PURPOSE: The dose distributions of intensity-modulated radiotherapy (IMRT) treatment plans can be shown to be significantly superior in terms of higher conformality if designed to simultaneously deliver high dose to the primary disease and lower dose to the subclinical disease or electively treated regions. We use the term "simultaneous integrated boost" (SIB) to define such a treatment. The purpose of this paper is to develop suitable fractionation strategies based on radiobiological principles for clinical trials and routine use of IMRT of head and neck (HN) cancers. The fractionation strate

PURPOSE: To investigate the role of intertumor heterogeneity in clinical tumor control datasets and the relationship to in vitro measurements of tumor biopsy samples. Specifically, to develop a modified linear-quadratic (LQ) model incorporating such heterogeneity that it is practical to fit to clinical tumor-control datasets.
METHODS AND MATERIALS: We developed a modified version of the linear-quadratic (LQ) model for tumor control, incorporating a (lagged) time factor to allow for tumor cell repopulation. We explicitly took into account the interpatient heterogeneity in clonogen number, radi

The risk of secondary cancer induction after a therapeutic irradiation with conventional photon beams is well recognised and documented. However, in general, it is totally overwhelmed by the benefit of the treatment. The same is true to a large extent for the combinations of radiation and drug therapy. After fast neutron therapy, the risk of secondary cancer induction is greater than after photon therapy. This can be expected from the whole set of radiobiological data, accumulated so far, which shows systematically a greater relative biological effectiveness (RBE) for neutrons for all the biol

Since the introduction of the high-dose rate afterloading therapy a higher biological effectivity of this method has been well known in comparison with the low-dose contact therapy. This will practically taken into account by reducing the dose and more fractionating of the total dose. With our test we want: firstly to prove the influence of the therapy break on the duration of the radiation reaction. Secondly we want to prove the influence of the dose rate on the efficiency of the radiation reaction. We have tried to answer the question by the animal model guinea pig skin. We examined early re

Radiobiological data have been used for an analysis of acute and late tissue responses on the basis of a linear-quadratic survival function SLQ = exp(-alpha d -beta d2). Isoeffect curves of late effects exhibit a steeper slope than those for the corresponding acute effects, and, therefore, late effects may become an essential aspect in the radiotherapy of many kinds of tumours. In this communication, the theoretical and radiobiological view will be presented, whereas in a second part clinical applications will play a dominant role: Thus a statistical analysis of clinical observations has revea

Respiratory motion management was crucial importance in radiotherapy. Our study aimed to explore the dosimetric and radiobiological benefits of various respiratory motion management strategies in liver tumor patients undergoing stereotactic body radiotherapy (SBRT). Respiratory motion images of 23 liver tumor patients were obtained by 4DCT scan under abdominal compression (AC) and free-breathing (FB). Group A included all 23 patients, while group B focused on 11 patients with tumors < 5 cm in diameter. Dosimetric and radiobiological parameters were recorded and compared across three treatment

PURPOSE: The use of Monte Carlo (MC) simulations capable of reproducing radiobiological effects of ionising radiation on human cell lines is of great importance, especially for cases involving protons and heavier ion beams. In the latter, huge uncertainties can arise mainly related to the effects of the secondary particles produced in the beam-tissue interaction. This paper reports on a detailed MC study performed using Geant4-based approach on three cancer cell lines, the HTB-177, CRL-5876 and MCF-7, that were previously irradiated with therapeutic proton and carbon ion beams.

BACKGROUND: Osteosarcoma (OS) is the most common primary malignancy of the bone and is notoriously resistant to radiation therapy. High-dose cytotoxic chemotherapy and surgical resection have improved the survival rate and prognosis of patients with OS. Nonetheless, treatment challenges remain when the tumor cannot be removed by surgery. Boron neutron capture therapy (BNCT) provides high linear energy transfer (LET) radiation, and its internal targeted characteristics make BNCT a novel therapy for removing OS and reducing radiation damage to adjacent healthy tissues.

