.
RESULTS: In all of 27 patients who were operated for radial nerve entrapment neuropathy,

CASE: A 30-year-old man sustained an injury to the right elbow after a fall from a bike. On clinical examination, he had preoperative radial nerve palsy with radiological diagnosis of the intercondylar distal humerus fracture. Intraoperatively, the patient had the radial nerve entrapped between the proximal metaphyseal fragment and distal lateral condyle of the humerus.
CONCLUSION: Preoperative radial nerve palsy associated with the intercondylar distal humerus fracture is very rare. Fractures with the significant anterolateral displacement of the proximal metaphyseal fracture segment may ent

PURPOSE: With radial nerve lesions, the results of nerve transfers and how they objectively compare with the outcomes of tendon transfers remain unstudied. We compared the results after nerve transfer in patients with less than 12 months since radial nerve injury with the results after tendon transfer in patients not eligible for nerve surgery because of longstanding paralysis (minimum of 15 months).
METHODS: In 14 patients with radial nerve lesions incurred less than 12 months previously, we transferred the anterior interosseous nerve to the nerve of the extensor carpi radialis brevis (ECRB)

High-resolution magnetic resonance neurography (MRN) is being increasingly employed for the evaluation of peripheral nerves. We describe a case of spiral groove syndrome (Saturday night palsy) and illustrate the role of advanced 3 Tesla MRN imaging techniques, including three-dimensional diffusion-weighted fat-suppressed reversed fast imaging with steady-state precession and diffusion tensor imaging.

PURPOSE: A suitable muscle motor in reconstruction after acquired never injury should have adequate strength to perform the desired task, be aligned in the direction of pull, have synergistic action, and not result in unacceptable functional loss. In radial nerve palsy, the pronator teres is the most common motor donor used to restore wrist extension. Although the pronator teres remains aligned to provide pronation, the force deficit of the transfer is not known.
METHODS: We used 6 cadavers and 6 patients to determine the loss of pronation strength both experimentally and clinically.
RESULTS

This case report describes a 44-year-old woman who suffered a radial nerve compression injury from self-administered myofascial release using a lacrosse ball. She underwent decompression of the right radial nerve at the spiral groove of the humerus with intraoperative nerve stimulation. At 18-month follow-up, the patient had regained partial function of the radial nerve. This case highlights mechanisms of radial nerve compression injuries and discusses nonoperative versus operative treatment options.

BACKGROUND: Intrafascicular nerve torsion is a rare and poorly studied type of nerve compression. This disease can be assigned to compression-induced neuropathies, but it has a distinctive feature. It is a spontaneous intrafascicular compression following internal local spiral deformation and compression of the nerve outside routine tunnels due to its torsion around its own axis. Understanding the pathogenesis of such spiral compression is essential in diagnosis, prognosis and treatment of these patients.
OBJECTIVE: To assess the effectiveness of various diagnostic techniques, the possibility

Malignant schwannoma (MPNST) is a rare tumor. Many of these cases occur in patients with neurofibromatosis type I (NF-1) and they usually present as a mass lesion which rapidly increase in size with or without neurological symptoms. Here we present an unusual case of MPNST along the radial nerve in which the patient has no underlying neurofibromatosis type I.