PURPOSE: MacCune-Albright syndrome (MAS) is a rare autosomal dominant osteo-hormonal disorder. MAS is characterized by a severe form of polyostotic fibrous dysplasia, 'café-au-lait' pigmentation of the skin and multiple endocrinopathies. MAS was shown to be caused by mosaic missense somatic mutations in the GNAS gene coding for the alpha-subunit of the stimulatory G-protein. MAS is also associated with radiation-induced malignant tumors, like osteosarcoma, fibrosarcoma and chondrosarcoma but their origin remains misunderstood. In parallel, bisphosphonates treatment was shown to improve the MAS

PURPOSE: The present report addresses the question of what could be the appropriate dose and dose rate for 125I and 103PD permanent seed implants for breast cancer as monotherapy for early stage breast cancer. This is addressed by employing a radiobiological methodology, which is based on the linear quadratic model, to identify a biologically effective dose (BED) to the prescription point of the brachytherapy implant, which would produce equivalent cell killing (or same cell survival) when compared to a specified external radiotherapy scheme.
METHODS: In the present analysis, the tumor and no

The efficiency of the radiobiological and the clinical planning of the combination of the intraoperative radiation therapy (IORT) and the external beam radiation therapy (EBRT) was assessed according to the incidence of local recurrences and to the level of radiation-induced damages during 5 years for patients with malignant tumors of head and neck, lung and soft tissues. Criteria of radiobiological planning for performing IORT + EBRT using the modified model of TDF (time-dose-fractionation) for calculating a single IORT dose and total radiation doses was defined among 169 patients of the stud

BACKGROUND: Radiation therapy planning (RTP) for abdominal tumors often requires multiple dose-volume constraints for each organ at risk (OAR), which vary with different fractionation schemes. This variability can complicate biologically optimized treatment planning.
PURPOSE: This study aims to extract fractionation-independent dose constraints based on the concept of equivalent uniform dose (EUD), utilizing published clinical data for a range of fractionation regimens. The goal is to derive organ-specific radiobiological parameters as well as the EUD for each OAR that can be used in biologic

INTRODUCTION: The continued reliance upon the traditional biologically effective dose (BEDT) formalism of BEDT=nd(1+d/(α/β)) may be one possible contributor to the poor clinical outcomes observed with single-fraction 19-20 Gy prescriptions in prostate high-dose-rate (HDR) brachytherapy because BEDT does not consider intrafraction sublethal damage repair (iSLDR). This, along with low α/β and repair half-times comparable to delivery time, could reduce the biological effect predicted using BEDT.
METHODS AND MATERIALS: BED was recalculated with a model accounting for iSLDR, using time-averaged un

Studies about radiation-induced human cataractogenesis are generally limited by (1) the poor number of epithelial lens cell lines available (likely because of the difficulties of cell sampling and amplification) and (2) the lack of reliable biomarkers of the radiation-induced aging process. We have developed a mechanistic model of the individual response to radiation based on the nucleoshuttling of the ATM protein (RIANS). Recently, in the frame of the RIANS model, we have shown that, to respond to permanent endo- and exogenous stress, the ATM protein progressively agglutinates around the nucl

BACKGROUND: The truncation level of human airways is an influential factor in the analysis of respiratory flow in numerical simulations. Due to computational limitations and limited resolution of diagnostic medical imaging equipment, a truncated geometry of airways is always investigated.
OBJECTIVE: This study aimed to employ image-based geometries with zero generation and 5th-generation truncation levels and assess bronchial airways truncation's effect on tracheal airflow characteristics.
MATERIAL AND METHODS: In this numerical study, computational fluid dynamics was employed to solve the r

To perform a comparison of the different stereotactic body radiotherapy (SBRT) plans between the Varian EDGE and CyberKnife (CK) systems for locally advanced unresectable pancreatic cancer. Fifteen patients with pancreatic cancer were selected in this study. The median planning target volume (PTV) was 28.688 cm3 (5.736-49.246 cm3). The SBRT plans for the EDGE and CK were generated in the Eclipse and Multiplan systems respectively with the same contouring and dose constrains for PTV and organs at risk (OARs). Dose distributions in PTV were evaluated in terms of coverage, conformity index (CI),
